Thyroid storm (thyrotoxic crisis) and myxedema coma (decompensated hypothyroidism) represent the two opposite, life-threatening extremes of thyroid gland dysfunction. In thyroid storm, a massive surge of active thyroid hormones (\(T_3\) and \(T_4\)) drives the body's metabolic engine into hyperdrive, causing extreme fever above \(40.0^\circ\text{C}\) (\(104.0^\circ\text{F}\)), racing heart rates above \(140\text{ beats per minute}\), atrial fibrillation, high-output heart failure, agitation, delirium, and fatal cardiovascular collapse. In myxedema coma, severe prolonged depletion of thyroid hormone slows every organ system to a near-standstill, resulting in profound hypothermia (core temperature often below \(35.0^\circ\text{C}\) or \(95.0^\circ\text{F}\)), dangerously slow heart rate, shallow hypoventilation with carbon dioxide buildup, low blood sugar, generalized non-pitting swelling, and coma. Because both conditions carry hospital mortality rates of 10 to 40 percent even with intensive care, immediate recognition, targeted temperature management (active cooling for thyroid storm versus passive insulation for myxedema coma), and avoiding deadly pre-hospital mistakes like giving aspirin or applying hot heating pads are critical to survival.
Two critical medication and warming traps that can kill thyroid crisis patients
- In Suspected Thyroid Storm: NEVER give aspirin (salicylates) to lower their high fever. Over 99 percent of thyroid hormone circulating in the blood is safely bound to carrier proteins. Aspirin chemically displaces thyroid hormone from those carrier proteins, instantly doubling or tripling the concentration of active free \(T_3\) and free \(T_4\) in the bloodstream and worsening the fatal thyroid storm. Use cool wet cloths, fans, and acetaminophen (paracetamol) instead.
- In Suspected Myxedema Coma: NEVER use electric heating blankets, hot water bottles, or hot baths to rewarm the cold patient. A patient in myxedema coma only stays alive by constricting all their skin blood vessels to keep their remaining blood near their heart and brain. Applying external heat forces their skin blood vessels to dilate suddenly, causing catastrophic rewarming shock and fatal cardiac arrest. Use passive external rewarming only (dry warm blankets and a warm room).
- Call your local emergency number immediately and state either "Suspected Thyroid Storm" or "Suspected Myxedema Coma." Dial 911 in the United States and Canada, 112 across the European Union, 999 in the United Kingdom, or see all أرقام الطوارئ. Explicitly report the patient's known thyroid history (Graves' disease, Hashimoto's thyroiditis, prior thyroid surgery, radioactive iodine therapy, or amiodarone/lithium use) and whether they are hyperthermic and racing or hypothermic and stuporous.
- Determine which extreme of thyroid emergency is occurring by checking temperature, pulse, skin, and mental state.
- Thyroid Storm (Hypermetabolic): Hot, drenched in sweat, fever \(\ge 38.5^\circ\text{C}\text{ to }41.0^\circ\text{C}\) (\(101.3^\circ\text{F}\text{ to }105.8^\circ\text{F}\)), pulse racing \(> 130\text{ to }180\text{ bpm}\) (often irregular), severe tremor, extreme agitation or delirium, vomiting, diarrhea, or yellow eyes (jaundice).
- Myxedema Coma (Hypometabolic): Ice-cold, dry, thick pale-yellowish skin, puffy eyelids and swollen tongue, body temperature \(< 35.5^\circ\text{C}\) (\(95.9^\circ\text{F}\)), pulse slow \(< 50\text{ bpm}\), slow shallow breathing, extreme drowsiness, confusion, or coma.
- For Suspected Thyroid Storm: Begin active external cooling immediately and NEVER give aspirin. Strip off heavy clothing, point a fan at the patient, mist their skin with cool (not freezing) water, and apply wrapped ice packs or cool wet towels to the neck, armpits, and groin. If the patient is fully awake and able to swallow, you may give acetaminophen (paracetamol) for fever, but strictly forbid aspirin, bismuth subsalicylate (Pepto-Bismol), or NSAIDs. Keep them sitting semi-upright (45 degrees) if they are breathless from heart failure.
