Part 1: Wild Mushroom Poisoning (Mycetismus) and the 8 Clinical Syndromes

Of the more than 10,000 known species of macrofungi worldwide, fewer than 100 are clinically poisonous to humans, and roughly a dozen species cause over 95 percent of all fatal mushroom poisonings. However, not a single folk rule or old wives' tale can distinguish a safe mushroom from a deadly one.

Debunking dangerous mushroom folk myths

  • Myth 1: "Boiling, cooking, drying, or freezing neutralizes mushroom toxins.": FALSE. Amatoxins (the lethal cyclic octapeptides in Death Cap and Destroying Angel mushrooms) are 100 percent heat-stable, freeze-stable, and acid-resistant. Boiling, frying, baking, dehydrating, or digesting in stomach acid does not reduce their lethality at all.
  • Myth 2: "Deadly mushrooms taste bitter, acrid, or foul, or tarnish a silver spoon in the cooking pot.": FALSE. Survivors of Amanita phalloides (Death Cap) poisoning universally report that the mushrooms smelled pleasant and tasted delicious, and they never tarnish silver or turn garlic or onions black.
  • Myth 3: "If squirrels, deer, rabbits, or slugs eat a mushroom, it is safe for humans.": FALSE. Slugs, insects, and many woodland mammals have different hepatic metabolism and routinely feed on Amanita phalloides without harm.
  • Myth 4: "Poisonous mushrooms have bright colors or caps that do not peel easily.": FALSE. The Destroying Angel (Amanita bisporigera / Amanita virosa) is pure, innocent snow-white, while the Death Cap (Amanita phalloides) has a plain greenish-olive or yellowish-tan cap that peels easily and frequently tricks experienced immigrants into thinking it is an edible Asian Paddy Straw mushroom (Volvariella volvacea) or button mushroom (Agaricus).
  • Myth 5: "A smartphone mushroom-identification app said it was 98% edible.": FALSE. Poison centers worldwide report a surge in severe poisonings caused by foragers trusting AI photo apps, which routinely fail to see the buried underground volva or spore print colors that separate an edible meadow mushroom from a lethal Amanita.

3. Orellanine(聯吡啶腎毒素) Cortinarius orellanus 和 Cortinarius rubellus(致命網帽)。
5.毒蕈鹼(膽鹼能症候群) Inocybe (Fiber Caps) and Clitocybe dealbata (Sweating Mushroom: grows on lawns). (Note: Despite its name, Amanita muscaria contains almost NO muscarine). 15 minutes to 2 hours (Rapid onset). Parasympathetic Nervous System ("SLUDGE" / DUMBBELS): Profuse Salivation, Lacrimation (tearing), Urination, Diarrhea, GI cramps, Emesis, drenching Sweating, pinpoint pupils (miosis), wheezing, and bradycardia. GI Tract Only: Nausea, projectile vomiting, abdominal cramping, and profuse watery or bloody diarrhea; resolves within 24 to 48 hours without liver or kidney damage. Oral or IV fluid and electrolyte replacement and antiemetics. (Must still monitor 48h liver enzymes if mixed wild species were eaten).

Deep dive into the 4 Stages of Amatoxin (Amanita phalloides) Poisoning

Because 90 percent of all mushroom fatalities worldwide are caused by alpha-amanitin in Amanita phalloides (Death Cap), Amanita bisporigera/virosa (Destroying Angel), and Galerina marginata, understanding its four deceptive clinical stages is important:

  • Stage 1: The Silent Latency Period (6 to 24 hours after ingestion): For the first 6 to 12 hours after eating the meal, the person feels 100 percent normal. During this silent window, amatoxins are absorbed from the gut, enter liver hepatocytes via OATP1B3 transporters, and irreversibly bind to RNA Polymerase II, halting all messenger RNA transcription and protein synthesis in the liver.

"Blind as a bat, mad as a hatter, red as a beet, hot as a hare, dry as a bone, full as a flask, fast as a fiddle": Huge dilated pupils (mydriasis), bone-dry skin/mouth, flushed hot feverish skin, florid visual hallucinations/delirium (picking at invisible bugs), urinary retention, tachycardia. Go to Emergency Department. Active cooling, quiet room, IV Benzodiazepines for agitation, bladder catheterization, and Intravenous Physostigmine (specific cholinesterase inhibitor antidote for severe delirium).
Ribosome-Inactivating Toxalbumins: Castor Bean (Ricinus communis) & Rosary Pea / Jequirity Bean (Abrus precatorius) Ricin (in Castor Beans) and Abrin (in bright red-and-black Rosary Peas used in tourist necklaces/bracelets). Irreversible 60S Ribosomal Inactivation: Halts cellular protein synthesis, causing severe capillary leak and multiorgan necrosis. (Note: Swallowing an intact, unchewed hard seed coat often passes harmlessly in stool). Chewing a crushed seed causes delayed (4 to 24h) severe bloody vomiting/diarrhea, hypovolemic shock, liver/kidney failure, and death. Go to Emergency Department immediately. Administer Activated Charcoal or Whole Bowel Irrigation (polyethylene glycol) for intact seeds, plus aggressive ICU fluid resuscitation.

