Medical Term & Visual Appearance What It Looks Like to a Layperson Anatomical Source of Bleeding Why It Looks That Way (Pathophysiology) Urgency Level
1. Frank Red Hematemesis (Vomiting bright red blood or clots) Bright crimson liquid blood or dark red jelly-like clots vomited up into a basin or toilet. Upper GI Tract (Esophagus, Stomach, or Duodenum, above the Ligament of Treitz): e.g., ruptured esophageal varices, arterial peptic ulcer, or Mallory-Weiss tear. Bleeding is so rapid and high-volume (arterial or high-pressure portal venous) that the stomach fills and vomits before gastric hydrochloric acid has time to turn the blood brown. CRITICAL LIFE-THREATENING EMERGENCY: Call 911 / 112 / 999 immediately. High mortality without urgent hospital resuscitation and endoscopy.
2. "Coffee-Ground" Emesis (Vomiting dark brown/black granular flecks) Dark brown or blackish fluid mixed with gritty specks that look exactly like wet used coffee grounds. Upper GI Tract (Stomach or Duodenum): e.g., slow-to-moderate bleeding peptic ulcer, erosive gastritis, or gastric tumor. Blood has sat in the stomach for at least 15 to 60+ minutes, allowing gastric hydrochloric acid (\(HCl\)) to oxidize the iron in red hemoglobin (\(Fe{2+}\)) into dark brown hematin / acid hematin (\(Fe{3+}\)). HIGH MEDICAL EMERGENCY: Call 911 / 112 / 999 or transport immediately to an emergency department. Can suddenly erupt into bright red bleeding.
3. Melena (Black, tarry, foul-smelling stools) Jet-black, shiny, sticky, tar-like or road-asphalt consistency stool that clings to the toilet bowl and has a distinctive, overwhelmingly foul metallic/rotting odor. Upper GI Tract in 90% of cases (or slow bleeding from the small intestine / right ascending colon). Requires only \(50\text{ to }100\text{ milliliters}\) (\(3\text{ to }7\text{ tablespoons}\)) of blood in the stomach to turn stools black. Blood remains in the digestive tract for at least 8 to 14 hours as it travels 6 meters (20 feet) through the small and large intestines; gut bacteria break hemoglobin down into black hematin and foul-smelling sulfur compounds. (Note: Stools can stay black for 3 to 5 days even after bleeding stops). URGENT EMERGENCY DEPARTMENT EVALUATION: Same-day hospital admission required for blood count check and upper endoscopy. Call 911 / 112 / 999 if dizzy, pale, or on blood thinners.
4. Hematochezia (Bright red or dark maroon blood from the rectum) Toilet water turned bright red, dark maroon clots filling the bowl, or bright red streaks on toilet paper. Lower GI Tract (Colon, Rectum, or Anus) in 85% of cases (diverticulosis, angiodysplasia, colitis, cancer, hemorrhoids). HOWEVER, 10% to 15% of massive hematochezia is actually a torrential Upper GI Bleed (>1,000 mL). Either the bleeding originates low down in the colon/rectum where there is no acid, OR an upper GI ulcer/varix is bleeding so fast (\(>1\text{ liter}\)) that liquid blood rushes through the entire gut in under 2 hours without turning black (cathartic effect of blood). CRITICAL IF LARGE VOLUME OR DIZZY: Call 911 / 112 / 999 for any maroon clots, bowl-filling red blood, or dizziness. Small paper streaks require doctor check.

False alarms: Distinguishing real blood from harmless food and pill staining ("Pseudomelena")

Before panicking over a dark or reddish stool in someone who feels completely healthy, has a normal pulse, and has zero dizziness or stomach pain, check whether they consumed one of these common mimics in the past 24 to 48 hours:

