Discovering a foul-smelling, draining crater on an older adult's lower back, buttock, or heel, whether you are caring for a parent at home or lifting the bedsheets during a visit to a hospital or skilled nursing facility, is an acute medical emergency. Many family caregivers inadvertently worsen pressure ulcers by scrubbing them with cytotoxic hydrogen peroxide or rubbing alcohol, sitting the patient on rubber "donut" cushions that strangle blood supply, or rolling the patient onto a 90-degree side-lying position that crushes the hip bone. This guide explains how to recognize systemic sepsis and spreading cellulitis, stage pressure injuries using the official NPIAP (National Pressure Injury Advisory Panel) criteria, dress and offload a deep wound safely until wound-care clinicians arrive, execute the 30-Degree Lateral Tilt repositioning schedule, and report nursing home neglect when a facility allows preventable Stage 3 or Stage 4 ulcers to develop.

Tee tämä nyt

  1. Call emergency services immediately if the person shows any signs of sepsis or deep spreading infection. If an older adult with a bedsore develops new confusion or sairaalan delirium, extreme lethargy, shivering chills, a fever above 38.0 degrees Celsius (100.4 degrees Fahrenheit) OR a dangerously low core body temperature below 36.0 degrees Celsius (96.8 degrees Fahrenheit), rapid breathing, a racing heart rate, or red streaks spreading outward from the wound, call your local emergency number (911 Yhdysvalloissa ja Kanadassa, 112 kaikkialla Euroopan unionissa, 999 Isossa-Britanniassa) käyttämällä meidän hätänumerot directory. Tell the dispatcher: "I have an older adult with an infected pressure ulcer showing clinical signs of acute sepsis."
  2. Take 100 percent of body weight off the ulcer immediately ("Zero Pressure Offloading"). No antibiotic or dressing on Earth can heal a bedsore while the patient is still lying or sitting on top of it. Gently roll and support the patient onto an intact side using pillows wedged behind their back at a 30-degree tilt, or float a sore heel completely in the air by placing pillows lengthwise underneath the lower calf from the knee to the Achilles tendon.
  3. Never put hydrogen peroxide, rubbing alcohol, povidone-iodine (Betadine), or harsh soap inside an open Stage 3 or Stage 4 bedsore. Under NPIAP / EPUAP and Agency for Healthcare Research and Quality (AHRQ) clinical guidelines, antiseptic chemicals like hydrogen peroxide and rubbing alcohol are cytotoxic: they burn and kill the fragile new granulation cells and fibroblasts trying to rebuild the wound bed. Flush an open pressure ulcer gently using only sterile normal saline (0.9% sodium chloride) or clean, body-temperature potable water, and cover it with a sterile moist saline gauze or foam dressing until a doctor evaluates it.
  4. Never use a ring-shaped "donut cushion" or sheepskin pad alone to offload a tailbone bedsore. Ring or donut cushions concentrate 100 percent of the person's body weight into a narrow ring around the wound, acting like a tourniquet that cuts off arterial blood flow to the center of the ulcer and expands tissue death.
  5. Inspect every bony pressure point from head to toe right now. Where there is one bedsore, there is almost always a second hidden one. Use a bright flashlight to check the sacrum/tailbone, both outer hips (greater trochanters), both sitting bones (ischial tuberosities), both heels, both outer ankles (malleoli), the shoulder blades, the elbows, the back of the skull, and behind the ears (where nasal oxygen tubing presses against the skin).
  6. If you discover an unreported Stage 3, Stage 4, or Unstageable black bedsore in a nursing home or hospital, photograph it with a ruler and demand an immediate physician / wound-care consult. In a long-term care facility, developing an avoidable Stage 3 or Stage 4 pressure ulcer is a serious regulatory violation (see our guide on nursing home abuse, neglect, and eviction). Place a clean metric ruler or coin next to the wound (without touching the open bed) to document its exact length and width before demanding an immediate transfer to an acute hospital if infection is present.