- For Suspected Myxedema Coma: Protect the airway in the recovery position and apply PASSIVE blanket insulation only. Because an enlarged swollen tongue (macroglossia), throat edema, and depressed brain respiratory drive rapidly cause suffocation, roll an unconscious or stuporous patient onto their side in the recovery position and monitor every breath. Wrap them in dry blankets and raise the room thermostat, never applying heating pads, electric blankets, or hot water bottles.
- Check blood sugar or look for overlapping Adrenal Crisis (Schmidt Syndrome). In myxedema coma, severe hypoglycemia is common and co-existing acute adrenal insufficiency occurs in 5 to 10 percent of cases. Never force food or liquid into the mouth of a drowsy patient; if they have a prescribed emergency hydrocortisone injection kit for co-existing Addison's disease, administer it per the adrenal crisis guide.
- Gather all prescription bottles, supplement containers, and recent hospital discharge papers for the paramedics. Look specifically for anti-thyroid pills (methimazole, carbimazole, propylthiouracil [PTU]), thyroid hormone pills (levothyroxine, liothyronine), heart rhythm drugs (amiodarone), psychiatric drugs (lithium), cancer immunotherapy cards, or unlabelled "weight-loss / energy" supplements containing hidden bovine/porcine thyroid extract.

Head-to-head comparison: Thyroid Storm vs. Myxedema Coma¶
Every organ system in the human body has thyroid hormone receptors. Review this clinical comparison table (based on the American Thyroid Association [ATA], Japan Thyroid Association [JTA], and European Thyroid Association [ETA] guidelines) to see how opposite hormone levels transform the body:
| Organ System / Feature | Thyroid Storm (Hyperthyroid Crisis) | Myxedema Coma (Hypothyroid Crisis) |
|---|---|---|
| Underlying Hormone Defect | Extreme excess of free \(T_3\) and \(T_4\) combined with heightened cellular sensitivity to adrenaline (catecholamines). | Extreme depletion of \(T_3\) and \(T_4\), causing cellular metabolism and oxygen consumption to grind to a halt. |
| ملف المريض النموذجي | Untreated or partially treated Graves' disease, toxic multinodular goiter, toxic adenoma, or recent stopped methimazole/PTU. | Long-standing Hashimoto's thyroiditis, prior thyroidectomy or radioiodine ablation who ran out of or stopped levothyroxine; mostly older women (\(> 60\text{ years}\)) in winter. |
| Core Body Temperature | Severe Hyperthermia: \(38.5^\circ\text{C}\text{ to }41.5^\circ\text{C}\) (\(101.3^\circ\text{F}\text{ to }106.7^\circ\text{F}\)). | Profound Hypothermia: Often \(32.0^\circ\text{C}\text{ to }35.5^\circ\text{C}\) (\(89.6^\circ\text{F}\text{ to }95.9^\circ\text{F}\)) without shivering; even \(36.5^\circ\text{C}\) (\(97.7^\circ\text{F}\)) can hide severe sepsis. |
| Heart Rate & Rhythm | Extreme Tachycardia: \(130\text{ to }200+\text{ bpm}\) (disproportionate to fever); atrial fibrillation in 30% to 40% of cases. | Severe Bradycardia: \(30\text{ to }55\text{ bpm}\); low-voltage ECG with prolonged QT interval and risk of Torsades de Pointes. |
| Blood Pressure & Heart Function | Wide pulse pressure initially (high systolic, low diastolic), progressing to high-output heart failure, pulmonary edema, and cardiogenic shock. | Narrow pulse pressure (diastolic hypertension early from intense vasoconstriction, followed by refractory hypotensive shock and pericardial effusion). |
