An acute aortic dissection and a ruptured abdominal aortic aneurysm (AAA) are two of the most time-critical cardiovascular emergencies in medicine, capable of causing fatal internal hemorrhage or organ failure within minutes. Because the aorta is the main high-pressure artery carrying oxygenated blood directly from the heart to the brain, arms, organs, and legs, any tear in its inner wall (dissection) or burst across a weakened segment (ruptured aneurysm) requires immediate emergency surgical intervention. First aid focuses on rapid recognition of red flag symptoms, calling emergency medical services with explicit vascular alerts, keeping the person completely still to prevent blood pressure spikes, and avoiding medications like aspirin that worsen bleeding.

Do not give aspirin for sudden tearing chest or back pain

While chewing aspirin is standard care for a suspected heart attack caused by a coronary blood clot, never give aspirin, ibuprofen, or any blood thinner if you suspect an aortic dissection or a ruptured aortic aneurysm. In an aortic emergency, the person is bleeding through a torn arterial wall, and antiplatelet or anticoagulant drugs prevent life-saving clot formation and dramatically increase surgical mortality. If chest pain is instantaneous, feels like a sharp ripping or tearing blade radiating into the back, or causes different pulses between the left and right arms, withhold aspirin and tell paramedics immediately.

Làm điều này ngay bây giờ

  1. Call your local emergency number immediately and declare a "Suspected Acute Aortic Emergency" or "Suspected Ruptured Aortic Aneurysm." Dial 911 in the United States and Canada, 112 across the European Union, 999 in the United Kingdom, or check all số khẩn cấp. Thông báo rõ ràng cho người điều phối nếu bệnh nhân đã biết rõ chứng phình động mạch chủ, hội chứng Marfan, van động mạch chủ hai mảnh hoặc đau ngực, lưng hoặc đau bụng đột ngột kèm theo ngất xỉu, để họ có thể điều động xe cấp cứu hỗ trợ sự sống tiên tiến và cảnh báo bệnh viện có khả năng phẫu thuật tim hoặc mạch máu.
CPR with chest compressions at 100 to 120 beats per minute.

Understanding the two life-threatening aortic emergencies

Although both conditions involve structural failure of the body's largest artery, acute aortic dissection and ruptured abdominal aortic aneurysm (AAA) happen through different mechanical processes, strike somewhat different patient profiles, and require distinct emergency hospital strategies.

1. Acute Aortic Dissection: The high-pressure inner tear

The wall of the healthy aorta consists of three microscopic layers: the thin inner lining (tunica intima), the thick elastic muscular middle layer (tunica media), and the tough outer fibrous jacket (tunica adventitia). In an aortic dissection, a sudden tear develops in the inner intimal lining, usually in the chest (thoracic aorta) where blood leaves the heart at high velocity.

Once blood surges through this inner tear under systolic pressure, it splits (dissects) the middle muscular layer apart, carving a "false lumen" (a second, abnormal blood channel) down or up the length of the aorta. As this false channel balloons with blood, it triggers four immediate life-threatening dangers: - Rupture into the pericardial sac (Cardiac Tamponade): If the dissection travels backward toward the heart into the pericardial sac surrounding the heart muscle, blood fills the sac and squeezes the heart until it cannot pump, causing sudden death. - Acute aortic valve regurgitation: The tear can unhinge the leaflets of the aortic valve, causing blood to pour backward into the left ventricle and flooding the lungs with fluid (flash pulmonary edema). - Coronary artery occlusion: The dissecting flap can shear across the opening of the right or left coronary artery, triggering an actual simultaneous heart attack. - Branch vessel malperfusion (Stroke, Paraplegia, Visceral Ischemia): As the false lumen spirals down the aorta, the bulging inner flap can pinch off the carotid arteries feeding the brain (causing an immediate stroke), the subclavian arteries feeding the arms (causing a missing wrist pulse), the spinal arteries feeding the spinal cord (causing sudden paralysis of both legs), or the mesenteric and renal arteries feeding the intestines and kidneys.