How to stage a bedsore: Official NPIAP / EPUAP classification matrix

Clinicians worldwide classify pressure injuries using the standardized National Pressure Injury Advisory Panel (NPIAP) and European Pressure Ulcer Advisory Panel (EPUAP) staging system. Identifying the exact stage tells you whether you are dealing with an early warning sign or a deep surgical emergency.

The "Finger Blanch Test" for Stage 1 detection

When you see a red or darkened patch of skin over a bony area (such as the tailbone, hip, or heel), press a clean, gloved fingertip gently against the center of the red spot for 3 seconds, then let go:

  • Blanchable Erythema (Normal Reactive Hyperemia): The spot turns white (blanches) for a second when pressed and then flushes pink again as blood rushes back in. The microcirculation is still intact; take all pressure off the spot immediately and the redness will fade within 30 to 60 minutes.
  • Non-Blanchable Erythema (Stage 1 Pressure Injury): When you lift your finger, the spot stays bright red, dark purple, or maroon and does not turn white. On darker skin tones (where redness may not show clearly), look for a localized patch that looks purplish, bluish, or darker than surrounding skin, feels hotter or cooler to the back of your hand, or feels hard/boggy (indurated) compared to nearby tissue. This is a Stage 1 Pressure Injury, meaning capillary damage has already begun.

NPIAP Pressure Injury Staging and Emergency Action Table

NPIAP Stage Visual Appearance & Tissue Depth Infection & Sepsis Risk Level Immediate Care & Clinical Intervention Required
Stage 1: Non-Blanchable Erythema of Intact Skin Skin is intact (no open break), with a localized area of non-blanchable redness (or purple/discolored warmth/firmness on dark skin) over a bony prominence. Low (Skin barrier is still unbroken, but tissue will die within hours if pressure continues). 100% Offloading immediately. Apply a multi-layer soft prophylactic silicone foam border dressing to reduce friction/shear, moisturize dry skin, and never massage or rub the red area (massaging crushes damaged capillaries).

  1. Pese kätesi huolellisesti saippualla ja vedellä 20 sekunnin ajan ja pue puhtaat kertakäyttöiset nitriilikäsineet.
  2. Käytä steriiliä 0,9 % natriumkloridi (normaali suolaliuos) haavanpesuainetta: Jos sinulla ei ole apteekista tölkkiä steriiliä suolaliuosta, voit valmistaa isotonista hätäsuolaliuosta kotona keittämällä **1 litraa (4 kuppia) puhdasta hana- tai pullotettua vettä sekoitettuna 2 grammaan suolaa ** 5 minuuttia (9 tl:ssa). kannelliseen kattilaan ja anna sen sitten jäähtyä kokonaan haaleaan kehon lämpötilaan (37 °C / 98,6 °F) ennen käyttöä. (Älä koskaan käytä kylmää vettä haavapohjalla, koska haavan jäähdyttäminen alle 33 °C:een pysäyttää valkosolujen ja mitoottisen paranemistoiminnan jopa 4 tunniksi).

    D --> B

How to position a patient in the 30-Degree Lateral Tilt step by step

  1. Always use a two-person "Draw Sheet Lift" (Never drag the patient across the sheet): Friction and shear (when the skin sticks to the cotton bedsheet while the skeleton slides downward inside the body) tear subcutaneous blood vessels. Place a folded cotton draw sheet (or low-friction nylon slide sheet) under the patient from shoulders to thighs, and lift the patient slightly off the mattress before shifting them.
  2. Keep the Head of the Bed (HOB) at or below 30 degrees: Unless the patient is actively eating, drinking, or receiving tube feedings (or has severe heart failure/COPD requiring upright breathing), never leave the head of the hospital bed cranked up at 45 to 60 degrees for hours. When the head of the bed is raised above 30 degrees, gravity pulls the patient's torso down toward the foot of the bed, grinding the sacrum and tailbone with massive shear forces.
  3. Wedge pillows behind the back and buttocks at a 30-degree angle:
    • Roll the patient gently toward you, fold two firm pillows lengthwise behind their shoulder blades and lower back/buttocks, and let them roll back onto the pillows so their pelvis rests at a 30-degree angle to the mattress.
    • Perform the "Hand-Slide Check": Slide your flat hand underneath the patient's lower back right over the sacrum/tailbone and over the lower hip bone (trochanter). In a true 30-degree tilt, your hand should slide freely past the sacrum (it is floating in the air) AND the patient's weight is resting on the fleshy gluteal muscle rather than directly on the point of the hip bone.
  4. Place a pillow lengthwise between the knees and ankles, and "Float the Heels":
    • Never let one bony knee or ankle rest directly on top of the other knee or ankle; place a long pillow between the legs from groin to foot.
    • To float the heels: Place one or two pillows lengthwise underneath both lower calves from just below the knee down to the Achilles tendon, so both heels hang completely suspended in mid-air over the edge of the pillow with zero contact against the bedsheet.

