Two fatal mistakes in hunting trauma: pulling out an arrow and laying a suspended victim flat too fast or sitting a bleeding victim up
- Never pull out an embedded broadhead arrow or hunting knife in the field: Unlike a smooth target field point, a hunting broadhead carries 2 to 4 surgical-steel razor blades inside the tissue. Pulling an arrow backward slices major arteries and veins that the shaft was actively plugging (tamponading), causing immediate, uncontrollable internal exsanguination. Leave it in place and stabilize it with bulky padding.
- Prevent orthostatic suspension trauma immediately: If you fall from a tree stand and are hanging in your full-body harness, you must deploy your suspension relief strap and pump your legs continuously against the tree trunk; hanging with relaxed, dangling legs allows tight leg straps to trap up to 60 percent of your blood volume in your lower limbs, leading to unconsciousness and cardiac arrest in under 30 minutes.
- Make sure scene safety and unload all firearms or crossbows first: Before touching an injured hunter or climbing down to help, engage the safety, point the muzzle in a safe direction, completely unload the chamber and magazine of every firearm or uncock any crossbow at the scene and place all weapons and open broadhead quivers at least 3 meters (10 feet) away so a panicked or spasming patient cannot trigger a second discharge.
- If hanging in a tree stand harness, deploy your suspension relief loop immediately: Pull the suspension relief strap (foot loop) from your harness pocket, clip or loop it at knee height, step into the loop, and stand up or pump your calf and thigh muscles every 5 to 10 seconds to drive venous blood back to your heart while you either climb back onto the platform, cut yourself down onto a ladder rung, or blow your rescue whistle and call for help.
- Stop massive arterial bleeding within 60 seconds (M in MARCH):
- Limb gunshot or broadhead wound (thigh, calf, arm): Apply a CoTCCC windlass tourniquet (CAT or SOFTT-W) 5 to 8 centimeters (2 to 3 inches) above the wound (or "high and tight" over hunting clothes if the exact wound site is hidden), crank the windlass until all bright red bleeding stops, and write the time on the strap; see سخت خونريزي ۽ tourniquets.
- Neck, armpit, or groin (junctional) wound: Immediately pack the wound cavity tightly with z-fold hemostatic gauze (or clean cotton cloth) pushed straight down onto the bleeding artery with your fingers, then hold firm two-handed direct pressure for 3 to 10 minutes.
- Stabilize impaled broadhead arrows or branches in place: Never remove an impaled arrow, bolt, or tree branch. If an arrow shaft protruding from a thigh, arm, or chest is too long to evacuate safely without snagging brush and levering the razor blades inside the body, carefully stabilize the shaft with two hands and use heavy wire cutters or a multi-tool saw to shorten the carbon/aluminum shaft 10 to 15 centimeters (4 to 6 inches) outside the skin without vibrating the broadhead, then tape bulky foam pads or rolled socks around the base.
- Seal open chest wounds immediately (A and R in MARCH): If a gunshot, arrow, or broken rib from a tree stand fall creates an open chest wound (or both an entry and exit wound on the torso), wipe away blood and apply a vented chest seal (or a 3-sided taped plastic square) over both the entry and exit wounds on exhalation to prevent a fatal tension pneumothorax. See tension pneumothorax and sucking chest wound first aid.
- Protect the spine, prevent hypothermia, and transmit exact GPS coordinates: Keep any hunter who fell from a tree stand flat and still on an insulated foam pad wrapped in dry jackets and a mylar blanket. Call your local emergency number (911 in the United States and Canada, 112 across the European Union, 999 in the United Kingdom; see all ايمرجنسي نمبر) or trigger your satellite SOS beacon, and read your exact decimal-degree GPS coordinates from your phone compass/mapping app (OnX Hunt, Garmin, Apple/Google Maps) to dispatch.

