Windows Time Ischemia Mafana vs. Mangatsiaka: Nahoana no manova ny famantaranandro ny fampangatsiahana rantsantanana iray vs¶
Mandra-pahoviana ny ampahany amin'ny vatana tapaka no velona tsy misy rà mandriaka alohan'ny hahafatesan'ny selany (ischemic necrosis)? Ny valiny dia miankina amin'ny zava-misy biolojika lehibe iray: raha misy hozatry ny taolana ny ampahany amin'ny vatana.
- Ny hozatry ny taolana (izay mameno ny sandriny, ny sandriny ambony, ny feny, ny tongotra ambany, ary ny felatana) dia mitaky oksizenina metabolika be dia be. Amin'ny mari-pana ao amin'ny efitrano (ischemia mafana), ny hozatra skeletal tapaka dia manomboka maty tsy azo iverenana intsony ary mamoaka potasioma sy myoglobin misy poizina rehefa afaka 4 hatramin'ny 6 ora.
- Ny rantsantanana sy ny ankihibe (isa), mifanohitra amin'izany, misy hozatra taolana ZERO. Ny hozatra izay manetsika ny rantsan-tànanao dia mipetraka eo amin'ny sandrinao sy ny felatananao, misintona amin'ny rantsantanana amin'ny tady fotsy lava (tendons). Mechanisms of amputation: Why a Guillotine Cut is easier to replant than an Avulsion or Crush¶
Microsurgeons examine how the part was severed because the mechanism determines how many centimeters of damaged artery must be trimmed or replaced with a vein graft:
- Sharp ("Guillotine") Amputation: Caused by a chef's knife, sheet metal, guillotine paper cutter, glass pane, or machete. The skin, bone, tendons, nerves, and arteries are sliced cleanly in a single narrow plane with minimal bruising on either side. Carries the highest replantation success rate (85% to 92%).
- Local Crush Amputation: Caused by a heavy steel beam, log splitter, car door, or hydraulic press dropping onto a finger or hand. The blood vessels are crushed over a 1-to-3-centimeter (0.5-to-1.2-inch) zone, requiring surgical vessel trimming and shortening of the bone or interposition vein grafts.
- Avulsion (Pull-Off) or "Ring Avulsion" Amputation: Caused when a limb is caught in a rotating auger, rope, PTO shaft, or when a wedding ring catches on a ladder, fence, or truck bed as the person jumps down, stripping the skin, nerves, and long coiled ribbons of digital arteries out of the finger like pulling off a glove. Even though the bone may still be intact inside a "degloved" ring avulsion, the microscopic inner lining (tunica intima) of the arteries is stretched and torn several centimeters up into the palm (the "Red Ribbon Sign" on the side of the vessel), requiring complex microsurgical vein transfers.
Step-by-step technique: Caring for the Patient and the Stump¶
Step 1: Control bleeding on the Stump (Direct Pressure vs. Windlass Tourniquet)¶
- For Finger, Thumb, or Toe Amputations:
- Have the patient sit or lie down and elevate the injured hand or foot above heart level.
- Place a thick stack of sterile gauze pads directly over the bleeding stump and press firmly with your gloved fingers for 5 to 10 minutes without peeking.
- Once active spurting subsides, wrap a snug (not constricting) sterile roller bandage over the stump and hand.
- Why you must NEVER tie a rubber band, string, hair tie, or clamping forceps onto a bleeding finger stump: Every digital artery runs side-by-side with the digital nerve in a tiny neurovascular bundle less than 2 millimeters wide. Clamping a hemostat blindly into a finger stump or tying a tight rubber band around the base of a finger crushes the digital nerve and healthy artery segment that the microsurgeon needs to sew the finger back on, and can cause gangrene of the remaining finger stump. Direct pressure and elevation will control 99 percent of digital amputations.
