  2. Call emergency medical services or go to an emergency department immediately: Call your local emergency number (911 in the United States and Canada, 112 across the European Union, 999 in the United Kingdom, 000 in Australia) or check our directory of noodnummers if the person passes dark brown, rust-colored, or tea-colored urine, cannot straighten or bend their swollen arms or legs, feels dizzy or confused, has chest palpitations, or stops urinating.
  3. Cool the body if exertion occurred in heat: If the person has been exercising or working in warm weather and feels hot, strip off excess gear or heavy athletic uniforms, mist their bare skin with cool water while fanning them, and apply cloth-wrapped ice packs to the neck, armpits, and groin. Do not pack ice directly onto swollen, cramping muscle bellies, which worsens tissue ischemia.
  4. Give plain water by mouth if awake, alert, and not nauseated: If emergency transport is on the way or you are traveling to a hospital and the person is fully conscious with no nausea or vomiting, have them drink 500 to 1,000 milliliters (16 to 34 fluid ounces) of plain cool water or a low-potassium oral rehydration solution over the first hour to keep urine flowing through the kidneys.

Roodbruine, donkerbruine, roestkleurige of ‘cola-/theekleurige’ urine (myoglobinurie), of aanzienlijk verminderde/afwezige urineproductie (oligurie/anurie) ondanks drinkwater.

Of het nu wordt veroorzaakt door extreem herhaalde excentrische contracties, inspanningshitteberoerte, verpletteringsverwonding, prolonged immobilization on a floor, or toxic drug reactions, the muscle cell exhausts its store of adenosine triphosphate (ATP). Without ATP, the microscopic ion pumps in the muscle cell wall (the sodium-potassium-ATPase and calcium-ATPase pumps) fail. Sodium and calcium flood into the muscle cell, drawing vast amounts of water out of the bloodstream into the muscle tissue and activating calcium-dependent destructive enzymes (proteases and phospholipases) that shred the muscle cell membrane (sarcolemma).

1. The immediate 1-to-12-hour threat: Hyperkalemia and cardiac arrest

Roughly 98 percent of all potassium in the human body is stored safely inside skeletal muscle cells. When hundreds of grams of muscle fibers rupture simultaneously, they dump a massive wave of intracellular potassium directly into the bloodstream (acute hyperkalemia), while ruptured cellular proteins bind up circulating blood calcium (acute hypocalcemia).

High blood potassium combined with low blood calcium destabilizes the electrical conduction system of the heart within hours, progressing from tall "peaked T-waves" on an electrocardiogram (ECG) to widened QRS complexes, ventricular fibrillation, and sudden cardiac arrest. This is why patients with suspected rhabdomyolysis must never consume potassium-containing fluids (like coconut water or orange juice) and need an immediate hospital ECG and blood potassium test.

2. The 6-to-48-hour threat: Third-spacing hypovolemia and compartment syndrome

Damaged muscle tissue acts like a giant dry sponge: up to 4 to 10 liters (1 to 2.6 gallons) of intravascular fluid can be pulled out of the circulating bloodstream and trapped inside swollen thigh, buttock, back, or arm muscles within 24 hours (third-spacing). This massive internal fluid shift leaves the bloodstream severely depleted (hypovolemic shock), starving the kidneys of blood flow just when they need high flow most.

At the same time, because skeletal muscles are wrapped inside tough, inelastic envelopes of connective tissue called fascia, rapid fluid swelling inside a muscle compartment spikes internal tissue pressure until it crushes the microscopic capillaries and nerves inside the limb, triggering acuut compartimentsyndroom.

3. The 12-to-72-hour threat: Myoglobinuric acute kidney injury (AKI)

Myoglobin is an iron-containing oxygen-storage protein inside muscle cells. Once released into the bloodstream, myoglobin filters freely through the kidney's glomeruli into the microscopic renal tubules. If the patient is dehydrated and their urine is concentrated and acidic (low pH), three destructive processes destroy the kidneys:

  1. Tubular cast obstruction: Myoglobin binds with a normal urinary protein called Tamm-Horsfall protein (uromodulin) to form thick, insoluble reddish-brown sludge plugs (pigmented casts) that physically cork the microscopic kidney tubules shut like concrete in plumbing pipes.
  2. Direct iron free-radical toxicity: In acidic urine, the heme iron ring inside myoglobin breaks down and releases free iron ions that generate toxic hydroxyl free radicals, burning and killing the tubular epithelial cells (acute tubular necrosis).
  3. Renal vasoconstriction: Myoglobin scavenges nitric oxide inside the kidney's blood vessels, causing the renal arteries to clamp down and starve the kidney cortex of oxygen.

