Hemoptysis is the coughing up of blood or blood-streaked mucus from the lower respiratory tract (the windpipe [trachea], bronchial airways, or deep lung tissue). While tiny streaks of blood mixed with phlegm during acute bronchitis are often non-life-threatening, massive hemoptysis (also called life-threatening hemoptysis) is an immediate airway crisis in which rapid bleeding into the bronchial tree threatens to drown the lungs long before the patient loses enough blood volume to bleed to death. Because the entire conducting airway system of both adult lungs (the trachea and bronchi, known as the anatomical dead space) holds only 150 milliliters (5 fluid ounces, or roughly two-thirds of a coffee cup), just 100 to 150 milliliters of brisk arterial bleeding can completely plug the main airways with blood clots and cause fatal asphyxiation (suffocation) within minutes. Immediate pre-hospital survival depends on keeping the patient calm, letting them cough blood out into a container, never laying them flat on their back, and positioning them Bleeding-Lung-Side DOWN if the bleeding side is known so gravity keeps the healthy upper lung clear to breathe.
People with massive hemoptysis die from suffocation, not blood loss: protect the healthy lung first
Unlike a severed femoral artery or a ruptured abdominal aneurysm where death is caused by hemorrhagic shock (losing liters of blood), 95 percent of deaths from massive hemoptysis are caused by asphyxiation when blood spills out of the bleeding lung across the main bronchial fork (the carina) and floods the healthy, non-bleeding lung. Never lay a patient coughing up blood flat on their back, and never give them cough-suppressant medication (codeine or dextromethorphan): suppressing their cough traps blood inside the windpipe where it clots into a solid branching plug (a bronchial cast) that suffocates both lungs.
Gerðu þetta núna¶
- Call your local emergency number immediately for ANY brisk, pure, or repeated coughing of blood, or any hemoptysis with shortness of breath. Dial 911 in the United States and Canada, 112 across the European Union, 999 in the United Kingdom, or see all neyðarnúmer. Tell the dispatcher: "The patient is coughing up bright red frothy blood (suspected massive hemoptysis) and needs immediate advanced airway ambulance transport."
- Position the person immediately to keep their healthy lung clear of blood:
- If you DO NOT know which lung is bleeding AND the person is awake: Sit them upright and leaning slightly forward (resting their forearms on a table or knees) so gravity helps them cough and spit blood forward out of their windpipe rather than letting it pool backward across both lungs.
- If you DO know which lung is bleeding (from a known right or left lung tumor, cavity, tuberculosis lesion, bronchiectasis lobe, recent lung biopsy, or localized gurgling chest pain): Roll the patient onto their side with the BLEEDING LUNG SIDE DOWN (Lateral Decubitus Toward the Pathology). Lying with the bleeding lung on the bottom uses gravity to trap the blood inside the sick bottom lung while keeping the healthy top lung high, dry, and completely free to breathe.
- If the person becomes drowsy or unconscious and the bleeding side is unknown: Roll them onto their side in the recovery position (never flat on their back).
- Give the person a clear cup, bowl, or basin to spit into, and encourage gentle, controlled clearing coughs (NEVER suppress the cough). Ask the patient to spit all coughed-up blood into a single container rather than flushing it down the sink or toilet. Seeing the exact volume and appearance of the blood (bright red and frothy versus dark brown coffee-grounds) tells emergency doctors instantly whether it is true arterial lung bleeding (hemoptysis) or stomach bleeding (hematemesis).
- Withhold all food, water, oral medications, and especially blood thinners or cough suppressants. Keep the person strictly Nothing by Mouth (NPO) because they may require immediate emergency endotracheal intubation, rigid bronchoscopy, or interventional radiology Bronchial Artery Embolization (BAE) upon hospital arrival. Do not let them take their next dose of aspirin, clopidogrel, warfarin, apixaban, or rivaroxaban.