- For Major Limb Amputations (Hand, Wrist, Forearm, Arm, Foot, Leg, Thigh):
- Immediately apply a commercial windlass tourniquet (CAT, SOFTT-W, or SAM XT) 5 to 7 centimeters (2 to 3 inches) proximal (above) the amputation wound directly onto bare skin if possible.
- Never place a tourniquet over the elbow or knee joint (where the two forearm or lower-leg bones shield the interosseous arteries from compression); if the amputation is just below the knee or elbow, place the tourniquet right above the knee or elbow on the single thigh or upper-arm bone.
- Tighten the windlass until all bright red bleeding stops completely, secure the rod in the clip, and write the exact time of application on the tourniquet label and on the patient's forehead.
- Cover the raw end of the stump with a sterile gauze pad moistened with sterile normal saline, topped with a dry bulky trauma dressing and wrapped snugly to protect the exposed bone and nerve ends from the air.
Step 2: Managing a Partial ("Near" or Incomplete) Amputation¶
When a saw, lawnmower, or crash leaves a finger, hand, or foot hanging by a bridge of skin, muscle, or tendon: 1. Never complete the amputation: Even a 1-centimeter (0.4-inch) strip of intact skin on the back of a finger often contains the dorsal veins and lymphatic vessels needed to drain blood from the finger after the surgeon repairs the palm-side artery. Cutting that skin bridge destroys the venous drainage and can doom the finger. 2. Gently untwist and align: If the partially severed hand, foot, or finger is twisted 90 or 180 degrees or folded backward, gently lift and support it back into its normal anatomical alignment so the blood vessels inside the skin bridge are unkinked. 3. Dress and splint: Cover the open laceration and exposed bone with sterile saline-moistened gauze, pad generously around the digit or limb with dry sterile dressings, and splint the entire hand/wrist or foot/ankle on a padded board or malleable splint so it cannot flop or twist during transport. 4. How do you cool a PARTIALLY amputated part that has no blood flow? If the partially attached hand or finger beyond the cut is completely white, cold, and pulseless (meaning the artery is severed and only skin/tendon remains attached), wrap the splinted part in a thick dry sterile dressing and towel first, and lay a cool pack outside the thick towel barrier next to the ischemic part so it stays gently cool without freezing either the part or the living skin bridge.
Step-by-step technique: The "Two-Bag Moist-Cold Method" for Severed Parts¶
Every person at the scene should know how to package a completely severed finger, thumb, hand, ear, nose, scalp, or tooth/bone fragment so it arrives at the hospital in peak biological condition:
Step 1: Locate and gently handle the severed part¶
- Assign a bystander to search the scene for ALL severed parts (if multiple fingers were cut by a saw, check inside work gloves, on the saw table, or in clothing folds: even a severely mangled finger that cannot be replanted itself is extraordinarily valuable to a microsurgeon as a "spare-parts bank" to harvest matching nerves, arteries, veins, skin, or bone grafts to save the patient's thumb or other fingers).
- Pick up the part with clean medical gloves. Never pinch or pull on delicate dangling blood vessels or nerve threads trailing from the cut end.
Step 2: Rinse gently ONLY if grossly contaminated (Never Scrub)¶
- If the part is clean, do not wash it.
- If it is covered in dirt, grass, or sawdust, hold it gently and pour a gentle stream of sterile 0.9% normal saline (or clean bottled drinking water if saline is unavailable) over it to float away loose debris.
- Never scrub, brush, or wipe the cut surface, and never touch the severed part with soap, rubbing alcohol, hydrogen peroxide, povidone-iodine (Betadine), chlorhexidine, or tap water baths. Antiseptics chemically cauterize and kill the delicate endothelial cells lining the microscopic arteries.
Step 3: Wrap in a Damp (Not Dripping) Sterile Gauze Layer¶
- Take a sterile gauze pad or clean lint-free cloth and moisten it with sterile normal saline (or clean potable water), then wring out the excess liquid so the gauze is lightly damp, NOT soaking wet.
- Wrap the damp gauze completely around the severed finger or limb. This damp layer prevents air desiccation while acting as an insulating cushion inside the bag.
