Finn nummeret for ditt land.

|---|---|---|---|
| 1. Beste valg (umiddelbar reimplantasjon) | Tilbake inne i pasientens egen tannkjøtthule | Beste langtidsprognose | Gjenoppretter umiddelbar blod- og næringstilførsel ved kroppstemperatur. |
| 2. Gullstandard væske | Hanks balanserte saltløsning (HBSS / Save-A-Tooth-sett) | Opptil 12 til 24 timer | Spesielt pH-balansert, isotonisk cellekulturvæske som finnes i førstehjelpsutstyr for idrett og skole. |
| 3. Beste husholdningsvæske | Kald pasteurisert kumelk (hel eller skummet) | Opptil 3 til 6 timer | Fysiologisk osmolalitet, nøytral pH og næringsstoffer holder periodontale ligamentceller i live uten hevelse. |
| Immediate dental emergency (within 2 to 6 hours). | Bacterial contamination of the exposed dental pulp increases every hour. Do not wipe or poke the red nerve dot; cover gently with sterile gauze, take oral ibuprofen or acetaminophen, and go straight to an emergency dentist for a protective pulp cap or root canal treatment. |
| Root fracture (Below the gumline) | The crown of the tooth wiggles back and forth as a single piece and bleeds from the gum collar when touched. | Urgent; see dentist within 6 to 12 hours. | Avoid biting on that side entirely; eat only liquids until a dentist takes a periapical X-ray and splints the crown. |
Lost fillings, dislodged crowns, and broken braces wires¶
When a restoration falls out or orthodontic hardware breaks after hours, temporary stabilization protects the exposed tooth structure and prevents accidental aspiration until your dentist opens:
- Lost filling: Rinse the cavity gently with warm salt water to clear food debris; never scrape inside the hole with a toothpick or pin. Press a small ball of over-the-counter temporary dental filling cement (zinc oxide/calcium sulfate paste sold at pharmacies) or a piece of softened sugar-free chewing gum into the cavity to block cold air and food (never use sugared gum, which causes intense pain inside open dentin tubules).
- Crown (cap) falls off intact: Save the crown and check inside it; if it is hollow and your underlying tooth stump is intact and not painful, you can test-fit the crown back over the tooth. If it slides on easily in one clear orientation, coat the inside of the crown with a thin smear of over-the-counter temporary dental cement, toothpaste, or denture adhesive and bite down gently to seat it until you see your dentist. Never use household superglue (cyanoacrylate) inside your mouth, as it is toxic, kills the tooth nerve, and ruins the crown margins. If the crown is loose or the patient is a child, leave the crown off in a zip-top bag so it is not swallowed or inhaled during sleep.
- Poking orthodontic braces wire: If an archwire slips out of a back bracket and stabs your inner cheek, try using the clean rubber eraser end of a pencil to gently push the flexible wire flat against the tooth, then dry the metal bracket with a tissue and press a pea-sized ball of orthodontic relief wax (or a tiny piece of wax from a babybel cheese rind or paraffin candle in a pinch) firmly over the sharp wire end. Never try to cut a heavy orthodontic wire inside the mouth with household wire cutters unless it is trapping the cheek, as the clipped metal end can shoot down the throat.
Severe toothache, dental abscesses, and Ludwig's angina¶
A severe, throbbing toothache that wakes you from sleep or worsens with hot liquids usually indicates irreversible pulpitis or a periapical dental abscess (a pocket of bacterial pus building up at the tip of the dead root inside the jawbone).
Red flags for Ludwig's angina and deep neck space infections¶
Because the roots of your lower molars sit right next to the submandibular and sublingual tissue spaces of the neck, and upper tooth roots sit millimeters below your sinuses and eye sockets, an untreated dental abscess can break through the thin jawbone plate and spread rapidly down the neck or up toward the brain:
- Ludwig's angina (bilateral floor-of-mouth cellulitis): If infection from a lower molar spreads under the tongue, the soft floor of the mouth becomes wooden-hard, swollen, and elevated, pushing the tongue upward and backward against the roof of the mouth and completely sealing off the airway within hours.
- Cavernous sinus thrombosis and orbital cellulitis: If an upper canine or molar abscess spreads upward, it causes rapid swelling and redness around one eye, drooping eyelid, double vision, or severe headache.
- When to go straight to a hospital emergency department (not a regular dental office): Seek immediate hospital ER care for intravenous antibiotics and airway protection if a toothache is accompanied by any swelling visible on the outside of the face, jaw, cheek, eye, or neck, difficulty opening your mouth wider than two fingers (trismus), a raised tongue or firm swelling under the tongue, drooling or painful swallowing, or fever and chills.
Safe pain and swelling relief while waiting for the dentist¶
- Take oral anti-inflammatory pain relievers: For adults who can safely take NSAIDs, combining scheduled oral ibuprofen (which targets prostaglandin inflammation inside the bone) with acetaminophen (paracetamol) according to package label directions provides stronger dental pain relief than either medication alone.