The deadly "Wild Carrot / Wild Parsnip" foraging trap: Hemlock

Every spring and summer, experienced gardeners and foragers are poisoned after mistaking Water Hemlock (Cicuta maculata) or Poison Hemlock (Conium maculatum) for edible wild members of the Apiaceae (carrot/parsley) family, such as Queen Anne's Lace (wild carrot), wild parsnip, wild celery, or anise/fennel.

  • How to recognize Poison Hemlock (Conium maculatum): While edible Queen Anne's Lace has a hairy green stem ("the Queen has hairy legs"), Poison Hemlock has a smooth, hairless hollow green stem covered with distinctive purple-red blotches or spots, a musty mouse-urine odor when crushed, and grows up to 2 to 3 meters (6 to 10 feet) tall.
  • Why Water Hemlock (Cicuta maculata) is the most acutely lethal plant in North America: Growing along stream banks, marshes, and wet ditches, Water Hemlock has chambered tuberous roots filled with yellowish, oily liquid saturated with cicutoxin. Taking a single bite of a Water Hemlock root mistaken for wild parsnip triggers violent grand mal seizures (status epilepticus) within 15 to 60 minutes, rapidly causing severe rhabdomyolysis, metabolic acidosis, cerebral hypoxia, and death unless immediate ambulance airway management and intravenous benzodiazepines are administered.

My 2-year-old toddler picked a small brown mushroom off our backyard lawn and took a bite. They look completely fine right now: can I just watch them at home?

Call Poison Control (1-800-222-1222 in the United States) immediately before deciding to stay home, and dig up the rest of that mushroom (and any identical ones nearby) in a paper bag so you can text clear photos of the cap, gills, and underground stem base to the poison specialist. While the majority of small brown lawn mushrooms ("LBMs" like *Panaeolus foenisecii*) cause little or no illness, deadly amatoxin-containing species (*Galerina marginata*, *Lepiota*, and *Amanita* species growing near yard trees) also grow in yards and cause ZERO symptoms for the first 6 to 24 hours. Poison Control will verify whether the mushroom is a harmless lawn species or requires an immediate trip to the emergency department for a single dose of activated charcoal.

Why must fresh wild mushrooms be placed in a brown paper bag instead of a plastic Ziploc bag when bringing them to the hospital?

Fresh mushrooms have a very high water content and active enzymes. When sealed inside an airtight plastic bag, condensation builds up rapidly and bacteria on the mushroom surface multiply, turning the cap, gills, and stem into a slimy, decomposed mush within a few hours. A breathable brown paper bag or wax-paper wrap allows moisture to evaporate so the mushroom retains its structural gills, ring, and base for accurate mycological identification at the hospital.

My child chewed on a leaf of our indoor Philodendron / Dumb Cane (Dieffenbachia) plant and started crying and drooling immediately. Is this life-threatening?

In the vast majority of cases, no. *Dieffenbachia*, *Philodendron*, *Pothos*, and *Peace Lily* leaves contain microscopic bundles of needle-shaped insoluble calcium oxalate crystals (*raphides*). The instant a child or pet bites into the leaf, those microscopic needles shoot into the sensitive lining of the lips and tongue, causing immediate stinging pain and drooling. Because it hurts on the very first chew, children almost always spit it out immediately without swallowing a significant amount. Wipe out the mouth with a wet cloth, offer cold milk, water, or a popsicle to soothe the burning, and call Poison Control; go to the emergency room only if severe lip/tongue swelling, difficulty swallowing saliva, or noisy breathing develops.

What happens if a child swallows one of the bright red berries from a Yew bush (Taxus) in the yard?

It depends entirely on whether the child swallowed the berry whole or chewed the hard inner seed. The soft, sweet red cup (aril) surrounding a yew berry is actually the only part of the yew plant that does NOT contain lethal taxine cardiotoxins. In addition, if a child swallows the hard inner seed completely intact without chewing it, the tough seed coat usually passes through the digestive tract unbroken. However, if the child chewed and crushed the inner seed (or chewed on green yew needles or bark), lethal taxine alkaloids are absorbed and can cause fatal heart rhythm disturbances. Always call Poison Control immediately after any yew berry ingestion.

This guide is for informational purposes and does not replace professional medical evaluation or hands-on training. Because severe plant and mushroom poisonings can trigger seizures or cardiac arrest, we recommend taking an accredited first aid and CPR course through the International Federation of Red Cross and Red Crescent Societies (IFRC), your national Red Cross society, St John Ambulance, or a certified local training provider.
