3. Best household liquid Cold pasteurized cow's milk (whole or skim) Up to 3 to 6 hours Physiological osmolality, neutral pH, and nutrients keep periodontal ligament cells alive without swelling.
4. Good medical alternative Sterile 0.9% normal saline or contact lens saline solution Up to 1 to 2 hours Isotonic salt balance prevents osmotic cell bursting, though it lacks cellular nutrients.
5. Emergency fallback (Conscious adult only) Patient's own saliva (spit into a small cup, or tucked inside the lower cheek vestibule) 30 to 60 minutes Keeps tooth moist, but lower osmolality and mouth bacteria degrade root cells after an hour; never put a loose tooth inside the mouth of a child or dazed patient who could choke on it.
6. Never use Plain tap water, bottled water, sports drinks, alcohol, or dry tissue/napkin Cells die in 15 to 30 minutes Tap water bursts cells through hypotonic shock; dry tissues strip away the ligament and desiccate the root instantly.

How to distinguish a baby tooth from a permanent adult tooth

Children between the ages of 6 and 12 have a "mixed dentition" containing both primary baby teeth and newly erupted permanent adult teeth:

  • Age 5 and younger: Almost certainly a primary baby tooth. Do not reinsert it.
  • Age 6 to 8 (Front upper incisors): The permanent upper and lower central front teeth usually erupt between ages 6 and 8. Newly erupted permanent incisors are noticeably larger, slightly yellower, and have three small rounded bumps (mamelons) along their biting edge, whereas baby teeth are smaller, chalky white, and have flat, worn biting edges.
  • When in doubt: Never force a questionable tooth back into a child's socket. Place the tooth immediately into a cup of cold milk and drive straight to an emergency dentist, who can identify it in seconds and reimplant it if it is permanent.

Displaced, loosened, or jammed teeth (dental luxation)

Often an elbow, fall, or steering wheel impact does not knock a tooth completely out of the mouth, but instead pushes it out of alignment while it remains partially attached to the gum socket:

  • Lateral luxation (pushed forward or backward) or Extrusion (hanging loosely partway out of the socket): Leave the tooth in the mouth. Do not yank a loose tooth out, and do not try to wrench a crooked tooth back into place if it feels locked in bone. Have the patient bite gently on a damp piece of gauze to steady it and see an emergency dentist within 2 to 6 hours so they can numb the gum, reposition the bone and tooth carefully, and bond a flexible wire splint across the teeth for 2 to 4 weeks.
  • Intrusion (tooth jammed up into the jawbone): Sometimes a hard impact drives a tooth vertically upward into the gum and bone so that the tooth appears shorter or disappears completely beneath swollen gum tissue, mimicking a knocked-out tooth. Never try to pull an intruded tooth down with your fingers or tools; urgent dental X-rays are required to check the root and socket floor.

Broken, chipped, or fractured teeth

How urgently a broken tooth needs treatment depends on which internal layer of the tooth is exposed to mouth bacteria:

Fracture classification What you see and feel Clinical urgency Immediate first aid steps
Enamel craze or minor chip White outer enamel edge is rough or slightly chipped; no bleeding and no sensitivity to cold air. Non-urgent; see dentist within 2 to 5 days.

Complicated crown fracture (Pulp / Nerve exposed) You can see a pinpoint pink or bright red bleeding dot in the center of the broken tooth structure; excruciating pain to air, touch, or temperature. Immediate dental emergency (within 2 to 6 hours). Bacterial contamination of the exposed dental pulp increases every hour. Do not wipe or poke the red nerve dot; cover gently with sterile gauze, take oral ibuprofen or acetaminophen, and go straight to an emergency dentist for a protective pulp cap or root canal treatment.
Root fracture (Below the gumline) The crown of the tooth wiggles back and forth as a single piece and bleeds from the gum collar when touched. Urgent; see dentist within 6 to 12 hours. Avoid biting on that side entirely; eat only liquids until a dentist takes a periapical X-ray and splints the crown.

