Rabies is an acute, progressive viral encephalomyelitis caused by neurotropic viruses in the genus Lyssavirus. Once the virus enters the central nervous system and clinical neurological symptoms begin, the disease is virtually 100 percent fatal. However, rabies is also 100 percent preventable when immediate physical and chemical wound decontamination is combined with timely Post-Exposure Prophylaxis (PEP). Every hour after a bite, scratch, or saliva contact on broken skin represents a critical window to destroy the viral envelope on the surface and neutralize viral particles before they enter peripheral nerve endings.
This guide provides step-by-step field protocols and clinical pathways aligned with guidance from the World Health Organization (WHO), the United States Centers for Disease Control and Prevention (CDC), the UK Health Security Agency (UKHSA), and the International Federation of Red Cross and Red Crescent Societies (IFRC). Whether you have been bitten by a stray dog abroad, scratched by a wild carnivore, or awakened to find a bat inside your room, following these steps immediately saves lives.
এখন এই কাজ¶
If you or someone nearby has been bitten, scratched, or licked on broken skin or mucous membranes by any wild mammal, bat, or unvaccinated or stray domestic animal:
- Move to safety and isolate the animal without touching it bare-handed: Step behind a solid door, fence, or vehicle immediately. Never attempt to catch, corner, or strike a potentially rabid mammal, as infected animals lose their natural fear and attack repeatedly. If a bat is inside a room where anyone was sleeping, close all interior doors and windows to trap the bat inside the room so animal control officers can test its brain tissue for rabies.
- Control life-threatening bleeding first: If a deep bite is spurting blood or pouring continuously, press a clean cloth or sterile gauze pad firmly over the wound. Call your local emergency number (911 in the United States and Canada, 112 across the European Union, 999 in the United Kingdom) or consult our জরুরী নম্বর directory immediately if bleeding is severe, if the bite involves the head or neck, or if the person shows signs of শক.
- Wash the wound continuously with soap and running water for 15 full minutes: Place the bite, scratch, or saliva-exposed skin directly under a strong stream of running tap water. Lather thoroughly with ordinary household bar soap, liquid hand soap, or dish detergent for a full 15 minutes timed on a clock. Soap dissolves the fragile outer fat envelope of the rabies virus, reducing the risk of viral survival by more than 50 percent before you reach a hospital.
- Irrigate with a virucidal antiseptic solution: After completing 15 minutes of soap-and-water flushing, rinse away all soap suds with clean water and apply povidone-iodine (1 percent to 10 percent aqueous solution), 70 percent isopropyl or ethyl alcohol, or chlorhexidine gluconate. If saliva splashed into the eyes, nose, or mouth, flush those mucous membranes with clean running water or normal saline for 15 minutes without soap.
- Cover the wound loosely with sterile gauze: Place a clean, dry gauze pad loosely over the site to protect it from dirt during transport. Never seal a bite wound shut with butterfly tape, skin glue, or tight bandages, as closing the wound traps virus particles and deep oral bacteria inside low-oxygen tissue planes.
- Go to a hospital emergency department the same day for Post-Exposure Prophylaxis (PEP): Seek emergency medical care immediately to receive Rabies Immune Globulin (HRIG) infiltrated directly into and around the wound and your Day 0 intramuscular rabies vaccine dose. Report the encounter to local public health authorities or animal control so the animal can be safely captured for laboratory testing or placed under a mandatory 10-day observation quarantine.
Rabies is nearly 100 percent fatal once symptoms start, so never wait to see if you feel sick
Because the rabies virus travels silently inside peripheral nerves for weeks or months without causing fever or pain at first, feeling completely fine after a bite or bat encounter does not mean you are safe. Once the virus reaches the brain and causes early symptoms like tingling at the healed bite site, fever, agitation, or difficulty swallowing, no medicine can cure the infection. Post-exposure prophylaxis must start before symptoms appear.

Virological mechanics: why the 15-minute soap wash saves lives¶
When an infected animal bites or scratches, it deposits saliva loaded with Lyssavirus particles into subcutaneous fat, dermis, and skeletal muscle tissue. Understanding what happens in the wound during the first several hours explains why first aid at the sink is just as important as the injections given at the hospital.
How soap destroys the rabies virus on contact¶
The rabies virus is a bullet-shaped, single-stranded RNA virus surrounded by a delicate host-derived lipoprotein (fat) envelope. Studded across the outside of this fatty membrane are glycoprotein spikes (G proteins). The virus relies on these surface spikes to recognize and bind to nicotinic acetylcholine receptors on human muscle cells and neural cell adhesion molecules at the neuromuscular junction.