- Check immediately for rare, catastrophic Tracheoinnominate Artery Fistula (TIF) if the patient has a neck tracheostomy tube. If a patient with a tracheostomy tube suddenly coughs or spurts bright red blood out of or around their neck tube, inflate the tracheostomy cuff to maximum pressure (\(50\text{ mmHg}\)) immediately and leave the tube in place; if massive bleeding continues around an uncuffed tube, trained caregivers must perform the Utley Finger Maneuver (compressing the innominate artery forward against the back of the breastbone).
- Keep the room quiet, coach slow breaths between coughs, and gather oncology, pulmonary, and anticoagulant records for paramedics. Panic and violent coughing spikes blood pressure inside the fragile high-pressure bronchial arteries. Reassure the patient calmly, loosen tight clothing, and locate any recent chest CT reports showing which lobe or lung has a tumor, cavity, or bronchiectasis.

Step 1: Is it really coming from the lungs? (Hemoptysis vs. Hematemesis vs. Pseudohemoptysis)¶
When blood suddenly appears in a person's mouth, basin, or bathroom sink, even experienced clinicians must first determine whether the blood was coughed up from the lungs (hemoptysis), vomited up from the esophagus or stomach (hematemesis), or drained down the back of the throat from a nosebleed or bleeding gums (pseudohemoptysis). Complicating matters further, a person who has a nosebleed or stomach bleed while sleeping may inhale a drop of blood onto their vocal cords and cough it out, while a person who coughs up lung blood in their sleep may swallow it into their stomach and vomit it up later.
Use this side-by-side clinical comparison table to identify the true source:
| Diagnostic Clue | True Hemoptysis (Lungs & Bronchial Airways) | Hematemesis (Esophagus, Stomach, or Duodenum) | Pseudohemoptysis (Nose, Nasopharynx, Mouth, or Gums) |
|---|---|---|---|
| How It Is Expelled | Coughed up following an irresistible tickle, warmth, or gurgling rattle deep behind the breastbone or in one side of the chest. | Vomited or retched up following waves of nausea, stomach cramping, or forceful abdominal heaving. | Cleared from the throat or spat out with a shallow "hawking" sound, without a deep chest cough or abdominal retching. |
| Color of the Blood | Bright crimson red (oxygenated arterial blood) or pink-red; may turn dark rusty-red after sitting in the lungs for days. | Dark mahogany red, maroon, or brown/black like wet "coffee grounds" (because stomach hydrochloric acid oxidizes hemoglobin into brown hematin), unless from a massive arterial variceal bleed. | Bright or dark red streaks coated on the surface of saliva, often accompanied by blood visible in one nostril or around inflamed gums. |
| Texture & Frothiness | Frothy, foamy, and bubbly (mixed with tiny microscopic air bubbles from the lungs and slippery mucus). | Non-frothy liquid or thick clots, often mixed with partially digested food particles and sour stomach liquid. | Watery saliva mixed with blood or a smooth jelly-like post-nasal blood clot; never foamy. |
| Chemical pH (Hospital Strip Test) | Alkaline (\(\text{pH} \ge 7.4\)) on litmus paper. | Acidic (\(\text{pH} < 4.0\text{ to }5.0\)) due to gastric hydrochloric acid. | Neutral (\(\text{pH} \approx 7.0\)). |
| Associated Stool Changes | Normal stool (unless the patient swallowed large amounts of coughed-up blood). | Frequently followed within hours by melena (jet-black, tarry, foul-smelling stools from digested blood passing through the intestines). | Normal stool. |
| Typical Medical History | Lung cancer, bronchiectasis, COPD, tuberculosis, pneumonia, fungal Aspergilloma cavity, pulmonary embolism, mitral stenosis, or recent bronchoscopy. | Peptic ulcer disease, liver cirrhosis with esophageal varices, heavy NSAID/aspirin or alcohol use, or violent vomiting (Mallory-Weiss tear). | Dry winter air, nasal trauma, hypertension, blood thinners, severe gingivitis, or recent dental extraction. |
