Souse blesi pwatrin ak efondre poumon premye swen¶
Nimewo ijans yo diferan selon peyi. Paj sa a sèvi ak 911, nimewo pou Etazini ak Kanada. Anpil lòt peyi itilize 112, 999, oswa yon lòt nimewo. Jwenn nimewo pou peyi w.
Respirasyon nòmal imen depann sou yon vakyòm ki byen fèmen, presyon negatif andedan kavite thoracic la. Ant sifas ekstèn nan poumon yo (plèv visceral) ak miray enteryè a nan kòt la (plèv parietal) manti espas mikwoskopik pleural la, kenbe nan yon presyon negatif anba-atmosferik apeprè -4 a -8 santimèt dlo. Lè yon kout zam, lam kouto, fragman shrapnel, kòt kase, oswa dlo nan poumon espontane dechire nan vakyòm sele sa a, lè a kouri nan espas pleural la epi retire poumon elastik la lwen miray pwatrin lan, sa ki lakòz yon poumon tonbe (pneumothorax).
Lè yon twou ekstèn nan miray pwatrin lan ase lajè (apeprè de tyè dyamèt trache a oswa pi gwo), lè a pran chemen ki pi piti rezistans ak souse dirèkteman nan miray pwatrin lan olye pou yo desann trachea a, kreye yon souse blesi nan pwatrin (louvri pneumotoraks). Pi mal toujou, si lè a antre nan kavite pwatrin lan ak chak souf oswa vin kwense anba yon bandaj konplètman sele ki pa gen okenn fason pou chape, presyon andedan pwatrin lan bati pi wo pase nivo atmosferik, kraze poumon an nèt ak koude gwo venn yo retounen san nan kè a. Ijans letal sa a, ki rele yon tansyon pneumothorax, lakòz arestasyon kadyak obstriktif fatal nan kèk minit sof si yo rekonèt ak dekonprese.
Fè sa kounye a¶
- Rele nimewo ijans lokal ou imedyatman epi mande paramedik Advanced Life Support (ALS). Fè 911 nan Etazini ak Kanada, 112 atravè Inyon Ewopeyen an, 999 nan Wayòm Ini a, 000 nan Ostrali, oswa tcheke anyè a. nimewo ijans. Rapòte klèman yon blesi nan pwatrin penetrasyon oswa sispèk ki tonbe nan poumon pou paramedik yo pote sele nan pwatrin, twous dekonpresyon zegwi, ak oksijèn.
- Ekspoze tout pwatrin lan AK dousman tcheke do a ak anbabra pou yon blesi sòti. Koupe oswa leve chemiz la nèt jiska kou a. Chak fwa ou jwenn yon blesi nan pwatrin sou devan an, imedyatman woule oswa santi anba zepòl yo, armpits (axillae), ak tounen lakay ou pou yon dezyèm twou, epi sele twou ki pi gwo oswa plis ebulisyon an premye.
- Kouvri yon blesi nan pwatrin ki te souse imedyatman ak yon men gan pandan ekzalasyon, Lè sa a, aplike yon sele nan pwatrin vantilasyon komèsyal oswa yon pansman oklizif twa bò. Siye san ak swe sou po a pou adezif la kole, mande pasyan an pou l respire (rann souf), epi lise yon sele pwatrin vantile komèsyal sou twou a.
- ** Chita yon pasyan konsyan, ki respire dwat oswa semi-rekline (oswa panche yon ti kras sou bò blese a) pou kite bon poumon an elaji.** Pa janm fòse yon pasyan konsyan k ap lite pou respire ak yon blesi nan pwatrin yo kouche plat sou do sof si imobilizasyon epinyè a egzije sa.
- Siveye respirasyon chak 60 segonn epi "BURP" sele pwatrin lan imedyatman si pasyan an vin pi kout souf, cyanotic, oswa pèdi batman radial yo. Si respirasyon pasyan an vin pi mal toudenkou apre yo fin aplike yon sele nan pwatrin lan, kale tounen yon kwen nan pansman an pou 3 a 5 segonn pandan ekzalasyon pou kite pwatrin li bloke soti nan nenpòt lè ki peze bouche. valv, epi re-lis sele a vantile.