- Mechanical flushing removes bulk saliva: Running tap water directed into the wound cavity for 15 minutes physically washes away free viral particles, clotted blood, and foreign debris before the virus can attach to tissue receptors.
- Amphiphilic lipid dissolution: Ordinary household soap and dish detergent consist of amphiphilic surfactant molecules with a water-loving head and a fat-loving tail. When you work a rich soap lather into the bite wound for 15 minutes, the fat-loving tails wedge themselves directly into the viral lipoprotein envelope.
- Complete structural collapse: As water rinses the soap micelles away, the viral lipid membrane is ripped apart. Without its outer lipid envelope and glycoprotein spikes, the naked viral core cannot bind to muscle cells or enter nerve axons. It is rendered permanently non-infectious.
- Chemical inactivation by iodine or alcohol: Following the 15-minute soap wash with aqueous povidone-iodine or 70 percent alcohol denatures any remaining surface proteins without burning healthy human tissue needed for wound healing.
Step-by-step field decontamination checklist¶
- Inspect all body surfaces under bright light: Many victims focus on the largest bite mark and miss smaller claw scratches or saliva-contaminated scrapes nearby. Every single break in the skin must be washed and later infiltrated with immune globulin.
- Remove rings, watches, and tight clothing: Cut away sleeve or pant fabric soaked with animal saliva (handle contaminated clothing with gloves and wash in hot water with detergent). Remove finger rings immediately before swelling starts.
- Use high-volume running water: Hold the wound under a faucet or shower nozzle at moderate pressure for 15 continuous minutes. In remote wilderness areas without plumbing, use a 20-milliliter to 50-milliliter irrigation syringe (or a clean plastic water bottle with a small hole pierced in the cap) to squirt a steady pressurized stream of potable water deep into puncture tracts.
- Wash gently without stiff scrubbing brushes: Work soap suds into the wound with clean fingers or soft gauze. Do not scrub the interior of raw puncture wounds with stiff nylon nail brushes; aggressive scrubbing damages exposed peripheral nerve endings and facilitates viral entry into nerve fibers.
- Flush eyes, nose, and mouth with water or saline only: If an animal barked, sneezed, or shook saliva into your eyes, nostrils, or mouth, rinse those mucous membranes continuously for 15 minutes using clean tepid water or sterile 0.9 percent normal saline. Never put soap or alcohol into the eyes.
What never to do to a suspected rabies wound
Never try to suck venom or saliva out of an animal bite with your mouth, as this exposes your oral mucous membranes directly to the rabies virus. Never cauterize a bite wound with a hot metal object, matches, carbolic acid, or nitric acid. Never apply chili powder, coffee grounds, mustard oil, soil, or herbal poultices. Finally, never allow a wound to be tightly sutured closed before Rabies Immune Globulin (HRIG) has been thoroughly infiltrated around the wound margins.

The bat encounter emergency protocol: why invisible bites kill¶
In countries where domestic dogs are routinely vaccinated (such as the United States, Canada, and across Europe), insectivorous microbats are responsible for the majority of locally acquired human rabies deaths. More than half of the patients who die of bat-variant rabies have no memory of being bitten.
Why bat bites go completely undetected¶
Unlike dogs or raccoons that inflict painful lacerations, small insect-eating bats (such as the silver-haired bat Lasionycteris noctivagans or the tricolored bat Perimyotis subflavus) weigh less than 10 to 15 grams (0.35 to 0.5 ounces) and possess razor-sharp, conical teeth that measure only 1 to 2 millimeters (0.04 to 0.08 inches) in length.
- Painless micro-punctures: A bat landing briefly on a sleeping person's bare arm, neck, or face can deliver a defensive nip that penetrates the epidermis into the dermis without waking a sound sleeper.
- No visible mark by morning: Because the puncture is microscopic, capillary bleeding is minimal or nonexistent, and the tiny skin slit closes completely within a few hours. Inspecting yourself in the mirror the next morning and seeing clear skin provides zero assurance that a bite did not occur.
Mandatory public health scenarios requiring bat testing or PEP¶
Public health agencies (including the US CDC, Public Health Agency of Canada, and UKHSA) classify the following situations as presumptive rabies exposures requiring either immediate brain testing of the captured bat or immediate initiation of full Category III PEP for everyone exposed:
- Direct physical contact: You felt a bat brush against your bare skin, land on your body, get tangled in your hair, or scratch you while awake, even if you see no puncture mark afterward.
- Waking up with a bat in the room: You wake up from sleep (even a brief daytime nap) and discover a live or dead bat inside the room.
- Bat in a room with an unattended child: You find a bat in a room with an infant, toddler, or young child who cannot reliably explain whether the bat touched them.