Common non-workout causes of rhabdomyolysis you must not miss

While athletes and fitness newcomers account for many exertional cases, over half of hospital admissions for rhabdomyolysis involve non-athletic triggers:

  • The "Long Lie" after an older adult falls: When an elderly person slips in the bathroom or bedroom, cannot get up on their own, and lies immobilized in one position on a hard floor for more than 1 to 2 hours, the sheer weight of their own body compresses the gluteal (buttock), thigh, back, and shoulder muscles, cutting off blood flow and causing massive pressure-induced rhabdomyolysis even if no bone was broken. Every person found on the floor after a prolonged fall must be evaluated in an emergency department for rhabdomyolysis and acute kidney injury.
  • Medications and statin interactions: Cholesterol-lowering statin medications (atorvastatin, simvastatin, rosuvastatin), especially when combined with fibrates (gemfibrozil) or certain antibiotics (clarithromycin), antifungals, or large amounts of grapefruit juice, can cause severe statin-induced myopathy and rhabdomyolysis.
  • Drug-induced hyperthermia and prolonged seizures: Serotonin syndrome and neuroleptic malignant syndrome, malignant hyperthermia, status epilepticus (continuous seizures), severe alcohol withdrawal (delirium tremens), and stimulants (cocaine, methamphetamine, MDMA).
  • High-voltage electrical shock and lightning strikes: High-voltage current traveling through a limb cooks deep muscle bellies along the bone while leaving minimal outer skin burns (elektrische schok).
  • Venomous bites and stings: Certain pit viper and elapid snakebites (such as sea snakes, Russell's vipers, and rattlesnakes) and mass hornet or bee stings inject myotoxins (phospholipase A2) that directly dissolve skeletal muscle fibers.
  • Sickle cell trait (SCT) exertional collapse: Athletes or military recruits with sickle cell trait who perform maximal all-out sprints in hot weather or at high altitude can experience hyperacute exertional sickling and fulminant rhabdomyolysis within 5 to 15 minutes (sickle cell crisis).

How to inspect urine color and distinguish dark urine causes

When someone passes red, pink, orange, or dark brown urine, checking the color, clarity, and recent diet or medication list helps narrow the cause while arranging medical evaluation:

Urine Appearance Meest waarschijnlijke oorzaak Belangrijke bijbehorende aanwijzingen
Bright red or smoky pink, often cloudy or containing visible stringy red blood clots Gross Hematuria (Whole red blood cells in the urine) from a kidney stone, bladder infection, kidney trauma, or urinary tract tumor. Usually accompanied by sharp flank pain radiating to the groin (kidney stone) or burning pain when urinating (UTI); blood clots never form in rhabdomyolysis because myoglobin is a dissolved protein, not whole blood.
Dark yellow to amber-honey colored, turning pale yellow after drinking 1 liter of water Simple Dehydration (Concentrated urobilin) Thirst and dry mouth, but no disproportionateset muscle swelling, no joint locking, and urine rapidly lightens once rehydrated.
Harmless bright orange, neon yellow-orange, or pinkish-red without muscle pain Food or Medication Dye: Eating beets (beeturia), blackberries, or rhubarb; or taking phenazopyridine (UTI pain reliever), rifampin (antibiotic), senna (laxative), or high-dose B-vitamins. Completely painless, normal muscle strength, and clear dietary or medication history within the past 12 hours.

How emergency departments confirm rhabdomyolysis in 5 minutes

On a standard chemical urine dipstick, the test pad for "Blood" reacts identically to both whole red blood cells (hemoglobin) and dissolved muscle protein (myoglobin). When emergency physicians see a urine dipstick that reads "Large Blood (3+)" on the chemical pad, but microscopic inspection of the spun urine reveals zero or almost no actual red blood cells (0 to 2 RBCs per high-power field), that mismatch ("dipstick-positive, microscopy-negative for blood") is pathognomonic for myoglobinuria from rhabdomyolysis, confirmed moments later by a blood creatine kinase (CK) draw.

Step-by-step prehospital first aid and acute kidney injury (AKI) prevention

If you or someone with you develops signs of exertional rhabdomyolysis or acute kidney injury risk, follow these evidence-based field protocols while transferring to medical care:

Step 1: Immediate rest, thermal unloading, and compartment protection

  1. Halt all muscular work: Have the person lie down comfortably. Do not allow them to "walk it off," stretch forcefully, or use percussive massage guns or foam rollers on suspected rhabdomyolysis muscles. Deep tissue massage or percussive therapy on acutely necrotic muscle physically crushes fragile myocytes, releasing a fresh surge of myoglobin and potassium into the blood.
  2. Cool the body core, but never ice the muscle compartment directly: If core temperature is elevated, cool the whole body with cool water mist and fans, and place cloth-wrapped ice packs at the neck, armpits, and groin. Never strap ice packs tightly onto a swollen, rock-hard calf, thigh, or forearm, because extreme local cold constricts already-compromised capillary blood flow and accelerates compartment syndrome.
  3. Check the "6 Ps" of acute compartment syndrome every 15 minutes: Inspect the swollen arms or legs for:
  4. 1. Pain out of proportion (especially severe pain when you gently stretch the fingers or toes passively).
  5. 2. Pressure / Tightness (the muscle belly feels rock-hard like a bowling ball or wood).
  6. 3. Paresthesia (pins-and-needles tingling or numbness in the fingers, hand, toes, or foot).
  7. 4. Paresis (weakness or paralysis when trying to wiggle the fingers or toes).
  8. 5. Pallor (pale or mottled skin distal to the swelling).
  9. 6. Pulselessness (a very late sign; compartment syndrome destroys muscle long before the wrist or ankle pulse disappears).
  10. If any of these appear, remove all watches, rings, compression sleeves, athletic tape, and tight pants immediately, and keep the limb flat at the level of the heart (neither elevated high nor dangling down).