- Apply cold only to the outside of the cheek: Hold a cold pack wrapped in a towel against the outside of the jaw for 15 minutes on, 15 minutes off. Never put a heating pad or hot water bottle against the outside of an abscessed jaw, as external heat dilates facial blood vessels and draws pus outward through the cheek skin.
- Rinse gently with warm salt water inside the mouth: Dissolve 2.5 milliliters (0.5 teaspoon) of table salt in 240 milliliters (8 ounces) of warm water, hold a mouthful over the sore gum for 30 seconds, and spit it out gently.
- Keep your head elevated on two or three pillows: Lying completely flat increases arterial blood pressure in the head and jaw, making a dental abscess throb much harder at night.
Warning
Never hold an aspirin tablet, crushed painkiller, clove oil concentrate, rubbing alcohol, or bleach against your gum or inside a broken tooth. Holding aspirin against the gum does not numb the tooth nerve; instead, salicylic acid burns the delicate oral mucosa and creates a painful white sloughing chemical ulcer ("aspirin burn") right on top of your toothache. Similarly, never try to pop or lance a gum boil with a sewing needle or knife.
Controlling severe mouth, lip, or tongue bleeding¶
The tongue, lips, and oral mucosa have an exceptionally rich blood supply, and a tiny drop of blood mixed with a mouthful of saliva makes oral injuries look much worse than they are.
- For post-extraction socket bleeding: Rinse the mouth gently once with cool water to clear loose jelly-like clots, roll a damp sterile gauze pad (or a moistened standard black tea bag, which contains natural astringent tannic acid that constricts capillaries and accelerates platelet clotting) into a firm cylinder, place it directly over the socket gap, and bite down with steady, uninterrupted pressure for 30 to 45 full minutes without opening your mouth to check it. Avoid spitting, rinsing, smoking, drinking hot liquids, or using a drinking straw for at least 24 hours.
- For lip, cheek, or tongue lacerations: Use a clean cloth or sterile gauze to pinch the bleeding lip or tongue firmly between your thumb and index finger for 10 to 15 continuous minutes. Seek medical or dental suturing within 6 hours if a lip cut crosses the vermilion border (the distinct line where the pink lip meets the facial skin, which requires precise stitching to prevent a permanent cosmetic notch), if a puncture goes all the way through the lip, or if a tongue cut is gaping, longer than 1 centimeter (0.4 inches), or continues bleeding briskly after 15 minutes of firm pinching.
Jaw fractures, temporomandibular joint dislocation, and facial bone trauma¶
High-energy impacts that knock out or fracture teeth frequently transfer severe mechanical force into the upper jaw (maxilla), lower jaw (mandible), cheekbone (zygoma), or the temporomandibular joint (TMJ) directly in front of the ear canal. Recognizing a jaw fracture or dislocation early prevents permanent bite malocclusion, nerve damage, and airway obstruction.
Clinical warning signs of a fractured mandible or maxilla¶
Perform a gentle visual and verbal check after any blow to the chin, cheek, or mouth:
- Acute malocclusion (teeth no longer fit together): Even a 1-to-2-millimeter shift in the jawbone makes the patient feel that their upper and lower teeth suddenly hit prematurely on one side or leave an open gap on the other side when they try to close their mouth gently.
- Step deformity or sublingual hematoma: Looking inside the lower mouth reveals a dark purple-black blood blister pooling on the floor of the mouth under the tongue (a classic clinical sign of a mandibular body fracture) or a visible stair-step misalignment in the row of teeth accompanied by a torn, bleeding vertical split in the gum tissue.
- Numbness of the lower lip and chin: The inferior alveolar nerve runs directly through a hollow canal inside the lower jawbone before exiting at the chin. If the lower lip or chin feels completely numb or tingly after a blow to the jaw (without dental anesthesia), the mandible is likely fractured across the nerve canal.
- Upper jaw mobility (Le Fort fractures): If grasping the upper front teeth gently reveals that the entire upper tooth arch or hard palate moves separately from the rest of the skull, or if clear watery fluid (cerebrospinal fluid) drips from the nose alongside severe midface bruising, the patient has suffered a high-energy maxillofacial fracture requiring immediate Level 1 trauma center transport.
First aid for a broken or dislocated jaw¶
- Keep the patient sitting upright and leaning slightly forward: If the lower jaw is broken in two places (such as a bilateral parasymphyseal fracture), the muscle attachments that hold the tongue forward lose their bony anchor, allowing the tongue to slide backward and block the throat if the patient lies flat on their back. Let them sit forward and drool blood and saliva into a cup or towel.
When a lower third molar (wisdom tooth) only pokes halfway through the gum at the very back of the jaw, a thick flap of gum tissue (called an operculum) hangs over the biting surface of the tooth. Food particles and plaque easily slip underneath this gum flap where a toothbrush bristle cannot reach, triggering a painful acute bacterial infection called pericoronitis.
