A complete joint dislocation (luxation) occurs when extreme mechanical force tears the fibrous joint capsule and supporting ligaments, driving the articular ends of two bones completely out of contact with one another. A partial dislocation (subluxation) leaves the joint surfaces partially touching but misaligned. Unlike a mid-shaft bone fracture where the injury sits in a straight cylinder of bone, a joint dislocation occurs right through the tight anatomical tunnels where major arteries and peripheral nerves cross from one limb segment into the next.
Because the displaced bone head often rests directly against a major blood vessel or nerve trunk, combined with unknown hidden articular fractures, attempting to yank, twist, or "pop" a dislocated joint back into place in the field is one of the most dangerous mistakes in trauma care. Knowing how to perform a 30-second distal Circulation, Sensation, and Motor (CSM) check, how to distinguish a true limb-threatening vascular emergency (such as a knee joint dislocation or hip dislocation) from a lateral kneecap slip, and how to immobilize every dislocated joint strictly in the position found using pillow-and-sling padding protects both the limb and its permanent nerve function.
Hadda samee tan¶
- Tell the injured person to stay completely still and support their limb manually in the exact angle they are holding it. Never try to straighten a bent dislocated knee, elbow, or hip, and never try to push an abducted dislocated shoulder flat against the ribcage.
- Never attempt to pull, twist, or "pop" a dislocated joint back into its socket. Up to 25 to 40 percent of joint dislocations have an associated fracture inside the joint socket; pulling on the limb without X-rays and muscle relaxation can drive jagged bone edges directly through major arteries and nerves.
- Check distal Circulation, Sensation, and Motor (CSM) function below the injured joint immediately. Palpate the radial pulse at the wrist (for shoulder or elbow injuries) or the dorsalis pedis and posterior tibial pulses at the foot (for hip, knee, or ankle injuries), press a fingernail or toenail to confirm pink capillary refill returns within 2 seconds, and check skin sensation and finger or toe movement.
- Call your local emergency number immediately if the hand or foot below the dislocation is pale, blue, cold, numb, or pulseless, or for any hip, knee joint, elbow, or open dislocation. Dial 911 in the United States and Canada, 112 across the European Union, 999 in the United Kingdom, 000 in Australia, or check the directory of nambarada degdega ah.
- Remove all rings, watches, bracelets, shoes, and tight sleeves or trousers from the injured extremity immediately. Joint dislocations trigger rapid intra-articular hemarthrosis (bleeding into the joint space) and distal venous swelling that can turn rings or boots into ischemic tourniquets within 20 minutes.
- Pad the empty space beneath the deformed joint and splint it strictly in the position found, then apply a cloth-wrapped ice pack. Use rolled towels, blankets, a pillow, or a moldable SAM splint bent to match the exact deformity, secure it above and below the joint without tying straps directly over the protruding bone, keep the patient NPO (nothing by mouth) for hospital sedation, and apply a cool pack for 15 to 20 minutes at a time.
True knee dislocation (tibiofemoral) vs. kneecap dislocation (patellar): a life-and-limb distinction
Bystanders frequently say "dislocated knee" when they mean the kneecap (patella) has slipped to the outer side of the knee. While a lateral patellar dislocation is painful, the blood vessels behind the knee remain untouched. A true knee joint (tibiofemoral) dislocation, where the shinbone (tibia) tears completely away from the thighbone (femur), is a catastrophic vascular emergency: in 20 to 40 percent of true knee dislocations, the popliteal artery behind the knee is stretched, clotted, or torn, which leads to above-knee amputation in over 80 percent of cases if arterial blood flow is not surgically restored within 6 to 8 hours. Even if a true knee dislocation pops back into place on its own before paramedics arrive, it still requires immediate trauma center transport and CT angiography.