- Bat in a room with an incapacitated adult: You find a bat in a room with someone who has a cognitive disability, dementia, sensory neuropathy, or was heavily intoxicated by alcohol, sedatives, or sleep medication.
How to safely capture and contain a bat inside your home¶
If a bat is found inside a bedroom or living space where human exposure may have occurred, your top priority (after moving children and pets out of the room) is to contain the bat without touching it bare-handed and without crushing its skull, so public health laboratories can perform a Direct Fluorescent Antibody (DFA) test on its brain tissue. If the lab confirms the bat is negative for rabies within 24 to 48 hours, no one in the household needs to undergo the PEP vaccine series.
- Isolate the room: Gently usher all people and household pets out of the room. Close the bedroom door, stuff a towel along the bottom gap of the door, and keep all windows closed so the bat cannot escape outdoors.
- Gear up before re-entering: Put on thick leather work gloves (never thin latex or nitrile medical gloves, which bat teeth easily pierce), long sleeves, long pants, and a hat or hoodie.
- Wait for the bat to land: Do not swing a broom, tennis racket, or shoe at a flying bat. Striking a bat crushes its tiny skull and destroys the brain architecture required for laboratory rabies testing. Turn on the room lights and wait quietly for 5 to 15 minutes until the bat tires and lands on a wall, curtain, or floor.
- Execute the box-and-cardboard trap:
- Approach the resting bat slowly holding a rigid plastic storage container, coffee can, or sturdy shoebox in one gloved hand and a flat piece of stiff cardboard in the other.
- Place the open mouth of the container squarely over the bat against the wall or floor.
- Slowly slide the flat sheet of cardboard between the wall and the container opening, gently nudging the bat's feet onto the cardboard until the opening is completely covered.
- Hold the cardboard tightly against the rim as you pull the container away from the wall, then tape the cardboard (or snap the container lid) securely shut all the way around.
- Punch a few tiny air holes (smaller than 2 millimeters / 0.08 inches so the bat cannot squeeze out; bats can squeeze through a gap as narrow as 6 millimeters / 0.25 inches) using the tip of a pen if the bat is alive.
- Store safely and call authorities immediately: Place the sealed container in a quiet, shaded spot away from children and pets. If the bat is already dead, use leather gloves or tongs to place it inside a sealed plastic bag inside a rigid container and keep it refrigerated on ice packs (do not freeze it solid), as freezing damages brain tissue for microscopy. Call your local animal control office or public health department immediately for pickup and expedited testing.
Pathophysiology and clinical stages of rabies in humans¶
Understanding how the rabies virus progresses through the human body clarifies why post-exposure treatment works during the incubation window and why bites to specific body parts require immediate care.
How the virus travels from wound to brain¶
- Phase 1: Local muscular replication (Hours to Weeks): After entering a bite or scratch, the rabies virus does not enter the bloodstream. Instead, viral particles remain in the local wound tissue, binding to nicotinic acetylcholine receptors on skeletal muscle fibers, where they replicate quietly at the inoculation site. This local residence window is when soap washing and injected Rabies Immune Globulin (HRIG) can intercept and destroy the virus.
- Phase 2: Neuromuscular junction entry and retrograde axonal transport (Days to Months): From the muscle fibers, the virus crosses the neuromuscular junction into peripheral motor or sensory nerve endings. Once inside the nerve axon, the virus becomes shielded from circulating antibodies. It hitches a ride on the neuron's internal dynein motor protein transport system, traveling backward toward the spinal cord dorsal root ganglia and brainstem at a speed of approximately 12 to 100 millimeters (0.5 to 4 inches) per day.
- Phase 3: Central nervous system amplification (Days): Once the virus reaches the spinal cord and brain, it spreads rapidly from neuron to neuron across synaptic junctions, causing severe disruption of neurotransmitters.
- Phase 4: Centrifugal spread to salivary glands and organs: From the infected brain, the virus travels outward along autonomic and cranial nerves to the salivary glands (where it is shed in saliva), cornea, skin hair follicles at the nape of the neck, heart, and adrenal glands.
Incubation period factors and clinical progression¶
The average incubation period from bite to symptom onset is 20 to 90 days (1 to 3 months), though documented cases range from 4 days to longer than 1 year. Bites on the face, scalp, or neck (close to the brainstem) or on richly innervated fingertips have the shortest incubation periods (sometimes under 10 days).
If untreated, clinical rabies unfolds in three stages before fatal cardiorespiratory arrest within 5 to 14 days:
- Prodromal stage (2 to 10 days): Flu-like fever, headache, malaise, and anxiety, accompanied in 50 to 80 percent of patients by neuropathic paresthesia at the healed bite site (burning, itching, tingling, or throbbing pain radiating from the old scar).
