Jigar etishmovchiligi chalkashliklari (jigar ensefalopatiyasi) va varikoz qon ketishi¶
Emergency numbers differ by country. This page uses 911, the number for the United States and Canada. Many other countries use 112, 999, or another number. Find the number for your country.
Patients living with chronic liver disease, cirrhosis, or acute liver failure face three life-threatening decompensation emergencies that can erupt without warning at home: ruptured gastroesophageal varices (massive upper gastrointestinal hemorrhage where engorged veins in the lower esophagus burst open), hepatic encephalopathy (acute neuropsychiatric confusion, personality change, flapping hand tremors, and coma caused by ammonia and gut neurotoxins bypassing the scarred liver and swelling the brain), and spontaneous bacterial peritonitis (SBP) (a rapid, life-threatening bacterial infection of the fluid inside a swollen abdomen).
Your immediate responsibilities when caring for a liver patient in crisis are to call emergency medical services immediately, protect the airway by rolling a vomiting or drowsy patient onto their side in the recovery position (as drowning in aspirated blood is a major cause of sudden death in variceal rupture), enforce strict NPO (nothing by mouth) if active bleeding or severe drowsiness is present, test for the telltale "flapping hand tremor" (asterixis) to identify early hepatic encephalopathy, and strictly avoid giving sleeping pills, sedatives, ibuprofen/NSAIDs, or excess acetaminophen.
Buni hozir qiling¶
- Zudlik bilan tez tibbiy yordam xizmatlariga qo'ng'iroq qiling: Mahalliy favqulodda yordam raqamiga qo'ng'iroq qiling (AQSh va Kanadada 911, Yevropa Ittifoqi bo'ylab 112, Buyuk Britaniyada 999, Avstraliyada 000) yoki bizning ma'lumotnomamizni tekshiring. favqulodda raqamlar. State clearly that the patient has cirrhosis or liver disease and is either vomiting blood, passing black tarry stools, acting confused/unrousable, or running a fever with belly pain.
- If vomiting blood or drowsy, place in the side-lying recovery position immediately: Never let a patient who is vomiting blood or drowsy from hepatic encephalopathy lie flat on their back. Roll them onto their left side with their chin tilted slightly downward toward the floor so gushing blood, dark "coffee-ground" vomit, or saliva drains straight out of the mouth rather than choking the lungs.
- Enforce strict NPO (nothing by mouth) during bleeding or severe drowsiness: Do not give water, milk, food, or oral pills if the patient is vomiting blood, passing black tarry stools, or too drowsy to sit up and converse clearly. Hospital gastroenterologists will need an empty stomach to perform emergency endoscopic variceal band ligation within hours of arrival.
- Check for the "flapping hand tremor" (asterixis) if the patient seems confused or irritable: Ask the awake patient to stretch both arms straight out in front of them, bend their wrists backward ("like a traffic officer stopping traffic"), and spread their fingers wide apart for 15 to 30 seconds. If their hands and wrists involuntarily drop forward and jerk back up in a rhythmic flapping motion like a bird's wings, they are in acute hepatic encephalopathy.
- Never give sleeping pills, anxiety sedatives, opioids, or NSAID painkillers: Never give a restless, insomniac, or agitated liver patient benzodiazepines (lorazepam, alprazolam, diazepam), sleep aids (zolpidem, diphenhydramine), opioid painkillers, or NSAIDs (ibuprofen, naproxen, aspirin). Sedatives plunge a patient with hepatic encephalopathy straight into an unrousable coma, while NSAIDs trigger fatal variceal bleeding and immediate kidney shutdown (hepatorenal syndrome).
- Keep the patient warm and gather all liver medications: Cover the patient with warm dry blankets to prevent hypothermia (which further paralyzes blood clotting in cirrhosis), and collect their bottles of lactulose, rifaximin, beta-blockers (propranolol, nadolol, carvedilol), and diuretics (spironolactone, furosemide) for paramedics.

