In any adult aged 50 or older, the sudden onset of a new temple headache, scalp tenderness when combing hair, cramping pain in the jaw muscles while chewing food (jaw claudication), transient dimming of vision, double vision (diplopia), or sudden blindness in one eye is a time-critical medical emergency caused by giant cell arteritis (GCA), also known as temporal arteritis or Horton disease. Call your local emergency number (911 in the United States and Canada, 112 across the European Union, 999 in the United Kingdom) or consult our بیړنۍ شمیرې immediately and go straight to a hospital emergency department for stat high-dose intravenous corticosteroids. While arranging transport, distinguish arteritic vision loss from non-arteritic thromboembolic central retinal artery occlusion (CRAO) eye stroke, review our broader sudden vision loss and retinal detachment guide, and check for concurrent signs of cerebral stroke.
Giant cell arteritis is a systemic, T-cell-mediated granulomatous vasculitis that attacks medium-sized and large arteries containing an internal elastic lamina, with a relentless predilection for the extracranial branches of the external and internal carotid arteries, especially the superficial temporal artery, maxillary artery, ophthalmic artery, and short posterior ciliary arteries. When inflammatory intimal hyperplasia swells the walls of the short posterior ciliary arteries until their lumens clamp shut, the optic nerve head suffers catastrophic infarction (arteritic anterior ischemic optic neuropathy [A-AION]), causing profound, irreversible blindness (often down to no light perception). Once vision is lost in the first eye, it rarely recovers even with aggressive therapy. However, without immediate high-dose corticosteroid treatment, 30 to 50 percent of patients will suffer permanent, total blindness in their second (fellow) eye within 24 hours to 7 days. International guidelines from the American College of Rheumatology (ACR), EULAR, and the British Society for Rheumatology (BSR) mandate that emergency physicians administer intravenous methylprednisolone (500 to 1,000 milligrams daily) or high-dose oral prednisone immediately upon clinical suspicion, without waiting hours or days for erythrocyte sedimentation rate (ESR) lab results, temporal artery ultrasound, or temporal artery biopsy.
- Go immediately to a hospital emergency department (not an urgent care clinic, dental office, or routine optometry clinic) if an adult aged 50 or older develops visual changes OR classic cranial arteritis symptoms. Treat transient misty vision (amaurosis fugax), a missing upper or lower half of vision (altitudinal defect), double vision (diplopia), or new headache with jaw claudication as an immediate threat to bilateral sight.
- Tell the emergency triage nurse and physician in your very first sentence: "We suspect Giant Cell Arteritis with visual risk and need immediate high-dose intravenous steroids to save the second eye." Do not wait passively in a hospital waiting room while holding a blurry eye. Every hour of delay before the first dose of intravenous methylprednisolone (500 to 1,000 mg IV) increases the risk that the inflammatory cascade will occlude the posterior ciliary arteries of the fellow eye.
- Never allow a physician to delay starting corticosteroids while waiting for a temporal artery biopsy or outpatient ultrasound. A common and devastating medical error is withholding prednisone or methylprednisolone out of fear that steroids will "ruin" the temporal artery biopsy result. Pathological granulomatous inflammation and giant cells remain clearly visible in the temporal artery wall for at least 10 to 14 days (and often up to 4 weeks) after starting high-dose corticosteroids. Steroids come first; the biopsy comes second.
- Recognize "jaw claudication" and "diplopia" as imminent pre-blindness warning signs. If your jaw muscles ache, cramp, or grow exhausted after 1 to 2 minutes of chewing meat, crusty bread, or tough food and feel better when you stop chewing, this is NOT temporomandibular joint (TMJ) disorder or a bad tooth. It is ischemic claudication of the masseter and temporalis muscles caused by inflammatory narrowing of the maxillary and facial arteries, and it carries the highest statistical correlation with impending blindness in GCA. Similarly, transient double vision precedes permanent blindness by hours or days in up to 15 percent of patients.
- Lay the patient flat (supine) if acute vision loss or transient dimming has already started. Lying flat maximizes systemic arterial perfusion pressure through the narrowed ophthalmic and posterior ciliary arteries feeding the optic nerve head while intravenous methylprednisolone is being prepared.
- Check for FAST stroke signs and vertebrobasilar symptoms. Because giant cell arteritis also inflames the vertebral arteries and internal carotid arteries in 3 to 7 percent of patients, check for dizziness, vertigo, unsteady gait, slurred speech, or one-sided weakness, and report these immediately to the emergency team.
Why Waiting for Blood Tests or a Biopsy Blinds the Second Eye
In untreated giant cell arteritis with unilateral vision loss, one-third to one-half of patients go permanently blind in their remaining good eye within 1 to 7 days, sometimes within less than 24 hours. In addition, up to 4 to 10 percent of biopsy-proven GCA patients have a normal ESR or normal CRP, meaning a normal blood test never rules out GCA when clinical symptoms are compelling. Emergency physicians must draw blood for ESR, CRP, and platelets and start 500 to 1,000 mg of intravenous methylprednisolone immediately on the spot, then schedule the temporal artery ultrasound and temporal artery biopsy within the following 7 to 14 days.

