Emergency numbers differ by country. This page uses 911, the number for the United States and Canada. Many other countries use 112, 999, or another number. Find the number for your country.
Recognizing exact, measurable red flags for both the birthing parent and the newborn saves lives. Whether you are evaluating soaking vaginal bleeding (postpartum hemorrhage), a sudden spike in blood pressure days after hospital discharge (postpartum preeclampsia), chest or leg pain (pulmonary embolism or deep vein thrombosis), surgical or uterine infection (puerperal sepsis), acute psychiatric crisis (postpartum psychosis), or a newborn with retractions, bilious green vomiting, or fever, acting immediately and stating the word "Postpartum" to emergency responders triggers specialized obstetric and neonatal protocols.
- Call your local emergency number immediately if the birthing parent or newborn shows any immediate life-threatening red flag: Dial 911 in the United States and Canada, 112 across the European Union, 999 in the United Kingdom, or 000 in Australia (see بیړنۍ شمیرې). Never wait for an obstetrician's office to return a voicemail when an urgent symptom appears.
- Say these exact words to the emergency dispatcher and triage nurse: "I had a baby on [Date], this is a postpartum emergency" (or "My baby is [X] days old"): Many general emergency department staff do not visually recognize that a patient who gave birth 5 or 14 days ago is postpartum. Stating that you recently gave birth (or had a miscarriage or cesarean section) immediately alerts doctors to screen for postpartum eclampsia, pulmonary embolism, peripartum cardiomyopathy, and uterine hemorrhage.
- If bleeding heavily, lie flat, elevate your legs, and perform firm uterine fundal massage if the lower abdomen feels spongy: If you are soaking through more than one heavy maxi pad per hour for 2 hours in a row or passing blood clots larger than a golf ball or plum (5 centimeters / 2 inches), lie flat on your back, empty your bladder if able, place one hand just above your pubic bone to support the lower uterus, and use your other hand to firmly massage the top of the uterus (near your navel) in circular motions until it firms up like a grapefruit while awaiting paramedics.
- If the birthing parent has a severe headache, vision changes, or Blood Pressure of 160/110 mmHg or higher, sit quietly in a dim room and go to the emergency room immediately: Do not take a nap to "sleep off" a pounding postpartum headache accompanied by blurred vision, flashing lights, upper-right abdominal pain, or sudden facial/hand swelling. This is a classic presentation of postpartum preeclampsia, which can trigger a stroke or grand mal seizure (eclampsia) within hours without intravenous antihypertensive and magnesium sulfate therapy.
- Take a rectal temperature on any newborn under 28 days old who feels warm, cool, or lethargic: If a newborn under 28 days (4 weeks) old has a rectal temperature of 38.0 degrees Celsius (100.4 degrees Fahrenheit) or higher, or a low rectal temperature below 36.0 degrees Celsius (96.8 degrees Fahrenheit), go straight to a hospital emergency department immediately. Never give fever-reducing medication (acetaminophen/paracetamol or ibuprofen) to a newborn before a doctor examines them.
- Check a distressed newborn's naked chest and abdomen for breathing retractions, grunting, or blue lips: Strip off the baby's swaddle and shirt under good light. If the baby is breathing faster than 60 breaths per minute continuously, making a rhythmic "ugh" grunting sound on every exhale, flaring their nostrils, sucking the skin deeply inward between their ribs or under their breastbone (retractions), or showing blue/gray color around the tongue and lips (central cyanosis), call emergency services right away.

1. Postpartum Hemorrhage (PPH): Primary vs Secondary¶
Normal postpartum vaginal discharge (lochia) starts bright red like a heavy menstrual period for the first 3 to 4 days (lochia rubra), then gradually lightens to pinkish-brown (lochia serosa) and yellowish-white (lochia alba) over 4 to 6 weeks. Bright red bleeding that suddenly increases rather than decreases is never normal.
