Sharp, stabbing center-left chest pain that worsens dramatically when lying flat on your back or taking a deep breath and feels noticeably better when sitting up and leaning forward, especially days or weeks after a viral respiratory infection, flu, stomach bug (gastroenteritis), or mRNA/viral immunization, is the classic hallmark of acute pericarditis (inflammation of the two-layered sac surrounding the heart) or myopericarditis (combined inflammation of the heart sac and the heart muscle itself). While uncomplicated viral pericarditis often responds rapidly to targeted anti-inflammatory therapy, isolated acute myocarditis (inflammation of the pumping heart muscle wall, the myocardium) is one of the leading causes of sudden cardiac arrest in young adults, children, and athletes, capable of triggering lethal ventricular arrhythmias (ventricular tachycardia or ventricular fibrillation), complete heart block, or rapid cardiogenic shock (fulminant myocarditis). in addition, fluid accumulating rapidly inside an inflamed pericardial sac can compress the heart chambers shut (හෘද tamponade). Immediate emergency evaluation (12-lead ECG, high-sensitivity cardiac troponin blood testing, and echocardiography), proper seated-forward positioning, avoiding aspirin/NSAIDs if myocarditis is suspected before a doctor's exam, and strict 100 percent physical exercise restriction save lives.
Any acute chest pain requires immediate emergency medical evaluation to rule out a heart attack, myocarditis, or tamponade
Never try to self-diagnose chest pain at home as "just benign pericarditis" without an emergency room 12-lead electrocardiogram (ECG), cardiac troponin blood test, and echocardiogram. Call your local emergency number (911 in the United States and Canada, 112 across the European Union, 999 in the United Kingdom) immediately if chest pain is accompanied by shortness of breath, dizziness, lightheadedness, or fainting (syncope), a racing, fluttering, or skipping heartbeat (palpitations), pale/sweaty skin, or pain radiating to the jaw, neck, back, or arms.
දැන් මේක කරන්න¶
- Stop all physical exertion immediately and seat the person upright, leaning slightly forward (\(30\text{ to }45\text{ degrees}\) forward over a pillow on a table or their knees).
- Why leaning forward relieves pericardial pain: When a person lies flat on their back (supine), gravity pulls the heart backward and stretches the inflamed parietal pericardium against the diaphragm and pleura, causing intense stabbing pain with every heartbeat and breath. Sitting upright and leaning forward (the tripod/pillow-hug position) pulls the heart slightly forward away from the posterior lung pleura and posterior mediastinum, reducing mechanical friction immediately.
- Why zero physical exertion is life-saving: If the heart muscle itself is inflamed (myocarditis), adrenaline (catecholamines) released by walking up stairs, carrying groceries, or exercising triggers lethal electrical short-circuits (ventricular fibrillation) in inflamed heart muscle fibers.
- Perform the 60-Second "Myocarditis & Cardiac Tamponade Red Flag Screen" and call 911 / 112 / 999 immediately:
- Dial 911 in the United States and Canada, 112 across the European Union, 999 in the United Kingdom, or see all හදිසි අංක.
- Myocarditis Red Flags (Heart Muscle Inflammation): Chest pain or tightness paired with unexplained shortness of breath at rest or when walking across a room, inability to lie flat without gasping for air (orthopnea), fast resting heart rate out of proportion to fever (sinus tachycardia \(>100\text{ to }120\text{ bpm}\)), palpitations/skipping beats, dizziness/fainting, or extreme flu-like exhaustion, vomiting, and abdominal pain (especially in children).
- Cardiac Tamponade Red Flags (Fluid Crushing the Heart): Rapidly worsening shortness of breath, bulging neck veins (jugular venous distension) while sitting upright, lightheadedness when breathing in deeply (pulsus paradoxus), and a weak, rapid pulse.
- Do NOT blindly take over-the-counter Ibuprofen, Naproxen, or Aspirin BEFORE an emergency room troponin blood test determines whether you have pure Pericarditis or Myocarditis.
