Emergency numbers differ by country. This page uses 911, the number for the United States and Canada. Many other countries use 112, 999, or another number. Find the number for your country.

1. Why the First CT Angiogram (CTA) Is Normal in Up to 30 Percent of RCVS Patients (The Centripetal March of Vasospasm)

A critical clinical pitfall in emergency neurology is ruling out RCVS when a patient presents on Day 1 or Day 2 of recurrent thunderclap headaches and has a normal CT angiogram (CTA) or MR angiogram (MRA): - Distal-to-Proximal (Centripetal) Progression: In RCVS, sympathetic dysregulation begins in the microscopic distal pial and leptomeningeal arterioles over the cerebral convexities (vessels \(< 0.5\text{ mm}\) in diameter, which are below the spatial resolution of CTA and MRA). - Why Convexity SAH Occurs Early (Days 1 to 5) While Stroke Occurs Late (Days 7 to 14): When those tiny distal pial arterioles abruptly constrict and then reperfuse over the surface of the brain during the first few days, they can rupture into a superficial cortical sulcus, causing a small non-aneurysmal convexity subarachnoid hemorrhage (cSAH) (seen on Brain MRI FLAIR/SWI in up to 30 percent of RCVS patients) while the large Circle of Willis arteries still look completely normal on CTA. - Peak Proximal "String-of-Beads" Narrowing at Days 10 to 16: Over the second week, the vasoconstriction marches centripetally (inward) into the large M1/M2 middle cerebral, A1/A2 anterior cerebral, and P1/P2 posterior cerebral arteries. Therefore, visible "string-of-beads" alternating constriction and ectasia on CTA/MRA often does not peak until 10 to 16 days after the first thunderclap headache, explaining why ischemic strokes in RCVS typically occur in Week 2 (often after the headaches are already improving). If initial CTA/MRA is normal in a patient with classic recurrent thunderclap headaches, repeat CTA/MRA or Transcranial Doppler (TCD) in 5 to 7 days.

2. Why PRES Strikes the Occipital and Parietal Lobes First (The Sympathetic Innervation Gradient)

Why does acute hypertension cause swelling in the back of the brain (parieto-occipital lobes) rather than the front of the brain? - Dense Anterior vs. Sparse Posterior Sympathetic Tone: The anterior cerebral circulation (derived from the internal carotid arteries supplying the frontal and temporal lobes) receives dense perivascular sympathetic adrenergic innervation from the superior cervical ganglion. When systemic blood pressure surges acutely, those sympathetic nerves constrict the anterior arterioles to shield the frontal capillary beds from high hydrostatic pressure. - The Vertebrobasilar Vulnerability: By contrast, the posterior vertebrobasilar circulation (supplying the occipital lobes, posterior parietal lobes, and cerebellum) has sparse sympathetic adrenergic fibers. When Mean Arterial Pressure surges above the autoregulatory ceiling (\(150\text{ to }160\text{ mmHg}\)), or even at mild blood pressure elevations (\(140/90\text{ mmHg}\)) when tacrolimus, cyclosporine, or eclampsia has already injured endothelial tight junctions, the posterior arterioles passively dilate ("autoregulatory breakthrough"), forcing protein-rich plasma across the blood-brain barrier into the parieto-occipital white matter (vasogenic edema).

Side-by-side comparison: RCVS vs. PRES vs. Aneurysmal SAH vs. Primary CNS Vasculitis (PACNS)