Lost fillings, dislodged crowns, and broken braces wires

When a restoration falls out or orthodontic hardware breaks after hours, temporary stabilization protects the exposed tooth structure and prevents accidental aspiration until your dentist opens:

  • Lost filling: Rinse the cavity gently with warm salt water to clear food debris; never scrape inside the hole with a toothpick or pin. Press a small ball of over-the-counter temporary dental filling cement (zinc oxide/calcium sulfate paste sold at pharmacies) or a piece of softened sugar-free chewing gum into the cavity to block cold air and food (never use sugared gum, which causes intense pain inside open dentin tubules).
  • Crown (cap) falls off intact: Save the crown and check inside it; if it is hollow and your underlying tooth stump is intact and not painful, you can test-fit the crown back over the tooth. If it slides on easily in one clear orientation, coat the inside of the crown with a thin smear of over-the-counter temporary dental cement, toothpaste, or denture adhesive and bite down gently to seat it until you see your dentist. Never use household superglue (cyanoacrylate) inside your mouth, as it is toxic, kills the tooth nerve, and ruins the crown margins. If the crown is loose or the patient is a child, leave the crown off in a zip-top bag so it is not swallowed or inhaled during sleep.
  • Poking orthodontic braces wire: If an archwire slips out of a back bracket and stabs your inner cheek, try using the clean rubber eraser end of a pencil to gently push the flexible wire flat against the tooth, then dry the metal bracket with a tissue and press a pea-sized ball of orthodontic relief wax (or a tiny piece of wax from a babybel cheese rind or paraffin candle in a pinch) firmly over the sharp wire end. Never try to cut a heavy orthodontic wire inside the mouth with household wire cutters unless it is trapping the cheek, as the clipped metal end can shoot down the throat.

Severe toothache, dental abscesses, and Ludwig's angina

A severe, throbbing toothache that wakes you from sleep or worsens with hot liquids usually indicates irreversible pulpitis or a periapical dental abscess (a pocket of bacterial pus building up at the tip of the dead root inside the jawbone).

Red flags for Ludwig's angina and deep neck space infections

Because the roots of your lower molars sit right next to the submandibular and sublingual tissue spaces of the neck, and upper tooth roots sit millimeters below your sinuses and eye sockets, an untreated dental abscess can break through the thin jawbone plate and spread rapidly down the neck or up toward the brain:

  • Ludwig's angina (bilateral floor-of-mouth cellulitis): If infection from a lower molar spreads under the tongue, the soft floor of the mouth becomes wooden-hard, swollen, and elevated, pushing the tongue upward and backward against the roof of the mouth and completely sealing off the airway within hours.
  • Cavernous sinus thrombosis and orbital cellulitis: If an upper canine or molar abscess spreads upward, it causes rapid swelling and redness around one eye, drooping eyelid, double vision, or severe headache.
  • When to go straight to a hospital emergency department (not a regular dental office): Seek immediate hospital ER care for intravenous antibiotics and airway protection if a toothache is accompanied by any swelling visible on the outside of the face, jaw, cheek, eye, or neck, difficulty opening your mouth wider than two fingers (trismus), a raised tongue or firm swelling under the tongue, drooling or painful swallowing, or fever and chills.

Safe pain and swelling relief while waiting for the dentist

  1. Take oral anti-inflammatory pain relievers: For adults who can safely take NSAIDs, combining scheduled oral ibuprofen (which targets prostaglandin inflammation inside the bone) with acetaminophen (paracetamol) according to package label directions provides stronger dental pain relief than either medication alone.
  2. Apply cold only to the outside of the cheek: Hold a cold pack wrapped in a towel against the outside of the jaw for 15 minutes on, 15 minutes off. Never put a heating pad or hot water bottle against the outside of an abscessed jaw, as external heat dilates facial blood vessels and draws pus outward through the cheek skin.
  3. Rinse gently with warm salt water inside the mouth: Dissolve 2.5 milliliters (0.5 teaspoon) of table salt in 240 milliliters (8 ounces) of warm water, hold a mouthful over the sore gum for 30 seconds, and spit it out gently.
  4. Keep your head elevated on two or three pillows: Lying completely flat increases arterial blood pressure in the head and jaw, making a dental abscess throb much harder at night.

Warning

Never hold an aspirin tablet, crushed painkiller, clove oil concentrate, rubbing alcohol, or bleach against your gum or inside a broken tooth. Holding aspirin against the gum does not numb the tooth nerve; instead, salicylic acid burns the delicate oral mucosa and creates a painful white sloughing chemical ulcer ("aspirin burn") right on top of your toothache. Similarly, never try to pop or lance a gum boil with a sewing needle or knife.

Controlling severe mouth, lip, or tongue bleeding

The tongue, lips, and oral mucosa have an exceptionally rich blood supply, and a tiny drop of blood mixed with a mouthful of saliva makes oral injuries look much worse than they are.