  • Harmless mimics of Black Melena ("Pseudomelena"):
  • Oral Iron Supplements (ferrous sulfate, ferrous gluconate, prenatal vitamins): Turns stool a dark greenish-black color, but the stool has a normal firm shape (not sticky tar) and lacks the putrid odor of digested blood.
  • Bismuth Subsalicylate (Pepto-Bismol, Kaopectate): Bismuth reacts with trace sulfur in saliva and the gut to form black bismuth sulfide, often turning both the tongue and the stool jet-black.
  • Activated Charcoal, black licorice, dark chocolate/Oreo cookies, or large bowls of dark blueberries/grape juice.
  • Harmless mimics of Red Hematochezia / Red Vomit:
  • Red beets (beetroot / betanin pigment), red gelatin (Jell-O), Kool-Aid / red sports drinks, red food coloring, dragon fruit, or large amounts of tomato sauce.
  • Safety Rule: If there is any doubt whatsoever, or if the person has stomach pain, weakness, sweating, or takes aspirin/blood thinners, always treat it as real blood and go to the emergency room (a 30-second hospital fecal occult blood test or hemoglobin fingerstick settles the question immediately).

Major causes of upper and lower gastrointestinal bleeding

Understanding what causes GI bleeding helps you give critical medical history to the paramedics and emergency physicians.

1. Upper GI bleeding causes (75% to 80% of all hospital GI bleed admissions)

  • Peptic Ulcer Disease (Gastric and Duodenal Ulcers: 40% to 50% of upper bleeds): Deep sores eroded into the lining of the stomach or the first part of the small intestine (duodenum), almost always caused by either Helicobacter pylori (H. pylori) bacterial infection or regular use of NSAID painkillers (ibuprofen, naproxen, aspirin, meloxicam, diclofenac). When an ulcer eats down into the submucosa and erodes the wall of a major artery (such as the gastroduodenal artery behind the duodenum or the left gastric artery), blood spurts under high arterial pressure into the gut.
  • Esophageal and Gastric Varices (10% to 20% of upper bleeds, highest mortality): In patients with liver cirrhosis (from alcohol use, hepatitis B or C, or fatty liver disease) or portal vein thrombosis, scar tissue in the liver blocks normal blood flow from the intestines. Pressure backs up in the portal vein (portal hypertension), forcing blood to detour through fragile, thin-walled veins lining the lower esophagus and upper stomach (varices), which swell like varicose veins. When a varix ruptures, it causes painless, explosive projectile vomiting of hundreds of milliliters of bright red blood and carries a 15 to 20 percent mortality rate per bleeding episode.
  • Mallory-Weiss Tear (5% to 15% of upper bleeds): A longitudinal laceration (tear) in the mucous membrane at the junction where the esophagus meets the stomach, triggered by violent retching, dry heaving, or forceful vomiting (after heavy alcohol binge drinking, food poisoning, hyperemesis gravidarum in pregnancy, or severe coughing fits). Typically starts with several episodes of normal vomit followed suddenly by bright red blood.
  • Erosive Gastritis, Esophagitis, and Dieulafoy Lesion: Diffuse pinpoint bleeding from severe acid reflux, alcohol, critical illness stress, or a Dieulafoy lesion (an abnormally large, tortuous submucosal artery in the upper stomach that erodes through a tiny mucosal defect and causes massive sudden bleeding without an ulcer).

2. Lower GI bleeding causes (Colon, Rectum, and Small Bowel)

  • Diverticular Bleeding (Diverticulosis: 30% to 50% of brisk lower bleeds): Small pouch-like herniations (diverticula) in the muscular wall of the colon, common in adults over age 50, form right where penetrating nutrient arteries (vasa recta) pierce the colon wall. If a vasa recta artery inside a diverticulum ruptures, it causes sudden, completely painless passing of large amounts of maroon or bright red blood and clots into the toilet.
  • Angiodysplasia (Vascular Ectasias): Fragile, twisted tangles of dilated capillaries and veins in the wall of the right colon or small intestine, common in older adults, patients with chronic kidney disease, or patients with aortic stenosis (Heyde's syndrome).
  • Ischemic Colitis, Inflammatory Bowel Disease (Ulcerative Colitis / Crohn's Disease), and Infectious Colitis: Unlike diverticular bleeding (which is usually painless), these conditions cause crampy lower abdominal pain paired with bloody diarrhea or mucus.
  • Colorectal Polyps and Colorectal Cancer: Often cause slow, chronic oozing leading to iron-deficiency anemia, or intermittent dark red streaks mixed into the stool fabric.
  • Hemorrhoids and Anal Fissures: Swollen veins in the anal canal (hemorrhoids) or a sharp tear in the anal lining (fissure, which feels like "passing broken glass" during a bowel movement) typically coat the outside of the stool or drip bright red blood onto the toilet paper after wiping.