Why an airtight four-sided bandage kills: the one-way valve trap
Lè yon bal oswa yon kouto pèse miray pwatrin lan, li prèske toujou chire tisi poumon ki kache a tou. Menm apre ou fin kouvri twou deyò a nan po a pou lè deyò a pa ka souse ankò, lè a kontinye ap koule soti nan poumon ki chire nan espas pwatrin lan chak fwa moun nan respire oswa touse. Si yon sekouris tape yon moso plastik oswa twal gaz ak vazine konplètman fèmen sou tout kat kote ki pa gen okenn kanal vantilasyon, lè nan poumon ki koule pa gen okenn kote pou ale. Nan kèk minit, lè kwense gonfle kavite pwatrin lan tankou yon kawotchou presyon wo, pouse kè a ak trache sou bò opoze a epi anpeche san retounen nan kè a (tansyon pneumothorax). Toujou sèvi ak yon sele nan pwatrin vantile oswa yon abiye twa-bò flutter, epi dwe pare pou "rote" li nan premye siy detrès vin pi grav.

Comparing life-threatening thoracic emergencies¶
Thoracic trauma and spontaneous pleural air leaks produce five distinct clinical syndromes. Distinguishing a simple collapsed lung from an open sucking chest wound, a tension pneumothorax, a massive hemothorax, or a flail chest determines both immediate positioning and life-saving decompression.
| Thoracic Condition | Underlying Anatomical Mechanism | Classic Visual & Auscultation Signs | Neck Veins & Percussion Note | Definitive First Aid & ALS Intervention |
|---|---|---|---|---|
| Simple Closed Pneumothorax (Spontaneous or Blunt Trauma) | Air leaks from a ruptured lung alveolus/bleb or fractured rib puncture into the closed pleural space without an external skin hole | Sudden sharp one-sided chest pain worse on inspiration, shortness of breath, decreased breath sounds on affected side | Venn kou nòmal; pèkisyon nan pwatrin se ipèrresonan (kre tankou tanbou) | Sit upright comfortably, give high-flow supplemental oxygen if available, avoid air travel or scuba diving, transport for chest X-ray/ultrasound |
| Open Pneumothorax ("Sucking Chest Wound") | Penetrating hole in chest wall \(\ge \frac{2}{3}\) the diameter of the trachea allows outside air to suck directly in and out of the pleural space | Audible sucking, hissing, or whistling sound on inhalation; frothy pink blood bubbling from wound on exhalation; severe respiratory distress | Neck veins normal unless tension develops; subcutaneous emphysema ("rice-krispies" crackles under skin) | Immediately apply a Commercial Vented Chest Seal or Three-Sided Occlusive Flutter Dressing at end-exhalation; check back for exit wound |
| Tension Pneumothorax (Open or Closed) | One-way valve traps air in pleural space under positive pressure, collapsing the lung and pushing the heart/vena cava to the opposite side (mediastinal shift) | Extreme air hunger, cyanosis, agitation/panic, completely absent breath sounds on injured side, asymmetric chest rise, rapidly falling blood pressure (obstructive shock) | Distended (bulging) Jugular Neck Veins (JVD); hyperresonant percussion; tracheal deviation away from injured side (late sign) | Immediately "BURP" or remove any chest seal; ALS paramedic performs immediate Needle Thoracostomy Decompression (5th ICS mid-axillary or 2nd ICS mid-clavicular) |
| Massive Hemothorax | Laceration of intercostal artery, internal mammary artery, lung hilum, or great vessel pours > 1,500 milliliters (3 pints) of blood into the chest cavity | Pale, cold, clammy skin (hypovolemic hemorrhagic shock), rapid weak pulse, decreased or absent breath sounds over lower/entire lung | Flat (collapsed) neck veins due to severe blood loss; chest percussion is dull / flat (like tapping a solid brick) | Seal any external bleeding wound, keep patient warm and flat/semi-reclined, rapid priority 1 transport to trauma surgeon for chest tube & autotransfusion/thoracotomy |
| Flail Chest & Pulmonary Contusion | 2 or more adjacent ribs fractured in 2 or more places, creating a floating "flail segment" of chest wall over bruised lung | Mouvman paradoks miray pwatrin lan: segman ki kase a souse ANDAN pandan w ap respire pandan rès pwatrin lan ap elaji, epi li gonfle deyò lè w respire. | Venn nan kou nòmal sof si asosye ak nemotoraks tansyon; krepitis sansib (griye zo) | Do NOT tape or sandbag the ribs tightly; have conscious patient hold a soft pillow gently against the flail area while sitting semi-upright; prepare for positive-pressure ventilation |
Pathophysiology of a sucking chest wound (Open Pneumothorax)¶
Under normal physiological conditions, when your diaphragm contracts downward and your intercostal muscles pull your ribcage outward, intra-thoracic pressure drops slightly below outside atmospheric pressure (from -4 down to -8 \(\text{cmH}_2\text{O}\)). Because the chest wall is completely sealed, the only pathway for air to enter the chest is down the mouth, trachea, and bronchi, inflating both lungs effortlessly.