Anatomy and physiology: Why lung bleeds are under high pressure and why 150 mL suffocates¶
1. The Lung's Dual Blood Supply: Why 90% of Massive Hemoptysis Comes from the Bronchial Arteries¶
Many people assume that bleeding inside the lungs comes from the pulmonary arteries that carry blood from the right side of the heart to pick up oxygen. In reality, the lungs have two completely separate arterial blood supplies: 1. The Pulmonary Arterial System (99% of blood volume, LOW pressure): Carries deoxygenated blood through the alveolar air sacs at a low resting pressure of only \(15\text{ to }25 / 8\text{ to }10\text{ mmHg}\). It accounts for fewer than 5 to 10 percent of massive hemoptysis cases (such as eroded Rasmussen's aneurysms inside old tuberculosis cavities or pulmonary artery catheter perforations). 2. The Bronchial Arterial System (1% of blood volume, HIGH systemic aortic pressure): Branches directly off the descending thoracic aorta to nourish the walls of the windpipe and bronchial tubes under full systemic arterial blood pressure (\(120/80\text{ mmHg}\) or higher). - When chronic airway inflammation (from bronchiectasis, tuberculosis, fungal Aspergilloma mycetoma balls, chronic lung abscesses, or bronchogenic lung cancer) damages a bronchial wall, the body grows twisted, fragile, dilated new collateral bronchial arteries (neovascularization). - When one of these fragile bronchial arteries erodes open into an airway lumen, blood spurts directly from the aorta into the bronchial tree at \(120+\text{ mmHg}\) systolic pressure.
2. The "150 mL Dead Space" Rule: How Asphyxiation Happens Before Shock¶
An average adult has roughly \(5,000\text{ milliliters}\) (\(5\text{ liters}\)) of circulating blood, and must lose \(750\text{ to }1,500\text{ mL}\) before showing signs of hemorrhagic stuð. - However, the entire branching tree of conducting airways connecting your mouth to your two lungs (the trachea, right and left mainstem bronchi, and lobar/segmental bronchi, known as anatomical dead space) has a total internal volume of only \(150\text{ milliliters}\) (\(5\text{ fluid ounces}\)). - Unlike the tiny elastic alveolar air sacs at the outer edge of the lung (which can hold liters of fluid locally), if \(100\text{ to }150\text{ mL}\) of blood pours into the main bronchial airways faster than the person can cough it out, the blood spills over the central ridge dividing the left and right lungs (the carina), runs down into the healthy lung, and clots into a solid rubber-like branching mold of the windpipe (a bronchial blood cast). - Once both mainstem bronchi are plugged by blood or clots, no oxygen can enter the bloodstream, and hypoxic cardiac arrest occurs within 3 to 5 minutes, even though the patient has lost less than 3 percent of their total blood volume.
How doctors define Mild, Moderate, and Life-Threatening (Massive) Hemoptysis¶
Historically, textbooks defined massive hemoptysis using 24-hour volume cutoffs (\(200\text{ to }600\text{ mL}\) per 24 hours). Modern emergency medicine and pulmonology guidelines notes that waiting 24 hours to measure a bucket of blood is useless in an acute emergency. Instead, triage severity by hourly rate, single-cough volume, and airway/breathing impact:
| Alvarleikastig | Estimated Volume & Rate of Blood | Breathing & Critical Sign Status | Strax aðgerða krafist |
|---|---|---|---|
| 1. Mild / Scant ("Streaky") Hemoptysis | Less than \(5\text{ to }20\text{ mL}\) in 24 hours (less than 1 tablespoon total): tiny red or rust-colored streaks mixed primarily with phlegm or mucus. | Normal breathing (\(\text{SpO}_2 \ge 95\%\)), clear voice, normal heart rate and blood pressure, no gurgling. | Urgent Medical Evaluation (Within 24 Hours): Keep upright, monitor closely, and see a physician for a chest X-ray/CT scan to identify the underlying cause. (Go to ER immediately if on blood thinners or if pure blood appears.) |