Clinicians classify aortic dissections using the Stanford Classification System, which dictates immediate survival care: - Stanford Type A Dissection: Involves the ascending aorta (the first segment rising directly out of the heart) and/or the aortic arch, regardless of how far down it travels. Type A dissections are surgical emergencies with a mortality rate that rises by roughly 1 to 2 percent for every hour surgical repair is delayed during the first 48 hours. - Stanford Type B Dissection: Begins in the descending thoracic aorta (beyond the left subclavian artery feeding the left arm) and travels downward toward the abdomen. Type B dissections are managed initially with aggressive intravenous blood pressure and heart rate control ("anti-impulse therapy"), plus endovascular stent-graft repair (TEVAR) if organs are starved of blood or the wall threatens to burst.

2. Ruptured Abdominal Aortic Aneurysm (AAA): The bursting weakened wall

An aneurysm is a permanent, localized widening (ballooning) of all three layers of the arterial wall caused by chronic degeneration of elastic fibers and collagen. While aneurysms can occur in the chest (thoracic aortic aneurysm), they are far more common in the lower abdomen below the kidney arteries (infrarenal abdominal aortic aneurysm, or AAA), where a normal adult aorta measures roughly 2.0 centimeters (0.8 inches) in diameter. When that segment stretches beyond 3.0 centimeters (1.2 inches), it is defined as an aneurysm; once it exceeds 5.0 to 5.5 centimeters (2.0 to 2.2 inches), the risk of spontaneous wall rupture climbs steeply.

Most abdominal aortic aneurysms grow silently over years without causing a single symptom until they suddenly expand or tear open (ruptured AAA). When an AAA bursts, blood pours out of the aorta into one of two spaces: - Free Intraperitoneal Rupture: The front wall of the aneurysm bursts directly into the open abdominal cavity. Massive exsanguination occurs within minutes, often causing immediate cardiac arrest before the ambulance arrives.

So sánh toàn diện: Bóc tách động mạch chủ, vỡ AAA và đau tim

Đặc điểm lâm sàng Acute Thoracic Aortic Dissection (Type A / Type B) Vỡ phình động mạch chủ bụng (Vỡ AAA) Nhồi máu cơ tim cấp tính (Đau tim) Đau bụng thận (sỏi thận) hoặc căng cơ lưng cấp tính
Speed of Pain Onset Tức thời ("sấm sét"): Đạt cường độ tối đa 10/10 ngay trong giây đầu tiên bắt đầu. Sudden and severe: Abrupt onset of deep, constant abdominal, lower back, or flank pain.

  3. "Pain Plus One" (Đau ngực/lưng kết hợp với các triệu chứng thần kinh hoặc chi): Bất cứ khi nào đau ngực hoặc đau lưng xảy ra do suy giảm thần kinh hoặc chi lạnh, hãy nghĩ đến bóc tách động mạch chủ trước:
  4. Đau ngực/lưng + Triệu chứng đột quỵ: Ăn sâu vào động mạch cảnh gây yếu một bên đột ngột, xệ mặt hoặc mất khả năng nói. (Cảnh báo nghiêm trọng của bệnh viện: Tiêm tĩnh mạch các loại thuốc làm tan cục máu đông như tPA/tenecteplase cho bệnh nhân đột quỵ bị đột quỵ do bóc tách động mạch chủ Loại A ẩn gần như gây tử vong.)
  5. Đau ngực/lưng + Liệt (Liệt hai chân): Cắt đứt động mạch cột sống (đặc biệt là động mạch Adamkiewicz) gây liệt và tê đột ngột cả hai chân.
  6. Đau ngực/lưng + Thiếu máu cục bộ chi cấp tính: Tắc nghẽn động mạch dưới đòn hoặc chậu khiến một tay hoặc chân trắng bệch, lạnh buốt, không mạch và đau đớn.

