Life-Threatening Emergency 1: Malignant Stimulant Hyperthermia ("Cooking From the Inside")

The number one cause of rapid death in acute methamphetamine, MDMA ("molly"), and synthetic cathinone ("bath salt") overdoses is runaway hyperthermia.

1. Why stimulants cause core body temperature to pass 41 degrees Celsius (106 degrees Fahrenheit)

Stimulants simultaneously turn up the body's internal furnace while breaking its radiator:

  • Massive Heat Production: Excess dopamine, serotonin, and norepinephrine trigger continuous skeletal muscle hyperactivity, jaw clenching, agitation, and mitochondrial uncoupling (UCP-3), generating enormous internal metabolic heat (especially in crowded, hot nightclubs, summer festivals, or poorly ventilated rooms).
  • Blocked Heat Dissipation: Simultaneously, norepinephrine triggers intense alpha-1 adrenergic peripheral vasoconstriction, clamping shut the blood vessels in the skin so hot core blood cannot reach the skin surface to cool off.
  • The 40.5C (105F) Multi-Organ Failure Threshold: Once core temperature exceeds 40.5 degrees Celsius (105 degrees Fahrenheit) for more than 20 to 30 minutes, cellular proteins denature, the intestinal barrier breaks down (dumping gut endotoxins into the blood), skeletal muscle melts (rhabdomyolysis), and the blood clotting system goes haywire (Disseminated Intravascular Coagulation / DIC), causing fatal bleeding from every organ.

2. Step-by-step Pre-Hospital Rapid Cooling Protocol

Do not wait for paramedics to start cooling a burning-hot stimulant patient, and never give acetaminophen (paracetamol / Tylenol) or ibuprofen/aspirin to lower a stimulant fever. Antipyretic fever pills work on the hypothalamus during infectious flu/bacterial fevers and do zero for drug-induced muscle hyperthermia (worse, acetaminophen accelerates liver failure during hyperthermia, and NSAIDs worsen kidney failure).

  1. Move to the Coolest Possible Environment: Bring the person into an air-conditioned room or shaded breeze and remove heavy shirts, hoodies, hats, and shoes down to underlayers.
  2. Apply Ice Packs to Major Arterial Junctions: Wrap ice packs, bags of ice cubes, or frozen food packages in a thin wet towel and pack them tightly against the neck (carotid arteries), armpits (axillary arteries), and groin creases (femoral arteries).
  3. Evaporative Spray-and-Fan Cooling (or Cold-Water Immersion):
    • Sponge or spray large areas of bare skin with cool water (15 to 20 degrees Celsius / 60 to 68 degrees Fahrenheit) while pointing the strongest available electric fan (or fanning vigorously with cardboard/towels) directly across the wet skin. Evaporation removes heat seven times faster than dry air.
    • If a bathtub is immediately available and the person is awake (or two strong helpers can hold the head safely out of the water), immerse the torso and limbs in cool tap water. (See Heat stroke vs. heat exhaustion.)

Life-Threatening Emergency 2: Cocaine and Methamphetamine Chest Pain (Heart Attack & Vasospasm)

Cocaine and methamphetamine trigger acute myocardial infarction (heart attack), aortic dissection, and sudden cardiac arrest even in 19-to-35-year-old users who have zero cholesterol plaque in their arteries.

1. The triple cardiac hit of cocaine (and Cocaethylene with alcohol)

  • Coronary Artery Vasospasm + Surging Oxygen Demand: Cocaine and meth cause the heart muscle to beat harder and faster while simultaneously spasming the coronary arteries shut, starving the heart muscle of oxygen.
  • Direct Sodium-Channel Blockade (Only Cocaine): Cocaine is a local anesthetic ("caine") that directly blocks cardiac fast sodium (Na+) channels, widening the heart's electrical QRS complex and triggering fatal ventricular tachycardia or ventricular fibrillation (which hospital doctors treat with intravenous sodium bicarbonate).
  • Accelerated Blood Clotting: Stimulants activate platelets, causing sudden blood clots inside the spasming coronary artery.
  • ** Ohun elo Cocaethylene Multiplier (Cocaine + Alcohol):** Nigbati ẹnikan ba mu ọti lakoko lilo lulú tabi kiraki kokeni, ẹdọ eniyan ṣe iyipada awọn oogun mejeeji sinu ẹkẹta, agbo-ara psychoactive ọtọtọ ti a pe ni **cocaethylene **. | WHY PLAIN WATER CAN KILL ON MDMA: |
| MDMA stimulates the brain to release huge amounts of Antidiuretic |
| Hormone (ADH / Vasopressin), causing SIADH: your kidneys are |
| chemically LOCKED and CANNOT produce urine to excrete excess water. |

Life-Threatening Emergency 4: Stimulant Stroke, Subarachnoid Hemorrhage, and Seizures

Nitori methamphetamine, kokeni, kiraki, ati awọn cathinones sintetiki fa lojiji, awọn spikes iwa-ipa ni titẹ ẹjẹ iṣan ara (200/120 to 260/150 mmHg) alongside cerebral blood-vessel spasm and platelet clumping, acute ischemic stroke and hemorrhagic brain bleeds (intracerebral hemorrhage or ruptured cerebral aneurysm) are frequent catastrophic complications of stimulant overamping, even in people in their twenties and thirties.