Pathophysiology: How autoimmune granulomatous vasculitis strangles the optic nerve¶
Understanding why giant cell arteritis targets specific arteries in adults over age 50 explains both its bizarre combination of symptoms (jaw pain, scalp tenderness, shoulder stiffness, and sudden blindness) and why standard clot-busting drugs cannot reverse the optic nerve infarction.
1. Why Giant Cell Arteritis strikes exclusively adults aged 50 and older¶
Giant cell arteritis virtually never occurs in individuals under age 50, and its incidence rises steeply with each decade past 50, peaking between ages 70 and 80 years (with highest prevalence in populations of Northern European and Scandinavian ancestry, and a 2:1 to 3:1 female-to-male ratio). - Elastic Lamina Senescence and Adventitial Dendritic Cells: Medium and large arteries possess an internal elastic lamina (a ring of elastic fibers separating the inner intima from the muscular media) and are nourished by microscopic blood vessels in their outer wall called the vasa vasorum. With aging past age 50, age-related structural changes in elastin fibers and immune senescence activate specialized toll-like receptors on indigenous adventitial dendritic cells residing in the outer wall of cranial arteries and the thoracic aorta. - T-Cell Macrophage Granulomatous Cascade: Activated dendritic cells recruit CD4+ T lymphocytes through the vasa vasorum via two distinct inflammatory pathways: 1. The Interleukin-6 (IL-6) and IL-17 (Th17) pathway, which drives systemic hepatic acute-phase reactants (skyrocketing ESR, CRP, fibrinogen, and reactive thrombocytosis) and causes fever, night sweats, weight loss, and polymyalgia rheumatica (PMR); and 2. The Interferon-gamma (IFN-\(\gamma\), Th1) and macrophage pathway, which triggers macrophages to fuse into multinucleated giant cells that release matrix metalloproteinases (destroying the internal elastic lamina) and platelet-derived growth factor (PDGF) and vascular endothelial growth factor (VEGF). - Luminal Occlusion by Intimal Hyperplasia (Not a Simple Blood Clot): Under the stimulation of PDGF, vascular smooth muscle cells migrate inward and proliferate rapidly, causing massive concentric intimal hyperplasia. The inner lining of the artery swells inward until the central channel (lumen) is squeezed down to a pinhole or 100 percent occluded.

2. Why posterior ciliary artery occlusion causes "Chalky-White" Optic Nerve Infarction (A-AION)¶
While the superficial temporal artery on the forehead gets the disease's nickname, the actual blinding culprit inside the orbit is almost always inflammatory occlusion of the short posterior ciliary arteries (SPCAs): - Between 10 and 20 short posterior ciliary arteries branch off the ophthalmic artery and pierce the sclera in a circle (circle of Zinn-Haller) directly around the optic nerve head (optic disc) to supply the prelaminar and laminar optic nerve head as well as the entire choroidal choriocapillaris.

- Occult "Silent" Systemic GCA (Fever of Unknown Origin): About 10 to 15 percent of GCA patients have zero headache or scalp tenderness and present instead with weeks of unexplained daily fevers, drenching night sweats, profound anorexia, 5 to 10 kilograms (11 to 22 pounds) of unintentional weight loss, and normocytic anemia, or they present with sudden "occult" painless blindness in one eye accompanied only by sky-high ESR and CRP.