- Acute neurological stage (2 to 7 days): Presents either as Furious Rabies (80 percent) with agitation, hallucinations, autonomic instability, hypersalivation, hydrophobia (painful pharyngeal spasms when attempting to swallow water), and aerophobia (spasms triggered by air drafts), or as Paralytic Rabies (20 percent) with ascending flaccid paralysis starting in the bitten limb and progressing to respiratory paralysis.
- Coma and death: Rapid progression to coma, cerebral edema, and cardiac arrest.
WHO Exposure Categories and Modern Post-Exposure Prophylaxis (PEP)¶
When you arrive at the emergency department, clinicians use the World Health Organization three-tier classification system to determine the exact medical regimen required.
| Category | Clinical Definition of Contact | Examples | Unvaccinated Patient Treatment | Previously Vaccinated Patient* Treatment |
|---|---|---|---|---|
| Category I | Touching or feeding animals; animal licks on intact, unbroken skin. | Petting a stray dog; a dog licking intact skin with no cuts or abrasions. | None. Wash exposed skin thoroughly with soap and water. | None. Wash skin with soap and water. |
| Category II | Nibbling of uncovered skin; minor scratches or abrasions without bleeding. | Superficial claw mark or tooth scrape that leaves a mark on skin but draws zero blood. | 1. 15-minute soap-and-water wash. 2. Full Rabies Vaccine series (Days 0, 3, 7, 14). (Bats always treated as Cat III). |
1. 15-minute soap-and-water wash. 2. 2-dose Booster series (Days 0 and 3). No HRIG. |
| Category III | Transdermal bites or scratches that draw blood; licks on broken skin; saliva in eyes, nose, or mouth; any bat contact. | Any puncture wound, bleeding scratch, saliva on a cut, or waking up with a bat in the room. | 1. 15-minute soap wash + antiseptic. 2. Infiltrate HRIG directly into/around all wounds. 3. Full Rabies Vaccine series. |
1. 15-minute soap-and-water wash. 2. 2-dose Booster series (Days 0 and 3). Never give HRIG. |
*Note: A "previously vaccinated person" has documented completion of a prior PrEP or PEP series using a modern cell-culture or embryonated-egg rabies vaccine, or a documented neutralizing antibody titer of at least 0.5 IU/mL.
Component 1: Rabies Immune Globulin (HRIG / ERIG) on Day 0¶
Because your immune system takes 7 to 14 days after your first vaccine shot to produce its own protective antibodies, Rabies Immune Globulin (RIG) bridges that two-week gap by providing immediate passive antibodies at the wound site on Day 0.
- Weight-based dosing: Human Rabies Immune Globulin (HRIG) is dosed at 20 International Units per kilogram (20 IU/kg) for adults and children (Equine RIG is dosed at 40 IU/kg). Never exceed the calculated dose, as excess RIG suppresses active vaccine response.
- Anatomical wound infiltration: Under current WHO and CDC guidelines, as much of the calculated HRIG dose as anatomically feasible must be infiltrated directly into the depth and around the edges of every single bite wound or scratch. Local infiltration neutralizes virus particles inside the tissue before they enter nerve axons. Any remaining volume that cannot fit into a tight anatomical space (such as a fingertip, where clinicians avoid excess pressure to prevent compartment syndrome) is injected intramuscularly at a site anatomically distant from the vaccine site.
- The 7-day HRIG cutoff and syringe separation: If HRIG is unavailable on Day 0, it can and must still be administered up to and including Day 7 after the first vaccine dose (after Day 7, active vaccine antibodies are present). Never mix HRIG and vaccine in the same syringe or inject them into the same muscle.
Component 2: Modern Cell-Culture Rabies Vaccine Schedule¶
Modern vaccines, including Human Diploid Cell Vaccine (HDCV) and Purified Chick Embryo Cell Vaccine (PCECV), are safe intramuscular injections.
- Injection site rule (Deltoid or Thigh only): Inject intramuscularly into the deltoid muscle of the upper arm in adults and older children, or the anterolateral thigh in young children under age 2. Never inject rabies vaccine into the gluteal (buttock) area, where fat tissue causes poor absorption and vaccine failure.
- Standard 4-Dose Regimen (Healthy Unvaccinated Patients): Given on Day 0, Day 3, Day 7, and Day 14 (where Day 0 is the day of the first dose).
- 5-Dose Regimen (Immunocompromised Patients): Patients with HIV, organ transplants, chemotherapy, or high-dose steroids receive a fifth dose on Day 28 (Days 0, 3, 7, 14, and 28) plus antibody titer verification.