1. Recognizing a stimulant-induced brain bleed or stroke (BE-FAST)

Maṣe yọ orififo nla kuro, ikọsẹ, tabi ọrọ sisọ lasiko ṣiṣe akikanju bi “o kan ga” tabi aini oorun:

  • The "Thunderclap" Headache (Ruptured Aneurysm / Subarachnoid Hemorrhage): If a person who just smoked crack, snorted cocaine, or used methamphetamine suddenly grabs their head and cries out with the worst, most explosive headache of their life (reaching maximum agony within 60 seconds), accompanied by a stiff neck, vomiting, sensitivity to light, or collapse, a blood vessel at the base of the brain has likely ruptured under extreme catecholamine blood pressure.
  • BE-FAST Neurological Stroke Red Flags:

2. Ṣiṣakoso ijagba ti o ni itunnu lakoko ti o nduro fun paramedics

Stimulants lower the brain's seizure threshold directly through sodium-channel effects (cocaine), cerebral vasospasm, hyperthermia, and hyponatremia (MDMA). Unlike a classic epileptic seizure that stops on its own after 60 seconds, drug-induced seizures frequently progress into continuous status epilepticus unless treated with intravenous or intramuscular benzodiazepines (midazolam, lorazepam, or diazepam):

  1. Clear the floor and cushion the head: Ease the person onto the floor, slide a folded jacket or pillow under their head, and push coffee tables, heaters, and glass objects out of reach.
  2. Place Ice Packs at the Neck, Armpits, and Groin: A convulsing stimulant patient generates massive muscle heat that can push core temperature past 41 degrees Celsius (106 degrees Fahrenheit) in minutes; start cooling the neck and armpits immediately while they are seizing.
  3. Never put anything in their mouth and never hold down their convulsing limbs: Forcing spoons, wallets, or fingers between clenched teeth breaks teeth and blocks the airway, and pinning down convulsing limbs fractures bones and worsens muscle breakdown (rhabdomyolysis).
  4. Roll into the Side-Lying Recovery Position the moment the jerking stops: Clear saliva or vomit from the mouth by rolling them onto their left side with the chin tilted slightly up so they can breathe freely until paramedics arrive.

Harm-Reduction Pre-Use and Mid-Binge Safety: Preventing Overamping and Fentanyl Poisoning

For individuals who currently use stimulants or family members supporting someone at risk, evidence-based harm-reduction steps dramatically cut the risk of fatal overdose:

Step-by-Step Harm-Reduction De-Escalation for Non-Critical Overamping

If you have checked every item in the triage table above and confirmed the person has no chest pain, no fever/hot skin, no seizures, no stroke symptoms, and normal breathing, they are experiencing a non-critical psychological/autonomic overamp (often triggered by taking slightly too much stimulant, mixing stimulants with caffeine, or being awake for 36 to 72 hours).

1. Environmental and physiological de-escalation checklist

  1. Change the Sensory Environment Immediately:
    • Move away from loud bass music, crowded rooms, bright lights, or stressful street corners into a cool, quiet bedroom with dim lighting.
  2. Cool the Vagus Nerve (Without Shocking the Body):
    • Place a cool, wet washcloth over their forehead, eyes, and the back of the neck, or let them take a lukewarm-to-cool (never freezing cold) shower while seated or supervised.
  3. Hydrate with Electrolytes and Simple Carbohydrates:
    • Stimulant binges deplete blood glucose, sodium, potassium, and magnesium while causing severe dry mouth ("cottonmouth") and jaw grinding (bruxism).
    • Offer small, steady sips of a chilled electrolyte drink, coconut water, milk, or a fruit smoothie, plus soft, easy-to-swallow snacks (banana, yogurt, applesauce, or broth) and a piece of chewing gum or a clean washcloth corner to prevent them from cracking their molars from jaw clenching.
  4. Encourage Gentle Movement OR Rest (Whichever Feels Safer to Them):
    • Some people who are overamping feel trapped if told to lie completely still; pacing slowly in a safe room, stretching, or sitting upright while practicing slow 4-second inhale / 6-second exhale breathing burns off nervous kinetic energy until the peak passes.

