Freagra Anaifiolacsas Scoile, Rochtain Epinephrine, agus Pleananna Éigeandála 504

Ní hionann uimhreacha éigeandála de réir tíre. Úsáideann an leathanach seo 911, uimhir na Stát Aontaithe agus Ceanada. Úsáideann go leor tíortha eile 112, 999, nó uimhir eile. Faigh an uimhir do do thír.

Cónaíonn beagnach 1 as gach 13 leanbh in aois scoile (thart ar bheirt dhalta i ngach seomra ranga) le hailléirge bia a diagnóisíodh go cliniciúil, agus tarlaíonn suas le 25 faoin gcéad de riaradh epinephrine den chéad uair ar scoil i leanaí a raibh ailléirge neamhdhiagnóisithe acu roimhe seo roimh lón scoile nó sneaiceanna ranga. Murab ionann agus gríos éadrom logánta, is easghluaiseachta mear ailléirgeach ilchórais atá bagrach don bheatha é anaifiolacsas, agus é spreagtha ag próitéiní bia (píseanna talún, cnónna crann, bainne, uibheacha, cruithneacht, soy, iasc, sliogéisc, nó sesame), venom feithidí (stingeanna beacha nó foichí), laitéis, nó cógais. I leanaí agus ógánaigh, tá anaifiolacsas scoile marfach nó gar-mharfach ceangailte go mór le dhá earráid inchoiscthe: moill ar an epinephrine ionmhatánach agus tú ag fanacht le fáil amach an n-oibríonn frithhistamíní béil, agus ** ag cur leanbh le turraing anaifiolachtach ina sheasamh agus ag siúl síos an halla go dtí oifig na n-altraí scoile (a chuireann faoi deara ** Siondróm marfach, titimeas tobann).

Chun dalta a chosaint le linn géarchéime ailléirgeach scoile tá gá le forghníomhú cliniciúil láithreach ag an mbord seomra ranga nó caifitéire in éineacht le cosaintí cearta sibhialta feidearálacha in-fhorghníomhaithe faoi Alt 504 den Acht Athshlánaithe agus An tAcht um Meiriceánaigh faoi Mhíchumas (ADA). Trí leideanna anaifiolacsais péidiatraiceacha subtle a aithint ("My tongue feels spicy or itchy"), epinephrine a instealladh isteach sa mhéis lár amuigh (vastus lateralis) ar an gcéad chomhartha de rannpháirtíocht sistéamach nó dhá-orgán, an leanbh a choinneáil cothrom ar a dhroim (supine) agus a chosa ardaithe, dara dáileog epinephrine a riaradh ag 5 nóiméad má leanann na hairíonna, agus ag forfheidhmiú reachtúil Student Epinephrine Self-Iompar agus Stoc Scoile Dlíthe Epinephrine Neamhainmnithe, is féidir le tuismitheoirí agus le teagascóirí aon suíomh scoile a chosc.

