Unlike thermal fire burns that stop cooking tissue the moment the flame is extinguished, chemical burns from hydrofluoric acid (\(HF\)), strong alkalis (sodium hydroxide lye, drain openers, oven cleaners, wet Portland cement, anhydrous ammonia), concentrated mineral acids (sulfuric, nitric, hydrochloric), and white phosphorus continue burrowing through human skin, fat, tendons, and bone for hours until every trace of the chemical is flushed away or neutralized. Most dangerous of all is Hydrofluoric Acid (\(HF\)): found not only in semiconductor and glass-etching factories, but also in household rust stain removers, aluminum/chrome wheel cleaners, and masonry brighteners: which passes painlessly through intact skin to strip calcium and magnesium out of the bloodstream, triggering sudden fatal ventricular fibrillation cardiac arrest from a burn no bigger than the palm of your hand.
This guide provides step-by-step field decontamination and clinical antidotal protocols aligned with directives from the World Health Organization (WHO), the United States Centers for Disease Control and Prevention (CDC / NIOSH), the Agency for Toxic Substances and Disease Registry (ATSDR), the American Burn Association (ABA), and the UK National Poisons Information Service (NPIS). Stripping contaminated clothing, brushing off dry powders before adding water, flushing with running water for 15 to 60 minutes, and massaging 2.5 percent calcium gluconate gel onto hydrofluoric acid burns saves limbs and lives.
Ivyo ubikore ubu¶
If you or someone nearby has splashed, spilled, or knelt in hydrofluoric acid, rust stain remover, wheel cleaner, drain opener, wet cement, lime, or any industrial acid or alkali:
- Put on chemical-resistant gloves and strip off all contaminated clothing and jewelry immediately: Never touch chemical-soaked clothing or skin with bare hands (use nitrile, butyl, or neoprene gloves). Use shears to cut away all wet clothing, socks, shoes, rings, and watchbands within 60 seconds, as trapped chemicals inside fabric or under rings cause deep circumferential full-thickness burns.
- Check the "Dry Powder First Rule" BEFORE turning on the water: If the skin is covered in a DRY chemical powder: such as dry quicklime (calcium oxide / \(CaO\)), dry Portland cement dust, dry sodium hydroxide lye flakes, elemental metals (sodium, potassium, lithium), or white phosphorus: BRUSH the dry powder off gently with a dry cloth or brush BEFORE applying water. Adding a trickle of water directly onto dry quicklime or dry lye crystals triggers a violent exothermic heat reaction that thermally cooks the skin on top of the chemical burn.
- Flush with high-volume running water immediately: Once dry powder is brushed off (or immediately if the splash was a liquid):
- For Hydrofluoric Acid (\(HF\)), Rust Remover, or Wheel Cleaner: Flush under a strong stream of cool running tap water (or Hexafluorine solution if available at an industrial site) for 15 to 30 minutes (or for 5 minutes if 2.5% Calcium Gluconate Gel is immediately on hand so you can start calcium binding right away).
- For Strong Alkalis (Lye, Drain Opener, Wet Cement, Ammonia): Flush continuously under running tap water for 30 to 60 full minutes (alkalis dissolve tissue fats into soap and penetrate far deeper than acids).
- For Chemical Eye Splashes: Flush the open eye under a gentle stream of running water or sterile saline for at least 30 to 60 minutes continuously (see ibikomere vy'amaso).
- Apply 2.5% Calcium Gluconate Gel immediately to any Hydrofluoric Acid (\(HF\)) or Rust Remover burn: Wearing nitrile gloves (so you do not absorb fluoride into your own fingers), massage 2.5 percent Calcium Gluconate Gel generously into the burned skin continuously for 30 to 45 minutes, reapplying every 15 minutes until the deep throbbing bone pain stops. If you do not have commercial Calcium Gluconate Gel at home: crush 10 tablets of regular Calcium Carbonate antacid (such as Tums) into a fine powder and mix with 30 milliliters (1 ounce / 2 tablespoons) of water-soluble K-Y jelly or aloe vera gel to make an emergency calcium paste, and rub it into the burn while paramedics are on the way.