Comparing major anatomical joint dislocations and their neurovascular traps¶
Every major joint in the body has a specific direction in which it most commonly dislocates, producing a classic limb posture and threatening a specific nerve or artery running directly across the joint capsule.
| Dislocated Joint | Most Common Direction & Mechanism | Classic Limb Posture & Visual Deformity | Primary Artery & Nerve at Immediate Risk | Field Splinting Method (In Position Found) |
|---|---|---|---|---|
| Shoulder (Glenohumeral) | 95% Anterior: fall onto an abducted, externally rotated arm. 4% Posterior: electric shock, tonic-clonic seizure, or direct blow to front of shoulder |
Anterior: Squared-off "epaulet" acromion, arm held slightly abducted (away from ribs) and externally rotated. Posterior: Arm locked tightly against belly in internal rotation; cannot turn palm up |
Axillary nerve (numbness over outer deltoid "regimental badge" patch) and axillary artery | Pillow-and-Sling: wedge a folded pillow or rolled blanket between the arm and ribcage, then support with a triangular sling and swathe |
| Collarbone / AC Joint (Acromioclavicular) & SC Joint (Sternoclavicular) | Direct fall onto the point of the shoulder or heavy chest impact | AC Joint: Visible step-off bump at outer tip of collarbone. Posterior SC Joint: Inner end of collarbone driven backward into throat (compresses trachea, esophagus, and great vessels) |
Posterior SC Dislocation: Trachea, subclavian/brachiocephalic vein, and aorta (true life-threatening airway emergency) | Standard arm sling supporting elbow weight upward; call EMS immediately if any difficulty swallowing, hoarseness, or shortness of breath (posterior SC) |
| Elbow (Ulnohumeral) | 90% Posterior: fall onto an outstretched hand (FOOSH) with elbow slightly flexed | Forearm appears shortened; prominent bony olecranon process protruding sharply backward; elbow locked at roughly 45 degrees of flexion | Brachial artery (risk of Volkmann ischemic contracture), median nerve, and ulnar nerve | Padded moldable splint (SAM splint or cardboard) bent to the exact 45-degree angle of the elbow, secured to upper arm and forearm |
| Toddlers' Radial Head Subluxation ("Nursemaid's Elbow") | Child aged 1 to 4 years lifted, swung, or pulled suddenly by the wrist or hand while forearm is pronated | Barar ama qallafsanaan muuqda ma jiro; canuga waxa uu diiday in uu isticmaalo cududda, isaga oo haysta ** inyar oo ka soo laabta dhinacooda calaacaduna hoos u soo jeedsanayso (u janjeersan)** | Xidhmada neerfaha xididada ayaa si buuxda u ahaanaysa; Seedda annular waxay kaliya ku xayiran tahay madaxa shucaaca | Ku taageer cudud si raaxo leh wadhaf fudud ama barkin; Booqashada rugta degdega ah halkaas oo takhtarku si tartiib ah u sameeyo hyperpronation ama hoos u dhigista-jilicsanaanta |

| Kneecap (Patellar) | Twisting on a planted foot while knee is slightly bent, or direct blow to inner kneecap | Kneecap visibly shifted to the outer (lateral) side of the knee; knee is stuck in a bent (flexed) position | Neurovascular bundle behind knee is completely spared | Pillow splint tucked underneath the bent knee to support it in flexion during transport to hospital |
| Ankle (Tibiotalar) | Severe inversion/eversion twist, almost always combined with malleolar fractures (fracture-dislocation) | Foot twisted sharply inward or outward; tense, shiny white ischemic skin stretched tightly over the protruding ankle bone | Posterior tibial artery, dorsalis pedis artery, and overlying skin necrosis (pressure necrosis within 1 to 2 hours) | Pillow splint wrapped around foot and lower leg and secured with bandages; immediate EMS transport for urgent reduction |
Why you must never try to "pop" a dislocated joint back in¶
In action movies and sideline sports myths, someone invariably grabs a dislocated arm or finger, yanks hard on it, and pops it back into place so the hero can keep going. In real clinical medicine, layperson attempts to reduce a dislocation without radiographs and analgesia frequently cause permanent disability:
- Up to 40 percent of dislocations contain hidden intra-articular fractures: When a shoulder, elbow, ankle, or finger dislocates, the impact frequently shears off a piece of the joint socket or bone head (for example, a greater tuberosity fracture or bony Bankart fracture in the shoulder, a coronoid process or radial head fracture in the elbow, or a malleolar fracture in the ankle). Yanking on the limb grinds sharp fracture fragments across articular cartilage and can convert a closed injury into an unstable comminuted fracture.