Mechanism Comparison Matrix: Tree Stand Falls vs. Ballistic vs. Broadhead Trauma¶
| Trauma Mechanism | Primary Pathophysiology & Hidden Lethal Risks | Time to Critical Deterioration | Immediate Field Stabilization | Critical Contraindication (What NOT to Do) |
|---|---|---|---|---|
| Harness Suspension After Tree Stand Fall | Leg straps compress femoral veins while motionless legs pool 50 to 60% of blood volume; causes cerebral hypoxia, vagal bradycardia, and cardiac arrest | 10 to 30 minutes if legs hang motionless without a foot relief loop | Step into suspension relief strap immediately; pump legs against tree trunk; lower victim to ground and transition gradually from sitting to supine | Never hang passively with relaxed legs; make sure rescuers cut or unhook the victim down onto a ladder rapidly |
| Free-Fall Ground Impact from Tree Stand (4.5 to 7.5 m / 15 to 25 ft) | Axial loading fractures calcanei (heels), tibial plateaus, pelvic ring and L1/T12 vertebrae; deceleration shears thoracic aorta or spleen/liver | 15 to 60 minutes from internal retroperitoneal or intrathoracic hemorrhage | Full spinal immobilization on insulated pad; pelvic binder around greater trochanters; vented chest seal if impaled on branch | Never stand or walk a hunter out who has back, neck, or hip pain after falling from a stand |
| High-Velocity Rifle or Shotgun Slug GSW | Permanent crush tract plus massive hydrostatic temporary cavitation wave (10x bullet diameter) that shatters bone and shreds adjacent arteries/bowel | 2 to 4 minutes (femoral/brachial artery); 10 to 20 minutes (tension pneumothorax) | Windlass tourniquet on limbs; hemostatic packing in junctional zones; check back/flanks for larger exit wound and seal both chest holes | Never forget to roll/check the patient's back for an exit wound; a small entry hole often hides a massive posterior exit bleed |
| Shotgun Birdshot / Buckshot at Close Range | Hundreds of pellets create a ragged, cratered tissue defect with embedded plastic wadding, clothing fragments, and diffuse vascular oozing | 3 to 15 minutes depending on distance and vessel involvement | Proximal windlass tourniquet for extremities; bulky hemostatic packing + elastic pressure dressing; occlusive chest seal for torso | Never waste time picking out individual lead or steel pellets in the field |
| Broadhead Arrow or Crossbow Bolt | 2 to 4 surgical razor blades create clean, deep stellate lacerations with zero hydrostatic shock; shaft acts as an internal vascular plug | Seconds if pulled out; stable for hours if immobilized cleanly in place | Leave embedded; pack and press around shaft base; pad with bulky split-gauze / foam rolls; trim excess shaft length carefully if needed for transport | NEVER pull a broadhead backward or push it forward through a limb (mechanical blades deploy wider when pulled back) |
Surviving a Tree Stand Fall: Harness Suspension Trauma & Ground Impact¶
1. The Physiology of Harness Suspension Trauma (Orthostatic Intolerance)¶
Modern hunting standards require wearing a Full-Body Fall Arrest System (FAS / TMA-certified harness) tethered to the tree from the moment your feet leave the ground via a lineman's belt or safety lifeline. However, catching your fall in a harness is only half the battle: - Why the "Venous Muscle Pump" Fails in Mid-Air: Normally, when you stand on the ground, contractions of your calf and thigh muscles squeeze deep veins and push deoxygenated blood upward through one-way valves back to your right atrium. When you hang suspended in mid-air after a tree stand collapses, gravity pulls blood down into your legs while the tight webbing of the harness leg straps compresses the femoral veins in your groin blocking venous return to the heart. - The 15-Minute Countdown: Within 5 to 15 minutes of hanging motionless, up to 60 percent of your circulating blood volume becomes trapped in your lower extremities. Cardiac output plummets, blood pressure crashes, and the brain triggers a sudden paradoxical vasovagal reflex (the Bezold-Jarisch reflex) that slows the heart rate, causing sudden loss of consciousness. Once unconscious in a vertical harness, the hunter's head slumps forward (compromising the airway) and fatal hypoxic cardiac arrest follows within 15 to 30 minutes.
3. Improvised Relief Loop (If You Lost Your Relief Strap): If you do not have a relief strap, take your lineman's climbing belt, deer drag rope, or binocular harness strap tie or loop it around the tree trunk or into your harness tether at knee height, and step one foot into the loop to take pressure off your groin straps. 4. Recover onto the Stand or Descend: While standing in the relief loop, hug the tree trunk to unweight your back tether, slide the tree tether strap downward 30 centimeters (12 inches) at a time to inch down to a climbing stick or ladder rung, or swing back onto the platform. 5. Post-Rescue Positioning After Prolonged Suspension: Once a hunter who has been hanging in a harness for more than 15 minutes is lowered to the ground: - If unconscious or in cardiac arrest: Lay them flat (supine) immediately, open the airway, and start - If conscious and alert: Modern Wilderness Medical Society (WMS) and occupational rescue guidelines advise transitioning the conscious patient carefully, monitoring their airway and important signs closely, loosening all harness straps immediately, and keeping them warm while evacuating to an emergency department to screen for crush-like rhabdomyolysis and hyperkalemia (muscle breakdown toxins and potassium released from ischemic legs).