Déan é seo anois

  1. Cuir Epinephrine isteach sa sliasaid lár amuigh LÁITHREACH má tá amhras ann faoi anaifiolacsas, glaoigh ar 911 / 112 / 999. Má thaispeánann leanbh **aon ** siomptóim thromchúiseach riospráide nó chardashoithíoch (ganntanas anála, wheezing, casachtach athchleachtach, stríoc, titim mhaol/flapach, nó ** siomptóim ata sa choirp, nó ** siomptóim ata thar an gcorp) coirceoga móide urlacan/cramps boilg) tar éis dóchúil a bheith nochta d’ailléirgin, instealladh Epinephrine Auto-Injector (0.3 mg le haghaidh >=25 to 30 kg / 55 to 66 lbs, 0.15 mg le haghaidh 15 to 25 kg / 33 to 55 lbs, nó 0.1 mg le haghaidh 7.5 to 15 kg / 16.5 to 33 lbs) nó Epinephrine Intranasal ceadaithe (neffy) láithreach, agus cuir an dara duine fásta glaoch ar d’uimhir éigeandála áitiúil (911 sna Stáit Aontaithe agus i gCeanada, 112 ar fud an Aontais Eorpaigh, 999 sa Ríocht Aontaithe, nó féach ar gach uimhreacha éigeandála) otharcharr Ardtacaíochta Saoil (ALS) a iarraidh.
  2. NÁ cuir páiste ailléirgeach ina sheasamh ná siúil go dtí oifig altra na scoile. Tabhair leat an Epinephrine Auto-Injector agus altra na scoile chuig an leanbh. Leag an mac léinn árasán ar a dhroim (supine) agus a chosa ardaithe 30 ceintiméadar (12 orlach) (nó greamaithe ar a thaobh sa suíomh téarnaimh má bhíonn urlacan á n-urlacan, nó suí le cosa leata má fhágann anacair riospráide thromchúiseach nach féidir luí go hiomlán cothrom); Nuair a sheasann leanbh ina seasamh le linn vasodilation anaifiolachtach, líonann sé an fhuil sna foircinní íochtaracha agus is féidir leis gabháil chairdiach láithreach do-aisiompaithe a chruthú.
  3. Ná tabhair frithhistamíní ó bhéal riamh (Benadryl / Cetirizine) nó análóir asma in ionad Epinephrine. Tógann frithhistamíní 30 go 60 nóiméad le hobair agus ní dhéanann siad ach tochas craicinn agus coirceoga a mhaolú; déanann siad nialas chun at aerbhealaigh marfach nó titim brú fola anaifiolachtach a aisiompú. Mar an gcéanna, ní féidir le análóir asma albuterol stop a chur le héidéime laringe an aerbhealaigh uachtair; instealladh i gcónaí Epinephrine CHÉAD (go háirithe i mic léinn a bhfuil plúchadh agus ailléirgí bia araon, a bhfuil an riosca is airde de anaifiolacsas marfach).
  4. Ullmhaigh an dara Epinephrine Auto-Injector agus instealladh é tar éis 5 nóiméad mura dtagann feabhas ar na hairíonna. Tabhair ** dhá dháileog Epinephrine le chéile i gcónaí (2-Pack). Má leanann nó má théann wheezing, tightness scornach, táimhe, nó urlacan in olcas ** 5 go 15 nóiméad** tar éis an chéad instealladh, a thabhairt ar an dara Epinephrine Auto-Injector sa thigh lár-smuigh eile.
  5. Seol an leanbh chuig Rannóg Éigeandála an Ospidéil in otharcharr fiú má fhéachann sé 100% air a ghnóthú. Caitheann Epinephrine amach i 15 go 30 nóiméad, agus taithí suas le 15 faoin gcéad go 20 faoin gcéad de na hothair Anaifiolacsas Déphasic, dara tonn contúirteach de shrianadh aerbhealaigh agus turraing a fhilleann nochtadh 1 go 12+ uair an chloig níos déanaí gan aon bhia. Tabhair an t-uath-instealltóir(í) agus an t-am insteallta cruinn scríofa ar théip do na paraimhíochaineoirí.
  6. Má chuir an scoil faoi ghlas do leanbh EpiPen, eisiata ó thuras allamuigh é, nó má dhiúltaigh siad socruithe ailléirge, agairt Alt 504 láithreach. Faoin Acht Leasuithe ADA 2008 agus fasach na Roinne Dlí agus Cirt / Oifig um Chearta Sibhialta (OCR) na SA, is míchumas faoi chosaint an dlí é ailléirge bia nó venom atá bagrach don bheatha; ní mór do scoileanna Plean Alt 504 ceangailteach a sholáthar (nó Plean Cúraim Sláinte Aonair Reachtúil na RA / Plean Dlí Sabrina Cheanada) a ráthaíonn rochtain neamhghlasáilte epinephrine agus cuimsiú iomlán.

Cén fáth ar féidir marfach a bheith ag seasamh nó ag siúl chuig oifig na n-altraí le linn anaifiolacsas

Le linn anaifiolacsas dian, sceitheann méideanna ollmhóra sreabhach amach as an sruth fola isteach sna fíocháin máguaird agus caolaíonn soithigh fola go tapa, rud a fhágann go dtiteann suas le 35 faoin gcéad den toirt fola a scaiptear go héifeachtach laistigh de 10 nóiméad. I staidéir fhóiréinseacha suntasacha ar anaifiolacsas marfach (Pumphrey, Journal of Ailléirge agus Imdhíoneolaíocht Chliniciúil), d’fhulaing othair ar cuireadh iallach orthu suí bolta ina seasamh, seasamh suas, nó siúl díreach 20 méadar (65 troigh) chuig clinic altra scoile nó sínteán otharchairr bás tobann laistigh de soicind ó "Siondróm Ventricle Folamh, cosc iomlán ar an tromaíocht ó chroí," toisc gur chuir an croí-thromchúis cosc ar ais go hiomlán ar an tromaíocht. folamh. Riail #1 d’fhreagairt anaifiolacsas na scoile: Cuir an dalta in áit cheart ag deasc an tseomra ranga nó ag an mbinse caifitéire áit ar thosaigh na hairíonna, leag cothrom ar an urlár iad agus na cosa ardaithe, agus rith an EpiPen CHUIG an dalta.