Fluoroquinolone Antibiotic Warning Regulatory agencies (US FDA, European EMA, UK MHRA) warn that systemic fluoroquinolone antibiotics (ciprofloxacin, levofloxacin) double short-term risk in high-risk patients. Same fluoroquinolone collagen-degradation warning applies to patients with known abdominal aortic aneurysms.

Step-by-step first aid and pre-hospital care while waiting for EMS

Step 1: Positioning for hemodynamic stability

  • Suspected Thoracic Aortic Dissection (chest/upper back pain, often high blood pressure): Help them lie down in the position that causes the least pain and easiest breathing, usually supine (flat on the back) or semi-Fowler's (head and shoulders gently propped at a 30-degree angle). Never force a patient who is struggling to breathe to lie completely flat.
  • Suspected Ruptured AAA or signs of cú sốc (pale, cold, clammy, dizzy, faint): Lay them completely flat on their back. Do NOT elevate the legs aggressively if it causes abdominal pain, and never allow them to sit upright or stand, as sitting up drains remaining blood away from the brain and heart.
  • Unconscious or actively vomiting: Gently roll them onto their side into the lateral recovery position as a single unit (log-roll style, without twisting the torso) so vomit cannot enter the lungs.

Step 2: Do NOT palpate or press deeply on a pulsating abdomen

If you notice or suspect a throbbing mass in the middle of the person's abdomen, do not press deeply on their stomach to check it, and do not allow well-meaning bystanders to push on the belly. Deep manual pressure over a thin-walled, leaking abdominal aortic aneurysm can mechanically disrupt the fragile retroperitoneal clot and trigger immediate free rupture.

Step 3: How to perform a two-arm blood pressure and pulse check

If a digital home blood pressure monitor is nearby and using it will not delay calling emergency services or agitate the patient: 1. Measure and write down the blood pressure and heart rate on the right upper arm. 2. Immediately move the cuff to the left upper arm and measure the blood pressure there. 3. What the numbers mean: In a healthy person, systolic blood pressure between the right and left arms differs by less than \(10\text{ mmHg}\). A systolic blood pressure difference greater than \(20\text{ mmHg}\) between the right and left arms (paired with acute chest or back pain) indicates that an aortic dissection flap may be blocking one of the subclavian arteries. Report both exact numbers to the paramedics as soon as they arrive.

Step 4: Physiological calming ("Verbal Beta-Blockade")

In the hospital emergency department, the first medical treatment for an aortic dissection is intravenous beta-blockers (such as esmolol or labetalol) to drop the heart rate below \(60\text{ beats per minute}\) and systolic blood pressure between \(100\text{ and }120\text{ mmHg}\), reducing the pounding force of each heartbeat against the torn aortic wall. At home, before IV medications arrive, panic and pain are the enemy: - Clear frightened crowds out of the room and coach the patient to take slow, gentle breaths, strictly avoiding holding their breath or bearing down (avoid the Valsalva maneuver, which spikes aortic pressure). - Keep the environment quiet and reassure them that help is minutes away.

Step 5: Managing thermal protection (Preventing the Lethal Triad)

In a patient with a ruptured AAA who is losing blood internally, body temperature drops rapidly. Hypothermia (body temperature below \(35^\circ\text{C}\) or \(95^\circ\text{F}\)) disables the blood's clotting enzymes and platelets, worsening internal bleeding. Cover the patient from chin to toes with dry blankets or coats, and place a blanket underneath them if they are lying on a cold floor.