The "Two-Thirds Tracheal Diameter" threshold¶
An adult human trachea has an internal diameter of roughly 18 to 24 millimeters (0.7 to 0.95 inches). If a knife blade, high-velocity rifle round, shotgun blast, or impaled fence spike creates an open hole through the chest wall whose cross-sectional diameter is two-thirds the diameter of the trachea (roughly 12 to 15 millimeters / 0.5 to 0.6 inches) or larger, the laws of fluid dynamics dictate that air follows the path of least resistance:
- Because the short hole through the ribcage offers far less airway resistance than the long, branching bronchial tree, every time the victim tries to inhale, outside room air rushes directly through the hole in the chest wall into the pleural space rather than coming down the windpipe into the lungs.
- As room air fills the pleural space on the injured side, the lung on that side collapses completely and cannot exchange oxygen.
- Even worse, as the victim inhales and exhales, stale carbon-dioxide-rich air sloshes back and forth uselessly between the collapsed lung and the healthy lung (Pendelluft or "pendulum air"), while the mediastinum (containing the heart and vena cava) wobbles side to side (mediastinal flutter), rapidly exhausting the patient and causing severe hypoxia and hypercapnia.
Classic signs of an open sucking chest wound¶
- Audible sucking, hissing, or slurping sound coming from the chest or back wound each time the casualty gasps for air.
- Frothy, bright pink or red blood bubbling or sputtering out of the wound during exhalation or coughing (unlike dark red venous oozing or bright red arterial spurting from an arm wound, frothy bubbles prove air from the respiratory tract or pleural space is mixing with blood).
- Subcutaneous emphysema: Air forced under the skin of the chest wall and neck creates a strange, crackling "Bubble Wrap" or "Rice Krispies" sensation (crepitus) when you press your fingers lightly on the skin around the collarbone or ribs.
Step-by-step technique: Applying a Vented Chest Seal or Three-Sided Dressing¶
The immediate goal of first aid for an open chest wound is to convert an open pneumothorax into a simple vented pneumothorax: blocking outside air from sucking in during inhalation while allowing trapped internal air and blood to escape during exhalation.
Method A: Commercial Vented Chest Seal (Gold Standard: CoTCCC & First Aid Guidelines)¶
Modern trauma kits carry purpose-built Vented Chest Seals (such as the HyFin Vent, SAM Chest Seal, Russell Chest Seal, Halo Vent, or Bolin/Asherman seal). These feature an aggressive medical hydrogel adhesive pad equipped with either a raised one-way dome valve or multiple laminar-flow vent channels that let air and blood escape outward while sealing flat against the skin when the patient inhales.
- Expose the wound and check the back: Cut the shirt away completely. Always check the back and armpits for an exit wound; if there are two holes (an entry and an exit), apply a vented chest seal to the primary sucking hole and a second seal (vented if available) to the exit hole.
- Wipe the skin around the wound completely dry: This is the step most rescuers forget under stress. Even the best hydrogel adhesive will slide right off skin coated in wet blood, sweat, mud, or rainwater. Use a dry gauze pad inside the chest seal package to briskly wipe a 10-centimeter (4-inch) circle of skin around the hole as dry as possible.