Phase 1: How children describe anaphylaxis & the 2-System Clinical Diagnostic Rule

Young children rarely say, "I am experiencing laryngeal edema and systemic hypotension." Instead, a 6-year-old at a cafeteria table will tug at their collar, scratch their ears, spit food onto their napkin, or use developmental phrases that every teacher, cafeteria monitor, and bus driver must be trained to recognize immediately.

How young children verbally describe early anaphylaxis

  • "My tongue is hot, burning, or feels spicy." (When eating non-spicy food)
  • "My mouth itches or feels like there is hair or a frog in my throat."
  • "My tongue feels too big or heavy for my mouth."
  • "Something is poking my throat or it hurts to swallow."
  • "My ears or the palms of my hands are really itchy."
  • "I feel dizzy, sleepy, or like something terrible is about to happen." (Sense of impending doom caused by sudden blood pressure drop)

Clinical criteria: When to inject Epinephrine immediately (NIAID / FARE / WAO Guidelines)

Under the Food Allergy Research & Education (FARE) Emergency Care Plan and National Institute of Allergy and Infectious Diseases (NIAID) clinical criteria, inject Epinephrine immediately if EITHER Trigger A OR Trigger B occurs within minutes to two hours of a known or suspected allergen exposure:

Diagnostic Trigger Symptoms Observed in the Student Immediate Clinical Action
Trigger A: ANY Single Severe Symptom in Lungs, Heart, or Throat/Mouth - Lung: Shortness of breath, wheezing, repetitive dry coughing (often mistaken for an asthma attack), chest tightness.
- Heart / Vascular: Pale, blue/grayish skin, fainting, weak/thread pulse, dizziness, sudden floppiness in a young child.
- Throat / Mouth: Tightness or hoarseness in throat, trouble swallowing, stridor (high-pitched inspiratory sound), significant swelling of tongue or lips.
1. INJECT EPINEPHRINE IMMEDIATELY.
2. Lay flat (legs elevated).
3. Call 911 / 112 / 999.
Trigger B: ANY Combination of Mild-to-Moderate Symptoms Across TWO OR MORE Body Systems For example, Skin + Gut happening together:
- Skin: Hives, widespread redness/flushing, itchy palms/ears, mild facial swelling.
- Gut (GI): Vomiting, severe abdominal cramps, sudden diarrhea.
- Nose/Eyes: Sudden profuse runny nose, sneezing fits, red itchy watery eyes.
1. INJECT EPINEPHRINE IMMEDIATELY.
(Notice: Even if the child has ZERO trouble breathing yet, having Hives + Vomiting equals TWO body systems = Anaphylaxis = Inject Epinephrine now).
Trigger C: Known History of Severe Anaphylaxis + Confirmed Allergen Ingestion Many allergists check the box on the student FARE Action Plan stating: "If child definitely ate [Peanut/Tree Nut/Dairy], inject Epinephrine immediately even before symptoms start." Follow the physician order on the FARE Plan and inject immediately.

There is no medical contraindication to giving Epinephrine in a suspected allergic emergency

Teachers and parents sometimes hesitate to use an EpiPen because they worry: "What if it is just a panic attack or a mild reaction, will the Epinephrine hurt the child?" Pediatric emergency medicine and allergy societies universally emphasize: when in doubt, give the Epinephrine. In a healthy child or teenager, a weight-appropriate intramuscular dose of epinephrine causes temporary shakiness, a fast heartbeat (like running a sprint), and pale skin for 20 minutes, whereas withholding or delaying epinephrine by 15 minutes during true anaphylaxis can result in hypoxic brain injury or death.

