Deep Vein Thrombosis (DVT) vs
(DVT, Blood Clot in Deep Vein) |

| Visible Swelling (Circumference) | None or minimal. Both calves measure the same distance around. | Noticeable one-sided (unilateral) swelling: Use a flexible tape measure 10 cm (4 inches) below the kneecap (tibial tuberosity), a calf circumference 3 centimeters (1.2 inches) or more larger than the other leg is the single strongest physical sign of a DVT. |
| Skin Temperature & Color | Normal skin temperature and normal skin color. | The swollen leg feels distinctly warmer to the back of your hand than the other leg, and the skin looks reddish, shiny, dusky purple, or darkened/flushed, often with dilated superficial surface veins. |
| Effect of Rest vs Standing | Feels better when resting still; hurts primarily when contracting that specific muscle. | Constant deep, throbbing heaviness or fullness that gets worse when standing still or letting the leg hang down (as venous blood backs up behind the clot). |
| Pitting Edema | Absent. | Pressing your finger gently over the shinbone or ankle for 5 seconds leaves a visible dimple/indentation ("pitting edema") that stays after you lift your finger. |
Calculating your clinical risk: the Modified Wells Score for DVT¶
In hospital emergency departments worldwide, physicians use the Wells Criteria for DVT to assess the probability that a painful or swollen leg is a blood clot. You can review these 10 clinical risk factors to understand why doctors take specific leg symptoms so seriously (each item adds +1 point):
- Active cancer (treatment within the past 6 months or palliative care): +1 point.
- Paralysis, paresis, or recent plaster cast / walking boot immobilization of the leg: +1 point.
- Recently bedridden for 3 or more days, OR major surgery within the past 12 weeks requiring general or regional anesthesia: +1 point.
- Localized tenderness along the deep venous system (down the center-back of the calf or along the inner thigh groin crease): +1 point.
- Entire leg swollen (from hip/thigh down to the ankle): +1 point.
- Calf swelling at least 3 centimeters (1.2 inches) larger in circumference than the symptom-free leg (measured 10 cm below the tibial tuberosity): +1 point.
- Pitting edema confined to the symptomatic leg: +1 point.
- Collateral superficial veins (visible, non-varicose surface veins standing out on the swollen leg as blood detours around the deep blockage): +1 point.
- Previously documented DVT in the past: +1 point.
- (Note: If an alternative diagnosis, such as cellulitis or a known tear, is at least as likely as DVT, physicians subtract 2 points; a score of 2 or higher indicates high clinical probability requiring immediate ultrasound imaging).
Beware of Phlegmasia Cerulea Dolens (The Blue Painful Leg)
In rare, severe cases of massive iliofemoral DVT where almost every deep and collateral vein draining the leg is completely clotted shut, venous pressure inside the leg climbs so high that arterial blood can no longer enter the limb. The entire leg swells massively, turns deep cyanotic blue-purple, becomes excruciatingly painful, and develops blisters or loss of sensation (Phlegmasia Cerulea Dolens). This is a limb- and life-threatening vascular surgical emergency that leads to venous gangrene, amputation, and hypovolemic shock within hours unless treated immediately in a hospital with catheter-directed thrombolysis or surgical thrombectomy.
Recognizing a Pulmonary Embolism (PE): when the clot reaches the lungs¶
When a fragment of a DVT breaks free (embolizes), it is swept upward through the large veins of the abdomen into the right side of the heart, which pumps it straight into the branching pulmonary arteries of the lungs.
Importantly, up to 40 percent of patients diagnosed with a life-threatening Pulmonary Embolism never had noticeable leg pain or swelling beforehand, either because the clot formed silently in the deep pelvic veins, or because the entire leg clot broke off at once and traveled to the lungs.
How a Pulmonary Embolism kills (Right Heart Strain, not just low oxygen)¶
A common misconception is that a Pulmonary Embolism only hurts you by lowering oxygen levels in a piece of lung tissue. In reality, a large Massive or Submassive ("Saddle") Pulmonary Embolism kills through acute Obstructive Shock and Right Ventricular Failure:
- When a large clot plugs 50% or more of the pulmonary arterial tree, the thin-walled Right Ventricle of the heart suddenly has to pump against a brick wall of pressure.