Clinical & Radiologic Feature Reversible Cerebral Vasoconstriction Syndrome (RCVS) Posterior Reversible Encephalopathy Syndrome (PRES) Aneurysmal Subarachnoid Hemorrhage (aSAH) Primary Angiitis of the CNS (PACNS / CNS Vasculitis)
Headache Onset & Pattern RECURRENT Thunderclap Headaches (\(2\text{ to }10+\) explosive 10/10 attacks over \(1\text{ to }14\text{ days}\), peaking in \(< 60\text{ sec}\)), triggered by orgasm, exertion, Valsalva, or emotion. Subacute, dull, throbbing holocephalic or occipital headache (rarely thunderclap unless overlapped with RCVS). Single catastrophic Thunderclap Headache + meningismus (stiff neck), photophobia, syncope, or coma. Insidious, chronic, dull progressive headache developing over weeks to months (almost never thunderclap).
Dominant Neurological Presentation Excruciating recurrent thunderclap headaches; focal deficits or visual symptoms occur if complicated by cSAH, lobar ICH, or Week-2 watershed stroke. Encephalopathy/Confusion (\(50\%\text{ to }80\%\)), Generalized Seizures (\(60\%\text{ to }75\%\)), and Visual Disturbances (\(40\%\text{ to }60\%\): cortical blindness, Anton syndrome, hemianopia). Cranial nerve III palsy (blown pupil), depressed consciousness, delayed cerebral ischemia (Days 4 to 14). Stepwise cognitive decline, multifocal subacute strokes, personality change over months.
Classic Precipitating Triggers SSRIs, SNRIs, Triptans, Nasal Decongestants, Cannabis, Cocaine, Amphetamines, Postpartum state, catecholamine tumors (pheochromocytoma). Acute Severe Hypertension, Pre-eclampsia / Eclampsia, Tacrolimus, Cyclosporine, Bevacizumab / Cisplatin, Renal Failure / Lupus (SLE). Rupture of a saccular berry aneurysm at Circle of Willis bifurcations (AComm, PComm, MCA); smoking, ADPKD. Autoimmune granulomatous or lymphocytic inflammation of small/medium CNS vessels; no vasoactive drug trigger.
Brain MRI (DWI / ADC / FLAIR / SWI) Findings Often normal initially; Convexity Sulcal SAH (cSAH) on FLAIR/SWI (\(30\%\)); watershed infarcts or lobar ICH in severe cases; \(10\%\text{ to }35\%\) have concomitant posterior PRES edema. Symmetric Parieto-Occipital (or Superior Frontal Sulcus / Cerebellar) T2/FLAIR Hyperintensity with HIGH ADC (Vasogenic Edema, vs. low ADC in cytotoxic stroke). Blood filling the basal cisterns, Sylvian fissures, and ventricles on non-contrast CT and FLAIR/SWI. Multiple bilateral subcortical and deep gray/white matter chronic and subacute infarcts of varying ages + leptomeningeal enhancement.
Vascular Imaging (CTA / MRA / DSA) & CSF Multifocal segmental "String-of-Beads" vasoconstriction and dilatation (peaks Day 10-16; resolves in \(\le 12\text{ weeks}\)). CSF is normal or near-normal (\(\text{WBC} < 10\), protein \(< 60\text{ mg/dL}\)). Normal cerebral arteries in \(65\%\text{ to }85\%\); concomitant RCVS-like vasoconstriction in \(15\%\text{ to }35\%\). Normal or mildly elevated CSF protein. Saccular berry aneurysm on CTA/DSA; xanthochromia and \(> 10,000\text{ RBCs/\mu L}\) in Tube 4 of LP. Irregular distal vessel narrowing that does not reverse in 12 weeks; Inflammatory CSF (\(> 90\%\) have lymphocytic pleocytosis and high protein).
Emergency Blood Pressure & Steroid Rule AVOID Hypotension (keep normal perfusion to prevent watershed stroke); give Nimodipine or Verapamil. STEROIDS ARE STRICTLY CONTRAINDICATED (Worsen Outcome). RAPIDLY Lower MAP by 20% to 25% in First 1 to 2 Hours using IV Nicardipine, Clevidipine, or Labetalol. Steroids do NOT reduce vasogenic edema. Lower SBP \(< 140\text{ to }160\text{ mmHg}\) before aneurysm is secured; give oral Nimodipine \(60\text{ mg}\) q4h. Step-by-Step Hospital Management Protocols for RCVS and PRES

Protocol A: Hospital Management of Reversible Cerebral Vasoconstriction Syndrome (RCVS)