| 2. Moderate / Submassive Hemoptysis | \(20\text{ to }100\text{ mL}\) in 24 hours (1 to 7 tablespoons, or coughing up small clots of pure bright red blood repeatedly over an hour). | Mild shortness of breath, anxiety, frequent coughing needed to clear the throat; vitals stable or mildly elevated pulse. | Immediate Emergency Department Transport: Call 911 / 112 / 999. High risk of sudden erosion into a larger bronchial artery ("sentinel bleed"). Keep NPO and position upright or bleeding-side down. |
| 3. Life-Threatening / Massive Hemoptysis | \(\ge 100\text{ to }150\text{ mL}\) in a single episode or within 1 hour (half a cup or more), OR \(\ge 200\text{ to }600\text{ mL}\) in 24 hours, OR ANY volume of blood that causes shortness of breath, hypoxia (\(\text{SpO}_2 < 90\%\)), or airway obstruction. | Airway & Respiratory Crisis: Gurgling breathing, inability to speak full sentences, bluish lips (cyanosis), panic, tachycardia (\(> 110\text{ bpm}\)), or falling blood pressure. | Code-Red Airway Emergency (Call 911 / 112 / 999 NOW): Position Bleeding-Lung-Side DOWN immediately (or upright leaning forward if side unknown); prepare for emergency single-lung intubation, rigid bronchoscopy, and Bronchial Artery Embolization. |
Detailed positioning guide: Mastering the "Bleeding-Lung-Side DOWN" rule¶
The single most important physical maneuver a bystander or paramedic can perform during massive hemoptysis is gravitational lung isolation:
Why "Bleeding-Side DOWN" works (And why "Bleeding-Side UP" kills)¶
Imagine the windpipe (trachea) splitting at the bottom (the carina) like an upside-down letter "Y" into the Right Mainstem Bronchus and the Left Mainstem Bronchus: - Suppose a patient has a bleeding lung tumor or bronchiectasis cavity inside their RIGHT lung. - If that patient lies on their left side (putting the bleeding right lung on TOP), gravity pulls every drop of blood pouring out of the upper right lung straight down across the carina and dumps it directly into the healthy lower LEFT lung. Within 2 minutes, both lungs are filled with blood and the patient suffocates. - Aftur á móti, ef þú veltir þeim sjúklingi á HÆGRI hlið hans (Blæðingar-lungna-Síða NIÐUR), heldur þyngdarafl blóðinu inni í háð hægra lunga neðst. Jafnvel þó að hægra lungað fyllist algjörlega af blóði, þá helst heilbrigða VINSTRI lungað sem situr hátt uppi 100 prósent þurrt og opið, og manneskja getur lifað þægilega af á einu heilbrigt lunga þar til læknar á spítala innsigla blæðinguna.
Hvernig geturðu sagt HVAÐA lunga (vinstri vs. hægri) blæðir fyrir sneiðmyndatöku?¶
Notaðu aðeins hliðarstöðuna Blæðingarhlið niður ef þú hefur áreiðanlega vísbendingu um hvaða lunga blæðir (ef þú hefur raunverulega ekki hugmynd hvaða lunga blæðir, skaltu halda vöku sjúklingi sitja uppréttan í 90 gráður halla sér örlítið fram svo hvorugt lungan halli yfir hinu): 1. Þekktur einhliða lungnasjúkdómur: Athugaðu útskriftarblöð sjúklingsins eða spyrðu: "Hvaða lunga hefur æxlið þitt, berklahola, berkjubólgu, sveppakúlu eða nýlega lungnavefsýni/skurðaðgerð?" 2. Eigin „brjóstgúrka“ tilfinning sjúklingsins: Það kemur á óvart að allt að 50 til 60 prósent vakandi sjúklinga með hröð blóðþurrð geta fundið nákvæmlega hvaða hlið brjóstsins blæðir. Spyrðu þá: "Áður en blóðið kemur upp í hálsinn á þér, finnurðu fyrir heitri kúlu, kitlandi eða gurglandi tilfinningu vinstra megin eða HÆGRI hlið á brjósti þínu?" 3. Einhliða brjóstverkur eða skrölt í hljóðsjá: Staðbundinn brjóstverkur í brjósti eða hávær freyðandi gróft brak heyrist á annarri hliðinni á brjósti.