Ruptured gastroesophageal varices: why cirrhosis causes catastrophic bleeding¶
In a healthy body, all nutrient-rich blood draining from the stomach, intestines, pancreas, and spleen flows through a large main vessel called the portal vein into the soft, spongy liver to be filtered before returning to the heart. When chronic liver disease (from alcohol, viral hepatitis B or C, metabolic dysfunction-associated steatohepatitis / MASH, or autoimmune disease) replaces soft liver tissue with hard, fibrous scar tissue (cirrhosis), blood can no longer flow easily through the liver.
Pressure inside the portal vein backs up like cars behind a blocked highway tunnel, a condition called portal hypertension. Desperate to find a detour back to the heart, pressurized portal blood forces open tiny, fragile, thin-walled veins lining the bottom of the esophagus (swallowing tube) and the top of the stomach. Over months, these delicate veins balloon into twisted, high-pressure ropes called esophageal and gastric varices.
Two reasons variceal bleeding is far more lethal than a normal stomach bleed¶
- High portal venous pressure: Unlike a small peptic ulcer, an esophageal varix is under continuous high hydrostatic pressure; when its thin wall ruptures, blood gushes into the esophagus and stomach at rates of 100 to 500 milliliters per minute.
- Coagulopathy and thrombocytopenia of liver failure: A healthy liver manufactures the body's blood-clotting proteins (prothrombin, fibrinogen, Factors V, VII, IX, X) while an enlarged spleen in portal hypertension traps and destroys blood platelets. Consequently, when a varix bursts in a cirrhotic patient, the blood lacks both the platelets and the clotting factors needed to plug the hole on its own.
Warning signs of ruptured varices (Even before blood is vomited)¶
A patient does not always vomit bright red blood right away when a varix begins leaking into the stomach:
- Hematemesis (Vomiting blood): Effortless, large-volume vomiting of bright red liquid blood, dark red maroon blood clots the size of plums, or dark brown granular material that looks like wet coffee grounds (gastrointestinal bleeding).
- Melena (Black, tarry, foul-smelling stools): Blood that pools in the stomach and passes down through the intestines is digested by stomach acid and gut bacteria into jet-black, sticky, shiny stools that look like road tar and have an unmistakable, pungent metallic odor. Passing black tarry stools in a patient with cirrhosis is an active upper GI hemorrhage, often the sole cause of sudden confusion or fainting even when zero vomiting has occurred.
- Hematochezia (Maroon or bright red blood from the rectum): If a variceal bleed is extraordinarily fast (over 1,000 milliliters), blood races through the entire intestinal tract so quickly that it exits the rectum still looking dark red or maroon, accompanied by profound dizziness and shock.
- Painless sudden fainting (Syncope) and cold sweat: Because blood filling the stomach acts as a powerful cathartic, many patients suddenly feel an urgent need to use the toilet, walk into the bathroom, and collapse unconscious on the bathroom floor before anyone realizes they are bleeding internally.
Step-by-step first aid for active variceal hemorrhage¶
- Summon paramedics immediately and unlock the front door: Every minute counts; hospital survival depends on receiving intravenous vasoactive drugs (octreotide or terlipressin to drop portal vein pressure), intravenous prophylactic antibiotics (ceftriaxone), cautious blood transfusion, and urgent endoscopic variceal band ligation within 12 hours.
- Protect the airway in the Left Lateral Recovery Position:
- Roll the patient onto their left side on the floor or bed.
- Angle their face slightly downward over a basin or towel so blood and clots pour straight out of the lower cheek.
- Use a gloved finger wrapped in a cloth to sweep large, jelly-like blood clots out of the front of the mouth if they are gagging. Never place a vomiting patient flat on their back, and never prop them sitting upright if they are dizzy or faint, as sitting upright causes cerebral hypoperfusion and sudden syncope with aspiration.
- Keep the patient warm with dry blankets: Wrap the torso and legs in warm blankets. Hypothermia below 36.0 degrees Celsius (96.8 degrees Fahrenheit) deactivates the few remaining clotting enzymes and platelets the patient has left.
- Never give water, ice chips, antacids, or blood pressure pills by mouth: Even if the patient begs for water to wash the taste of blood out of their mouth, only wipe their lips with a damp cloth. Giving water fills the stomach with fluid that obscures the gastroenterologist's endoscopic camera and triggers repeat vomiting and aspiration.