  1. Immediate Trigger Elimination & Bed Rest:
  2. Stop every serotonergic, adrenergic, and vasoactive substance immediately (triptans, ergots, SSRIs, SNRIs, nasal decongestants, cannabis, nicotine patches, amphetamines, cocaine).
  3. Place the patient on strict physical rest in a quiet room; prohibit Valsalva maneuvers, heavy lifting, sexual activity, and straining at stool (prescribe scheduled senna/docusate/polyethylene glycol stool softeners and antitussives if coughing).
  4. Calcium Channel Blocker Vasospasm Therapy & Blood Pressure Preservation:
  5. Administer Oral Nimodipine (\(30\text{ to }60\text{ mg}\) PO every 4 hours) or Sustained-Release Oral Verapamil (\(120\text{ to }240\text{ mg/day}\)) (or IV Nicardipine / Nimodipine if unable to take oral medications).
  6. Critical Hemodynamic Rule: Monitor blood pressure after each dose of nimodipine or verapamil. Never allow systolic blood pressure to drop below \(110\text{ to }120\text{ mmHg}\) (or MAP to drop \(> 20\%\) in a patient with severe proximal M1/A1/P1 vasoconstriction), because hypoperfusion across a constricted artery causes bilateral watershed infarction. Only treat acute hypertensive spikes (\(> 180/110\text{ mmHg}\)) gently.
  7. Safe Analgesia for Severe Headaches (Zero Triptans, Zero Steroids):
  8. Treat severe pain with IV Acetaminophen (Paracetamol), IV Magnesium Sulfate (\(1\text{ to }2\text{ g}\)), and short-term IV/Oral Opioids (Hydromorphone or Morphine) if necessary. Never give triptans, ergotamines, indomethacin (which reduces cerebral blood flow), or corticosteroids.
  9. Refractory Severe RCVS with Progressive Ischemic Deficits:
  10. Monitor daily or every-other-day Transcranial Doppler (TCD) mean flow velocities in the middle cerebral arteries. If a patient develops progressive focal neurological deficits (hemiparesis, aphasia, cortical blindness) from severe proximal vasospasm despite oral nimodipine and normotension, transfer to the Neuro-ICU for induced normotensive/mildly hyperdynamic euvolemia and endovascular intra-arterial vasodilator infusion (intra-arterial Verapamil, Nicardipine, or Milrinone) or cautious balloon angioplasty.

Protocol B: Hospital Management of Posterior Reversible Encephalopathy Syndrome (PRES)

  1. Immediate Controlled Blood Pressure Reduction (The 20% to 25% Rule):
  2. In the 70 to 80 percent of PRES patients who present with moderate-to-severe acute hypertension, immediately start a continuous, titratable intravenous antihypertensive infusion:
    • First-Line: Intravenous Nicardipine starting at \(5\text{ mg/hour}\) IV, titrated by \(2.5\text{ mg/hour}\) every 5 to 15 minutes (maximum \(15\text{ mg/hour}\)).
    • Alternative First-Line: Intravenous Clevidipine (\(1\text{ to }2\text{ mg/hour}\) IV, doubled every 90 seconds to target) or Intravenous Labetalol (\(10\text{ to }20\text{ mg}\) IV bolus over 2 minutes, repeated or infused at \(2\text{ to }8\text{ mg/minute}\)).
    • Avoid Sodium Nitroprusside and Nitroglycerin: Both cause direct cerebral venodilation and raise intracranial pressure (ICP), which can worsen cerebral edema in severe PRES.
  3. Target: Reduce Mean Arterial Pressure (MAP) by 20% to 25% within the first 1 to 2 hours, aiming for a systolic blood pressure of \(140\text{ to }160\text{ mmHg}\) and diastolic blood pressure of \(85\text{ to }100\text{ mmHg}\). Do not drop MAP by more than 25 percent in the first 2 hours, especially if concomitant RCVS vasoconstriction is present.
  4. Discontinue or Dose-Reduce Endothelial Cytotoxins (Tacrolimus / Cyclosporine / Bevacizumab):
  5. Note that 20 to 30 percent of PRES patients have normal or only mildly elevated blood pressure at presentation because their PRES is driven by direct endothelial tight-junction injury from tacrolimus, cyclosporine, chemotherapy, or sepsis rather than sheer hydrostatic pressure.
  6. Check a STAT whole-blood Tacrolimus or Cyclosporine trough level (though tacrolimus-induced PRES frequently occurs even when blood levels are within the "therapeutic" range). Coordinate immediately with the transplant or oncology team to hold the calcineurin inhibitor or anti-VEGF agent and transition to an alternative immunosuppressant (such as mycophenolate mofetil or a cautious calcineurin switch).
  7. Aggressive Seizure Termination and Airway Protection:
  8. Because non-convulsive status epilepticus (especially occipital lobe focal status epilepticus causing prolonged confusion or visual hallucinations/blindness) is common in PRES, obtain continuous EEG monitoring in any patient who remains encephalopathic after a seizure.
  9. Load Intravenous Levetiracetam (\(40\text{ to }60\text{ mg/kg}\), up to \(3,000\text{ to }4,500\text{ mg}\) IV over 15 minutes) after terminating active convulsions with IV Lorazepam (\(0.1\text{ mg/kg}\), up to \(4\text{ mg}\)).
  10. If Pregnant or Postpartum (Eclamptic PRES): Intravenous Magnesium Sulfate (\(4\text{ to }6\text{ g}\) IV load over 20 minutes, then \(1\text{ to }2\text{ g/hour}\)) is vastly superior to phenytoin or diazepam and is the mandatory gold standard, alongside urgent obstetric delivery if antepartum.