Step 4: Manage incontinence moisture and boost protein/calorie nutrition immediately

  1. Incontinence-Associated Dermatitis (IAD) and Fecal Contamination: Liquid stool contains digestive enzymes (proteases and lipases) that digest human skin on contact, and when urine mixes with fecal bacteria, urease converts urea into caustic ammonia that raises skin pH and burns the dermis. Check briefs at least every 2 hours, cleanse gently with a pH-balanced no-rinse perineal cleanser (never harsh bar soap), apply a thick zinc oxide or dimethicone moisture barrier paste over intact skin, and ask the physician whether a temporary external urinary catheter (such as a PureWick female external suction wick or male condom catheter) is appropriate while a sacral ulcer heals.
  2. Emergency Nutritional Support (NPIAP Guidelines): Healing a Stage 3 or Stage 4 bedsore requires massive metabolic energy, as the body is literally trying to regrow muscle and skin while losing protein in wound exudate. Under NPIAP / EPUAP nutritional guidelines, an older adult with a pressure ulcer needs 30 to 35 kilocalories per kilogram of body weight per day and 1.25 to 1.5 grams of protein per kilogram of body weight per day (roughly 85 to 105 grams of protein daily for a 68 kg / 150 lb senior, unless restricted by un-dialyzed end-stage kidney failure), plus high-protein oral nutritional supplements enriched with L-Arginine, Zinc, and Vitamin C.

Advocating against nursing home or hospital neglect (Stage 3 & 4 "Never Events")

In the United States, the Centers for Medicare & Medicaid Services (CMS) classifies an unstageable, Stage 3, or Stage 4 pressure ulcer acquired after admission to a hospital or skilled nursing facility as a Hospital-Acquired Condition (HAC) / Serious Reportable "Never Event." Under the federal Nursing Home Reform Act (42 CFR § 483.25(b)(1)), a nursing home must verify that a resident who enters the facility without pressure ulcers does not develop them unless the resident's clinical condition demonstrates that they were unavoidable, and that a resident with pressure ulcers receives necessary treatment and services (including a Group 2 Alternating-Pressure Low-Air-Loss Mattress or Group 3 Air-Fluidized Bed) to promote healing and prevent infection.

If you visit a loved one in a nursing home or hospital and discover a deep, foul-smelling bedsore that staff hid or downplayed as a "little rash":

  1. Demand to see the facility's Weekly Skin Assessment and 2-Hour Turning Logs: Ask the Director of Nursing (DON): "When was this pressure ulcer first staged in the chart, what stage is documented, when did the wound-care physician last examine it, and why is my parent still lying on a standard foam mattress instead of a powered alternating-pressure low-air-loss mattress?"
  2. Insist on immediate hospital transfer if any infection or sepsis signs are present: Nursing homes sometimes hesitate to send a resident with a Stage 4 infected bedsore to the hospital emergency room because hospitalizing the resident triggers automatic state health department scrutiny. Remember that you do not need the nursing home's permission to call 911 from your cell phone while standing at your parent's bedside if they are septic, feverish, or delirious.
  3. File a formal complaint with your State Long-Term Care Ombudsman and Department of Health Survey Agency: Read our full guide on nursing home abuse, neglect, and eviction for the exact state reporting hotlines and evidence-preservation steps.

