Imuduro Pigugu Pelvic, Isopọ Ayika, ati Ẹjẹ inu¶
A high-energy pelvic ring fracture is one of the most lethal injuries in trauma medicine, carrying a mortality rate of 15 to 50 percent when accompanied by hemodynamic instability. Unlike a long bone such as the shin or forearm, the human pelvis is a rigid bony ring made up of two large innominate (hip) bones joined in front at the pubic symphysis and anchored in back to the sacrum by the strongest ligaments in the body. When a high-speed car crash, motorcycle collision (where the pelvis slams forward into the fuel tank), pedestrian strike, crush injury, or fall from a height breaks this ring in two places and tears the posterior sacroiliac ligaments, the pelvis hinges open like the covers of a book (an "open-book" pelvic fracture).

- NEVER "spring," rock, push, or compress the hip bones to check if the pelvis is broken. Pushing down or inward on an unstable pelvis grinds razor-sharp bone fragments through the sacral venous plexus and internal iliac arteries, dislodging the fragile initial blood clots that are keeping the patient alive. If the mechanism of injury or symptoms suggest a pelvic fracture, assume the pelvis is broken and bind it.
- NEVER log-roll a suspected pelvic fracture victim onto their side unless their airway is actively blocked by vomit or blood. Rolling a casualty onto the side of a broken pelvis crushes the iliac wing inward and shears the retroperitoneal vessels. Keep the patient flat on their back (supine) and use a multi-rescuer flat vertical lift ("bridge lift") or scoop stretcher if movement is unavoidable.
- Apply a commercial Pelvic Binder (SAM Pelvic Sling, T-POD, or Prometheus) or a folded Bedsheet Binder centered strictly over the GREATER TROCHANTERS (the bony bumps on the outer upper thighs/hips), NOT over the waist or upper hip crests. Sliding a folded sheet smoothly underneath the hollow of the knees and pulling it up to the level of the greater trochanters and pubic symphysis leverages the femur heads inward to close the "open book" pelvis.

The #1 error in pelvic binding: Placing the binder too high over the Iliac Crests (waist)
Clinical audits in emergency departments worldwide show that 40 to 50 percent of prehospital pelvic binders and improvised sheets are placed too high, wrapped around the top of the hip bones (the iliac crests) or the soft abdomen like a waist belt. Wrapping a binder around the upper iliac crests or belly is completely ineffective at closing the pubic symphysis, squeezes the diaphragm and abdominal organs, and in certain lateral-compression fractures can actually lever the lower pelvic ring wider apart. Always center the middle of the binder lower down than you think: directly over the greater trochanters (the hard bony prominences you feel on the outside of your upper thighs where your trouser pockets sit, level with the pubic bone in front and the middle of the buttocks in back).