How Diffusion-Weighted MRI (DWI and ADC Map) Separates Reversible PRES from Permanent Stroke

Both acute posterior cerebral artery (PCA) ischemic stroke and PRES cause bright T2/FLAIR white-matter signal in the occipital lobes and cortical blindness. The Apparent Diffusion Coefficient (ADC) map on Brain MRI immediately tells them apart: - Acute Ischemic Stroke (Cytotoxic Edema): Energy failure causes water to move inside dying cells, restricting Brownian motion. DWI is BRIGHT and the ADC map is DARK (low ADC); tissue is irreversibly infarcted. - PRES (Vasogenic Edema): Hydrostatic fluid leaks into the extracellular interstitial space between intact brain cells, increasing water mobility. T2/FLAIR is BRIGHT and the ADC map is BRIGHT (elevated ADC); when blood pressure is lowered and the offending toxin is stopped, the fluid reabsorbs and vision returns completely within 3 to 14 days. (If tiny patches of dark ADC appear inside a bright PRES lesion, cytotoxic infarction has begun and prognosis is guarded).

How can doctors tell a Thunderclap Headache caused by RCVS apart from a Ruptured Brain Aneurysm (Subarachnoid Hemorrhage)?

Every patient with a first thunderclap headache must immediately undergo a non-contrast head CT and CT angiogram (CTA) to rule out a ruptured saccular berry aneurysm. However, two bedside clues point strongly toward Reversible Cerebral Vasoconstriction Syndrome (RCVS): (1) Recurrence, while a ruptured aneurysm typically causes one devastating headache with neck stiffness or loss of consciousness, RCVS characteristically causes 2 to 10 separate, explosive thunderclap headaches over 1 to 2 weeks, each triggered by sexual orgasm, straining on the toilet, coughing, exertion, or taking a hot/cold shower; and (2) Medication history, over half of RCVS patients recently took serotonergic or vasoactive substances such as SSRI antidepressants, migraine triptans, over-the-counter nasal decongestants, or cannabis, or gave birth within the past 6 weeks.

Because RCVS produces a "string-of-beads" narrowing of brain arteries on CT or MR angiography that looks visually similar to Primary CNS Vasculitis (autoimmune inflammation of brain blood vessels), doctors used to give high-dose intravenous steroids "just in case." Landmark clinical studies by Singhal and colleagues proved that steroids do not help RCVS; in fact, giving glucocorticoids in RCVS triples the risk of clinical deterioration, worsens cerebral vasoconstriction (by increasing blood vessel sensitivity to adrenaline and noradrenaline), and causes permanent stroke or death. Using the bedside RCVS2 score allows doctors to confirm RCVS and withhold steroids safely.

Why does PRES cause sudden blindness even though the patient's eyes and optic nerves are completely healthy?

The blood vessels at the back of the brain (the vertebrobasilar system, which supplies the occipital lobes where the primary visual cortex lives) have far fewer protective sympathetic nerve fibers than the blood vessels at the front of the brain. When blood pressure spikes dangerously high or a drug like tacrolimus damages the blood-brain barrier, fluid leaks selectively into the occipital lobes at the back of the head. Because the occipital cortex is the "movie screen" that processes vision for both eyes, swelling there causes acute cortical blindness, the patient's pupils still react normally to light and their retinas look normal, but their brain cannot process the image until blood pressure is lowered and the occipital swelling resolves.