- Critical Medical Distinction: While high-dose NSAIDs (like ibuprofen or aspirin) plus colchicine are the first-line hospital treatment for pure uncomplicated pericarditis (when the heart muscle troponin test is 100% normal), NSAIDs are contraindicated or restricted in acute viral MYOCARDITIS (when the heart muscle is inflamed and troponin is elevated). Cardiology studies (European Society of Cardiology [ESC] and American Heart Association [AHA]) show that taking NSAIDs during active viral myocarditis impairs myocardial healing, increases viral replication in heart muscle, and worsens heart failure. Wait until emergency physicians run a cardiac troponin test (or give a single \(325\text{ mg}\) chewed aspirin only if an acute coronary හෘදයාබාධය is strongly suspected by emergency dispatchers in an older adult).
- Check for Recent Viral, Autoimmune, or Medication Triggers (1 to 4 Weeks Prior) and write them down for the ER physician:
- Did the patient have a common cold, sore throat, COVID-19, influenza, hand-foot-and-mouth disease (Coxsackievirus B), or viral stomach flu (diarrhea/vomiting) in the past 1 to 4 weeks?
- Did the patient recently receive an immunization (especially within the past 1 to 7 days in adolescent or young adult males) or start a cancer immunotherapy (immune checkpoint inhibitor like pembrolizumab or nivolumab) or psychiatric medication (clozapine)?
- Does the patient have kidney failure (uremia), lupus, rheumatoid arthritis, or recent heart surgery/heart attack (Dressler syndrome)?
- Keep an Automated External Defibrillator (AED) nearby if available and prepare for CPR if the patient feels faint or has palpitations. Because acute myocarditis can cause sudden ventricular fibrillation without warning, never let a patient with suspected myocarditis drive themselves to the hospital or walk unassisted to the car.

- Why pain radiates specifically to the top ridge of the left shoulder ( the Trapezius Ridge): The central pericardium and diaphragm are innervated by the Phrenic Nerve, which originates from spinal nerve roots C3, C4, and C5 in the neck. Because the supraclavicular nerves that supply skin sensation over the top ridge of the shoulder (trapezius muscle) also come from C3 and C4, the brain interprets pericardial pain as sharp aching right along the trapezius ridge. (Clinical pearl: Heart attack pain radiates down the inner left arm/biceps [T1-T4] or into the jaw, whereas pain radiating to the top trapezius ridge of the shoulder is highly specific for pericarditis.)
- The Myocardium (The Thick Muscular Pumping Wall, Affected in Myocarditis):
- Beneath the thin pericardial sac lies the thick muscular wall of the left and right ventricles (the myocardium), which also houses the heart's electrical wiring system (the His-Purkinje conduction fibers).
- Interestingly, the heart muscle itself has almost no "sharp/positional" pain receptors, when Myocarditis occurs (from direct viral invasion by Coxsackievirus B, Adenovirus, Parvovirus B19, Influenza, SARS-CoV-2, or an autoimmune reaction), immune T-cells attack the muscle fibers, causing myocardial edema (swelling) and patchy cell necrosis.
- This produces dull, heavy, heart-attack-like chest tightness (or no chest pain at all.), leaks cardiac troponin protein into the bloodstream, weakens the heart's pumping squeeze (acute systolic heart failure / cardiogenic shock), and creates irritable electrical scars that trigger sudden ventricular tachycardia, ventricular fibrillation, or complete AV heart block.
- Myopericarditis and Perimyocarditis (When Both Layers Are Inflamed):
- Because the inner visceral pericardium sits directly on top of the outer heart muscle, inflammation frequently spills across the border:
- Myopericarditis: Primarily pericarditis (sharp positional pain) with mild spillover inflammation into the superficial heart muscle (causing a positive cardiac troponin test, bust normal heart pumping function on echocardiogram).
- Perimyocarditis: Primarily dangerous myocarditis (troponin elevation plus weakened heart pumping / wall motion abnormalities on echocardiogram) accompanied by pericardial pain.