Ifiwera awọn oriṣi Young-Burgess mẹrin ti awọn fifọ oruka ibadi¶
Orthopedic trauma surgeons classify pelvic ring fractures by the direction of the crushing force (the Young-Burgess classification). Understanding how the pelvis breaks explains why an "open-book" fracture causes the legs to flop outward and why internal bleeding is so ferocious.
| Fracture Pattern | Mechanism of Force | Idalọwọduro Anatomical | External Leg & Hip Appearance | Ewu Ẹjẹ ti iṣan & Ipa Binder |
|---|---|---|---|---|
| Anteroposterior Compression (APC / "Open-Book" Pelvis) | Ijamba ọkọ ayọkẹlẹ ori-lori (ikolu dasibodu si awọn ekun), Ipa gaasi alupupu-ojò, ẹṣin ja bo sori ẹlẹṣin, tabi ikolu straddle | Pubic symphysis tears wide open (> 2.5 cm / 1 in) in front, and the sacrospinous and anterior sacroiliac ligaments tear in back, hinging the two halves of the pelvis outward like an open book | Both legs fall outward into external rotation ("frog-leg" or Charlie Chaplin posture); wide gap may be visible or felt over the pubic bone | HIGHEST BLEEDING RISK: Pelvic volume increases sharply; tears the presacral venous plexus and internal iliac branches. Pelvic binder is lifesaving (closes the book and reduces pelvic volume) |
| Lateral Compression (LC) (Most common: 50% to 60% of cases) | T-bone side-impact motor vehicle collision or pedestrian struck from the side | Sideways blow crushes the pubic rami inward across the midline and compresses the sacrum | Ẹsẹ ti o ni ipalara le yi pada ** sinu ** ju ita lọ; oṣuwọn giga ti ** àpòòtọ tabi puncture uretral ** nipasẹ didan pubic rami shards | Moderate-to-high bleeding (plus associated spleen/liver/brain injury). Apply commercial auto-stop binder snugly for stabilization without over-crushing |
| Vertical Shear (VS) | Fall from a height (> 3.5 meters / 12 feet) landing on one extended leg, or heavy load falling onto shoulders | One entire half of the pelvis (hemipelvis) is sheared straight upward through both the pubic rami in front and the sacroiliac joint in back | The leg on the sheared side looks noticeably SHORTER than the other leg and is externally rotated | ** Ẹjẹ PUPO & EWU NERVE:** Omije plexus nafu ara lumbosacral ati awọn ohun-elo iliac pataki. Why is pushing on the hip bones ('rocking the pelvis') to check for a fracture now considered a life-threatening mistake?
When the bony ring of the pelvis breaks in a crash or fall, the sharp jagged bone edges tear a dense cobweb of large veins pressed against the sacrum and pubic bone. In the first few minutes after impact, the body tries to save itself by forming soft, fragile blood clots over those torn veins. If a rescuer places their hands on the hip bones and pushes down or rocks the pelvis side-to-side to 'test if it is broken,' the broken halves of the pelvis shift and grind against those veins, instantly ripping off the protective blood clots and restarting massive, unstoppable internal bleeding. Always judge a pelvic fracture by the crash mechanism, pain, and leg position, and never rock or push on the hips.
How can I find the right spot (the greater trochanters) for a pelvic binder or bedsheet, and why does putting it at the waist fail?
Most people mistakenly wrap a pelvic binder around the waist or the top curved rim of the hip bones (the iliac crests) where a belt sits. That is 10 to 15 centimeters (4 to 6 inches) too high: pushing inward on the top wings of the pelvis does not close the split front of the pelvic ring and can actually hinge the bottom of the pelvis wider open. Instead, slide your hands down the outer sides of the upper thighs to the exact level of the pubic bone (crotch) in front and the middle of the buttocks in back. The hard bony knobs you feel on the outer upper thighs are the greater trochanters, the tops of the thighbones where they plug into the hip sockets. Centering the binder directly over the greater trochanters pushes both thighbone heads straight into the hip sockets, locking the broken pelvic ring shut like closing the covers of a book.
Why do you have to turn the victim's feet inward ('pigeon-toed') and tie their ankles and knees together when they have a broken pelvis?
In an 'open-book' pelvic fracture, the front joint of the pelvis (the pubic symphysis) splits wide open. Because the thighbones are attached to the left and right halves of the pelvis at the hip sockets, the heavy weight of the victim's legs naturally rolls outward, which pulls the two halves of the broken pelvis even farther apart and expands the internal bleeding cavity. Gently rotating both legs inward so the big toes point toward each other, and tying a padded figure-of-eight strap around the ankles and knees, uses the long thighbones as internal levers to rotate both halves of the pelvis inward and hold the broken ring closed.
How do I slide a bedsheet or pelvic binder under a trauma victim if I'm not allowed to roll them onto their side?
Never log-roll a suspected pelvic fracture victim onto their side, because rolling them puts their entire body weight onto the broken hip wing and crushes the pelvis inward. Instead, take advantage of the natural arch underneath the human knees: slide your folded bedsheet or commercial pelvic binder flat through the hollow space underneath both knees while the patient lies flat on their back. Then, with one person holding the left end of the sheet and a second person holding the right end, gently saw the sheet back and forth in small 5-centimeter (2-inch) seesaw motions while sliding it smoothly upward beneath the thighs and lower buttocks until it rests directly under the greater trochanters.