- Peel the backing and apply at the end of exhalation: If the patient is conscious, ask them to breathe out (exhale) and pause for one second (exhaling pushes extra air out of the pleural space before you seal it). Center the one-way valve or laminar vent channels directly over the hole and press the adhesive firmly from the center outward to eliminate wrinkles.
Metòd B: Enpwovize twa-bò oklizif Flutter-Valv abiye¶
If you do not have a commercial vented chest seal in your first aid kit, you must construct a Three-Sided Occlusive Dressing:
- Sterile petrolatum (Vaseline) gauze covered with a layer of foil or plastic.
- In the first 10 seconds before materials are ready: Slap your gloved palm directly over the sucking hole during exhalation, lifting the heel of your hand slightly when the patient exhales to let air out, and sealing it flat when they inhale.
- Cut the plastic square at least 5 centimeters (2 inches) wider than the wound on all sides: If the plastic is too small, the suction of inhalation will pull the entire piece of plastic into the wound hole.
- Wipe the surrounding skin dry and place the plastic over the wound as the patient exhales.
- Tape THREE sides completely airtight, leaving the BOTTOM corner or fourth side completely UNTAPED:
- Apply wide medical tape or duct tape completely overlapping the top edge, left edge, and right edge of the plastic square so no air can leak under those three edges.
- Leave the bottom edge (or the lowest gravity-dependent corner) completely free of tape.
- How the flutter valve works: When the patient inhales, negative pressure inside the chest pulls the loose plastic sheet flat and wet against the skin, sealing the hole airtight so outside air cannot enter. When the patient exhales or coughs, positive pressure inside the chest pushes trapped air and blood out from underneath the untaped bottom edge like a one-way flapper valve.
Recent ILCOR nuance: what if blood clots glue your three-sided dressing shut?
International Liaison Committee on Resuscitation (ILCOR) and Red Cross guidelines emphasize a critical real-world hazard: as sticky blood oozes out from under a three-sided plastic dressing (or even a commercial chest seal), coagulating blood clots frequently glue the untaped fourth side firmly to the skin, silently turning your three-sided dressing into a deadly four-sided unvented trap. For this reason, some civilian first aid protocols permit leaving a non-sucking or actively bleeding chest wound loosely covered with plain gauze if a rescuer cannot continuously monitor a three-sided seal. Whenever you apply any chest seal or three-sided dressing, you must watch the patient's breathing continuously and "burp" the dressing immediately if it clots shut.
Recognizing and treating Tension Pneumothorax ("Burping the Seal")¶
A tension pneumothorax is an immediately fatal mechanical catastrophe that can develop either from a closed rib fracture / lung laceration or when an open chest wound dressing becomes clogged or sealed on all four sides.
How tension pneumothorax kills (Obstructive Shock)¶
With every breath, 50 to 100 milliliters of air leaks out of the lacerated lung into the trapped pleural space and cannot escape back into the bronchi or out through the chest wall: 1. Pressure inside the injured side of the chest rises from negative (-5 \(\text{cmH}_2\text{O}\)) to strongly positive (+15 to +30 \(\text{cmH}_2\text{O}\)). 2. The injured lung is crushed down into a tiny airless ball at the hilum, while the diaphragm on that side is pushed downward into the abdomen. 3. The expanding pocket of high-pressure air shoves the entire mediastinum (heart, aorta, superior vena cava, inferior vena cava, and trachea) sideways into the uninjured side of the chest, compressing the remaining good lung. 4. The fatal step: Because the thin-walled superior and inferior vena cava return deoxygenated blood to the right side of the heart at very low venous pressures (only 2 to 8 mmHg), the mediastinal shift kinks both great veins like a bent garden hose. Blood can no longer enter the heart (preload drops to zero), cardiac output plummets, and the patient suffers Pulseless Electrical Activity (PEA) cardiac arrest from pure mechanical obstructive shock.