- The right ventricle balloons outward, strains, and fails, so almost no blood can pass through the lungs to reach the Left Ventricle.
- Because the left ventricle comes up empty, systemic blood pressure to the brain and coronary arteries crashes to zero, causing sudden fainting (syncope), crushing chest pain, and Pulseless Electrical Activity (PEA) cardiac arrest.
Symptoms of Pulmonary Embolism vs Heart Attack comparison table¶
| Symptom Dimension | Acute Pulmonary Embolism (PE) | Acute Myocardial Infarction (Heart Attack) |
|---|---|---|
| Hallmark #1 Symptom | Sudden, unexplained Shortness of Breath (Dyspnea) at rest or with minimal exertion (present in 80%+ of cases), starting within seconds or minutes. | Central chest pressure, squeezing, fullness, or tightness ("elephant sitting on chest"), often radiating to the left arm, jaw, neck, or back. |
| Character of Chest Pain | Sharp, stabbing "Pleuritic" pain (usually in the side of the chest, under the ribs, or under the shoulder blade) that hurts sharply every time you take a deep breath in, cough, or laugh (unless it is a massive central PE, which causes central heavy retrosternal strain). | Dull, heavy, aching, or crushing pressure that does NOT change when you breathe in or out deeply. |
| Heart Rate & Breathing Rate | Persistent Tachycardia (>100 BPM) and Tachypnea (>20 breaths/min), the heart and lungs race to compensate for the blocked lung bed. | Heart rate may be fast, normal, or dangerously slow (bradycardia in inferior heart attacks), accompanied by cold clammy sweat (diaphoresis). |
| Cough & Sputum | Dry cough or coughing up blood-tinged sputum (hemoptysis) caused by a wedge of peripheral lung tissue dying from lack of blood flow (pulmonary infarction). | Usually no cough (unless acute heart failure fills lungs with frothy fluid). |
| Fainting (Syncope) on Standing | Major Red Flag for Massive PE: Fainting or near-fainting upon standing up or walking to the bathroom is often the very first sign of a massive saddle pulmonary embolism blocking the main lung trunk. | Can occur with lethal ventricular arrhythmias. |
| Accompanying Limb Clue | One calf or thigh may be swollen, warm, or aching (or history of recent surgery, flight, pregnancy, cancer, or COVID-19). | History of high blood pressure, smoking, diabetes, or high cholesterol. |
Understanding Virchow's Triad: who is at highest risk for blood clots?¶
In 1856, physician Rudolf Virchow identified the three physiological conditions ("Virchow's Triad") that cause blood to clot abnormally inside a human vein. Almost every DVT and PE results from one or more of these three triggers:
- ** venous stasis (የቀዘቀዘ ወይም የተቀላቀለ የደም ፍሰት)**:

በተደጋጋሚ የሚጠየቁ ጥያቄዎች¶
Should I take an adult Aspirin (325 mg) if I think I have a DVT in my leg instead of going to the hospital?
**No, do NOT try to self-treat a suspected DVT or Pulmonary Embolism at home with aspirin instead of going to the emergency room.** There is a fundamental pharmacological difference between an **arterial clot** (in a heart attack, where high-pressure blood flow is driven by sticky **platelets**, which aspirin blocks) and a **venous clot (DVT / PE)** (in slow-moving low-pressure veins, where clots are built out of a dense mesh of **fibrin protein** driven by the coagulation cascade). Aspirin is very weak against venous fibrin clots and **cannot stop an existing DVT from growing and breaking loose into a fatal Pulmonary Embolism**. True DVT and PE treatment requires prescription **anticoagulant medications**, such as Direct Oral Anticoagulants (DOACs like apixaban/Eliquis or rivaroxaban/Xarelto) or injectable Low-Molecular-Weight Heparin (enoxaparin/Lovenox), started immediately after an emergency ultrasound or CT angiogram.
Do "blood thinners" (anticoagulants like Eliquis, Xarelto, or Warfarin) actually dissolve the blood clot right away?