Complete a caregiver first aid and safe patient handling course

Caring for an immobile or bedbound family member at home places heavy physical and clinical demands on family caregivers. Consider taking a hands-on Home Caregiver and First Aid course through the Red Cross, St John Ambulance, or your local Area Agency on Aging / hospital wound clinic to practice two-person draw-sheet turning, skin inspection, and infection recognition safely.

Usein kysytyt kysymykset

Why did the doctor say my parent's bedsore got "bigger and deeper" right after the wound clinic cleaned it?

When a Stage 3 or Unstageable pressure ulcer is covered by a cap of black **eschar** or thick yellow **slough** (dead, necrotic tissue), the dead tissue acts like a plug hiding the true depth and width of the crater underneath, while preventing healthy tissue from growing and feeding bacteria. Once a wound surgeon or certified wound nurse performs **sharp or enzymatic debridement** to remove the dead tissue, the true dimensions of the underlying cavity are revealed. Seeing clean, beefy-red, slightly pebbled tissue (**granulation tissue**) at the base of a larger opening after debridement is actually the necessary first step toward healing from the bottom up.

How often should an immobile older adult sitting upright in a wheelchair or recliner be repositioned?

Sitting upright in a wheelchair or geriatric recliner concentrates **far higher pressure** onto the two small sitting bones (**ischial tuberosities**) and tailbone than lying flat in bed. Under **NPIAP guidelines**, a wheelchair-bound or chair-bound patient who cannot shift their own weight must be repositioned **at least every 1 hour** (and if they can use their arms, they should be taught to perform a small weight-shift lean every **15 minutes**). If an older adult already has an open Stage 3 or Stage 4 ulcer on their sitting bone (ischium) or sacrum, **limit chair sitting strictly** (or avoid sitting on the ulcer completely) and place a prescription **air-cell or gel/foam pressure-redistributing wheelchair cushion (such as a ROHO cushion)** on the chair, never a standard pillow or donut ring.

Can I use over-the-counter Triple Antibiotic Ointment (Neosporin) or honey inside a deep Stage 3 or Stage 4 bedsore?

Do not pack over-the-counter petroleum-based Triple Antibiotic Ointment (Neosporin/Polysporin) into a deep Stage 3 or Stage 4 pressure ulcer cavity; OTC neomycin ointments are designed only for minor surface scrapes, cause frequent allergic contact dermatitis in up to 10 percent of users, do not penetrate deep necrotic slough, and trap anaerobic fluid in deep craters. While sterile, gamma-irradiated **medical-grade Manuka honey dressings** (such as Medihoney) are legitimately used by wound clinics for autolytic debridement of sloughy ulcers, **never put raw grocery-store kitchen honey into an open wound**, as raw food honey can contain *Clostridium botulinum* spores.

How do I get Medicare or Medicaid to pay for an alternating-pressure air mattress for my bedbound parent at home?

Under **Medicare Part B Durable Medical Equipment (DME) Local Coverage Determinations (LCD L33820: Pressure Reducing Support Surfaces)**, Medicare covers a **Group 1 Support Surface** (alternating-pressure pad, gel overlay, or high-density foam mattress) if the patient is completely immobile OR has limited mobility plus any stage pressure ulcer on the trunk/pelvis and at least one risk factor (incontinence, malnutrition, or altered sensory perception). Medicare covers a **powered Group 2 Alternating-Pressure / Low-Air-Loss Mattress System** when the patient has **multiple Stage 2 ulcers** that have not improved after 30 days on a Group 1 surface, OR has a **large or single Stage 3 or Stage 4 pressure ulcer on the trunk or pelvis** (sacrum, ischium, or trochanter). Ask your parent's primary care physician or wound clinic to submit a **Face-to-Face DME Order for a Group 2 Low-Air-Loss Mattress (HCPCS Code E0277)** to a Medicare-enrolled DME supplier today.