7 drapo wouj klinik yo nan Tansyon Pneumotoraks¶
Gade chak pasyan chòk nan pwatrin tankou yon malfini pou siy sa yo nan ogmante presyon entratoracik:
| Siy klinik | Etap | Sa w wè ak tande |
|---|---|---|
| 1. Souf kout ki vin pi vit (Dypne) ak Takipne | Bonè | Respirasyon vin rapid (> 28 a 30 souf/min), pa fon, ak travay; pasyan an parèt pè, san pwoblèm, oswa li di "Mwen pa ka pran okenn lè" |
| 2. Pwatrin asimetri leve ak tonbe | ** Bonè rive nan mitan ** | Bò ki blese nan pwatrin lan parèt ipè elaji (barik soti) ak san mouvman, pandan y ap sèlman bò kote ki pa blese a deplase monte ak desann. |

| 4. Takikardi pwogrese nan ipotansyon (Pèt batman radial) | Mitan an reta | Ras batman kè a > 120 a 140 bpm pandan batman radial la nan ponyèt la ap fèb epi disparèt pandan retounen venn nan kè a ap kwense. |
| 5. Distansyon venn jugulaire (JVD) | Mitan an reta | Venn jigular ekstèn yo sou kote kou a gonfle tankou kòd epè ble menm lè pasyan an chita a 45 degre (nòt: JVD ka absan si pasyan an te pèdi tou gwo kantite san) |
| 6. Sianoz (lèv ble, lang, ak zong) | Anreta | Saturasyon oksijèn (\(\text{SpO}_2\)) desann anba 85% malgre efò sekou. |

Etap-pa-etap: Ki jan yo "BURP" yon sele nan pwatrin imedyatman¶
Si yon viktim ki mete yon sele pwatrin komèsyal oswa yon pansman plastik ki gen twa bò montre nenpòt nan siy avètisman ki anwo yo (grangou lè a pi mal, ajitasyon, bouch ble oswa batman febli):
- Atann yon kwen sele nan pwatrin lan oswa pansman plastik ak dwèt gan epi retire li tounen sou po a pou ekspoze twou blesi a nèt.
- **Peze dousman sou po a bò kote blesi a oswa rete tann pandan pasyan an ekspire pou 3 a 5 segonn: ** Lè yon nemotoraks tansyon vre prezan, ou pral imedyatman tande oswa santi yon ** gwo prese oswa sifle nan lè kwense ak san kimen eksplozif soti nan blesi a **, swiv nan kèk segonn pa pasyan an pran yon gwo souf ak amelyore koulè yo ak batman kè yo.
- Siye ploge san ki gen kayo a: Sèvi ak yon twal gaz esteril pou siye nenpòt boul epè ki sanble ak jele ki tap bouche tras blesi a oswa valv yon-direksyon sele pwatrin lan.
- **Re-lise sele pwatrin lan (oswa aplike yon nouvo pwòp) nan fen exhalation: ** Si valv sele nan pwatrin nan san espwa bouche ak ou pa gen okenn sele vantile rezèv, kite kwen an pa tape oswa kite blesi a kouvri sèlman ak yon twal gaz ki lach, ki pèmeyab nan lè a pandan w ap kontwole senyen.
Kijan Paramedik ak Doktè ALS trete Pneumotoraks Tansyon Fèmen¶
Si yon pasyan devlope yon nemotoraks tansyon ki pa gen twou ekstèn nan miray pwatrin lan (pa egzanp, apre yon enpak sou volan, aksidan motosiklèt, oswa rupture bleb espontane), pa gen okenn sele nan pwatrin yo rute. Paramedik ak klinisyen chòk nan sipò lavi avanse (ALS) fè Dekonpresyon Thoracostomy Zegwi: * Mete yon katetè ki long, gwo twou (10-a-14-kalib, 8.25-santimèt / 3.25-pous) sou zegwi a dirèkteman sou bò anwo nan kòt ki pi ba a (pou evite atè entèkostal la, venn, ak nè ki kouri nan renur nan sou anba a ** 5 ** chak kwen) nan espas ki nan kostal la. liy anteryè/mid-axillary (sit modèn ATLS 10yèm edisyon an pi pito pou granmoun) oswa 2yèm espas entèkostal la nan liy mitan-klavikilè a, imedyatman vantilasyon lè presyon an ak yon sifle ki ka tande jiskaske yo mete yon tib pwatrin definitif (tib torakostomi).