Hepatic encephalopathy (HE): why liver failure causes confusion and coma¶
Sog'lik nuqtai nazaridan, yo'g'on ichakda yashovchi bakteriyalar oziq-ovqat oqsillarini parchalaydi va zaharli azot chiqindilarini, birinchi navbatda, ammiak (\(NH_3\)), merkaptanlar, qisqa zanjirli yog' kislotalari va marganetsni ishlab chiqaradi. Odatda, portal vena qoni bu ammiakning barchasini to'g'ridan-to'g'ri jigarga olib boradi, u erda sog'lom jigar hujayralari (gepatotsitlar) karbamid sikli orqali toksik ammiakni zararsiz karbamidga aylantiradi, shuning uchun buyraklar uni siydik bilan chiqarib yuborishi mumkin.
sirroz yoki o'tkir jigar etishmovchiligida ikkita narsa noto'g'ri bo'ladi: chandiqli jigar hujayralari ammiakni tozalash qobiliyatining 80 foizini yo'qotadi va portal gipertenziya ichak qonini kollateral shuntlar (yoki jarrohlik yo'li bilan o'rnatilgan TIPS stent: transjugular intrastemik shunt) orqali jigarni butunlay chetlab o'tishga majbur qiladi. Filtrlanmagan ammiak to'g'ridan-to'g'ri tizimli arteriyalarga tushadi va qon-miya to'sig'ini kesib o'tadi. Miya ichida astrositlar deb ataladigan qo'llab-quvvatlovchi miya hujayralari glutamatni glutamin ga aylantirish orqali ammiakni zararsizlantirishga harakat qiladi. Glutamin astrositlar ichidagi osmotik shimgich kabi harakat qilib, miya hujayralariga suv tortadi va diffuz miya shishi (miya shishi) va kuchli neyrokimyoviy inhibisyonga (jigar ensefalopatiyasi) sabab bo'ladi.

Never accuse a slurring, stumbling cirrhosis patient of drinking alcohol
Grade 2 and Grade 3 hepatic encephalopathy produce slurred speech, unsteady stumbling gait (ataxia), glassy eyes, mood swings, and sometimes a sweet, musty breath odor (fetor hepaticus, caused by dimethyl sulfide and ketones escaping through the lungs). Bystanders and even uninformed responders frequently mistake acute hepatic encephalopathy for acute alcohol intoxication and leave the person to "sleep it off." Leaving a patient in Grade 2 or 3 hepatic encephalopathy to "sleep it off" on a couch is frequently fatal, as their brain swelling worsens into Grade 4 coma or an underlying silent gastrointestinal bleed or sepsis kills them overnight. Always treat new slurred speech or confusion in a liver patient as a life-threatening medical emergency.
How to perform the bedside "Asterixis" (Flapping Tremor) test¶
Asterixis is not actually a tremor of contracting muscles; neurophysiologically, it is negative myoclonus, a brief, sudden 50-to-200-millisecond electrical pause (shutoff) in the motor cortex centers that maintain posture against gravity, caused by ammonia poisoning.
- Have the patient sit upright in bed or a chair (or lie propped up) and extend both arms straight out in front of their chest with their elbows straight.
- Instruct them to bend both wrists sharply backward (dorsiflexion, palms facing forward as if pushing against an invisible wall) and spread their fingers wide apart.
- Tell them to close their eyes and hold that exact posture steadily for 30 seconds.
- What a positive test looks like: After a latency of 5 to 15 seconds, the fingers and wrist suddenly drop forward a few centimeters as muscle tone vanishes for a split second, then immediately jerk back upward as the patient catches it, repeating rhythmically like a bird flapping its wings ("liver flap").
- Alternative test for weak patients: If the patient is too weak to hold both arms up in the air, let their forearms rest flat on the mattress and simply lift their hands upward at the wrist for 20 seconds, or ask them to squeeze two of your fingers steadily ("milkmaid's grip").