Step 2: Safe oral rehydration protocol before intravenous access

In the hospital, physicians treat rhabdomyolysis by infusing large volumes of potassium-free intravenous isotonic saline (1.0 to 1.5 liters per hour initially) titrated to produce a high urine output of 200 to 300 milliliters per hour (roughly 1 cup of clear urine every hour), flushing myoglobin casts out of the kidneys before they solidify.

While waiting for an ambulance or during transport from a remote trail or athletic field:

  • Who can drink by mouth: Only give oral fluids if the patient is wide awake, alert, speaking clearly, able to swallow normally, and not vomiting.
  • What and how much to drink: Have them sip 500 to 1,000 milliliters (16 to 34 fluid ounces / 2 to 4 cups) of plain cool water over the first hour.
  • If in a remote wilderness setting hours from a hospital: If you are hiking hours from a road and the person is sweating heavily, mixing half a teaspoon (2.5 grams) of plain table salt (sodium chloride) and half a teaspoon of baking soda (sodium bicarbonate) with 2 tablespoons of sugar into 1 liter (1 quart) of plain water provides sodium to expand blood volume and mild bicarbonate to help keep the urine alkaline (higher pH prevents myoglobin from binding to Tamm-Horsfall protein in the kidneys) without adding any potassium.
  • When to stop oral fluids (Fluid Overload / Anuria Warning): If the person has already drunk 1 liter of water but has passed zero urine over 4 to 6 hours (anuria), or if they develop puffiness around the eyes, swollen ankles, or shortness of breath when lying flat, stop giving large amounts of water by mouth. Once the kidneys have completely shut down (anuric acute kidney injury), excess fluid cannot be excreted and backs up into the lungs (pulmonary edema), requiring hospital dialysis rather than oral water.

Step 3: Strict medication and beverage nephrotoxin blackout

When the kidneys are filtering myoglobin or recovering from acute kidney injury, certain everyday pills and beverages act as direct kidney poisons (nephrotoxins) or cardiac toxins:

Veelgestelde vragen

How can I tell the difference between normal post-workout muscle soreness (DOMS) and rhabdomyolysis?

Normal delayed-onset muscle soreness (DOMS) peaks at 24 to 48 hours, feels tolerable at rest, and loosens up with gentle movement. Exertional rhabdomyolysis causes severe, throbbing pain that worsens relentlessly past 48 hours, dramatic swelling that makes the muscle feel tight or wooden, inability to fully straighten or bend the joint (such as "T-Rex arms" locked at 90 degrees after pull-ups), profound weakness, and often dark reddish-brown or "cola-colored" urine.

Why is taking ibuprofen (Advil) or naproxen (Aleve) dangerous when you have severe muscle breakdown?

Nonsteroidal anti-inflammatory drugs (NSAIDs) like ibuprofen and naproxen block protective prostaglandins that keep the blood vessels feeding your kidneys open. When your kidneys are already struggling to flush out toxic myoglobin protein from ruptured muscle cells, taking an NSAID clamps down renal blood flow and rapidly triggers acute kidney failure.

Why must someone with suspected rhabdomyolysis avoid coconut water, orange juice, and high-potassium sports drinks?

When skeletal muscle cells rupture during rhabdomyolysis, they release massive amounts of intracellular potassium directly into the bloodstream (hyperkalemia), which can disrupt the heart's electrical rhythm and cause sudden cardiac arrest. Drinking potassium-rich fluids like coconut water or fruit juice adds even more potassium to the blood; stick strictly to plain water until hospital blood tests check potassium levels.

Can you have serious rhabdomyolysis even if your urine looks clear or normal yellow?

Yes, visible dark brown or tea-colored urine occurs in only 30 to 50 percent of patients with clinically confirmed rhabdomyolysis because myoglobin is cleared from the blood faster than creatine kinase and requires high urinary concentrations to change urine color visibly. Never rule out rhabdomyolysis based on clear urine alone if severe muscle swelling, weakness, or inability to extend a limb is present.

Why does lying on the floor for several hours after a fall cause rhabdomyolysis in older adults?

When a person falls and remains immobilized in one position on a hard floor for more than 1 to 2 hours (a "long lie"), the continuous pressure of their own body weight crushes the capillaries feeding their buttock, thigh, and back muscles. Starved of oxygen, those compressed muscle fibers break down and release myoglobin and potassium into the bloodstream as soon as the person is lifted up, requiring immediate hospital intravenous fluids to protect their kidneys.