Pi bon pozisyon pasyan ak jere Flail pwatrin¶
Ki jan yo pozisyon yon pasyan ki gen yon blesi nan pwatrin¶
- Konsyan ak pou l respire: Pèmèt pasyan an chita dwat oswa semi-rekline nan yon ang 45 a 60 degre (pozisyon Fowler), oswa apiye yon ti kras pi devan sou jenou yo (pozisyon trépied). Chita dwat sèvi ak gravite pou rale ògàn nan vant yo desann sou dyafram nan epi kenbe san plèvral (emotoraks) nan pati anba a nan kavite nan pwatrin lan pou lòb anwo poumon yo ka toujou respire.
- Pinke nan direksyon kote ki blese a (lè li pito): Si yon pasyan gen yon blesi nan pwatrin ki sele oswa yon kòt kase sou yon bò epi li prefere apiye, panche yo yon ti kras nan bò kote ki blese a atèl kòt ki fè lapenn yo sou branka oswa zòrye a epi li pèmèt **wo zòrye ki pa blese a elaji nan poumon an san libète nèt sou tout machin nan san koule nan machin nan san yo pa koule nèt. poumon.
- San konesans ak respirasyon (san blesi nan kolòn vètebral): Mete pasyan an nan pozisyon rekiperasyon lateral la ak bò BLESYE BASAN (poumon an sante sou tèt). Mete bò blese a kont tè a kenbe senyen entèn andedan poumon domaje a soti nan gaye atravè bronch prensipal la ak nwaye nan poumon an sante, pandan y ap kite tèt poumon an sante elaji lib.
Premye swen pou pwatrin flail (plizyè zo kòt kase ak mouvman paradoks)¶
Lè yon enpak ki gen gwo enèji kase de oswa plis zo kòt adjasan nan de oswa plis kote, seksyon rektangilè nan kòt kòt la pèdi koneksyon zo li ak rès miray pwatrin lan. * Kisa sa sanble (Respirasyon Paradoks): Lè moun nan respire AN, presyon negatif andedan pwatrin lan souse segman kòt ki kase a k ap flote ANDAN pandan rès pwatrin lan deplase deyò; lè yo respire SOTI, segman ki kase a soufle deyò. * Poukisa nou PA TApe kòt yo ankò oswa mete sak sab lou sou yon pwatrin fleo: Sa gen plizyè dizèn ane, sekouris yo te vlope pansman elastik byen sere sou tout kòt la oswa mete yon sak sab lou sou yon segman fleo. Esè klinik yo te pwouve ke atache miray pwatrin lan oswa pondération li ak sak sab mete restriksyon sou poumon an sante, vin pi mal atelektaz (effondreman alveolatè), ak rapidman presipite echèk respiratwa. * Swen modèn ki baze sou prèv pou Flail Chest: 1. Pa janm vlope tep sikonferansyal oswa pansman elastik alantou pwatrin lan. 2. Sèvi ak "Self-Splinting" dou ak yon zòrye mou: Chita yon pasyan konsyan semi-dwat epi ba yo yon zòrye kabann mou, dra ki plwaye, oswa jakèt peluche pou kenbe dousman sou kòt blese yo ak pwòp bra yo, espesyalman lè yo touse oswa monte sou boul. 3. Prepare pou ede respire: Paske vrè menas lavi a nan pwatrin flail la se gwo twou san fon kontusyon nan poumon (kraze, tisi nan poumon ki plen likid) dirèkteman anba zo kòt yo kase, pasyan yo souvan fatige epi yo mande pou vantilasyon presyon pozitif sak-valv-mask (BVM) ak entibasyon lopital.
Entèvansyon premye swen pou yon blesi nan pwatrin souse se youn nan egzanp ki pi klè kote yon aksyon senp nan de premye minit yo dirèkteman anpeche arè kadyak. Konsidere pran yon premye swen sètifye, Stop the Bleed, oswa yon kou taktik swen ijans pou aksidan atravè sosyete nasyonal Kwa Wouj oswa Kwasan Wouj ou, American College of Surgeons, oswa Anbilans St John pou pratike aplikasyon sele pwatrin ak yon enstriktè.