Comprehensive comparison: Pericarditis vs. Myocarditis vs. Heart Attack (STEMI) vs. Pulmonary Embolism¶
Because all four conditions bring patients to the emergency department with acute chest pain, use this clinical comparison table to understand how physicians tell them apart:
| සායනික ලක්ෂණය | Acute Pericarditis | Acute Myocarditis | Acute Heart Attack (Myocardial Infarction / STEMI) | Acute Pulmonary Embolism (PE) |
|---|---|---|---|---|
| Pain Character | Sharp, stabbing, knife-like, and pleuritic (hurts sharply when taking a deep breath, coughing, or swallowing). | Varies widely: Can be dull heavy chest pressure mimicking a heart attack, sharp positional pain (if myopericarditis), or completely painless (presenting only with shortness of breath or fainting). | Heavy, crushing, squeezing pressure or tightness ("elephant sitting on chest"); not sharp or positional. | Sudden-onset sharp pleuritic chest pain (usually on one side of the chest) paired with sudden shortness of breath. |
| Positional Effect (The Leaning-Forward Sign) | HALLMARK SIGN: Significantly worse when lying flat on the back (supine); markedly relieved by sitting upright and leaning forward. | Minimal positional relief unless the outer pericardium is also inflamed. Lying flat may cause orthopnea (breathlessness from fluid backing up into lungs). | No change when leaning forward or lying flat. | Worse with deep inspiration; not relieved by leaning forward (unlike pericarditis). |
| Classic Radiation Pattern | Radiates specifically to the left trapezius ridge (top ridge of the shoulder/neck via the C3-C5 phrenic nerve). | May radiate to left shoulder/arm or remain diffuse across the chest. |

Special Pericarditis Variants: Uremic, Purulent, and Dressler Syndrome¶
- Post-Cardiac Injury Syndrome (Dressler Syndrome): Occurs 1 to 6 weeks after a heart attack (myocardial infarction), open-heart surgery, pacemaker lead placement, or cardiac ablation, when the immune system mounts a delayed autoimmune attack against pericardial proteins released during the earlier injury. Presents with recurrent pleuritic chest pain, fever, and pleural/pericardial effusions.
- Uremic Pericarditis (Kidney Failure Emergency): In patients with advanced chronic kidney disease or missed dialysis sessions, accumulated nitrogenous toxins inflame the pericardium, causing a friable, bleeding "bread-and-butter" exudate. (Note: Uremic pericarditis frequently lacks classic ST elevation on ECG and is an indication for urgent hemodialysis without heparin to prevent hemorrhagic tamponade.)
- Purulent (Bacterial) Pericarditis: High spiking fevers (\(>39^\circ\text{C}\) [\(102.2^\circ\text{F}\)]), rigors, and toxic appearance in a patient with pneumonia, empyema, chest surgery, or sepsis requires immediate emergency surgical pericardial drainage and intravenous antibiotics.
Hospital Treatment & Why the "3 to 6 Month Sports Ban" Saves Lives¶
1. How Hospitals Treat Acute Uncomplicated Pericarditis (ESC Guidelines)¶
Once emergency physicians confirm pure acute pericarditis (normal troponin, no myocarditis, no tamponade, and no bacterial/tuberculous cause): - Dual Anti-Inflammatory Therapy: High-dose Ibuprofen (\(600\text{ to }800\text{ mg}\) every 8 hours) or Aspirin (\(750\text{ to }1,000\text{ mg}\) every 8 hours) paired with a stomach-protecting proton pump inhibitor, PLUS low-dose Colchicine (\(0.5\text{ mg}\) once or twice daily for 3 full months). (Why Colchicine is important: Landmark clinical trials [COPE and ICAP trials] proved that adding 3 months of colchicine cuts the risk of painful recurrent pericarditis in half, from 37% down to 16%.) - Why Oral Corticosteroids (Prednisone) Are Avoided First-Line in Viral Pericarditis: Giving oral prednisone for simple viral pericarditis makes pain vanish in 24 hours, but causes a massive rebound recurrence rate once tapered because steroids promote viral persistence. Corticosteroids are reserved strictly for autoimmune pericarditis, renal failure, pregnancy, or refractory cases.
2. Why Athletes and Active Adults MUST Obey the Strict Exercise Ban¶
Whether you are a competitive athlete, a teenager in school sports, a military recruit, or a casual jogger/gym-goer, international cardiology guidelines (American Heart Association / American College of Cardiology [AHA/ACC] and European Society of Cardiology [ESC]) enforce a strict physical activity ban after pericarditis or myocarditis: - After Pure Acute Pericarditis: Complete restriction from competitive sports and strenuous exercise (keep heart rate below \(100\text{ bpm}\) / light walking only) until all symptoms have resolved AND blood inflammatory markers (CRP/ESR), 12-lead ECG, and echocardiogram have returned to 100 percent normal (minimum 30 days to 3 months). - After Acute Myocarditis (or Myopericarditis with Elevated Troponin): Complete 100 percent ban on all competitive sports, running, heavy lifting, and moderate-to-vigorous exercise for 3 to 6 FULL MONTHS. - Why exercising during the recovery window can be fatal: Even after chest pain disappears in 3 days, microscopic patches of inflammatory edema and healing scar tissue remain inside the heart muscle for 90 to 180 days. A surge of exercise adrenaline can trigger fatal ventricular fibrillation (sudden cardiac arrest) on a soccer field or treadmill. Before returning to exercise at 3 to 6 months, a cardiologist must verify normal troponin, normal echocardiogram/cardiac MRI, a 24-to-48-hour Holter monitor showing zero dangerous arrhythmias, and a normal maximal exercise stress test.