- Nerves and arteries can be looped around the displaced bone head: During a dislocation, the brachial artery, sciatic nerve, axillary nerve, or median nerve is often stretched taut over the neck of the displaced bone. Pulling or levering the bone forcefully without anatomical technique can trap the nerve or artery inside the joint space or shear the blood vessel wall (intimal tear), causing acute arterial thrombosis and limb loss.
- Violent protective muscle spasm defeats brute force: Within minutes of a dislocation, the large muscles crossing the joint (such as the deltoid, pectoralis major, quadriceps, or hamstrings) lock into an involuntary protective tetanic spasm. Trying to overpower spasming muscles in an awake, unmedicated person requires dangerous mechanical force that can snap the humerus shaft or tear the rotator cuff completely.
Step 1: Performing the Distal CSM (Circulation, Sensation, Motor) Check¶
Whenever you encounter a suspected dislocation or fracture, you must assess and record the Distal CSM status (also called PMS: Pulse, Motor, Sensation) before you apply a splint AND immediately after you tie the last splint strap. If a splint is tied too tightly or shifts the bone slightly, checking CSM afterward catches the problem immediately so you can loosen the straps.
How to check CSM in the Upper Extremity (Shoulder, Elbow, Forearm, Wrist)¶
| Assessment Step | What to Check | What a Normal Result Looks Like | What an Abnormal Result Indicates |
|---|---|---|---|
| 1. Circulation (Distal Perfusion) | 1. Palpate the radial pulse on the thumb side of the inner wrist. 2. Pinch the tip of a fingernail for 5 seconds until it blanches white, then release (capillary refill). 3. Feel skin temperature and check color of fingers. |
Strong radial pulse; nail bed turns pink again in less than 2 seconds; fingers are warm and pink. | Axillary or brachial artery compression: absent radial pulse, capillary refill > 2 seconds, or pale/bluish cold fingers (Immediate Priority 1 EMS). |
| 2. Sensation (4 Major Nerve Zones) | Lightly touch (without moving the joint): 1. Outer shoulder ("Regimental Badge" patch over lateral deltoid) 2. Pad of the index finger 3. Pad of the little (pinky) finger 4. Web space on the back of the thumb |
Patient feels light touch clearly and equally compared to the uninjured arm, without pins-and-needles tingling. | 1. Outer shoulder numb = Axillary nerve (classic in shoulder dislocation) 2. Index finger numb = Median nerve 3. Little finger numb = Ulnar nerve 4. Back of thumb numb = Radial nerve |
| 3. Motor (3 Distal Hand Checks) | Ask the patient to gently move ONLY their fingers: 1. Hitchhiker's thumbs-up (extend thumb) 2. OK sign (pinch thumb tip to index tip) 3. Spread fingers wide apart like a fan |
Patient can perform all three gentle finger movements without pain at the joint. | 1. Cannot do thumbs-up = Radial nerve 2. Cannot pinch OK sign = Median (AIN) nerve 3. Cannot spread fingers = Ulnar nerve |
How to check CSM in the Lower Extremity (Hip, Knee, Patella, Ankle)¶
| Assessment Step | What to Check | What a Normal Result Looks Like | What an Abnormal Result Indicates |
|---|---|---|---|