- Primary Postpartum Hemorrhage occurs within the first 24 hours after delivery, usually while still in the hospital or after a home birth, most commonly caused by uterine atony (the uterus fails to contract tightly to pinch off the blood vessels where the placenta was attached, accounting for 70 to 80 percent of cases), genital tract lacerations, retained placental tissue, or blood clotting disorders.
- Secondary (Delayed) Postpartum Hemorrhage occurs between 24 hours and 12 weeks after birth (most often peaking around days 7 to 14 when the placental eschar sloughs off inside the uterus, or due to retained placental fragments or uterine infection).
- Exact clinical thresholds to call emergency services:
- Soaking through 1 or more heavy sanitary pads per hour for 2 hours in a row (never use tampons or menstrual cups during the first 6 weeks postpartum, as they can introduce lethal bacteria into the healing uterus and mask heavy bleeding).
- Passing blood clots larger than a golf ball or plum (5 centimeters / 2 inches).
- Signs of hypovolemic shock (which can occur even if external vaginal bleeding looks moderate because up to 1,000 milliliters / 1 quart of blood can pool invisibly inside a relaxed uterus): a racing pulse (over 110 beats per minute), lightheadedness when sitting up, pale or cold clammy skin, blurred vision, confusion, or a feeling of impending doom.
- First aid while waiting for the ambulance:
- Have the patient lie flat on her back and elevate her feet 20 to 30 centimeters (8 to 12 inches) on pillows to maintain blood flow to the brain and heart. Keep her warm with a blanket.
- Perform External Bimanual / Fundal Uterine Massage: Feel the lower abdomen around the level of the belly button (umbilicus). Normally, the contracted postpartum uterus feels firm and round like a hard grapefruit. If it feels soft, boggy, or spongy like dough, place one hand flat just above the pubic bone to brace the lower uterine segment, and use the palm of your other hand to firmly rub and squeeze the top of the uterus (the fundus) in circular motions. Expect a gush of pooled blood or clots to exit as the muscle contracts; keep massaging firmly until the uterus feels rock-hard and paramedics take over.
(ACOG/NICE/WHO):
- A fever of 38.0 degrees Celsius (100.4 degrees Fahrenheit) or higher, or an abnormally low body temperature below 36.0 degrees Celsius (96.8 degrees Fahrenheit), accompanied by shaking chills (rigors).
5. Baby Blues vs Postpartum Depression vs Postpartum Psychosis¶
Typical Onset & Prevalence | Key Symptoms | Reality Testing (Insight) | Required Action |
|---|---|---|---|---|
| Postpartum "Baby Blues" | Starts Days 2 to 3 after birth; peaks Day 5; resolves within 2 weeks (Affects 50% to 80% of parents) | Tearfulness, mood swings, feeling overwhelmed, irritability, trouble sleeping even when tired | 100% Intact: Parent knows their feelings are hormonal tiredness and remains deeply bonded and realistic | Rest, uninterrupted 4-hour sleep blocks handled by partner/family, hydration, emotional support. |
| Postpartum Depression (PPD) / Postpartum OCD | Starts anytime within the first year; lasts longer than 2 weeks (Affects 10% to 15% of parents) | Persistent deep sadness, numbness, inability to bond, overwhelming guilt, panic attacks, or intrusive ego-dystonic scary thoughts (where the parent is horrified by accidental harm thoughts and takes extreme steps to keep baby safe) | منظم: مور او پلار مداخله کونکي فکرونه د ناغوښتل شوي اندیښنې په توګه پیژني او هیڅ لیوالتیا نلري چې ماشوم ته زیان ورسوي | د OB/GYN، قابله یا د زیږون د رواني روغتیا متخصص سره سمدستي ملاقات؛ د زیږون وروسته د ملاتړ نړیوال سره اړیکه ونیسئ (1-800-944-4773). |

- Jaundice in the first 24 hours of life: Any yellowing of the skin or eyes visible within the first 24 hours after birth is always abnormal (often caused by ABO or Rh blood group incompatibility breaking down red blood cells rapidly) and requires immediate hospital bilirubin testing and phototherapy.