- NEVER attempt to "neutralize" an acid burn with baking soda or an alkali burn with vinegar: Pouring vinegar onto a lye burn or baking soda onto a sulfuric/hydrochloric acid burn triggers an intense exothermic acid-base heat reaction right on the raw flesh, superimposing a severe thermal burn on top of the chemical injury. (Calcium gluconate on Hydrofluoric Acid is NOT an acid-base neutralization: it is a non-thermal ionic calcium-fluoride binding reaction and is the ONLY exception).
- Call 911 / 112 / 999 immediately for Hydrofluoric Acid, Eye Burns, or Deep Alkali Burns: Call your local emergency number via our inomero z'ivyihutirwa directory. Every single Hydrofluoric Acid (\(HF\)) exposure requires hospital emergency department evaluation and continuous ECG cardiac monitoring, even if the skin looks completely normal and does not hurt yet, because dilute \(HF\) (
< 20%) takes 6 to 24 hours to start hurting while silently dropping blood calcium to lethal levels.
Why a palm-sized Hydrofluoric Acid burn can cause sudden cardiac arrest
Unlike sulfuric or hydrochloric acid (which coagulate surface proteins and stop penetrating), Hydrofluoric Acid (\(HF\)) is a weak acid whose uncharged molecule slips effortlessly through intact skin and lipid cell membranes. Once deep inside your tissue and bloodstream, each fluoride ion (\(F^-\)) acts as a metabolic scavenger: it latches onto positively charged Calcium (\(Ca^{2+}\)) and Magnesium (\(Mg^{2+}\)) ions in your blood and nerves, precipitating them into insoluble calcium fluoride (\(CaF_2\)). Deprived of calcium and flooded with potassium released from dying cells (hypocalcemia, hypomagnesemia, and hyperkalemia), the heart's electrical system collapses into refractory QT prolongation, Torsades de Pointes, and fatal Ventricular Fibrillation. A concentrated (> 50%) \(HF\) burn covering just 1 percent to 2.5 percent of body surface area (the size of one palm) can be fatal without immediate intravenous and local calcium rescue.

Pathophysiology: Coagulative (Acids) vs. Liquefactive (Alkalis) vs. Fluoride Scavenging (\(HF\))¶
Why does a splash of alkaline drain cleaner or wet concrete cause a much deeper, more destructive burn than a splash of battery acid of the same strength, and why does hydrofluoric acid behave differently from both?
1. Strong Acids (\(\text{pH} < 2\)): Coagulative Necrosis (Self-Limiting Crust)¶
When concentrated sulfuric acid (car battery acid / industrial drain cleaner), hydrochloric acid (muriatic pool/masonry acid), nitric acid, or trichloroacetic acid hits human skin, hydrogen ions (\(H^+\)) instantly denature and precipitate surface tissue proteins into a tough, leathery, impermeable crust called an eschar (coagulative necrosis):
- Because this coagulated protein crust acts as a physical barrier, most strong mineral acids limit their own deep penetration once the surface layer is burned (provided the acid is flushed off with water within the first few minutes).
- Ibimenyetso vy’amabara ku rukoba: Aside nitrique ihindura urukoba rwaturitse umuhondo ukeye gushika ku muhondo w’umuhondo, aside sulfurique ihindura urukoba umuhondo mwiza gushika ku mwirabura, aside hydrochlorique ihindura urukoba umweru gushika ku mvi-muhondo.
2. Ivyuma bikomeye vy’amazi (\(\text{pH} > 11.5\text{ gushika }12\)): Necrose y’amazi (Ugucukura bikomeye bitagira aho bigarukira)¶
When sodium hydroxide (\(\text{NaOH}\) / lye), potassium hydroxide (\(\text{KOH}\)), calcium hydroxide (wet Portland cement or mortar), anhydrous ammonia, or industrial oven/drain cleaners touch human skin or the cornea of the eye, hydroxide ions (\(OH^-\)) do not form a protective crust. Instead, they cause liquefactive necrosis:
- Guhindura ibinure vy’umuntu bibe isabune: Alkali ikorana n’ibinure biri mu ngingo z’umubiri w’umuntu be n’ibinure biri munsi y’urukoba, mu buryo nyabwo ihindura utugingo ngengabuzima twawe ubwawe tukaba isabune y’amazi (guhindura isabune) mu gihe ihindura poroteyine za kolajeni zikaba poroteyine ziyoroshe kandi zifise gelatine.