| 1. Circulation (Distal Perfusion) | 1. Palpate the dorsalis pedis (DP) pulse on top of the midfoot and the posterior tibial (PT) pulse just behind the inner ankle bone (medial malleolus). 2. Check great toenail capillary refill (< 2 seconds), skin color, and warmth. |
Strong DP and PT pulses; toenail refills pink in under 2 seconds; foot is warm. | Popliteal or tibial artery occlusion: absent foot pulses, mottled/white/blue cold toes (Immediate Priority 1 EMS). |
| 2. Sensation (2 Major Nerve Zones) | Lightly touch: 1. Web space between the 1st (big) and 2nd toes on top of the foot 2. Sole (plantar surface) of the foot |
Clear, normal touch sensation without burning or numbness. | 1. Numb 1st-2nd toe web space = Deep peroneal nerve 2. Numb sole of foot = Tibial nerve (both are branches of the sciatic nerve) |
| 3. Motor (2 Distal Toe Checks) | Ask the patient to gently move ONLY their toes: 1. Pull big toe upward toward their nose (dorsiflexion) 2. Curl toes downward like pressing a gas pedal (plantarflexion) |
Smooth, symmetric toe motion. | 1. Cannot pull toes up (Foot Drop) = Common peroneal / sciatic nerve (classic in posterior hip and true knee dislocations) 2. Cannot curl toes down = Tibial nerve |
Step-by-step immobilization for specific dislocated joints¶
The golden rule of prehospital dislocation care is "Splint it in the position found by bringing the padding to the limb, never forcing the limb to the splint."
1. Dislocated Shoulder (Glenohumeral Joint)¶
Qiyaastii 95 boqolkiiba leexashada garabka ayaa ah ** kala-bax hore ** (madaxa xumerus ayaa horay uga soo baxaya godka glenoid-ka-gacmeedka wuxuuna hoos fadhiyaa habka coracoid), halka 4 boqolkiiba ay yihiin ** kala-baxyada dambe** (waxaa keena qalal, shoog koronto, ama dharbaaxo dhinaca hore ee garabka).
- Waa maxay sababta dharbaaxada gacan fidsan ee caadiga ahi ay u dhaawacdo goynta garabka hore: Kala-bax hore, madaxa xumeral ee barokacay ayaa cududda wax yar ka xira ** ka fog dhinaca jidhka (waxaa la afduubay 15 ilaa 30 darajo)**. Isku dayga in uu suxulka qofka si siman ugu riixo feedhaha si uu ugu xidho kabaallayaasha dharbaaxada adag ee caadiga ah madaxa xumeral ee ka soo horjeeda cirifka glenoid iyo neerfaha axillary, taasoo keenta xanuun aad u daran.
- ** Farsamada Barkin-iyo-Dhardho (Tallaabo-Tallaabo):**
- Bukaanku ha u fadhiisto mid toosan ama fadhiyo badheed halka caawiye uu ku taageerayo cududdooda foorarsan xagal saxda ah ee ay hayaan.
- Take a soft bed pillow, folded fleece jacket, or thick rolled blanket and gently tuck it into the open triangular gap between the injured inner arm/elbow and the side of the ribcage. This fills the dead space so the arm can rest in its natural abducted position without being pushed inward.
- Ku duub faashad weyn oo saddex-xagal ah (xagal gacaliye) cududda hoosteeda iyo xagga dambe ee qoorta, kuna xidh guntinka dhinaca qoorta (waligaa si toos ah ha uga dul marin lafaha afka ilmo-galeenka) oo geli suuf jilicsan oo qoorta hoostiisa ah.

2. Jilibka Jilibka oo Go'ay (Dislocation Patellar Lateral) vs. Jilibka Jilibka dhabta ah¶
Distinguishing these two knee injuries takes only a gentle visual inspection after slicing open the trouser leg:

Su'aalaha soo noqnoqda¶
Why shouldn't I pop a dislocated shoulder or finger back into place right away while it is still numb?
