  2. Rewarm the affected extremities slowly at normal room temperature. Allow the limbs to warm passively in air between 20 °C and 22 °C (68 °F to 72 °F); never immerse non-freezing cold injuries in hot water baths.
  3. Wash the skin with extreme gentleness using mild soap and lukewarm water. Cleanse accumulated sweat, bacteria, and grime without scrubbing, as friction tears weakened, waterlogged skin.
  4. Pat the skin completely dry with a soft, clean towel. Blot moisture away gently; never rub, buff, or vigorously massage the affected toes, soles, or fingers.
  5. Elevate the feet or hands slightly above heart level. Propping the limbs on pillows or rolled blankets reduces gravitational blood pooling and relieves painful throbbing edema.
  6. Air-dry the bare skin in clean, dry air before applying loose, clean wool socks. Keep the affected limbs exposed to circulating room air for at least 30 to 60 minutes, making sure the skin is completely dry before applying loose, non-constricting footwear.

  • Never immerse trench foot or chilblains in hot water. Unlike frostbite, which requires a 37 °C to 40 °C (99 °F to 104 °F) bath, placing non-freezing cold injuries in hot water causes excruciating neurovascular pain and accelerates tissue necrosis.
  • Never rub, massage, or vigorously scrub cold, macerated skin. The waterlogged stratum corneum is fragile; scrubbing shears the epidermis from the dermis, causing open ulcers and secondary bacterial infections.
  • Never apply direct radiant heat from fires, heating pads, or hair dryers. Damaged peripheral sensory nerves cannot feel burns; radiant heat causes severe full-thickness thermal burns on numb skin.
  • Never pop, peel, or debride blistered skin in the field. Broken blisters expose devitalized dermal capillary beds to aggressive environmental pathogens like Pseudomonas and Staphylococcus.
  • Never put wet or damp socks back onto cold feet. Trapping moisture against cold skin restarts the cycle of capillary constriction, tissue hypoxia, and maceration.
  • Never smoke cigarettes or use nicotine products during recovery. Nicotine stimulates intense peripheral arteriolar vasoconstriction, cutting off important microvascular perfusion to healing toes and fingers.

Pathophysiology of non-freezing cold injury: vasospasm and maceration

Non-freezing cold injuries differ fundamentally from frostbite in their biophysical mechanisms, duration of exposure, and long-term neurovascular sequelae.

Microvascular dysfunction and the hunting reaction failure

The human body normal physiological response to cold exposure is cyclic:

When skin temperature drops, local smooth muscle fibers in cutaneous arterioles contract (cold-induced vasoconstriction) to prevent warm core blood from losing heat through the skin.

Periodically, the autonomic nervous system triggers brief periods of vasodilation, known as the Lewis hunting reaction (cold-induced vasodilation), which flushes warm blood into the digits for 5 to 10 minutes to deliver oxygen and nutrients before constricting again.

In susceptible individuals or during sustained damp cold exposure, the hunting reaction fails:

Cutaneous arterioles remain locked in sustained, intense vasospasm.

Capillary blood flow grinds to a halt, depriving the vessel endothelium and surrounding tissues of oxygen.

Vascular endothelial cells undergo hypoxic swelling, while perivascular mast cells and sensory nerve endings become hyper-sensitized.

When the chilled tissue is subsequently rewarmed too quickly, an abnormal inflammatory response ignites:

Arterioles dilate abruptly, while downstream venules remain constricted.

This dramatic pressure mismatch forces fluid, red blood cells, and inflammatory cytokines outward through damaged capillary walls into the dermis, producing severe edema, intense itching, inflammatory papules, and microvascular thrombosis.

The biophysics of epidermal maceration

Trench foot requires prolonged moisture. When feet are enclosed inside non-breathable rubber boots, plastic waders, or damp leather boots, the skin of the feet produces sweat that cannot evaporate:

A healthy human foot contains approximately 250,000 sweat glands, capable of secreting up to 250 ml (8.5 ounces) of moisture per day under moderate physical exertion.

Trapped inside impermeable footwear, the stratum corneum (the outermost layer of dead, keratin-filled skin cells) absorbs water like a sponge.

Normal skin has a water content of 15 to 20 percent; under continuous moisture exposure, water content surpasses 50 percent, a process known as hyperhydration or maceration.