- Jaundice spreading down to the palms of the hands and soles of the feet: Jaundice progresses from head to toe (cephalocaudal progression). When yellowing reaches the lower legs, palms, or soles, or the skin looks bright orange-yellow, bilirubin levels are dangerously high.
- Jaundice accompanied by Chalky White / Pale Stools or Dark Tea-Colored Urine: If a jaundiced baby has pale, clay-colored, gray, or chalky-white poop (instead of normal yellow, green, or brown), it is a warning sign of biliary atresia (blocked bile ducts in the liver), which requires prompt pediatric surgical intervention.
- Signs of Acute Bilirubin Encephalopathy (Brain Toxicity): If a jaundiced baby becomes extremely difficult to wake up for feedings (lethargic), has a high-pitched, shrill cry, feels either completely floppy (hypotonic) or stiffly arches their neck and back backward (
opisthotonos), go to the emergency room immediately.
4. Bilious (Dark Green) Vomiting, Dehydration, and Umbilical Cord Infection¶
- Normal spit-up vs Bilious Green Vomiting (Surgical Emergency): Almost all babies spit up small amounts of white or curdled breastmilk/formula. However, if a newborn vomits liquid that looks bright grass-green, forest-green, or fluorescent yellow-green (like dark bile), go to a hospital emergency department immediately. In a newborn, bilious green vomit is the classic sign of intestinal malrotation with midgut volvulus, a surgical emergency where the baby's intestine has twisted around its own blood supply and can die within hours without emergency surgery.
- Dehydration and Wet Diaper Counts: By Day 4 to Day 5 of life, a well-hydrated newborn should produce at least 6 heavy wet diapers every 24 hours with pale, nearly clear urine ( roughly 1 wet diaper on Day 1, 2 on Day 2, 3 on Day 3, 4 on Day 4, and 6+ from Day 5 onward). Note that in the first 2 to 3 days, orange-pink crystals in the diaper ("brick dust" urate crystals) are common as milk comes in, but if brick dust crystals persist past Day 4, or the baby goes more than 8 to 12 hours without a wet diaper, has a dry mouth, crying without tears, or a deeply sunken soft spot (anterior fontanelle) on top of the head, seek urgent medical care. Conversely, a tense, bulging soft spot when the baby is calm and upright is a red flag for increased pressure inside the skull (meningitis).
- Omphalitis (Umbilical Cord Stump Infection): It is normal for the drying umbilical cord stump to look dark brown/black and leave a tiny spot of dried blood when it falls off between Days 7 and 14. However, if a ring of bright redness and swelling spreads onto the baby's belly skin around the base of the cord (larger than a 5-millimeter / quarter-inch margin), feels hot or hard, or oozes foul-smelling yellow pus, go to the emergency room immediately. Because a newborn's umbilical vessels connect directly to the portal vein and interior abdominal wall, omphalitis is a medical emergency requiring intravenous antibiotics.
Safe Sleep Rule: Never fall asleep with a newborn on a couch, armchair, or recliner
Extreme postpartum exhaustion creates a massive hidden danger: accidentally falling asleep while holding a newborn on a sofa, cushioned armchair, or nursing recliner. According to the American Academy of Pediatrics (AAP), sleeping with an infant on a couch or armchair increases the risk of accidental suffocation, wedging, and Sudden Unexpected Infant Death (SUID) by up to 60-fold. If you feel your eyelids drooping while feeding your baby at night, either hand the baby to another awake adult or place the baby flat on their back on a firm, flat, bare crib or bassinet mattress with zero pillows, blankets, bumpers, or toys before closing your eyes.
How do I tell the difference between normal postpartum bleeding (lochia) and a delayed postpartum hemorrhage?