Recognizing hypovolemic shock: Why normal blood pressure can be a lie

An average adult body holds roughly 5,000 milliliters (5 liters / 10.5 pints) of blood. When a person bleeds into their stomach or colon, the body immediately unleashes a rush of adrenaline to constrict blood vessels and speed up the heart, which keeps systolic blood pressure looking deceptively normal until the patient has already lost 30 percent (1.5 liters) of their blood.

Use the American College of Surgeons Advanced Trauma Life Support (ATLS) hemorrhage classification to recognize early shock before blood pressure crashes:

Hemorrhage Class Estimated Blood Volume Lost (Adult 70 kg / 154 lb) Heart Rate (Pulse) Supine Blood Pressure Postural (Sitting/Standing) Hemodynamic Signs & Mental Status
Class I (Mild: <15% loss) Up to \(750\text{ mL}\) (\(<1.5\text{ pints}\)) Normal (\(<100\text{ bpm}\)) Κανονικό Normal; mild anxiety. No dizziness when standing.
Class II (Moderate: 15% to 30% loss) \(750\text{ to }1,500\text{ mL}\) (\(1.5\text{ to }3\text{ pints}\)) Elevated (\(100\text{ to }120\text{ bpm}\)) Normal when lying flat (narrowed pulse pressure) Postural dizziness / fainting when sitting or standing; pale cool hands and feet; delayed capillary refill (\(>2\text{ sec}\)); mild-to-moderate anxiety.
Class III (Severe: 30% to 40% loss) \(1,500\text{ to }2,000\text{ mL}\) (\(3\text{ to }4\text{ pints}\)) Fast & thready (\(120\text{ to }140\text{ bpm}\)) Dropped (\(<90\text{ mmHg}\) systolic) Marked confusion, restlessness, agitation; cold clammy sweat; rapid respiratory rate (\(30\text{ to }40\text{ breaths/min}\)); scant urine.
Class IV (Exsanguinating: >40% loss) Over \(2,000\text{ mL}\) (\(>4\text{ pints}\)) Over \(140\text{ bpm}\) (or terminal bradycardia) Critically low or unmeasurable Lethargy, stupor, coma, ashen-gray skin, gasping breathing, imminent cardiac arrest.

Two Diagnostic Traps: Beta-Blockers and the Early Hemoglobin Illusion

  1. Beta-Blocker Masking: Many older patients with heart disease or liver cirrhosis (who take propranolol, nadolol, or carvedilol to prevent variceal bleeding, or metoprolol/atenolol for blood pressure) cannot raise their heart rate above 70 or 80 beats per minute even when losing 2 liters of blood. Never assume a patient on beta-blockers is stable just because their pulse is 72 bpm: look for pale cold skin, sweating, and postural dizziness.
  2. The Early Hemoglobin Illusion: If someone suddenly loses 1.5 liters of whole blood from an ulcer, they lose red blood cells and liquid plasma in the exact same ratio. If a paramedic or doctor checks their hemoglobin concentration during the first hour, it may read a completely normal \(14\text{ g/dL}\) (\(140\text{ g/L}\)); hemoglobin only drops hours later after interstitial fluid shifts into the veins or IV saline dilutes the remaining blood.