The Backcountry MARCH Algorithm for Gunshot and Broadhead Trauma¶
When a high-powered hunting rifle, shotgun, muzzleloader, or compound bow/crossbow injures a hunter in remote timber, follow the tactical MARCH sequence (Massive hemorrhage, Airway, Respiration, Circulation, Hypothermia/Head):
1. M, Massive Hemorrhage (First 60 Seconds)¶
- Tourniquet Placement for Extremity Wounds: Do not waste precious seconds cutting off three layers of insulated hunting bibs to see a spurting femoral artery if blood is pouring down the pant leg. Place a CoTCCC windlass tourniquet (CAT or SOFTT-W) "high and tight" at the very top of the thigh or upper arm over smooth clothing (making sure no cell phone, knife, or ammo box is in the pocket beneath the band), pull the self-adhering band as tight as humanly possible, and twist the windlass rod until bright red bleeding stops completely. Once the scene stabilizes, expose the wound and, if needed, place a second tourniquet directly on the skin 5 to 8 centimeters (2 to 3 inches) above the wound before ever loosening a high-and-tight strap.
- Improvised Backcountry Hunting Tourniquet: If you do not carry a commercial tourniquet, use a 5-centimeter (2-inch) wide canvas rifle sling, binocular strap, or folded blaze-orange vest strip wrapped above the wound and twisted tightly with a metal bolt, cleaning rod section, or stout hardwood stick as a windlass. Never use a thin paracord or bootlace alone without a wide fabric band underneath, as thin cord slices skin without occluding the deep femoral artery.
- Wound Packing for Groin, Buttock, Axilla, or Neck Wounds: If a bullet or broadhead severs a vessel in the groin crease (inguinal junction), armpit, or base of the neck, wrap a finger in hemostatic gauze (QuikClot Combat Gauze or Celox) or clean cotton t-shirt strips and feed it tightly inch-by-inch directly onto the bleeding vessel at the bottom of the wound cavity until packed slightly above skin level, then lean down with both hands for 3 to 5 full minutes (hemostatic) or 10 minutes (plain gauze).
2. Special Protocol: Broadhead Arrow and Crossbow Bolt Impalements¶
Modern hunting arrows and crossbow bolts are tipped with fixed-blade or mechanical (expandable) broadheads engineered to maximize hemorrhage via 2, 3, or 4 surgical-grade razor blades spanning 2.5 to 6 centimeters (1 to 2.5 inches) in cutting diameter: 1. Why Pulling a Broadhead Out Is Fatal: Inside the body, the carbon arrow shaft and blades are pressing against the sliced walls of veins and arteries (internal tamponade). Also, mechanical broadheads have rear-facing barbed blades that open like an umbrella when pulled backward and fixed-blade broadheads rotate as the flexible spine of the arrow bends. Pulling an arrow out in the woods slices adjacent intact nerves, tendons, and major arteries wide open. 2. How to Stabilize an Impaled Arrow in the Field: - جيڪٽس ۽ شرٽس چوڌاري تير واري شافٽ کي ٽراما شيئرز يا چاقو سان ڪٽايو ته جيئن ڪپڙا شافٽ تي نه لڪي. - Build a log-cabin or doughnut bolster around the arrow up to a height of 8 to 10 centimeters (3 to 4 inches) using rolled socks, spare fleece hats, foam pad strips, or bulky gauze rolls, and tape the padding securely to the body so the arrow cannot pivot during litter transport. 3. When and How to Shorten the Arrow Shaft: If a 75-centimeter (30-inch) arrow shaft sticks out of a hunter's thigh or chest in thick brush, every tree branch will strike the feathered fletching end and lever the razor broadhead inside the patient's organs. - Have a partner hold the arrow shaft in a vise-like grip right near the skin entry point with both hands (or clamp it tightly with locking pliers) to absorb 100 percent of vibration. - Use heavy-duty wire cutters, a rotary tubing cutter, or a fine-tooth multi-tool saw to cut the hollow carbon or aluminum shaft 10 to 15 centimeters (4 to 6 inches) above the skin. Wrap a piece of duct tape over the ragged cut carbon-fiber end so sharp carbon splinters do not injure rescuers.