Macerated skin turns stark white, swollen, wrinkled, and deeply pleated.

The physical intercellular lipid bonds that cement keratinocytes together dissolve, causing the skin barrier to lose its structural tensile strength.

Frictional movement inside the boot shears the softened epidermis away from the underlying papillary dermis, opening microscopic fissures that invite invasive fungal and bacterial pathogens (such as cellulitis, wet gangrene, and trench foot ulcers).

Chilblains (pernio): clinical presentation and management

Chilblains (pernio or perniosis) represent acute or chronic inflammatory skin lesions triggered by abnormal neurovascular responses to non-freezing damp cold. They frequently affect outdoor workers, mountaineers, military personnel, and individuals with underlying autoimmune conditions or poor peripheral circulation.

Chilblains (Pernio) Anatomical Distribution:
--------------------------------------------------------------------------------
Primary Sites: Dorsal surfaces of toes, fingertips, heels, ears, nose.
Visual Appearance: Erythematous (bright red) to violaceous (purple) swollen papules.
Key Symptoms: Intense itching, burning pain upon rewarming, localized edema.
Distinction: Skin remains soft and pliable; NO hard wooden freezing.
--------------------------------------------------------------------------------

Clinical signs and symptoms of chilblains

Chilblains develop within 2 to 14 hours following exposure to damp, cold air:

  • Erythematous to violaceous papules. The lesions begin as discrete, swollen red patches that rapidly transition into dusky purple or violaceous nodules. They appear most commonly on the dorsal aspects of the toes, the sides of the feet, the tips of the fingers, the bridge of the nose, and the earlobes.
  • Intense pruritus (itching) and burning pain. The hallmark symptom of chilblains is agonizing itching that intensifies dramatically when the individual moves into a heated room or gets into a warm bed. As blood flushes into the vasospastic tissue, itching gives way to deep, throbbing burning sensations.
  • Blistering and ulceration in severe cases. In severe or recurring pernio, intense localized edema lifts the epidermis, forming small, tense clear blisters. If friction is applied, these blisters rupture, leaving shallow, painful, slow-healing erythematous ulcers prone to secondary staphylococcal infection.

First aid and medical care for chilblains

Managing chilblains centers on soothing the abnormal neurovascular response and protecting compromised skin:

  1. Rewarm the skin passively. Move indoors into a comfortably heated room (20 °C to 22 °C / 68 °F to 72 °F). Let the skin warm naturally. Never place chilblains near a radiator, electric heater, or open hearth, and never soak them in hot water, as rapid heat spikes intensify itching and inflammatory edema.
  2. Avoid all scratching and friction. Scratching chilblains tears the thin epidermis over the papules, leading to open ulcers and bacterial colonization. Keep fingernails short and apply loose cotton or wool coverings.
  3. Apply topical corticosteroid cream. Applying a moderate-potency topical corticosteroid (such as 1 percent hydrocortisone or 0.1 percent triamcinolone acetonide) twice daily reduces localized capillary inflammation, decreases edema, and relieves itching.
  4. Protect against friction inside shoes. If the patient must wear footwear, apply a hydrocolloid blister bandage or silicone gel toe sleeve over the lesions to buffer the nodules against mechanical rubbing.
  5. Systemic medical therapy for chronic or refractory cases. In severe, recurring winter chilblains, physicians may prescribe oral calcium channel blockers (such as nifedipine, 20 mg to 60 mg daily). Nifedipine relaxes vascular smooth muscle, dilates peripheral arterioles, and accelerates the resolution of chronic violaceous lesions.

Trench foot (immersion foot): the four clinical stages

Trench foot (immersion foot) is a far more destructive clinical condition than chilblains. It represents a combined neurovascular crush and ischemic injury affecting the deep tissues of the feet. First recognized on a massive scale among soldiers standing in waterlogged trenches during World War I, trench foot remains a constant hazard among modern soldiers, hikers, unhoused populations, and disaster survivors trapped in floodwaters.

Trench foot progresses through four distinct physiological stages, each requiring specific first aid and clinical management.

    B -->|Elevation, Bed Rest, Pain Management| C["Stage 3: Post-Hyperemic (Chronic Neuropathic)"]
    C -->|Long-Term Nerve Recovery: 6 to 24 Months| D["Stage 4: Permanent Sequelae / Raynaud / Autonomic"]

Stage 1: The pre-hyperemic (ischemic) stage

The pre-hyperemic stage occurs during the cold, wet exposure phase and immediately following rescue:

The foot is cold, swollen, pale, cyanotic, or mottled gray.