Critical warnings: Liver cirrhosis, blood thinners, and aortoenteric fistula

If the bleeding person belongs to any of these three high-risk groups, inform the 911 / 112 / 999 dispatcher and paramedics in your very first sentence:

  1. Known or Suspected Liver Disease / Cirrhosis (Variceal Bleed): Look for yellow eyes/skin (jaundice), a swollen fluid-filled belly (ascites), spider-like red blood vessels on the chest (spider angiomata), or red palms (palmar erythema). Variceal hemorrhage requires immediate hospital intravenous terlipressin or octreotide (to lower portal vein pressure), prophylactic IV antibiotics (ceftriaxone), and emergency endoscopic variceal band ligation within 12 hours.
  2. Patients Taking Anticoagulants ("Blood Thinners") or Dual Antiplatelet Therapy: Check if the patient takes warfarin (Coumadin), Direct Oral Anticoagulants (apixaban [Eliquis], rivaroxaban [Xarelto], edoxaban [Savaysa], dabigatran [Pradaxa]), or antiplatelets (clopidogrel [Plavix], prasugrel, ticagrelor, aspirin). Hospital emergency departments carry specific intravenous reversal antidotes (4-factor Prothrombin Complex Concentrate [Kcentra/Beriplex] + IV Vitamin K for warfarin; andexanet alfa for apixaban/rivaroxaban; idarucizumab [Praxbind] for dabigatran) that can stop fatal bleeding if the medical team knows which pill the patient takes and when the last dose was swallowed.
  3. Prior Abdominal Aortic Aneurysm (AAA) Graft Surgery ("Herald Bleed" of an Aortoenteric Fistula): If a person who has ever had an open or endovascular surgical graft placed on their abdominal aorta develops any gastrointestinal bleed (even a small, self-stopping episode of hematemesis or melena), it is a surgical emergency until proven otherwise. A synthetic aortic graft can slowly erode into the wall of the adjacent third part of the duodenum, creating an aortoenteric fistula; a small "herald bleed" is often followed hours or days later by catastrophic, fatal exsanguination directly from the aorta into the gut unless repaired in the operating room.

Even so, a doctor must evaluate the child to rule out true mucosal bleeding, Meckel's diverticulum (painless brick-red rectal bleeding in toddlers), or juvenile polyps.

  1. Two Large-Bore Intravenous Lines and Restrictive Blood Transfusion: Emergency nurses immediately place two large-bore intravenous cannulas (16-gauge or 18-gauge) into the forearm or antecubital veins, draw blood for a complete blood count, coagulation panel (INR), lactate, and blood type/crossmatch, and infuse warm balanced crystalloid fluids (Ringer's lactate or \(0.9\%\) normal saline). Modern randomized clinical trials (validated by the ACG, ESGE, and UK NICE) show that a restrictive packed red blood cell transfusion threshold (transfusing when hemoglobin drops to \(7\text{ g/dL}\) [\(70\text{ g/L}\)] in most patients, or \(8\text{ g/dL}\) [\(80\text{ g/L}\)] in patients with active coronary heart disease) saves more lives than over-transfusing, because over-transfusing raises portal and arterial blood pressure and blows off newly formed clots.
  2. Glasgow-Blatchford Score (GBS) and Intravenous Medication: Doctors calculate the Glasgow-Blatchford Score (using blood urea nitrogen, hemoglobin, systolic blood pressure, pulse rate, melena, syncope, liver disease, and heart failure) to determine severity. Patients receive high-dose intravenous proton pump inhibitors (such as pantoprazole or esomeprazole) to neutralize stomach acid so platelets can clot, prokinetic erythromycin (given 30 to 60 minutes before endoscopy to clear blood clots from the stomach view), and octreotide or terlipressin plus IV ceftriaxone if liver cirrhosis is suspected.
  3. Endoscopic Hemostasis, Interventional Radiology, or Balloon Tamponade: Within 12 to 24 hours (or immediately after airway intubation in unstable patients), a gastroenterologist passes a flexible fiber-optic esophagogastroduodenoscope (EGD) down the throat to locate the bleeding vessel and seal it using metal hemoclips, thermal multipolar electrocoagulation, epinephrine injection, hemostatic powder spray (Hemospray), or elastic variceal rubber band ligation. If bleeding cannot be stopped endoscopically, interventional radiologists perform transcatheter arterial embolization (TAE) (threading a microcatheter through the femoral or radial artery to plug the bleeding branch with coils) or place a transjugular intrahepatic portosystemic shunt (TIPS) for refractory varices.