3. A & R, Airway and Respiration (The Entry-and-Exit Wound Rule)¶
High-velocity hunting rifle bullets (.270, .308, .30-06, 6.5 Creedmoor) almost always pass completely through a human torso, creating a small, neat entry wound on the front and a much larger, ragged exit wound on the back or flank. - Check the "Box" (Neck to Navel, Front, Sides, and Back): Sweep your gloved hands inside the hunter's vest across the entire front, armpits, and back. - Apply Vented Chest Seals: Any penetrating wound between the collarbones and the navel (front or back) can collapse a lung (pneumothorax). Wipe blood and sweat off the skin and apply a vented commercial chest seal over the entry wound AND a second chest seal over the exit wound at the end of an exhalation. If improvising with a heavy plastic Ziploc bag or snack wrapper, tape down three sides squarely and leave the bottom corner untaped so trapped air can burp out of the chest without being sucked back in. - Watch for Progressive Tension Pneumothorax: If a patient with a sealed chest wound becomes progressively more short of breath, develops bluish lips, loses their radial wrist pulse, and has extreme anxiety or distended neck veins, burp the chest seal by peeling up one corner for 3 to 5 seconds to let trapped high-pressure air hiss out of the pleural cavity, then smooth the seal back down.
4. C & H, Circulation, Pelvic Binding, and Hypothermia Prevention¶
- Stop the Trauma Triad of Death: A bleeding trauma patient in a cold November forest loses body heat rapidly. Once core temperature drops below 35 degrees Celsius (95 degrees Fahrenheit) blood platelets and clotting factors stop working (hypothermic coagulopathy), causing wounds to start bleeding uncontrollably again.
- Immediate Thermal Enclosure: Even if the air feels mild, get the injured hunter off the cold dirt or snow immediately by sliding a closed-cell foam seat cushion, backpack, or thick bed of pine boughs underneath them. Wrap them in dry down layers, a blaze-orange outer shell, chemical hand warmers placed in the armpits and chest (over a thin shirt layer, never on bare skin), and a windproof mylar space blanket.
Coordinating Remote Backcountry Evacuation & Helicopter Landing Zones (LZ)¶
When calling emergency dispatch (911 / 112 / 999) or messaging via a satellite SOS device (Garmin inReach, Zoleo, SPOT, or iPhone/Android Satellite SOS) from deep woods:
1. Exact Information to Transmit in the First 30 Seconds¶
- Exact GPS Coordinates in Decimal Degrees or Degrees-Minutes-Seconds: Open your hunting mapping app (OnX Hunt, BaseMap, Gaia GPS, Garmin Explore, or Apple/Google Compass) and read the exact latitude and longitude (for example,
45.67892, -111.04567), plus the name of the nearest logging road, trailhead gate, or creek drainage. - Mechanism and Critical Interventions Applied: Clearly state "Hunting gunshot wound to right thigh with windlass tourniquet applied at 07:40 AM" or "6-meter (20-foot) tree stand fall with back paralysis." Asking specifically for air medical helicopter dispatch (Medevac) and a backcountry Search and Rescue (SAR) litter team cuts 30 to 60 minutes off rural response times because rural ambulance crews cannot carry a 100-kilogram hunter 3 kilometers through timber alone.
- Mark Every Trail Junction with Blaze Orange: If a second companion is available, hang blaze-orange hunting vests, surveyor's flagging tape, or glowsticks at the main logging road gate and at every fork in the trail leading to the patient so incoming rangers and paramedics can run straight to the scene without getting lost.
جيڪڏهن هڪ طبي يا SAR هيلي ڪاپٽر ٻيلو صاف ڪرڻ، ميڊو، يا وسيع لاگنگ روڊ تي آهي: - ** گھٽ ۾ گھٽ صاف ڪرڻ جا طول و عرض**: - Daytime LZ: At least 25 by 25 meters (75 by 75 feet) and ideally 30 by 30 meters (100 by 100 feet) for larger helicopters, completely clear of trees, stumps taller than 45 centimeters (18 inches), and overhead power lines. - Nighttime LZ: At least 45 by 45 meters (150 by 150 feet). - Ground Slope: The touchdown spot must be firm (not deep bog mud or thin lake ice) with a slope of less than 8 to 10 degrees (an incline steeper than 10 degrees causes the main rotor blades to strike the upslope hillside).