The 6 hidden triggers that precipitate hepatic encephalopathy¶
Hepatic encephalopathy almost never appears out of thin air in a previously stable cirrhosis patient; in more than 80 percent of hospital admissions, a specific acute medical insult triggered the ammonia surge. Emergency physicians search immediately for these six classic precipitants (and caregivers should tell paramedics which ones occurred over the past 72 hours):
- Silent Gastrointestinal Bleeding (A massive "protein meal" of blood): Blood is packed with hemoglobin protein (100 milliliters of blood contains roughly 15 to 20 grams of pure protein). When a varix or stomach ulcer bleeds silently into the digestive tract, gut bacteria feast on the digested blood protein and convert it into a massive wave of ammonia. Always check the toilet or ask about black, tarry stools (melena) whenever a liver patient becomes confused.
- Infection (Especially Spontaneous Bacterial Peritonitis, UTI, or Pneumonia): Systemic inflammation and bacterial toxins make the blood-brain barrier leaky and multiply ammonia neurotoxicity fivefold.
- Constipation and missed Lactulose doses: If stool sits inside the colon for 24 to 48 hours instead of being flushed out 2 to 3 times daily, colonic bacteria produce and absorb vastly more ammonia.
- Dehydration, Over-Diuresis, Vomiting, or Diarrhea (Electrolyte & Kidney Crisis): Taking too high a dose of prescribed water pills (furosemide and spironolactone), sweating in heat, or having a stomach bug causes acute kidney injury, hypokalemia (low blood potassium), and metabolic alkalosis. Low blood potassium directly stimulates the kidneys to manufacture extra ammonia, while alkalosis converts ammonium ions (\(NH_4^+\), which cannot cross into the brain) into uncharged ammonia gas (\(NH_3\), which crosses straight into the brain).
- Central Nervous System Depressant Medications: Taking even a single dose of a benzodiazepine (lorazepam, diazepam, clonazepam), sleeping pill (zolpidem), sedating antihistamine (diphenhydramine, hydroxyzine), or opioid painkiller (oxycodone, morphine, tramadol).
- Acute Portal Vein Thrombosis or New TIPS Shunt: A fresh blood clot blocking the portal vein or a recently placed liver shunt.
Understanding Lactulose and Rifaximin (And when NEVER to give oral Lactulose)¶
Patients with cirrhosis and prior hepatic encephalopathy are routinely prescribed two daily maintenance medications to clear ammonia from the gut:
- Lactulose (or Lactitol): A thick, sweet, non-absorbable synthetic sugar syrup. When lactulose reaches the colon, beneficial gut bacteria ferment it into lactic acid and acetic acid. This acidification drops the pH inside the colon, which chemically traps diffusible ammonia (\(NH_3\)) by adding a hydrogen ion to turn it into charged ammonium (\(NH_4^+\)). Because charged ammonium cannot pass through the intestinal wall into the bloodstream, it remains trapped inside the stool and is flushed out of the body by lactulose's osmotic laxative action. Doctors titrate daily lactulose doses so the patient passes 2 to 3 soft bowel movements every single day.
- Rifaximin: A non-absorbable oral antibiotic that stays strictly inside the intestines and reduces the population of ammonia-producing gut bacteria.
Critical safety rule during an acute encephalopathy crisis at home¶
- If the patient is in early Grade 1 or mild Grade 2 HE (wide awake, sitting up, coughing normally, and able to drink water without choking) and has missed their lactulose or is constipated, calling their hepatologist or emergency dispatch may include giving their prescribed oral sick-day lactulose dose while arranging medical evaluation.
- NEVER pour oral lactulose (or any liquid) into the mouth of a Grade 3 (stuporous/drowsy) or Grade 4 (comatose) patient: Once a patient is drowsy, slurring heavily, or hard to wake up, their gag reflex is blunted. Pouring thick, sticky lactulose syrup into their mouth causes immediate pulmonary aspiration into the lungs. Paramedics and hospital nurses treat Grade 3 and Grade 4 patients safely using lactulose retention enemas or a protected nasogastric tube after securing the airway.
Spontaneous Bacterial Peritonitis (SBP) and tense ascites emergencies¶
Many patients with decompensated cirrhosis accumulate liters of straw-colored fluid inside their abdominal cavity (ascites), causing a tight, swollen, distended belly. Because ascitic fluid has very low levels of protective immune proteins (complement), normal bacteria from the patient's own intestines can migrate microscopically through the bowel wall (bacterial translocation) and infect the belly fluid without any bowel perforation occurring. This life-threatening infection is called Spontaneous Bacterial Peritonitis (SBP) and carries a 20 to 30 percent mortality rate if intravenous antibiotics are delayed by even a few hours.