Despite the popular nickname "blood thinners," standard anticoagulant medications **do not actually thin the viscosity of your blood and do not directly dissolve the existing clot on contact**. Instead, anticoagulants immediately **halt the clotting cascade so the clot stops growing bigger and cannot form a fragile new tail that breaks off into your lungs**, giving your body's own natural internal clot-busting enzyme (*plasmin / fibrinolysis*) the next **2 to 12 weeks** to gradually dissolve, shrink, and remodel the clot safely against the vein wall. (Only in life-threatening *Massive PE with shock* or limb-threatening *Phlegmasia* do hospital ICU doctors infuse aggressive "clot-buster" *thrombolytic* drugs like tPA or use mechanical suction catheters, because thrombolytics carry a high risk of brain bleeding).
Can you get a DVT blood clot in your arm, shoulder, or neck instead of your leg?
**Yes.** About **5 to 10 percent of all Deep Vein Thromboses occur in the upper extremities** (the axillary, subclavian, or internal jugular veins in the arm, shoulder, and neck), and they can also break loose and cause a Pulmonary Embolism. Upper-extremity DVT happens in two primary groups: **(1) Medical patients with indwelling central venous catheters, PICC lines, chemotherapy ports, or cardiac pacemaker wires** running through the vein under the collarbone; and **(2) Young athletes, weightlifters, swimmers, baseball pitchers, or painters/mechanics ("Paget-Schroetter Syndrome" / Effort Thrombosis)** who perform repetitive overhead arm movements where the collarbone and first rib pinch the subclavian vein (*Thoracic Outlet*). Warning signs include **sudden swelling of one entire arm and hand, a heavy aching shoulder/arm, bluish/reddish skin tint on that arm, and visible dilated veins across the front of the chest and shoulder**.
Why are pregnant women and new mothers in the first 6 weeks after giving birth at such high risk for DVT and PE?
Pregnancy and the **postpartum period (especially the first 6 weeks after vaginal delivery or Cesarean section)** represent the highest-risk window for venous blood clots in young women, carrying a clot risk **5 to 20 times higher** than non-pregnant women of the same age. All three parts of Virchow's Triad converge at once: (1) the placenta and hormones massively increase clotting factors (fibrinogen and Factor VIII) while lowering natural Protein S to protect the mother from bleeding to death when the placenta detaches; (2) the heavy uterus physically compresses the **left common iliac vein** in the pelvis (which is why **over 80% of pregnancy DVTs occur in the LEFT leg**); and (3) childbirth or C-section surgery causes pelvic vein endothelial trauma and reduced mobility. Any pregnant or postpartum mother with **one-sided leg/groin swelling or sudden shortness of breath** must be evaluated immediately in an emergency room (never dismissed as "normal pregnancy swelling or tiredness").
What is "Homans' Sign" (pulling the toes up to check for calf pain) and why do doctors say not to rely on it?
Older first aid books taught **Homans' Sign**, forcibly flexing the patient's foot upward at the ankle (*dorsiflexion*) to see if it causes pain in the back of the calf. Modern clinical medicine has completely abandoned Homans' Sign for two critical reasons: **(1) It is wildly inaccurate**: Clinical studies proved Homans' Sign is absent in over **60% of people who actually have a dangerous DVT**, and positive in over **50% of people who just have a pulled muscle or sciatica**, giving a false sense of security. **(2) Theoretical dislodgement risk**: Violently jerking the foot upward stretches the deep calf veins around a fresh thrombus. Never use toe-pulling tests to "rule out" a blood clot, only a D-dimer blood test and a venous duplex ultrasound can safely rule out a DVT.
What is the difference between a superficial blood clot (Superficial Thrombophlebitis) and a Deep Vein Thrombosis (DVT)?
Your legs have two vein systems: **superficial veins** right under the skin (including varicose veins) and **deep veins** buried deep inside the calf and thigh muscles next to the bones. **Superficial Thrombophlebitis** is a clot and inflammation in a surface vein, you can usually feel it with your fingertips as a **tender, warm, red, cord-like hard lump right under the skin**, without swelling of the entire lower leg. However, while superficial phlebitis used to be considered completely benign, ultrasound studies show that **up to 20 to 25 percent of people with extensive superficial clots in the thigh (especially in the Great Saphenous Vein within 3 to 5 cm of the groin crease) also have an underlying DVT or see the clot grow inward into the deep vein system**. Therefore, any suspected superficial vein clot still requires a medical ultrasound evaluation.