Kesyon yo poze souvan¶
Why can't I just tape a regular airtight bandage on all four sides over a sucking chest wound?
When a bullet, knife, or sharp object punctures the chest wall, it almost always tears the lung underneath as well. If you tape an airtight piece of plastic or petroleum gauze down on all four sides with no vent valve, outside air stops entering through the skin, but air leaking out of the torn lung inside the body has nowhere to escape. With every breath, more air gets pumped into the sealed chest cavity like a bicycle tire. Within minutes, this trapped air crushes the injured lung and shoves the heart and major veins to the opposite side of the chest, pinching off blood return to the heart and causing fatal cardiac arrest (tension pneumothorax). Always use a commercial vented chest seal or tape an improvised plastic square on three sides only.
What does it mean to "burp" a chest seal, and when should I do it?
Even when you apply a commercial vented chest seal or a three-sided plastic flutter dressing correctly, thick clotting blood oozing from the wound can plug the vent channels or glue the untaped fourth edge shut. If a patient wearing a chest seal becomes progressively more short of breath, anxious, pale or bluish, loses breath sounds on the injured side, or develops a weak rapid pulse, trapped air is building up inside their chest. "Burping" the seal means peeling back one corner of the dressing for 3 to 5 seconds during exhalation so the trapped pressurized air and foamy blood can hiss out of the chest, wiping away any blood clots, and smoothing the vented seal back down.
What if I don't have a commercial chest seal or any plastic or tape right away?
In the first few seconds after discovering a sucking chest wound, your own gloved hand (or even the victim's own hand) is an immediate life-saving valve. Press your gloved palm firmly over the open wound to stop air from sucking into the chest when the person inhales; if they begin having more trouble breathing, lift the edge of your hand slightly as they exhale to let trapped air escape. If you have no commercial vented seal and no clean plastic or foil wrapper to build a three-sided flutter valve (or if a three-sided dressing keeps clotting shut), modern first aid guidelines advise covering the wound loosely with a clean gauze pad to absorb blood while leaving the wound able to vent air until paramedics arrive.
Should I lay someone with a collapsed lung or chest wound flat on their back?
No, never force a conscious, breathing person with a chest wound or collapsed lung to lie flat on their back unless you must immobilize their spine. Lying flat pushes the abdominal organs up against the diaphragm, makes it much harder for the remaining healthy lung to expand, and allows blood inside the chest to pool across the entire lung field. Instead, let a conscious patient sit upright or semi-reclined at a 45-degree angle (or leaning slightly toward the injured side). If the victim becomes unconscious but is still breathing, place them in the recovery position lying on their INJURED side so the healthy lung is on top and can expand freely without blood draining into it.
Why shouldn't we wrap an elastic bandage tightly around broken ribs or a flail chest anymore?
For decades, people used to wrap wide elastic bandages or tape around the ribcage to splint broken ribs. Clinical studies proved that strapping the chest wall prevents the lungs from expanding fully on the uninjured side, traps mucus in the lower airways, collapses microscopic air sacs (atelectasis), and rapidly triggers pneumonia or acute respiratory failure. Instead, leave the chest wall free to expand and let the conscious patient gently hug a soft pillow or folded blanket against the painful ribs to splint the area comfortably when they breathe or cough.
Can someone get a collapsed lung (pneumothorax) without being stabbed, shot, or hit in the chest?
Yes, a primary spontaneous pneumothorax happens when a tiny, thin-walled air blister (bleb or bulla) at the top of the lung ruptures on its own without any trauma, most commonly in tall, thin young adults between 15 and 35 years old or in smokers and people with COPD or asthma. It causes a sudden, sharp, stabbing pain on one side of the chest that hurts worse when taking a deep breath, accompanied by sudden shortness of breath. Even though there is no external wound on the skin, a spontaneous pneumothorax still requires urgent emergency department evaluation and a chest X-ray, and patients with a known pneumothorax must never fly in an airplane or scuba dive until cleared by a physician.