2. How to talk to someone experiencing stimulant paranoia or sleep-deprivation psychosis

After 48 to 96 hours without sleep on methamphetamine or crack cocaine, up to 40 to 60 percent of users experience transient stimulant-induced psychosis: auditory hallucinations (hearing whispers or police radios outside), visual shadow-people in peripheral vision, tactile hallucinations ("meth mites" / formication under the skin), or intense paranoia that people are watching the windows.

  • Never argue with or mock their delusion, and never pretend you see the delusion either:
    • Wrong: "You're crazy, there is nobody outside the window, sit down" (triggers anger and convinces them you are part of the plot).
    • Wrong: "Oh yeah, I see the spies outside too" (escalates their terror).
    • Right (Validate the emotion, ground in safety): "I can see how terrifying that feels right now. You are safe here with me in this room, the door is locked, and I am going to stay right here and watch out for you while you sit down and drink this cold water."
  • Keep your hands visible, stand at a 45-degree angle at least 2 meters (6 feet) away, and never block the exit doorway: A paranoid person who feels cornered in a room with someone blocking the door will panic and bolt or fight. Let them sit where they can see the door clearly.
  • Avoid giving unprescribed street "landing gear" pills: Never give someone who is overamping a street "Xanax" bar or opioid pill to put them to sleep; counterfeit street alprazolam bars are heavily laced with fentanyl, bromazolam, and xylazine, turning a survivable stimulant overamp into a fatal respiratory arrest while everyone goes to sleep.

Does Narcan (naloxone) work on a methamphetamine, cocaine, or crack overdose?

**Naloxone does NOT reverse the stimulant effects of pure methamphetamine or cocaine (such as racing heart, overheating, or agitation), because stimulants act on dopamine/norepinephrine receptors rather than opioid receptors.** HOWEVER, public health agencies (CDC, SAMHSA, and WHO) strongly instruct bystanders to **ALWAYS administer naloxone if someone who used cocaine, crack, meth, or MDMA is unresponsive and breathing slowly**, because a massive fraction of the illicit stimulant supply is contaminated with **fentanyl**. If they are unconscious and not breathing normally, it is almost certainly fentanyl contamination (or post-cardiac arrest), and naloxone plus rescue breathing can save their life with zero downside.

Why did my friend's urine turn dark brown like Coca-Cola the day after a heavy meth or MDMA binge?

Dark brown, tea-colored, or reddish-brown urine after stimulant use is the classic hallmark of **acute rhabdomyolysis**: severe overheating, muscle rigidity, and vasoconstriction have caused skeletal muscle fibers to break down and burst, dumping massive quantities of the muscle protein **myoglobin** into the bloodstream. Myoglobin clogs and poisons the microscopic filtering tubules of the kidneys, rapidly causing **acute kidney failure** and releasing fatal levels of **potassium (`hyperkalemia`)** into the heart. **Go to a hospital Emergency Department immediately** for aggressive intravenous fluid flushing (`200 to 300+ mL/hr` normal saline) to save their kidneys. (See [Acute kidney injury, rhabdomyolysis, and dark urine exertion triage](../first-aid/acute-kidney-injury-rhabdomyolysis-and-dark-urine-exertion-triage.md).)

Is it safe to throw someone having a stimulant overdose into an ice-cold shower by themselves to wake them up or cool them down?

**Never put an intoxicated, dizzy, or semi-conscious person into a shower alone, and avoid sudden freezing water if they are merely anxious rather than burning up with true hyperthermia.** If the person is truly burning hot (`> 39.5C / 103F`) with medical hyperthermia, you must actively cool them with ice packs at the neck/armpits/groin, cool water mist + fans, or a supervised cool bath while waiting for 911. However, putting an unsteady stimulant user alone in a shower frequently causes them to faint, slip on wet tile, and suffer a fatal traumatic brain bleed, or triggers sudden cold-shock vasoconstriction that spikes blood pressure during a cocaine chest-pain crisis.

What happens during the "Crash" (Days 1 to 5 after stopping a meth or cocaine binge), and when is it dangerous?

Unlike alcohol or benzodiazepine withdrawal, acute stimulant withdrawal (the **"crash"**) rarely causes physical withdrawal seizures, **but it carries an extreme risk of acute suicidal depression and profound hypersomnia (sleeping 16 to 24 hours a day)**. Because the binge exhausted the brain's vesicular stores of **dopamine and serotonin**, the person wakes up experiencing intense **anhedonia** (inability to feel any pleasure or hope), exhaustion, ravenous hunger, and crushing despair. During the first 72 hours of a stimulant crash, remove all firearms, lethal medications, and sharp hazards from the home, let them sleep and eat high-protein foods, check on their breathing every few hours while they sleep, and connect with the **988 Suicide & Crisis Lifeline** (US/Canada) or local crisis services if suicidal thoughts emerge.