Normal lochia gradually tapers down in volume and shifts color from bright red (Days 1 to 4) to pinkish-brown (Days 4 to 10) and then yellowish-white. A delayed postpartum hemorrhage occurs when bleeding suddenly turns bright red again and surges in volume, soaking through one or more full-sized sanitary maxi pads per hour for 2 consecutive hours, passing blood clots larger than a golf ball or plum (5 centimeters / 2 inches), or causing dizziness, a racing heart, or faintness. Note that a mild, brief increase in spotting after nursing or overexertion can happen around Day 7 to 10 as the placental scab heals, which should slow down after resting for 1 hour; if it continues soaking pads, go to the emergency room immediately.
Can I develop postpartum preeclampsia even if my blood pressure was completely normal during my entire pregnancy and delivery?
Yes. Roughly one-third to one-half of women diagnosed with postpartum preeclampsia had zero history of high blood pressure during pregnancy. After birth, several liters of extra fluid accumulated in your body tissues during pregnancy shift back into your bloodstream over Days 3 to 10, which can trigger a sudden, dangerous spike in blood pressure alongside blood vessel inflammation. Any blood pressure reading of 140/90 mmHg or higher after birth requires same-day medical attention, and any reading of 160/110 mmHg or higher, or a throbbing headache with blurred vision or upper-right belly pain, requires an immediate emergency room visit.
If my 2-week-old newborn has a rectal temperature of 38.1°C (100.6°F) but is acting completely happy and feeding normally, do we really need to go to the Emergency Room at 2:00 AM?
Yes, without exception. In an infant under 28 days old (and up to 60 days old), you cannot rely on how "happy" or comfortable the baby looks initially. Up to 10 to 12 percent of febrile newborns under 28 days old have a serious invasive bacterial infection (such as a urinary tract infection, bloodstream bacteremia, or bacterial meningitis) that can deteriorate catastrophically within hours. Do not give the baby infant Tylenol (acetaminophen/paracetamol) to mask the fever, and do not wait until your pediatrician's office opens in the morning, go straight to a hospital emergency department (ideally a dedicated Children's Hospital ER).
What is the difference between scary intrusive thoughts (Postpartum OCD) and Postpartum Psychosis?
The defining clinical difference is whether the parent retains "reality testing" (insight). Up to 50 percent of new parents, especially those with Postpartum Anxiety or Postpartum OCD, experience sudden, unwanted intrusive images of accidental harm (such as "What if I drop the baby down the stairs?"). A parent with Postpartum OCD is horrified and sickened by these thoughts, recognizes them as irrational anxiety, and actively avoids the stairs to protect the baby. In contrast, a parent in Postpartum Psychosis has lost contact with reality: they have gone days without sleep, hear voices, or hold fixed delusions (such as believing the baby is evil, possessed, or must be harmed to "save" its soul) and believe those delusions are completely true. Postpartum Psychosis is a life-threatening emergency requiring immediate 911 / hospital intervention.
Why are a newborn's hands and feet sometimes blue even when they are breathing normally?
Bluish hands and feet in the first few days of life, called acrocyanosis, are completely normal and harmless, caused by the newborn's immature circulation prioritizing blood flow to critical internal organs whenever their extremities get slightly cool. To check if a baby is getting enough oxygen, look at their central mucous membranes: their lips, tongue, gums, and central chest should always be warm and bright pink. If the tongue, lips, or central face turn blue, purple, or gray, call your local emergency number immediately.
My newborn had a tiny smear of pinkish-red or orange stain in their diaper on Day 2, is my baby bleeding?
In the first 2 to 4 days of life, two harmless causes frequently look like blood in the diaper. First, in both male and female newborns, concentrated uric acid crystals in the urine ("brick dust" urate crystals) leave a chalky orange-pink or rust-colored stain on the diaper pad; this resolves once mature breastmilk comes in around Day 4. Second, female newborns frequently have a tiny drop of milky or blood-tinged vaginal discharge ("pseudomenstruation") during the first week due to withdrawal from the mother's pregnancy hormones, which is completely normal. However, if brick dust crystals continue past Day 4, or if there is frank bright red blood in the stool or bleeding from a circumcision site larger than a 2.5-centimeter (1-inch) coin, call your pediatrician immediately.