Step 4: Seal Inside Watertight Bag #1 (The Dry Inner Barrier)¶
- Place the gauze-wrapped body part inside a clean, watertight zip-top plastic bag (or a sterile specimen cup/container for a fingertip, ear, or nose; or a clean heavy-duty plastic trash bag for a forearm or foot).
- Gently press out excess air and seal the bag completely watertight so no melting water from the outside can leak in and soak the tissue.
Step 5: Submerge Bag #1 into Bag #2 Containing a 50/50 Ice-and-Water Slush Bath¶
- Take a second larger plastic bag, bucket, or cooler and fill it with roughly equal parts regular crushed/cubed ice AND cold tap water (an ice-water slush bath).
- Place the sealed Bag #1 inside Bag #2 so it floats in the ice-water mixture.
The Physics of Why You Must Add Water to the Ice (4°C vs. -18°C)
Why do microsurgeons insist on an ice-plus-water slush bath rather than a bag of straight freezer ice cubes? * Ice cubes pulled fresh out of a home or commercial freezer are at -18 to -20 degrees Celsius (0 to -4 degrees Fahrenheit), well below the freezing point of human tissue (-0.5°C / 31°F). If you pack a plastic bag containing a thin finger directly against dry -18°C freezer ice cubes, the finger will freeze solid within 20 to 30 minutes, forming sharp intracellular ice crystals that shred the capillary walls (frostbite necrosis). Once a severed finger has been frozen, replantation is 100 percent impossible: if a surgeon sews a frozen-and-thawed finger back on, the dead vessels immediately clot off and the finger turns black. * By contrast, by the laws of thermodynamics, a mixture of melting ice floating in liquid water automatically stabilizes at 0 to 4 degrees Celsius (32 to 39 degrees Fahrenheit), and with the inner plastic bag and damp gauze wrap, holds the severed tissue at the exact gold-standard preservation temperature of 4 degrees Celsius (39.2 degrees Fahrenheit) without ever freezing. * Never use Dry Ice (solid carbon dioxide, -78.5°C / -109°F) and never use chemical instant-cold packs pressed directly against the inner bag without a thick cloth barrier, as chemical cold packs can drop below 0°C (32°F).
Step 6: Label and Transport Together¶
- Write the patient's full name, the body part, and the exact time of the amputation on a piece of tape on the outside of Bag #2.
- Keep the patient warm (wrapped in blankets to prevent systemic hypothermia and vasospasm), strictly NPO (nothing by mouth) so emergency anesthesia can begin immediately, and send the cooled severed part in the exact same ambulance with the patient.
Special amputation scenarios: Scalp, Ear, Nose, Pediatric Fingertips, and Why Nicotine/Caffeine Must Be Avoided¶
1. Scalp Avulsions, Severed Ears, Noses, and Lips¶
When long hair gets caught in industrial machinery or a dog bite or crash severs an ear, nose, or lip: * Scalp Avulsion: The scalp has one of the richest arterial networks in the body and bleeds profusely. Control scalp bleeding with firm direct pressure and a snug head wrap, and preserve the entire avulsed scalp (including the hair) using the exact same Two-Bag Moist-Cold Method. Microsurgeons can reconnect a single superficial temporal artery and vein to restore the entire hair-bearing scalp. * Ears, Noses, and Lips: Preserve every detached piece of cartilage and skin using the Two-Bag Moist-Cold Method. Even if microvascular anastomosis of a tiny ear vessel is not possible, plastic surgeons use the preserved cartilage framework for composite reconstruction.
2. Pediatric Fingertip Amputations (Car Doors and Hinges)¶
Young children frequently suffer fingertip amputations when a heavy door slams on their finger: * Children have extraordinary regenerative and microvascular healing capacity. Even a tiny fingertip cap severed beyond the last knuckle (distal phalanx) in a child can often be reattached as a composite graft or microsurgical replant if cooled properly in a damp gauze inside a sealed bag on an ice-water bath.