Phase 2: Step-by-step administration for all 4 school Epinephrine devices

Modern school health offices and student backpacks contain four main types of FDA/EMA/Health Canada-approved epinephrine devices. Store all epinephrine devices at controlled room temperature between 20 and 25 degrees Celsius (68 and 77 degrees Fahrenheit), never freezing them or leaving them in a hot car or bus, and check the viewing window monthly to confirm the liquid solution remains clear and colorless. Because each device operates slightly differently during an emergency, review the exact mechanism below:

Comparison of Epinephrine Auto-Injectors and Nasal Spray (EpiPen, Auvi-Q, Generic, neffy)

Device Name & Dosing Tiers Exact Step-by-Step Administration Technique Hold Time in Thigh & Post-Injection Safety
1. EpiPen / EpiPen Jr (and Mylan/Teva Generics)
- Yellow (0.3 mg): >= 30 kg (66 lbs)
- Green (0.15 mg): 15 to 30 kg (33 to 66 lbs)
1. Flip open yellow/green carrier tube cap and slide auto-injector out.
2. Grip barrel in fist with orange tip pointing down ("Blue to the sky, Orange to the thigh," never put thumb over either end).
3. Pull BLUE safety release straight up.
4. Swing and push ORANGE tip firmly into mid-outer thigh (vastus lateralis) at a 90-degree angle (through jeans/clothing is fine, avoiding seams/pockets) until it clicks.
Hold firmly against outer thigh for 3 full seconds (1-1000, 2-1000, 3-1000).
(Orange needle cover automatically extends to cover needle when removed).
2. Auvi-Q / Allerject (Voice-Guided Rectangular Device)
- 0.3 mg: >= 30 kg (66 lbs)
- 0.15 mg: 15 to 30 kg (33 to 66 lbs)
- 0.1 mg: 7.5 to 15 kg (16.5 to 33 lbs)
1. Pull Auvi-Q out of its outer rectangular case (this activates automated voice instructions that talk you through every step).
2. Pull RED safety guard straight down and off.
3. Place BLACK base against the middle of the outer thigh at a 90-degree angle, then press firmly until you hear a click and hiss.
Hold firmly in place for 2 full seconds (2-second countdown beep).
Needle automatically retracts back inside the black base after 2 seconds so you never see an exposed needle.
3. Adrenaclick / Impax / Lineage Generic Epinephrine
(Two-Cap Cylinder)
1. Remove device from protective carrying case.
2. Tarraing amach Caipín #1 (Liath) ó fhoirceann amháin, ansin tarraing amach Cap #2 (Dearg/Glas) ón taobh eile.
3. Cuir an Leid dhearg in aghaidh na pluide lár amuigh agus brúigh síos go dian go dtí go dtiocfaidh an tsnáthaid isteach sa mhatán (ní gá luascadh).
Hold firmly in the outer thigh for 10 full seconds, then remove carefully (needle remains exposed; place back in tube).
4. neffy (Epinephrine Nasal Spray, Needle-Free)
- 2 mg: >= 30 kg (66 lbs)
- 1 mg: 15 to <30 kg (33 to <66 lbs) (ages 4+)
1. Peel open blister pack and remove nasal spray device.
2. Do NOT prime or test spray into the air (contains one single dose).
3. Insert nozzle straight into ONE nostril until your fingers touch the bottom of the child nose.
4. Press plunger firmly all the way up to spray.
Mura dtagann feabhas ar na hairíonna nó mura dtéann siad in olcas tar éis 5 nóiméad, tabhair dara branda nua neffy gléas isteach sa nostril CÉANNA.

Nóta maidir le leanbh atá ag streachailt: Nuair a bhíonn instealladh ceille ionmhatánach á thabhairt do leanbh óg atá scanraithe, cuir barróg an linbh go réidh ar an dara duine fásta agus coinnigh glúin/cos níos ísle an linbh go daingean in aghaidh an urláir ionas nach ndéanann an leanbh a chos a chreimeadh le linn na fuinneoige insteallta 2 go 3 soicind (is féidir leis an masa a mhaolú nó an gátar a dhíbirt roimh am).