Use these clear clinical scripts so emergency dispatchers and hospital triage nurses immediately perform a 12-lead ECG and high-sensitivity troponin test:
- For Positional Chest Pain (Suspected Pericarditis / Myopericarditis):
"I am at emergency triage with a [Age]-year-old patient who has acute sharp center-left chest pain that worsens significantly when lying flat or breathing deeply and improves when sitting up and leaning forward [plus pain radiating to the left trapezius shoulder ridge]. The patient had a [viral illness / immunization] [Number] days ago. We have NOT given any ibuprofen or NSAIDs yet so you can check a stat 12-lead ECG, high-sensitivity cardiac troponin, and bedside echocardiogram to rule out acute myocarditis and pericardial effusion."
- For Suspected Acute Myocarditis (Post-Viral Shortness of Breath, Tachycardia, or Syncope):
"I need an emergency ambulance at [Exact Address]. The patient is a [Age]-year-old recovering from a recent viral infection who now has [chest tightness / severe shortness of breath when lying down / unexplained resting heart rate of ___ bpm / palpitations / fainting]. We suspect Acute Viral Myocarditis and need immediate cardiac monitor transport and emergency department resuscitation."

නිතර අසන ප්රශ්න¶
Why does acute pericarditis hurt so much worse when lying flat on your back, and why does leaning forward make it feel better?¶
When you lie flat on your back, gravity pulls the heart backward against the spine and stretches the inflamed outer pericardial sac against the diaphragm and lung lining (pleura), while increased venous blood returning from your legs expands the heart chambers against the raw sac with every beat. Sitting upright and leaning forward at a 30-to-45-degree angle shifts the heart forward away from the posterior lung pleura and reduces venous stretch, immediately easing the sharp rubbing pain.
Why shouldn't I take Ibuprofen (Advil/Motrin) right away at home if I think I have post-viral pericarditis?¶
From symptoms alone, it is impossible to tell whether inflammation is limited strictly to the outer sac (pure pericarditis) or has also invaded the heart muscle itself (myopericarditis or myocarditis). While ibuprofen and colchicine are the standard hospital treatment for pure pericarditis, taking NSAIDs like ibuprofen when the heart muscle is actively inflamed (myocarditis) has been shown in cardiology studies to impair heart muscle healing, increase viral damage, and worsen heart failure; always wait until an emergency room blood test confirms your cardiac troponin level is normal.
How does pain radiating to the top ridge of the left shoulder (trapezius ridge) point specifically to pericarditis instead of a heart attack?¶
The pericardial sac and central diaphragm are wired by the phrenic nerve, which plugs into the spinal cord at neck levels C3, C4, and C5, the exact same spinal segments that receive sensory nerves from the skin over the top ridge of your shoulder (the trapezius muscle). Because the brain confuses signals arriving at C3-C5, pericarditis sends sharp referred pain right along the top ridge of the shoulder, whereas a heart attack sends pain via thoracic nerves (T1-T4) down the inner side of the arm or to the jaw.
Why do cardiologists ban all sports and workouts for 3 to 6 months after myocarditis even when you feel 100 percent back to normal?¶
Even if your chest pain and fatigue disappear within a week of having myocarditis, microscopic patches of swelling and healing scar tissue inside the heart muscle remain electrically unstable for 3 to 6 months. If you run, lift weights, or play competitive sports during this healing window, the surge of adrenaline and heart-wall stretch can trigger a sudden lethal short-circuit called ventricular fibrillation (sudden cardiac arrest); waiting until a cardiologist clears you with a Holter monitor and stress test protects your life.