3. Absolute Prohibition on Smoking, Nicotine, Vaping, and Caffeine¶
In the prehospital setting and for weeks after replantation surgery, the patient must be strictly instructed: zero cigarettes, zero nicotine vapes or pouches, and zero caffeine: * Nicotine is one of the most potent peripheral arterial vasoconstrictors known. Taking even a single puff of a cigarette or vape triggers violent spasm of the repaired 0.8-millimeter digital arteries, dropping blood flow to the replanted finger by more than 80 percent and causing immediate thrombosis and death of the reattached part. * For the exact same reason, keep the patient's core body warm with blankets in the ambulance; shivering and cold exposure trigger sympathetic vasoconstriction in the hands and feet.
Calm, methodical hemorrhage control and proper two-bag cold preservation give microsurgeons the best possible chance to restore a person's hand or limb. Consider taking a certified first aid and Stop the Bleed course through your national Red Cross or Red Crescent society, the American College of Surgeons, or St John Ambulance.
Frequently asked questions¶
Why can't I put a severed finger directly into a bag of ice or a cup of water to keep it fresh?
Putting a severed finger directly onto ice or into water are the two most common mistakes that ruin any chance of reattaching the finger. Freezer ice sits at minus 18 degrees Celsius (0 degrees Fahrenheit); direct contact freezes the tiny blood vessels inside the finger, creating microscopic ice crystals that burst the cells and cause irreversible frostbite, and a frozen finger can never be sewn back on. On the other hand, submerging a finger directly into a cup of water or saline causes osmosis to swell, waterlog, and burst the exposed cells (maceration), making the delicate blood vessels turn mushy so they cannot hold surgical stitches. Always wrap the finger in lightly dampened sterile gauze, seal it inside a dry watertight plastic bag, and place that sealed bag into a second container of ice AND water.
Should I bring a severed finger to the hospital even if it looks badly crushed or too damaged to sew back on?
Yes, always bring every single severed piece of tissue, bone, or finger to the hospital, no matter how badly crushed or mangled it looks. Even if a severely crushed finger cannot be fully reattached in its original spot, hand microsurgeons routinely use non-replantable fingers as an invaluable 'spare-parts bank' during the operation: they harvest living segments of microscopic arteries, veins, nerves, tendons, bone grafts, or skin flaps from the damaged finger and transplant them to save and reconstruct the patient's thumb or other injured fingers.
How long after an accident can a severed finger, thumb, or hand still be sewn back on?
It depends on whether the severed part contains muscle and how fast it was cooled to 4 degrees Celsius (39 degrees Fahrenheit). Because fingers and thumbs contain no muscle tissue at all (only bone, tendons, nerves, blood vessels, and skin), they tolerate lack of blood flow much longer than an arm or leg: a severed finger or thumb can be successfully replanted up to 12 hours at room temperature, or up to 24 hours (and sometimes longer) if properly cooled using the two-bag ice-water method right after the accident. By contrast, a severed hand, forearm, or leg contains large amounts of muscle tissue that dies quickly without oxygen; a major limb amputation must be cooled immediately and reach a microsurgical operating room within 4 to 6 hours (or up to 10 to 12 hours if cooled rapidly).
Why shouldn't I wrap a tight rubber band or string around the base of a bleeding finger stump?
Never put a rubber band, string, wire, or shoelace tourniquet around a finger. Almost all bleeding from a severed finger or thumb can be stopped safely by elevating the hand above the heart and pressing firmly on the stump with a thick pad of sterile gauze for 5 to 10 minutes. Tying a narrow rubber band around the base of a finger often tightens just enough to pinch off the low-pressure veins while still letting high-pressure arterial blood pump in, which actually makes the finger bleed MORE, while simultaneously crushing the delicate digital nerves and blood vessels right where the surgeon needs to sew the finger back on. Rubber bands also frequently get hidden under bloody bandages and forgotten, causing gangrene and loss of the rest of the finger.