Céim 3: Dlíthe Féin-Iompar Epinephrine Mac Léinn agus Stoc Scoile ("Neamhainmnithe") Epinephrine

San am a chuaigh thart, d'éiligh scoil a bhí as dáta "Beartas Neamhfhulaingt Drugaí" gach cógas, lena n-áirítear EpiPens tarrthála agus análóirí asma, a chur faoi ghlas taobh istigh de chomh-aireachta in oifig altra na scoile. Nuair a chuaigh altra na scoile chuig an lón, nuair a chlúdaigh sí foirgneamh eile, nó nuair a d’fhág sé ag 15:00 roimh chleachtadh spóirt iarscoile, fuair leanaí bás lasmuigh de dhoirse an chlinic faoi ghlas.

1. Ceart Iomlán Reachtúil do Leanbh chun Féin-Iompar (Gach 50 Stát de chuid na SA, Ceanada, an RA)

  • Dlíthe Uilíocha Féin-Iompar & Féinriaracháin: Gach 50 Stát Aontaithe (móide Washington, D.C.), cúigí Cheanada (faoi Dhlí Sabrina in Ontario agus treoracha cúige comhthreomhara), agus an Ríocht Aontaithe tar éis reachtanna a achtú a ráthaíonn go bhfuil an ceart dlíthiúil ag mac léinn ar bith a bhfuil riosca deimhnithe ag dochtúir aige/aici ar anaiifiolacsas a riaradh uathfhrith-riaracháin agus féin-riaracháin. (agus análóir tarrthála asma) ar a bpearsa** i seomraí ranga, caifitéire, rang giomnáisiam, busanna scoile agus turais allamuigh i gcónaí.
  • Na Rudaí is Féidir leis an Scoil A Dhíth (agus Ní Féidir A Dhéanamh):
  • D’fhéadfadh go n-éileoidh an scoil: (1) údarú scríofa sínithe ag an dochtúir linbh ag dearbhú gur tugadh treoir don dalta conas an gléas a úsáid, agus (2) foirm thoilithe tuismitheora sínithe (agus instealladh uathoibríoch cúltaca coimeádta san oifig sláinte).
  • Ní féidir leis an scoil **** iallach a chur ar dhalta meánscoile nó ardscoile cáilithe (nó ar dhalta bunscoile lánfhásta arna cheadú ag a ailléirgeoir) a EpiPen a fhágáil faoi ghlas san oifig, ná ní féidir le múinteoir crios waist liachta mac léinn a choigistiú (SPIbelt) nó pouch ailléirge le linn scrúdaithe nó rang aclaíochta.

2. Dlíthe Epinephrine "Stoc / Neamhshainithe" Scoile (An tAcht um Rochtain Scoile Chónaidhme ar Epinephrine Éigeandála)

Cad a tharlóidh má itheann dalta 1ú grád nach bhfuil diagnóisíodh riamh le hailléirge bia** dó caisiú nó má chuirtear seaicéad buí air ar an gclós súgartha agus má théann sé i turraing anaifiolachtach gan aon EpiPen pearsanta i gcomhad? - Faoin Acht um Rochtain Scoile ar Epinephrine Éigeandála (agus dlíthe stáit ar fud beagnach gach ceann de na 50 stát, Alt 238A de Rialacháin Leasú Leigheasra Daonna na Ríochta Aontaithe 2017, agus reachtanna cúige Cheanada), tá scoileanna údaraithe (agus i go leor stát sainordaithe) chun fianna Neamhshainithe / "Spála" a sholáthar faoi phrótacal Uath-Insteallta Epinephrine. - Soláthraíonn na reachtanna seo díolúine soiléir ó dhliteanas sibhialta Samárach Maith d'aon altra scoile, múinteoir, cóitseálaí nó ball foirne a riarann stoc Instealladh Epinephrine de mheon macánta ar aon mhac léinn nó cuairteoir campais a chreidtear a bheith ag fulaingt anaifiolacsas.

Céim 4: Tógáil Ironclad Plean Alt 504 maidir le hAilléirgí Bia Géar nó Venom

Many schools hand parents of allergic children a 1-page medical sheet called an Individualized Healthcare Plan (IHP) or Emergency Action Plan (EAP) written solely by the school nurse, while telling the parent: "Your child gets straight A's on their report card, so they do not qualify for a legal Section 504 Plan."