3. Hydrofluoric Acid (\(HF\)): The Double-Action Ion Trap and Deep Bone Pain¶
Asidi y’amazi (\(HF\)) ni ubumara budasanzwe buhenda bufise inzira zibiri:
- Why dilute \(HF\) (
< 20%) doesn't hurt at first: In household rust stain removers (which contain 6% to 12% \(HF\) or ammonium bifluoride), aluminum/chrome wheel cleaners, and brick/masonry cleaners, the hydrogen ion (\(H^+\)) concentration is too weak to burn the surface skin nerves immediately. A person gets rust remover on their fingers or under their fingernails, feels zero pain and sees zero redness, and keeps working for hours. - Deep fluoride nerve and bone destruction: Meanwhile, uncharged \(HF\) molecules diffuse straight through the intact epidermis into the deep dermis, digital nerve sheaths, and finger bone (phalangeal periosteum), where they dissociate and release free fluoride ions (\(F^-\)). Fluoride strips all calcium out of the cell membranes, forcing voltage-gated potassium channels open and dumping massive amounts of intracellular potassium (\(K^+\)) directly onto bare sensory nerve endings.
- Agonizing "Throbbing Bone Pain" out of proportion to the exam: Suddenly, 6 to 24 hours after a dilute \(< 20\%\) exposure (or within 1 to 8 hours for a 20% to 50% exposure, and immediately for \(> 50\%\) industrial \(HF\)), the victim develops excruciating, relentless, deep throbbing pain that feels like their finger bones are being crushed in a vise, even before white/gray blisters or blue-black necrotic patches appear on the surface. Standard morphine or fentanyl opioids barely touch this pain: only replacing the stolen calcium with Calcium Gluconate stops the nerve depolarization and relieves the pain.

Deep dive 1: Complete Hydrofluoric Acid (\(HF\)) Rescue Protocol (Home to ICU)¶
Because household rust stain removers (sold in small plastic squeeze bottles in supermarket laundry aisles at 6% to 12% Hydrofluoric Acid) and automotive aluminum/magnesium wheel cleaners rarely print "HYDROFLUORIC ACID" in giant letters on the front label, thousands of homeowners and detailers suffer severe finger and hand burns every year without realizing they have touched one of the most dangerous chemicals on earth.
Step 1: Immediate Homemade Calcium Gluconate Gel Recipe¶
Every workplace that uses \(HF\) is required to keep unexpired tubes of 2.5 percent Calcium Gluconate Gel next to the sink. If a rust-remover or wheel-cleaner exposure happens at home where commercial calcium gluconate gel is unavailable:
- Flush under cool running tap water for 5 to 15 minutes while a family member calls 911 / 112 / 999 and Poison Control (1-800-222-1222 in the US).
- Mix an emergency Calcium Carbonate Slurry/Gel in 60 seconds:
- Take 10 regular-strength (500 mg to 1,000 mg) Calcium Carbonate antacid tablets (such as Tums or Rolaids).
- Crush them between two spoons or inside a clean plastic bag with a rolling pin into a fine powder.
- Mix the powder thoroughly with 30 milliliters (2 tablespoons / 1 ounce) of a water-soluble gel (such as K-Y Jelly, plain water-based aloe vera gel, or 1 to 2 tablespoons of tap water) to create a thick paste. (Never use petroleum jelly / Vaseline, because oil-based ointments trap fluoride ions and block water-soluble calcium ions from entering the skin).
- Use the "Nitrile Glove Poultice" technique for finger and hand burns:
- Put a nitrile glove on your own hand and massage the calcium gel into the burned area continuously.
- Iyo intoke canke ukuboko kw’uwakozweko icaha kwaturitse, uzuze igitambaro c’amaboko c’agaciro kanini cane c’ibara ry’agahama n’igikombe c’ubuntu c’i Calcium Gluconate Gel (canke gel yawe ya Tums yajanjaguwe) hanyuma ushire ukuboko kw’uwo muntu kwaturitse imbere muri ico gitambaro c’amaboko cuzuye gel. - Flush for a minimum of 30 to 60 continuous minutes for any alkali, cement, lime, or hydrofluoric acid splash (and at least 15 to 30 minutes for acids/bleach).