| Respiratory System | Tachypnea (rapid breathing \(> 25\text{ breaths/min}\)) and shortness of breath from pulmonary fluid overload and respiratory muscle fatigue. | Bradypnea and Hypoventilation: Slow, shallow breathing (\(< 8\text{ to }10\text{ breaths/min}\)) causing severe carbon dioxide retention (\(\text{CO}_2\) narcosis), hypoxia, and pleural effusions. |
| Central Nervous System | Hyperactive Delirium: Extreme agitation, anxiety, restlessness, emotional lability, psychosis, coarse tremor, seizures, and eventual stupor/coma. | Progressive CNS Depression: Extreme lethargy, slowed hoarse speech, memory loss, "myxedema madness" (paranoia), areflexia (delayed relaxation of reflexes), seizures, and coma. |

| Key Laboratory Hallmarks | Undetectable \(\text{TSH}\) (\(< 0.01\text{ mIU/L}\)), sky-high Free \(T_4\) / Free \(T_3\), high liver enzymes/bilirubin, mild hyperglycemia, and hypercalcemia. | Sky-high \(\text{TSH}\) (\(> 50\text{ to }100\text{ mIU/L}\), unless pituitary cause), undetectable Free \(T_4\), severe hyponatremia (\(\text{Na}^+ < 120\text{ mmol/L}\)), hypoglycemia, and high \(\text{PaCO}_2\). |
| Pre-Hospital Thermal Rule | ACTIVE COOLING (ice packs, cool mist, fans, acetaminophen) + NO ASPIRIN. | PASSIVE REWARMING ONLY (dry blankets, warm room) + NO HEATING PADS OR HOT WATER BOTTLES. |
Part 1: Deep dive into Thyroid Storm (Thyrotoxic Crisis)¶
Why simple hyperthyroidism suddenly explodes into Thyroid Storm¶
Most patients with overactive thyroid glands (hyperthyroidism) live for months with mild weight loss, warm hands, and a slightly fast pulse without immediate danger. Thyroid storm occurs when an acute physical stress or medical trigger overwhelms the body's ability to tolerate excess hormone, causing a sudden spike in unbound (free) \(T_3\) and \(T_4\) alongside a massive upregulation of beta-adrenergic receptors on the heart and blood vessels.
Common triggers that precipitate thyroid storm include: 1. Abruptly stopping anti-thyroid medication (methimazole, carbimazole, or propylthiouracil [PTU]) due to cost, illness, or side effects. 2. Acute bacterial or viral infection (pneumonia, urinary tract infection, strep throat, COVID-19, or sepsis). 3. Massive iodine load (Jod-Basedow phenomenon): Receiving iodinated intravenous CT contrast dye, taking amiodarone (each \(200\text{ mg}\) amiodarone tablet contains \(75\text{ mg}\) of organic iodine, equal to 500 times the daily human iodine requirement), or taking kelp/seaweed supplements. 4. Non-thyroidal surgery, trauma, childbirth, or Diabetic Ketoacidosis (دكا). 5. Vigorous palpation or trauma to an enlarged goiter, or recent radioactive iodine (\(^{131}\text{I}\)) therapy. 6. Factitious thyrotoxicosis: Accidental or intentional overdose of prescription levothyroxine/liothyronine or unregulated weight-loss capsules containing desiccated animal thyroid gland.
Recognizing the Burch-Wartofsky Point Scale (BWPS) clinical criteria¶
Emergency physicians diagnose thyroid storm clinically at the bedside (without waiting hours for thyroid hormone lab results) using the Burch-Wartofsky Point Scale (BWPS) and Japan Thyroid Association (JTA) criteria. A patient has life-threatening thyroid storm when thyrotoxicosis combines with multi-organ decompensation across these four domains:
- Thermoregulatory Failure (Fever \(\ge 38.0^\circ\text{C}\) to \(> 40.0^\circ\text{C}\) [\(100.4^\circ\text{F}\) to \(> 104.0^\circ\text{F}\)]): Unlike a normal infection where a \(39^\circ\text{C}\) fever raises the heart rate to roughly \(100\text{ to }110\text{ bpm}\), in thyroid storm the heart rate is wildly out of proportion to the fever, often racing at \(140\text{ to }190\text{ bpm}\) while sweat pours off the skin.