  • For post-extraction socket bleeding: Rinse the mouth gently once with cool water to clear loose jelly-like clots, roll a damp sterile gauze pad (or a moistened standard black tea bag, which contains natural astringent tannic acid that constricts capillaries and accelerates platelet clotting) into a firm cylinder, place it directly over the socket gap, and bite down with steady, uninterrupted pressure for 30 to 45 full minutes without opening your mouth to check it. Avoid spitting, rinsing, smoking, drinking hot liquids, or using a drinking straw for at least 24 hours.
  • For lip, cheek, or tongue lacerations: Use a clean cloth or sterile gauze to pinch the bleeding lip or tongue firmly between your thumb and index finger for 10 to 15 continuous minutes. Seek medical or dental suturing within 6 hours if a lip cut crosses the vermilion border (the distinct line where the pink lip meets the facial skin, which requires precise stitching to prevent a permanent cosmetic notch), if a puncture goes all the way through the lip, or if a tongue cut is gaping, longer than 1 centimeter (0.4 inches), or continues bleeding briskly after 15 minutes of firm pinching.

Jaw fractures, temporomandibular joint dislocation, and facial bone trauma

High-energy impacts that knock out or fracture teeth frequently transfer severe mechanical force into the upper jaw (maxilla), lower jaw (mandible), cheekbone (zygoma), or the temporomandibular joint (TMJ) directly in front of the ear canal. Recognizing a jaw fracture or dislocation early prevents permanent bite malocclusion, nerve damage, and airway obstruction.

Clinical warning signs of a fractured mandible or maxilla

Perform a gentle visual and verbal check after any blow to the chin, cheek, or mouth:

  • Acute malocclusion (teeth no longer fit together): Even a 1-to-2-millimeter shift in the jawbone makes the patient feel that their upper and lower teeth suddenly hit prematurely on one side or leave an open gap on the other side when they try to close their mouth gently.
  • Step deformity or sublingual hematoma: Looking inside the lower mouth reveals a dark purple-black blood blister pooling on the floor of the mouth under the tongue (a classic clinical sign of a mandibular body fracture) or a visible stair-step misalignment in the row of teeth accompanied by a torn, bleeding vertical split in the gum tissue.
  • Numbness of the lower lip and chin: The inferior alveolar nerve runs directly through a hollow canal inside the lower jawbone before exiting at the chin. If the lower lip or chin feels completely numb or tingly after a blow to the jaw (without dental anesthesia), the mandible is likely fractured across the nerve canal.
  • Upper jaw mobility (Le Fort fractures): If grasping the upper front teeth gently reveals that the entire upper tooth arch or hard palate moves separately from the rest of the skull, or if clear watery fluid (cerebrospinal fluid) drips from the nose alongside severe midface bruising, the patient has suffered a high-energy maxillofacial fracture requiring immediate Level 1 trauma center transport.

First aid for a broken or dislocated jaw

  1. Keep the patient sitting upright and leaning slightly forward: If the lower jaw is broken in two places (such as a bilateral parasymphyseal fracture), the muscle attachments that hold the tongue forward lose their bony anchor, allowing the tongue to slide backward and block the throat if the patient lies flat on their back. Let them sit forward and drool blood and saliva into a cup or towel.
  2. Never tie or bandage the jaws tightly shut: Old first aid manuals used to recommend tying a bandage tightly under the chin and over the top of the skull. Modern trauma protocols forbid strapping a fractured jaw shut, because if the patient vomits from concussion or swallowed blood while their teeth are tied together, they will aspirate vomit into their lungs and suffocate. Instead, let the patient support the bottom of their chin gently in the palm of their own hand or hold a cool compress lightly against the swelling.
  3. Acute TMJ dislocation (jaw locked wide open): Sometimes yawning wide, vomiting, or taking a blow while the mouth is open causes the rounded condyle of the lower jaw to slip forward over the articular eminence of the skull and lock in front of it. The patient cannot close their mouth at all, speaks with a garbled voice, and feels severe muscle spasm in front of the ears. Never try to slap or force a locked jaw shut with your hands, as this can snap the condylar neck; take the patient to an emergency department or oral surgeon where clinicians can provide muscle relaxants or local nerve blocks and perform a controlled downward-and-backward manual reduction.

Post-extraction dry socket (alveolar osteitis) and pericoronitis

Not every severe dental emergency happens at the moment of an injury; two of the most painful acute oral conditions develop days after a tooth extraction or around an erupting wisdom tooth.

Dry socket (alveolar osteitis) after tooth extraction

When a permanent molar or wisdom tooth is extracted, a dark red blood clot forms inside the empty bony socket to protect the raw jawbone and nerve endings while new gum tissue grows across. If that protective blood clot dissolves prematurely or is sucked out by negative pressure (such as smoking, vaping, spitting forcefully, or drinking through a straw), the bare, whitish alveolar bone is left directly exposed to air, food, and saliva.