- Check beneath the upper and lower eyelids (fornices) for trapped lime or cement grit: In plaster, mortar, or dry lime splashes, tiny white chunks of calcium hydroxide lodge deep under the upper eyelid and continue dissolving the eye even after 30 minutes of flushing until a physician flips (everts) the upper eyelid and sweeps the fornices clean with a moistened cotton swab while checking the tear film pH with litmus paper until it stays neutral at pH 7.0 to 7.4.
Ibibazo bikunda kubazwa¶
I used a household liquid rust stain remover on some laundry without gloves two hours ago. My fingers looked completely normal when I finished, but now they are starting to throb with a deep, severe ache inside the bone. Why is this happening, and is it an emergency?
Yes: go to a hospital emergency department immediately and bring the rust remover bottle with you. Almost all commercial fabric rust stain removers contain 6% to 12% Hydrofluoric Acid (HF) or ammonium bifluoride. At concentrations under 20%, hydrofluoric acid causes zero pain and zero redness on contact, allowing the fluoride molecules to soak silently through your skin and under your fingernails over 2 to 12 hours. The deep, throbbing "bone pain" you feel right now is caused by fluoride ions stripping calcium out of your finger nerves and bone. While someone drives you to the ER (or while waiting for an ambulance), crush 10 regular Tums (calcium carbonate) tablets into 2 tablespoons of K-Y jelly or water, rub the paste generously over your fingers inside a nitrile glove, and tell the ER triage nurse the moment you walk in: "Hydrofluoric acid burn from rust remover: needs Calcium Gluconate and an ECG."
Why is it a dangerous mistake to pour vinegar onto a lye/drain-cleaner burn, or baking soda onto a battery-acid burn, to "neutralize" the pH?
In a glass beaker in chemistry class, mixing a strong acid and a strong base neutralizes the pH: but it also releases a massive burst of thermal heat (an exothermic neutralization reaction). When you pour vinegar onto concentrated sodium hydroxide lye (or baking soda paste onto concentrated sulfuric acid) sitting on raw human skin, the chemical reaction generates intense heat right on the wound surface, cooking the already-damaged dermis and converting a superficial chemical burn into a deep third-degree thermal-chemical burn. High-volume cool running water (for 30 to 60 minutes) safely dilutes and washes away both the chemical and any heat of dilution without cooking the tissue.
What is the difference between Calcium Gluconate and Calcium Chloride, and why must Calcium Chloride NEVER be injected under the skin for a Hydrofluoric Acid burn?
Both medications supply calcium ions ($Ca^{2+}$) intravenously through a central line during cardiac resuscitation, but Calcium Chloride ($CaCl_2$) contains three times more ionized calcium per milliliter and is fiercely hyperosmolar and caustic to soft tissues. If Calcium Chloride is infiltrated subcutaneously under the skin or leaks out of a peripheral IV vein (extravasation), it causes severe, full-thickness tissue sloughing and necrosis. Therefore, ONLY **Calcium Gluconate** is ever used for topical gel, subcutaneous infiltration, or intra-arterial infusion in hydrofluoric acid burns.
Why do you have to brush dry lime or dry cement powder off the skin BEFORE turning on the safety shower, instead of just jumping straight into the water?
Dry quicklime (calcium oxide) and dry sodium hydroxide lye crystals react with water in a fiercely exothermic hydration reaction ($CaO + H_2O \rightarrow Ca(OH)_2 + \text{Heat}$). If your arms and shirt are caked with thick dry lime powder and you splash a small amount of water on them, the wet powder turns into a boiling-hot, pH-13 caustic paste that sticks to your skin and burns you thermally and chemically at the same time. Taking 15 seconds to strip off the dusty shirt and brush off the loose dry powder with a dry towel or brush first, and THEN flooding the skin with a massive, high-volume deluge shower for 30 minutes, prevents that thermal heat spike.
This guide is for informational purposes and does not replace professional medical evaluation or hands-on training. Because severe chemical burns and hydrofluoric acid exposures can trigger sudden cardiac arrhythmias, we strongly recommend taking an accredited first aid and CPR course through the International Federation of Red Cross and Red Crescent Societies (IFRC), your national Red Cross society, St John Ambulance, or a certified local training provider.