Red flags of Spontaneous Bacterial Peritonitis (SBP)¶
Unlike surgical peritonitis from a burst appendix (which makes the belly board-hard), the fluid cushion of ascites in a cirrhosis patient prevents the abdominal muscles from feeling rigid. Look for any one of these subtle warning signs in a patient with ascites:
- Any fever above 37.8 degrees Celsius (100.0 degrees Fahrenheit) or hypothermia below 36.0 degrees Celsius (96.8 degrees Fahrenheit), or unexplained shivering chills.
- New diffuse abdominal pain, cramping, discomfort, or tenderness when pressing gently on the swollen belly, or a belly that suddenly feels tighter and more tense than usual.
- New or worsening confusion / hepatic encephalopathy with zero obvious cause (in up to 30 percent of SBP cases, confusion is the only outward symptom, with no fever and no belly pain).
- Unexplained drop in blood pressure, dizziness, or sudden decrease in urine output (hepatorenal syndrome, where SBP infection triggers the kidneys to shut down completely).
- **Darhol harakat: ** To'g'ridan-to'g'ri tez yordam bo'limiga boring. Kasalxona shifokorlari ultratovush tekshiruvi ostida ingichka igna yordamida qorin suyuqligining kichik namunasini olishadi (diagnostik parasentez) oq qon hujayralarini hisoblash uchun (bir kub millimetrga 250 polimorfonukulyar hujayralar SBPni tasdiqlaydi) va darhol vena ichiga uchinchi avlod sefalosporin albomi, shuningdek, buyraklar uchun antibiotiklarni tejaydilar.
Agar kindik churrasidan suyuqlik oqsa nima qilish kerak ("Toshqin sindromi")¶
Og'ir, tarang astsit bilan og'rigan bemorlarda qorin bo'shlig'idagi bosim ko'pincha qorin tugmachasini yupqa terili kindik churrasi ga chiqaradi. Agar qorin bo'shlig'i ustidagi terida qora qoraqo'tir yoki yara paydo bo'lsa va to'satdan yorilib ketsa, kiyimga bir necha litr shaffof sariq astsit suyuqlik oqishi mumkin (Toshqin sindromi).
- Kindik churrasining yorilishida tez yordam:
- Mahalliy shoshilinch raqamingizga qo'ng'iroq qiling yoki darhol tez yordam bo'limiga olib boring (agar bakteriyalar bu teshikdan * ichkariga kirsa, bir necha soat ichida halokatli peritonit paydo bo'ladi va suyuqlikning tez yo'qolishi qon bosimini pasaytiradi).
- Bemorni chalqanchasiga yotqizib yoki ozgina egilib yotqiz, shunda oqayotgan joy yuqoriga qaratiladi.
- Oqayotgan teshikni qalin, steril doka yoki yangi yuvilgan toza sochiq bilan mahkam bog'lab qo'ying (teshikka hech narsani surmang) va shifoxona xodimlari vena ichiga antibiotiklar, albumin yuborish va jarrohlik ta'mirlashni o'tkazmaguncha kaftning tashqi bosimini yumshoq ushlab turing.
Siroz bilan og'rigan har bir kishi uchun qat'iy dori xavfsizligi qoidalari¶
Yaralangan jigar dorilarni asta-sekin metabolizatsiya qiladi, portal gipertenziya esa oshqozon shilliq qavati va buyraklarni juda mo'rt qiladi. Siroz bilan og'rigan bemorga g'amxo'rlik qiluvchi har bir shifokor ushbu uchta uy dorivor qoidalariga rioya qilishi kerak:
- Barcha NSAIDlarni (Ibuprofen, Naproksen, Aspirin, Meloksikam, Diklofenak) mutlaq taqiqlash: Sirozda hatto bir nechta ibuprofen yoki naproksen tabletkalarini qabul qilish ham oshqozon shilliq qavatidan himoya prostaglandinlarni yo'q qiladi (halokatli varikozli arteriya yoki gipertenziyali arteriyalarni qo'zg'atadi) (o'tkir, ko'pincha o'limga olib keladigan gepatorenal sindromni qo'zg'atadi).