- Central Nervous System Dysfunction (The single strongest diagnostic clue): Ranges from mild extreme restlessness and emotional agitation to frank delirium, psychosis, extreme confusion, seizures, or coma. If a patient has a fast heart rate and fever plus altered mental status, suspect thyroid storm immediately.
- Gastrointestinal-Hepatic Dysfunction: Profuse watery diarrhea, severe nausea and vomiting, diffuse abdominal pain, and unexplained jaundice (yellowing of the whites of the eyes and skin) caused by congestive liver hypoxia and hepatocyte necrosis.
- Cardiovascular Decompensation: Racing sinus tachycardia or rapid atrial fibrillation (\(> 140\text{ bpm}\)) that causes the heart chambers to fill poorly, leading to pulmonary edema (frothy pink sputum, inability to lie flat, crackling lungs) and sudden circulatory shock.
Beware of 'Apathetic Thyroid Storm' in Older Adults
In adults over age 65, thyroid storm frequently presents in a disguised form called Apathetic Thyrotoxicosis. Instead of appearing hyperactive, agitated, and trembling, an older adult in thyroid storm may look calm, severely depressed, lethargic, weak, or apathetic, with weight loss, drooping eyelids, new-onset atrial fibrillation, and congestive heart failure. Never rule out thyroid storm in an elderly patient just because they are not visibly hyperactive.
Why hospital treatment of Thyroid Storm must follow a strict 4-step timed order¶
Understanding the hospital sequence helps caregivers advocate for the patient and understand why doctors ask specific questions at triage: 1. Step 1: Block Beta-Adrenergic Receptors (Propranolol, Esmolol, or Landiolol): Given immediately via IV or mouth to slow the racing heart rate below \(100\text{ bpm}\) and prevent fatal arrhythmias. (Critical caveat: If the patient already has severe low-ejection-fraction dilated cardiomyopathy, giving a long-acting beta-blocker like propranolol can cause sudden cardiac arrest; ICU doctors use ultra-short-acting intravenous esmolol or diltiazem backed by bedside ultrasound.) 2. Step 2: Block New Hormone Synthesis (Propylthiouracil [PTU] or Methimazole): Thionamide drugs stop the thyroid peroxidase (TPO) enzyme from making new thyroid hormone; PTU is often favored in the first 24 hours because it also blocks peripheral conversion of inactive \(T_4\) into active \(T_3\) by 40 percent. 3. Step 3: Block Release of Pre-Formed Hormone with Iodine (Lugol's Solution or SSKI), Strictly AT LEAST 1 HOUR AFTER Step 2: High-dose inorganic iodine locks the thyroid gland shut (the Wolff-Chaikoff effect). However, if iodine is given BEFORE the thyroid gland is blocked by PTU or methimazole, the thyroid gland uses that iodine as raw fuel to manufacture a massive new batch of thyroid hormone, pouring gasoline on the thyroid storm. 4. Step 4: Block \(T_4\)-to-\(T_3\) Conversion and Protect Adrenals with IV Hydrocortisone (\(100\text{ mg}\) IV every 8 hours): Because thyroid storm burns through the body's natural cortisol faster than the adrenal glands can make it (causing relative adrenal insufficiency) and because steroids block peripheral \(T_4 \to T_3\) conversion, every thyroid storm patient receives stress-dose intravenous corticosteroids.
Bedside Burch-Wartofsky Point Scale (BWPS) clinical reference table¶
Emergency physicians use the Burch-Wartofsky Point Scale to calculate the probability of thyroid storm at the bedside. A total score of 45 points or higher strongly indicates active thyroid storm, 25 to 44 points indicates impending thyroid storm, and below 25 points makes thyroid storm unlikely:
| Diagnostic Domain | Specific Clinical Finding at Triage | BWPS Score Points |