+-------------------------------------------------------------------------+
Why temporal artery biopsy length (\(\ge 1.5\text{ to }2.0\text{ cm}\)) matters: The "Skip Lesion" phenomenon¶
Giant cell arteritis does not inflame the entire length of an artery uniformly; instead, it attacks in patchy, discontinuous segments called "skip lesions," where a 5-millimeter stretch of severely inflamed artery with multinucleated giant cells and fragmented elastic lamina sits right next to a 5-millimeter stretch of completely normal artery wall. - Therefore, when a vascular surgeon, ophthalmologist, or otolaryngologist performs a superficial temporal artery biopsy (a safe 30-minute procedure under local anesthesia along the hairline on the symptomatic side), guidelines require harvesting an arterial segment at least 1.5 to 2.0 centimeters long (after formalin fixation shrinkage) and examining multiple serial cross-sections so skip lesions are not missed. - Even if a temporal artery biopsy returns negative (which occurs in 10 to 15 percent of true GCA cases due to skip lesions or purely extracranial/posterior ciliary involvement), a negative biopsy never overrides a classic clinical presentation with elevated ESR/CRP and visual loss; clinicians continue full corticosteroid and tocilizumab treatment.
Fast-track vascular ultrasound ("Halo Sign") and high-resolution MRI¶
Many modern rheumatology and emergency centers now perform fast-track color duplex ultrasonography of the superficial temporal arteries (both frontal and parietal branches) and axillary arteries within 24 to 48 hours of presentation: - Active granulomatous edema of the arterial wall produces a characteristic dark (hypoechoic), circumferential ring around the color Doppler blood flow lumen called the "Halo Sign," which fails to flatten when the ultrasound probe presses down on the skin (positive "Compression Sign"). - Per EULAR and ACR guidelines, a positive temporal/axillary ultrasound halo sign in a patient with high clinical probability confirms the diagnosis of GCA with high specificity, while high-resolution 3-Tesla cranial vessel-wall MRI or whole-body FDG-PET/CT can detect large-vessel aortitis and subclavian arteritis.
Exact communication script for hospital triage¶
When arriving at a hospital emergency department with an older adult who has visual symptoms or classic cranial signs of giant cell arteritis, read this script directly to the triage nurse and emergency physician:
"I am here with a [Age]-year-old patient who developed sudden [vision loss / brief dimming of vision / double vision] in the [left/right] eye today, preceded by [a new temple headache / scalp pain when brushing hair / jaw cramping pain after chewing food for a minute]. We strongly suspect Giant Cell Arteritis (Temporal Arteritis) with acute ischemic optic neuropathy risk. Because 30 to 50 percent of untreated patients go permanently blind in their second eye within days, ACR and EULAR guidelines recommend immediate high-dose intravenous methylprednisolone (500 to 1,000 mg IV) right now in the emergency room BEFORE waiting for ESR and CRP blood results or a temporal artery biopsy. Please have an emergency physician evaluate the patient immediately."
Red-flag checklist for patients with Polymyalgia Rheumatica (PMR) or tapering steroids¶
If you or an older family member have already been diagnosed with Polymyalgia Rheumatica (PMR) or are currently tapering down oral prednisone for Giant Cell Arteritis, tape this checklist to your refrigerator and return to the emergency room immediately if any item occurs:
- Any brief dimming, blurriness, fog, or curtain over one eye (even if it clears up after 2 minutes).
- Any episode of double vision (seeing two images side-by-side or on top of each other), even if temporary.
- New fatigue, ache, or cramping in the jaw muscles or tongue while chewing food.
- Return of temple headache or tenderness when touching the scalp or putting on glasses.
- Never stop or skip your prescribed prednisone or tocilizumab doses abruptly: Stopping steroids suddenly during active GCA can trigger both an immediate blindness relapse and life-threatening acute adrenal crisis (see our adrenal crisis emergency guide).

Taking a certified hands-on first aid course from the Red Cross, St John Ambulance, or a local emergency training provider prepares you to recognize sudden neurological and vascular emergencies, assess visual and stroke red flags, and advocate effectively for rapid hospital care.
Why do doctors give massive intravenous steroid doses for Giant Cell Arteritis before doing the temporal artery biopsy?¶
In Giant Cell Arteritis, once the blood supply to the optic nerve is blocked in the first eye, vision in that eye is almost always permanently lost, and without immediate steroid treatment, 30 to 50 percent of patients will lose sight in their remaining good eye within 24 hours to 7 days. Starting high-dose intravenous methylprednisolone immediately shuts down the vascular inflammation to save the second eye, and pathologists can still clearly identify the arterial wall inflammation on a temporal artery biopsy performed up to 2 weeks after steroids are started.
How can I tell the difference between Giant Cell Arteritis jaw claudication and ordinary TMJ jaw joint pain or a toothache?¶
Temporomandibular joint (TMJ) disorder and dental infections cause immediate sharp pain the moment you bite down hard, open your mouth wide, or tap on a specific tooth, often accompanied by a clicking jaw joint. Jaw claudication from Giant Cell Arteritis is an arterial blood supply problem just like heart angina: your first few bites of food feel completely fine, but after 1 to 2 minutes of continuous chewing (especially meat or crusty bread), your jaw muscles develop a deep, cramping ache and exhaustion that forces you to stop chewing, and the pain vanishes after 1 to 2 minutes of rest.