Why does vomiting dark "coffee-ground" material mean someone is bleeding internally?

When blood leaks slowly or moderately into the stomach from a peptic ulcer or gastritis, the hydrochloric acid and digestive enzymes inside the stomach chemically react with the iron inside red hemoglobin molecules, oxidizing bright red hemoglobin into dark brown acid hematin within 15 to 60 minutes. When vomited up, these clumped, oxidized blood proteins look identical to wet, dark brown used coffee grounds and represent a true upper gastrointestinal bleeding emergency that requires hospital evaluation.

How can I tell if jet-black stools are real internal bleeding (melena) or just from taking Pepto-Bismol or iron pills?

True melena caused by digested blood (from at least 50 to 100 milliliters of bleeding in the stomach or duodenum) has a very distinct appearance and smell: the stool is shiny, sticky, and loose like wet road tar, clings stubbornly to the porcelain toilet bowl, and produces an unmistakable, overwhelmingly foul metallic odor caused by bacterial breakdown of hemoglobin. In contrast, dark stools caused by iron supplements, bismuth subsalicylate (Pepto-Bismol), or dark foods are usually greenish-black or dull gray-black, keep a normal firm stool shape, and do not have a foul metallic odor; however, if you have any doubt or feel dizzy, weak, or have stomach pain, always go to the emergency room.

Why shouldn't I give a glass of cold water or milk to someone who just vomited blood to rinse their mouth and calm their stomach?

You must keep anyone with suspected upper gastrointestinal bleeding strictly Nothing by Mouth (NPO) for two life-saving reasons. First, pouring liquid into a bleeding stomach triggers nausea and further vomiting while washing away fragile blood clots. Second, to stop the bleeding, hospital gastroenterologists must perform an emergency upper endoscopy (passing a camera down the throat under sedation or general anesthesia); if the patient has water or milk sitting in their stomach, they can regurgitate and inhale that liquid into their lungs during intubation. If their mouth tastes terrible, let them gently swish a tiny sip of water and spit it completely out without swallowing.

Can bright red blood in the toilet bowl ever come from a bleeding stomach ulcer instead of hemorrhoids or the colon?

Yes. While roughly 85 percent of bright red or maroon rectal bleeding (hematochezia) comes from the lower digestive tract (colon, rectum, or anus), about 10 to 15 percent of massive bright red rectal bleeding actually comes from a torrential upper GI bleed in the stomach or duodenum. When an arterial ulcer or esophageal varix bleeds more than 1,000 milliliters rapidly, the liquid blood acts as a natural laxative and rushes through the entire small and large intestine in under 2 hours before stomach acid or bacteria can turn it black; these patients are almost always dizzy, pale, sweaty, and have a fast heart rate.

Why is taking ibuprofen, naproxen, or aspirin so dangerous if someone has stomach pain and dark stools?

Nonsteroidal anti-inflammatory drugs (NSAIDs) like ibuprofen (Advil, Motrin), naproxen (Aleve), meloxicam, diclofenac, and aspirin work by blocking cyclooxygenase (COX-1) enzymes. In the stomach, COX-1 produces the protective prostaglandins that maintain the stomach's mucus shield and bicarbonate layer; in the bloodstream, COX-1 allows platelets to stick together to plug bleeding vessels. Taking an NSAID or aspirin during an active ulcer bleed strips away the stomach's defense and disables blood clotting at the exact same time, which can turn a minor ooze into a fatal arterial hemorrhage.

What is the safest position for someone with a severe GI bleed while waiting for the ambulance?

Positioning depends entirely on whether they are vomiting. If the person is actively vomiting blood, retching, or becoming drowsy or unconscious, roll them onto their left side in the lateral recovery position with their chin angled downward so blood drains out of their mouth and cannot be inhaled into their lungs. If they are fully awake, alert, and NOT nauseated or vomiting, lay them flat on their back on the floor or bed and keep them warm with a blanket; never let them sit upright on the toilet or try to walk to the front door, as standing up can cause immediate fainting and circulatory collapse.