The skin feels clammy and waterlogged, showing deep, white, pleated maceration folds along the plant surface and between the toes.

Peripheral arterial pulses (dorsalis pedis and posterior tibial) are exceptionally weak or completely impalpable due to intense arteriolar spasm.

Sensation is profoundly diminished. The patient describes the foot as feeling heavy, dead, or like "walking on wooden pegs." Tactile sensation, light touch, and pinprick response are completely absent across the toes and distal forefoot.

Damage during this stage is centered on endothelial cell swelling, microvascular stasis, and early axonal degeneration in peripheral sensory and autonomic nerve fibers.

Stage 2: The hyperemic (inflammatory reperfusion) stage

The hyperemic stage begins hours after the foot is removed from the cold and warmed to room temperature. It represents an explosive inflammatory reperfusion reaction that lasts from several days to several weeks:

The foot transforms into an intensely hot, dry, bounding red extremity. Peripheral pulses become full, bounding, and hyper-dynamic.

Massive edema develops, causing the foot, ankle, and lower leg to swell to twice their normal volume.

Large, tense blisters filled with serous or hemorrhagic fluid lift the macerated skin. Areas of superficial skin slough away, revealing raw, weeping dermis.

Severe, agonizing pain sets in. Patients describe intense, burning, throbbing pain and sudden electric-shock sensations (shooting paresthesias) that reach excruciating levels, particularly at night.

Pain during this stage is caused by chemical inflammation irritating regenerating sensory nerve endings and acute reperfusion hyperemia stretching ischemic tissue beds.

Stage 3: The post-hyperemic stage

The post-hyperemic stage develops as the acute inflammation and bounding hyperemia subside, typically lasting from 2 to 12 months:

Swelling and redness gradually resolve, and blisters dry into hard crusts.

However, the foot remains profoundly abnormal. The skin becomes cold, pale, and hypersensitive to temperature changes.

Hyperhidrosis (excessive, cold perspiration) develops due to damaged sympathetic autonomic nerve fibers that uncontrollably trigger sweat glands.

Patients suffer chronic, intractable neuropathic pain, characterized by allodynia (pain caused by non-painful stimuli, such as a bedsheet brushing against the toes) and cold intolerance. Walking even short distances causes deep, aching bone pain and severe fatigue.

Stage 4: Permanent chronic sequelae

In severe cases of trench foot where exposure exceeded 48 to 72 hours, permanent structural and functional disabilities remain for life:

Extensive loss of peripheral unmyelinated and myelinated nerve axons leads to permanent sensory neuropathy and patchy numbness.

Secondary Raynaud phenomenon occurs, where mild exposure to cool air triggers sudden, painful digital blanching and cyanosis.

Patients experience premature osteoarthritis in the small joints of the foot, muscular atrophy in the intrinsic foot muscles (causing hammer toes and pes cavus deformities), and persistent hyperhidrosis that increases susceptibility to recurring fungal and bacterial infections.

Remove wet boots and socks immediately, wash the feet gently with mild soap and lukewarm water without scrubbing, and pat them dry with a clean towel. Allow the bare feet to air-dry in circulating room-temperature air for at least 30 to 60 minutes so the softened skin can firm up. Elevate the feet to reduce swelling, and put on loose, dry wool socks once completely dry.

Recognizing and treating non-freezing cold injuries like chilblains and trench foot requires practical knowledge of environmental medicine, skin barrier physiology, and gentle wound care. Learning how to distinguish non-freezing trauma from deep frostbite, avoiding dangerous rapid-heating mistakes, and executing proper field drying and splinting can prevent permanent neurovascular disability in outdoor workers, hikers, and disaster survivors.

We strongly encourage everyone who works outdoors, participates in winter recreation, or leads outdoor groups to complete accredited certification in wilderness first aid (WFA) or standard first aid and cardiopulmonary resuscitation (CPR). Recognized providers such as the American Red Cross, St John Ambulance, the British Red Cross, and the Canadian Red Cross provide comprehensive training in environmental cold injury prevention, maceration management, and trauma first aid.