Diúltaigh an diúltú sin láithreach i scríbhinn. Cé gur leathán treorach cliniciúil tábhachtach é IHP/EAP, is doiciméad altranais inmheánach é IHP amháin a bhfuil fiacla forghníomhaithe cearta sibhialta feidearálacha nialasach ann. A Plean Alt 504 (faoi Alt 504 den Acht Athshlánaithe, 1973, 29 USC. 794, agus Teideal II/III den Acht um Meiriceánaigh faoi Mhíchumas) is conradh um chearta sibhialta cónaidhme atá ceangailteach ó thaobh dlí arna fhorfheidhmiú ag Roinn Oideachais na SA ** An Oifig um Chearta Sibhialta (OCR)**.

Why your child qualifies for a Section 504 Plan even if their Allergy is "In Remission" When Avoiding Food

Under the ADA Amendments Act of 2008 (ADAAA, 42 U.S.C. 12102(4)(D)): 1. "Episodic Conditions" Rule: An impairment that is episodic or in remission (such as a severe food allergy, asthma, epilepsy, or diabetes) is legally a disability under Section 504 and the ADA if it would substantially limit a major life activity (such as breathing, eating, or immune/digestive system function) when active. 2. "No Mitigating Measures" Rule: Schools must evaluate whether the allergy substantially limits breathing/eating without considering the ameliorative effects of mitigating measures (such as strict allergen avoidance or carrying an EpiPen). 3. Grades Are Irrelevant: Section 504 eligibility is not limited to learning disabilities or low grades; a straight-A student with a life-threatening peanut, dairy, or bee-sting allergy has an absolute federal right to a formal Section 504 Plan.

Mandatory accommodations to include in your child Food Allergy Section 504 Plan

  • 1. Unlocked Epinephrine Access & Proximity Rule: Student Epinephrine Auto-Injectors (2-pack) must be kept in an unlocked emergency cabinet in the health office AND carried by the student (or by the classroom teacher in a hip-pack for Pre-K to Grade 3) so that Epinephrine can reach the student within 60 to 90 seconds anywhere on campus (playground, gym, art room, cafeteria).
  • 2. No-Walk Emergency Protocol: Explicit mandate that if the student exhibits any allergic symptoms or reports suspect ingestion, the student shall NOT be sent walking to the nurse office; staff must call a "Code Medical" to bring the nurse and Epinephrine directly to the student.
  • 3. Cafeteria & Classroom Allergen-Safe Zones:
  • Provision of an Allergen-Safe Table (cleaned before lunch with a dedicated fresh cloth and EPA-approved detergent/wipe; note that hand sanitizer gels do NOT remove food allergen proteins, and only soap and running water or friction commercial wipes remove peanut/dairy proteins from hands and tables), making sure the student is never isolated alone in a corner as "punishment" and can invite classmates eating safe lunches to sit with them.
  • Strict No-Food Classroom Curriculum Rule: Prohibiting the use of the child allergens in classroom math counters, science experiments, sensory bins (such as birdseed containing peanuts, dried pasta/wheat playdough, or egg cartons), or unapproved bake-sale treats.
  • Handwashing Protocol: All students in the elementary classroom wash hands with soap and water (or wet friction wipes, not alcohol gel) upon entering the classroom after lunch/snack.
  • 4. Substitute Teacher & Bus Driver Notification: Mandatory "Sub Folder" red alert sheet handed to every substitute teacher at morning check-in, and trained emergency communication protocols on the school bus (see school bus accident, field trip emergency, and parent-child reunification).
  • 5. Full Field Trip Inclusion (Zero Mandatory Parent Chaperone Conditioning):
  • Important Federal Rule: Under U.S. Department of Education OCR enforcement rulings, a public school cannot tell a parent: "Your allergic or diabetic child can only go on the class field trip if you take a day off work to come chaperone them yourself."
  • While parents are always welcome to volunteer as chaperones, the school district is legally required under Section 504 to provide a trained staff member or nurse to carry and administer the student Epinephrine on every school-sponsored field trip, regardless of whether the parent attends.
  • 6. Anti-Bullying Protection: Explicit protocol treating any intentional waving of an allergen in the student face, threatening to smear peanut butter on their desk, or mocking their allergy as a severe Disability Harassment / Title II / Section 504 violation (see student bullying, hazing, and cyberbullying).