- Symptoms: The gum flap at the back corner of the jaw becomes swollen, bright red, and tender; biting down crushes the swollen flap with the upper wisdom tooth; pus may ooze from under the flap; and inflammation spreading into the nearby chewing muscles (the medial pterygoid and masseter) makes it painful and difficult to open the mouth (trismus).
- Immediate home care and red flags: Fill a curved-tip plastic irrigation syringe (or rinse forcefully) with warm salt water (2.5 milliliters / 0.5 teaspoon of salt in 240 milliliters / 8 ounces of warm water) after every meal to flush trapped debris out from under the gum flap, and take scheduled oral ibuprofen. See a dentist within 24 to 48 hours for professional sub-operculum irrigation and antibiotics if needed. If pericoronitis causes swelling under the jawline, fever, trouble swallowing saliva, or inability to open your mouth wider than 2 centimeters (0.8 inches), go to an emergency room immediately.
Dental trauma prevention and mouthguard assembly¶
Over 60 percent of traumatic tooth avulsions and crown fractures occur during contact sports, cycling, skateboarding, and scooter riding. Preparing a compact dental rescue kit for your home, school nurse office, or sports sideline bag saves critical minutes when an impact occurs:
- Wear a custom-fitted or boil-and-bite mouthguard: A flexible thermoplastic mouthguard absorbs and distributes impact forces across the entire dental arch, cushioning the upper front incisors and reducing the risk of both tooth avulsion and mandibular condyle fractures.
- Stock an ADA-accepted Save-A-Tooth (HBSS) container: Keep a sealed container of Hank's Balanced Salt Solution in your sports team first aid kit alongside sterile 5-by-5-centimeter (2-by-2-inch) gauze sponges, orthodontic relief wax, temporary zinc-oxide filling paste, and nitrile medical gloves.
Ta et kurs
Dental and facial injuries often happen alongside sports collisions, bicycle falls, and car crashes. Completing a hands-on first aid and CPR course with the Red Cross, St John Ambulance, or your local emergency medical service teaches you how to check for concussions, protect the cervical spine, and keep an injured person's airway clear.
Ofte stilte spørsmål¶
Why shouldn't I store a knocked-out permanent tooth in tap water?
Plain tap water and bottled drinking water have very low salt concentration (hypotonic osmolality) compared to human cells. When a knocked-out tooth is submerged in water, water rushes through the cell membranes of the delicate periodontal ligament cells on the root, causing them to swell and burst within 15 to 30 minutes. Cold cow's milk has a compatible osmolality and neutral pH that keeps those root cells alive for several hours.
What if I accidentally dropped a knocked-out tooth on a dirty sidewalk or soccer field?
Pick the tooth up strictly by the white chewing crown (never touch the root) and gently pour cold milk, sterile saline, or cold running tap water over it for a maximum of 10 seconds to wash away loose grit. Never scrub, brush, or wipe the root with a cloth or soap, even if tiny specks of dirt remain attached, because scrubbing strips away the living root ligament; the dentist will clean it safely and prescribe antibiotics plus a tetanus booster if needed.
Why is it dangerous to put a knocked-out baby tooth back into a child's gum?
Directly underneath the roots of every primary (baby) tooth sits the developing crown of the permanent adult tooth inside its bony crypt. Pushing a knocked-out baby tooth back into the socket can crush, infect, or scar the developing permanent tooth bud underneath, causing the adult tooth to erupt deformed, discolored, or fused to the bone.
What should I do if I think someone swallowed or inhaled a knocked-out tooth or dental crown?
If a tooth or crown goes missing during an accident and cannot be found on the ground, observe the person closely. A tooth that is swallowed into the stomach usually passes harmlessly through the digestive tract within a few days, whereas a tooth that is aspirated (inhaled) into the windpipe or lungs causes coughing, wheezing, or silent lung infection and requires a chest X-ray and bronchoscopy removal. Always go to an urgent care clinic or ER for a quick neck and chest X-ray if a missing tooth cannot be accounted for after trauma.
Can emergency rooms pull an abscessed tooth or perform a root canal?
Unless it is a major university trauma hospital with an on-call oral and maxillofacial surgeon, standard hospital emergency rooms do not have dental chairs or drills and do not perform routine tooth extractions or root canals. However, you **must** go to a hospital ER immediately if you have facial or neck swelling, fever, or trouble swallowing, because ER physicians provide life-saving intravenous antibiotics, airway protection, CT imaging, and surgical drainage of deep neck abscesses before referring you to a dentist to remove the offending tooth.
How long will a dentist leave a splint on a knocked-out permanent tooth after putting it back in?
After reimplanting an avulsed permanent tooth and verifying its alignment with an X-ray, the dentist bonds a thin, flexible passive wire and composite splint across the injured tooth and its stable neighbors for **2 weeks** (or up to 4 weeks if the surrounding bony socket wall was also fractured). Unlike rigid metal splints used decades ago, a slightly flexible splint allows microscopic physiologic movement during healing, which stimulates the periodontal ligament fibers to reattach without fusing rigidly to the jawbone (ankylosis), and the dentist typically initiates root canal therapy within 7 to 10 days for fully developed adult roots.