Algengar orsakir blóðþurrðar og alvarlegra rauðra fána¶
Þó að bráð berkjubólga og lungnabólga valdi mörgum vægum blóðfleinum hósta, leita bráðalæknar strax að sex lífshættulegum orsökum blóðhýsis:
- Lungnasegarek (PE, blóðtappi í lungum):
- Þegar blóðtappi berst úr djúpri bláæð í fótleggnum (DVT) inn í lungun og veldur því að fleyg lungnavefs dregur upp, fær sjúklingurinn skyndilegan brjóstverk í brjósti (skarpa verki þegar andað er), skyndilega mæði, hröðum hjartslætti (\(> 100\text{ bpm}\)) og blæðingarbólga, sem oft fylgir skurðaðgerð á flugi/bólga í flugi eða nýlegri skurðaðgerð.
- Berkjavaldandi lungnakrabbamein eða æxli með meinvörpum:
- Allir fullorðnir eldri en 40 ára (sérstaklega núverandi eða fyrrverandi reykingamenn) sem hósta jafnvel smáar blóðrákir í meira en 1 til 2 vikur, eða upplifir óútskýrt þyngdartap, háa rödd eða þrálátan hósta, þurfa bráða **smásneiðmynd af brjósti og berkjuspeglun, alveg eðlilega, ** röntgenmynd af brjósti missir allt að 20 prósent æxla í miðlægum öndunarvegi).
- Berkkjabólgu, berklar (TB) og sveppasveppaæxli (Aspergilloma):
- Á heimsvísu eru virkir eða gamlir læknaðir lungnaberklar og berkjubólgur eftir sýkingu númer eitt orsök stórfelldrar, lífshættulegrar blæðingar í berkjuslagæð. Ef grunur leikur á virkum berklum (langvarandi hósti \(> 3\text{ vikur}\), nætursviti, þyngdartap, hiti), láttu sjúklinginn halda vefju fyrir munninn þegar hann hóstar ofan í vaskinn og tryggja góða loftræstingu í herberginu (eða notaðu N95/FFP2 öndunargrímu ef hann er til staðar) á meðan hann sinnir honum.
- Bráð hjartabilun í vinstri slegli eða míturþrengsli ("bleik froðuhráki"):
- Þegar hár þrýstingur vinstra megin í hjartanu þvingar vökva og rauð blóðkorn yfir lungnablöðrurnar (hjartalungnabjúgur), berst sjúklingurinn fyrir andanum þegar hann liggur flatur (orthopnea) og hóstar upp mikilli, vatnsmikilli, bleiklituðu froðufroðu frekar en þykkum rauðum blóðtappa. Setjið þessa sjúklinga bolta upprétta með fæturna dinglandi niður.
- Dreifðar alveolar blæðingar (DAH) og sjálfsofnæmisæðabólga:
- Aðstæður eins og Goodpasture syndrome (and-GBM sjúkdómur), granulomatosis með polyangiitis (GPA / Wegener's), lupus eða crack kókaíninnöndun valda blæðingum frá smásæjum háræðum í báðum lungum samtímis.
- Blóðþynningarlyf eða blóðflöguhemjandi blóðþynningarlyf ("coagulopathic hemoptysis"):
- Sjúklingar sem taka warfarín, apixaban, rivaroxaban, dabigatran, edoxaban, klópídógrel eða tvíþætta blóðflöguhemjandi meðferð geta breytt minniháttar berkjuertingu í gríðarlega blóðhýsa; bráðamóttökur gefa samstundis markvissa viðsnúningalyf samhliða stranexamsýru í úðagjöf eða í bláæð (TXA).