Phase 5: University and college campus dining hall & dormitory allergy rights (DOJ Lesley University Precedent)

Food allergy rights do not end at high school graduation. When a student with severe food allergies or celiac disease enrolls at a college or university that requires freshmen to live in dormitories and purchase a mandatory $3,000-per-semester campus meal plan:

  • The U.S. Department of Justice Lesley University Landmark Settlement: The U.S. Department of Justice Civil Rights Division established under the Americans with Disabilities Act (ADA) that severe food allergies and celiac disease are protected disabilities in higher education.
  • Má ghearrann ollscoil táille plean bia éigeantach, ní mór don ollscoil: (A) béilí sábháilte tras-teagmhála saor ó ailléirgin a sholáthar (cosúil le stáisiún tiomnaithe saor ó ailléirginí, béilí cistine réamhordaithe, agus lipéadú soiléir ar chomhábhair), (B) seomra/sraith phríobháideach dorm a sholáthar don mhac léinn le cistin/cuisneoir/micreathonn sábháilte (deontas iomlán airgeadais) ** chun díolúine iomlán a thabhairt don mhac léinn. an plean bia éigeantach don champas**.
  • College Resident Advisors (RAs) and Campus Police should also be notified via the Campus Disability Services Office of where backup Epinephrine is stored in the student dorm room.

Because severe allergic reactions can progress to respiratory arrest within minutes, we strongly recommend that all parents, teachers, coaches, and babysitters complete a hands-on First Aid, CPR, and Anaphylaxis / Epinephrine Auto-Injector training course through the Red Cross, St John Ambulance, or a certified local provider.

Ceisteanna coitianta

My child school says they will give my child liquid Benadryl first if they accidentally eat a peanut at lunch, and then "watch them for 20 minutes" before using the EpiPen. Is that safe?

No, that "wait-and-see with Benadryl" approach is dangerous and directly contradicts current clinical guidelines from the American Academy of Pediatrics (AAP), NIAID, and Food Allergy Research & Education (FARE). Oral antihistamines like diphenhydramine (Benadryl) or cetirizine (Zyrtec) take 30 to 60 minutes to reach peak effect and ONLY treat mild skin hives and itching; they cannot stop fatal airway swelling or anaphylactic blood pressure collapse. Worse, waiting 20 minutes to see if Benadryl works masks early skin symptoms while life-threatening internal respiratory or cardiovascular shock progresses silently. Epinephrine is the ONLY first-line medication for anaphylaxis and must be injected immediately.

Does hand sanitizer (like Purell) remove peanut butter, milk, or egg allergens from a child hands or a classroom desk?

No. Clinical studies published in the Journal of Allergy and Clinical Immunology prove that alcohol-based hand sanitizer gels disinfect bacteria and viruses, but do NOT break down or remove food allergen proteins, as they simply smear intact peanut or dairy protein around on the skin. To actually remove food proteins from hands after eating, students must wash their hands with **soap and running water** (or use a commercial wet wipe with physical friction). Classroom and cafeteria tables must be cleaned with soap/detergent and water or commercial cleaner using a clean disposable towel to prevent cross-contact.

The school principal told me that my 3rd-grader with a severe tree nut allergy cannot go on the class zoo field trip unless I take off work and go with them to hold the EpiPen. Is that legal?

No. Under Section 504 of the Rehabilitation Act and the Americans with Disabilities Act (ADA), public schools (and private schools receiving federal funds or covered by ADA Title III) are strictly prohibited from conditioning a disabled student participation in a school field trip on their parent serving as an unpaid medical chaperone. While a parent may *choose* to attend voluntarily, the school is legally required to send a school nurse or a trained, delegated staff member who carries the child Epinephrine Auto-Injector and is trained to administer it throughout the trip.

If we use an EpiPen on a student at school and their breathing and hives completely clear up within 5 minutes, do we still have to call 911 and send them to the hospital in an ambulance?

Yes, 100 percent of the time. Calling 911 and transporting the student to the Emergency Department by ambulance is mandatory after every school epinephrine administration for two reasons: First, the first dose of epinephrine may not be enough, and paramedics carry advanced airway equipment, oxygen, IV fluids, and additional epinephrine. Second, epinephrine metabolizes out of the body in 15 to 30 minutes, and up to 20 percent of patients suffer a "Biphasic Reaction," a sudden, severe return of anaphylaxis 1 to 12 hours later without any new exposure to the allergen. Hospital observation (typically 4+ hours) is required to make sure the reaction does not rebound.