Although people often believe popping a joint back in immediately on the sports field or trail is easy and safe, up to 25 to 40 percent of joint dislocations also have a hidden fracture inside the socket or bone head. Pulling, twisting, or levering a dislocated shoulder, elbow, or finger without an X-ray and proper muscle relaxation can grind sharp bone fragments into joint cartilage, tear the rotator cuff or volar plate ligaments, or trap a major nerve or artery (such as the axillary nerve or brachial artery) directly inside the joint socket, causing permanent paralysis or loss of blood flow. Always immobilize the joint in the position found and let a clinician reduce it safely after neurovascular checks and imaging.
What is the difference between a dislocated kneecap and a true dislocated knee?
A dislocated kneecap (patellar dislocation) happens when the small shield-shaped bone on the front of the knee slips onto the outer side of the joint, leaving the knee stuck slightly bent; while painful, it does not endanger the major blood vessels behind the knee. A true knee dislocation (tibiofemoral dislocation) happens when all the main ligaments inside the knee tear so completely that the shinbone (tibia) disconnects from the thighbone (femur). In 20 to 40 percent of true knee dislocations, the popliteal artery running directly behind the knee is torn or clotted shut, which causes amputation of the leg in over 80 percent of cases if surgical blood flow is not restored within 6 to 8 hours.
What does "splint in the position found" actually mean in practice?
When a joint is dislocated, the displaced head of the bone is mechanically wedged outside its normal socket, locking the arm or leg at an awkward bent or angled posture. Trying to straighten a bent dislocated elbow or knee, or trying to push a dislocated shoulder flat against the ribs so it fits into a regular straight splint, uses the long limb bone like a crowbar to crush the nerves and blood vessels stretched across the joint. Splinting in the position found means leaving the joint at the exact angle the victim is holding it, stuffing pillows, rolled towels, or folded blankets into the empty spaces around the deformity, and molding the splint around the limb rather than forcing the limb to fit the splint.
How can I tell if a dislocated shoulder has pinched the axillary nerve?
In a shoulder dislocation, the humeral head pops out of the socket right where the axillary nerve wraps around the surgical neck of the upper arm bone. You can check the sensory branch of the axillary nerve in 5 seconds without moving the patient's arm by lightly touching the round patch of skin over the outer side of the upper arm (over the lateral deltoid muscle, right where a military uniform has a shoulder patch, known clinically as the "regimental badge" area) and comparing it to the uninjured shoulder. If that patch of skin feels numb or tingles, tell the paramedics and emergency room physician immediately so they can document the axillary nerve status before and after reducing the shoulder.
Why can't I give someone with a dislocated shoulder or hip a glass of water or pain pills on the way to the hospital?
To put a dislocated shoulder, hip, elbow, or ankle gently back into its socket without tearing muscles or causing severe pain, emergency room doctors typically give the patient short-acting intravenous procedural sedation (putting the patient briefly to sleep so their spasming muscles relax completely). Having water or food in the stomach increases the risk of vomiting and inhaling stomach acid into the lungs (aspiration) while sedated. Keeping the patient strictly NPO (nothing by mouth) ensures the hospital team can sedate them and fix the joint as quickly and safely as possible.
My two-year-old toddler suddenly stopped using their arm after I lifted them up by the wrist, but nothing looks swollen. Is it dislocated?
This classic presentation in a child between 1 and 4 years old is almost certainly "nursemaid's elbow" (radial head subluxation). In toddlers, the ring-shaped ligament holding the radius bone at the elbow is still loose; a quick upward pull on an extended arm slips a tiny fold of that ligament into the joint. Because it is not a full bone dislocation, there is no swelling or deformity, and the child feels no pain as long as they hold their arm still against their side with the palm turned down. Do not try to twist the arm at home; support it gently in a sling and visit an urgent care or emergency clinic, where a doctor can slip the ligament back in seconds with a gentle rotation maneuver.