- Asetaminofen (Paratsetamol) uchun qat'iy doza chegarasi: Yuqori dozali asetaminofen (>4000 mg/kun) jigar etishmovchiligiga olib kelgan bo'lsa-da, gepatologiya bo'yicha ko'rsatmalar (AASLD va EASL) surunkali sirozli bemorlar uchun spirtli ichimliklarni iste'mol qilmaydigan, past dozali asetaminofenning chegaralangan dozasi 0 milligramm dan ortiq emasligini ta'kidlaydi. Qisqa kurslar uchun jami 24 soat uchun 500 mg muntazam ta'sirga ega tabletkalar NSAIDlarga qaraganda xavfsizroqdir, **agar bemorda o'tkir jigar etishmovchiligi, spirtli ichimliklarni faol iste'mol qilish yoki to'yib ovqatlanmaslik/ro'za tutish, bu holda nol asetaminofen to'g'ridan-to'g'ri shifokor ko'rsatmasisiz berilishi kerak.
- **Qusish, diareya yoki chalkashlik paytida diuretiklarni ("suv tabletkalari") ushlab turing: ** Agar bemorda bosh aylanishi, sarosimaga tushish, qusish yoki najas kuniga 4 dan ortiq bo'sh bo'lsa, shifokor qonda natriy, kaliy kislotasi va krep miqdorini tekshirmaguncha, spironolakton yoki furosemid ning ertalabki dozalarini hech qachon bermang.

Tez-tez beriladigan savollar¶
Why is vomiting blood or passing black tarry stools so dangerous in someone with cirrhosis?
In cirrhosis, scar tissue blocks normal blood flow through the liver (portal hypertension), forcing blood into fragile, swollen veins in the lower esophagus and stomach called varices. When a varix ruptures, high venous pressure combined with the liver's inability to make normal blood-clotting factors causes massive, rapid bleeding that appears as vomiting bright red blood, coffee-ground material, or passing jet-black tarry stools (melena). Call your local emergency number immediately and place the person on their left side so they do not choke on blood.
What is hepatic encephalopathy, and how do you check for the "flapping hand tremor" (asterixis)?
Hepatic encephalopathy occurs when a scarred liver can no longer filter toxic ammonia produced by gut bacteria out of the bloodstream, allowing ammonia to reach the brain and cause swelling, confusion, reversed sleep patterns, slurred speech, and coma. To check for asterixis, ask the person to hold both arms straight out in front of them, bend their wrists backward as if stopping traffic, and spread their fingers wide for 30 seconds; a positive test shows rhythmic dropping and jerking of the hands like a bird flapping its wings.
Why must you never pour oral lactulose syrup into the mouth of a very drowsy or unconscious liver patient?
While oral lactulose syrup is the main treatment for mild hepatic encephalopathy because it traps ammonia in the stool, a patient who has progressed to severe drowsiness or stupor (Grade 3 or Grade 4 encephalopathy) loses their protective gag and swallowing reflexes. Pouring thick lactulose syrup into the mouth of a drowsy patient causes them to inhale the sticky liquid into their lungs (aspiration pneumonia and suffocation); keep them on their side with nothing by mouth so hospital staff can administer lactulose safely via an enema or protected tube.
Why can a blood bleed in the stomach or constipation trigger sudden confusion in liver disease?
Whole blood is extremely rich in protein; when a stomach ulcer or esophageal varix bleeds into the digestive tract, intestinal bacteria digest that blood protein and release a massive surge of ammonia into the blood. Similarly, constipation keeps waste inside the colon longer, increasing bacterial ammonia absorption, which overwhelms the failing liver and triggers acute hepatic encephalopathy.
Why are ibuprofen (Advil) and naproxen (Aleve) strictly forbidden for people with cirrhosis?
In cirrhosis, the kidneys depend entirely on protective chemicals called prostaglandins to keep their blood vessels open against extreme circulatory stress. Nonsteroidal anti-inflammatory drugs (NSAIDs) like ibuprofen and naproxen block prostaglandins, which can instantly shut down the kidneys (hepatorenal syndrome) and cause life-threatening stomach or variceal bleeding.