Sérstakt skurðaðgerðarslys: Blæðing frá barkastómaslöngu (tracheoinnominate artery fistel)¶
Ef þú sinnir sjúklingi heima sem er með barkastómslöngu í hálsi og hann hóstar skyndilega upp blóði eða blæðir í kringum barkastóma sinn, verður þú að þekkja regluna um "Sentinel Bleed" og Tracheoinnominate Fistel (TIF):

Hvað gerist á sjúkrabílnum og sjúkrahúsinu: TXA í úðagjöf, þræðing í einni lungu og uppbót¶
Að vita hvernig bráðalæknar og inngripsgeislafræðingar stöðva stórfellda blóðblóðbólgu hjálpar fjölskyldum að skilja sjúkrahúsferilinn:

- Að tryggja öndunarveginn (stórborinn barkahólkur eða sértæk innþræðing á meginstofni):

Nákvæm samskiptahandrit fyrir 911 / 112 / 999 og sjúkrahúsþrenging¶
Use this clear clinical script when calling emergency medical services:
"I need an emergency ambulance at [Exact Address]. The patient is a [Age]-year-old [male/female] who has coughed up approximately [Amount in mL / tablespoons / cups] of bright red frothy blood from the lungs over the past [Minutes/Hours]. [He/She] has a history of [lung cancer in the right/left lung / bronchiectasis / tuberculosis / blood thinners / a tracheostomy tube]. We have positioned [him/her] [sitting upright leaning forward / lying with the bleeding right/left lung side DOWN] and saved the coughed-up blood in a container for the paramedics."

Taking a certified hands-on first aid and basic life support (BLS) course from the Red Cross, St John Ambulance, or a local emergency training provider gives you the practical skills to manage airway emergencies, position patients safely, and perform rescue breathing and CPR.
Algengar spurningar¶
Why should a person who is coughing up blood lie with their bleeding lung facing DOWN toward the bed instead of up?¶
When someone bleeds inside one lung, the greatest danger to their life is not losing blood volume, but rather having that blood spill across the central fork of the windpipe (the carina) and drown their other, healthy lung. Lying on your side with the bleeding lung on the bottom (Bleeding-Side Down) uses gravity to keep all the blood pooled inside the sick lower lung while keeping the healthy upper lung high, dry, and open to breathe oxygen.
Why is it dangerous to take cough syrup (like dextromethorphan or codeine) to stop coughing when you are coughing up blood?¶
When blood is leaking into your bronchial airways, your cough reflex is the only mechanism keeping your windpipe open so you can breathe. Taking cough-suppressant medication stops you from coughing the liquid blood out, allowing the blood to pool inside your windpipe and main bronchi where it quickly congeals into a solid, branching rubbery clot (a bronchial cast) that blocks airflow to both lungs and causes sudden suffocation.
How can I tell if blood in my mouth was coughed up from my lungs (hemoptysis) or thrown up from my stomach (hematemesis)?¶
Blood coughed up from the lungs (hemoptysis) is preceded by a chest tickle or gurgling cough, looks bright crimson red, and is frothy or foamy because it is mixed with tiny lung air bubbles. In contrast, blood vomited from the stomach (hematemesis) is preceded by nausea and abdominal retching, is never frothy, often contains food particles, and frequently looks dark maroon or dark brown like wet coffee grounds because stomach acid chemically darkens the blood.
If I only coughed up a tiny streak of blood in my phlegm once or twice and feel completely fine, do I still need to see a doctor?¶
Yes, any unexplained blood in your sputum (especially if it lasts more than a few days or occurs in an adult over age 40, a current or former smoker, someone taking blood thinners, or someone with a history of cancer, blood clots, or tuberculosis) requires a prompt medical evaluation and chest imaging. Even a tiny streak of blood can be the earliest curable warning sign of a central airway tumor, bronchiectasis, pulmonary embolism, or a "sentinel bleed" from a fragile blood vessel.