Understanding ambulance and hospital concepts: Permissive Hypotension and Impulse Control

Knowing what paramedics and emergency physicians will do helps family members communicate clearly during an aortic crisis:

1. Why paramedics practice "Permissive Hypotension" in Ruptured AAA

In most medical emergencies involving low blood pressure, paramedics infuse large bags of intravenous saline rapidly to bring blood pressure back up to \(120/80\text{ mmHg}\). In a ruptured abdominal aortic aneurysm or bleeding aortic dissection, giving large volumes of IV fluid to normalize blood pressure before the surgeon clamps the aorta is dangerous. - Raising systolic blood pressure above \(90\text{ to }100\text{ mmHg}\) with saline dilutes natural clotting factors, cools the blood, and hydraulically blows off the temporary clot ("popping the clot") that is keeping the patient alive. - Therefore, international guidelines (NICE NG156 in the United Kingdom, Society for Vascular Surgery in the United States, and European Society for Vascular Surgery) direct paramedics to practice permissive hypotension: so long as the patient is awake, talking, and maintaining a systolic blood pressure around \(70\text{ to }90\text{ mmHg}\), fluids are restricted while the ambulance drives directly to a vascular surgery operating room.

2. Why hospital bypass to an Aortic Center saves lives

Not every local hospital has a cardiothoracic surgeon (for Type A aortic dissection) or a vascular surgeon with an endovascular hybrid operating suite (for ruptured AAA) on call 24 hours a day. When you clearly state "Suspected Aortic Dissection" or "Known Abdominal Aortic Aneurysm with Sudden Pain and Syncope" during your initial emergency call, regional EMS systems can activate a Direct-to-OR "Code Rupture" or "Code Aorta" pathway, bypassing smaller clinics to take the patient straight to a surgical center.

When speaking to an emergency dispatcher or triage nurse, use this specific clinical language:

  • For Suspected Aortic Dissection:

"I need an emergency ambulance at [Exact Address]. The patient is a [Age]-year-old [male/female] who developed instantaneous, 10-out-of-10 tearing [chest / upper back] pain at [Time]. [He/She] also has [a blood pressure difference of X mmHg between the right and left arms / sudden leg weakness / Marfan syndrome / a bicuspid aortic valve]. We suspect an Acute Aortic Dissection. We have NOT given aspirin."

  • For Suspected Ruptured AAA:

"I need an emergency ambulance at [Exact Address]. The patient is a [Age]-year-old [male/female] with [a known Abdominal Aortic Aneurysm / a history of smoking and hypertension] who suddenly developed severe [abdominal / lower back / flank] pain and [fainted / is pale, sweaty, and dizzy]. We suspect a Ruptured Abdominal Aortic Aneurysm. We are keeping [him/her] lying flat and still."

Triage preparation checklist while the ambulance is en route

  • Exact time of pain onset written down: Surgeons need to know the exact minute the tearing pain or fainting occurred.
  • Both arm blood pressures recorded (if measured): Write down Right Arm BP and Left Arm BP on a piece of paper.
  • Prior aortic imaging or surgical cards located: Grab any discharge summary showing previous aneurysm size (e.g., "4.8 cm infrarenal AAA"), stent-graft implant cards, or heart valve cards.
  • Complete list of blood thinners identified: Immediately tell paramedics if the patient takes warfarin, apixaban, rivaroxaban, dabigatran, edoxaban, clopidogrel, or daily aspirin, so the hospital can prepare emergency reversal agents (such as 4-factor prothrombin complex concentrate [PCC], idarucizumab, or andexanet alfa) before surgery.
  • Nothing by mouth confirmed: Tell paramedics the time of the patient's last meal or drink for safe anesthesia planning.

Prevention and screening: Catching aneurysms before they rupture

Because an intact aortic aneurysm can be repaired electively with a 95 to 98 percent survival rate, whereas a ruptured aneurysm carries a mortality rate of 70 to 80 percent, ultrasound screening and blood pressure management are life-saving:

  1. Siêu âm bụng một lần:

A heart attack is caused by a blood clot blocking a coronary artery, so chewing aspirin stops that clot from growing. In contrast, an aortic dissection or ruptured aneurysm is a mechanical tear in the wall of the aorta; giving aspirin disables platelets, prevents protective clot formation, and causes fatal bleeding into the chest, abdomen, or pericardial sac.

How can I tell the difference between a ruptured abdominal aortic aneurysm and a kidney stone in an older adult?

Both can cause sudden one-sided flank pain radiating into the groin because retroperitoneal blood irritates the same nerves and ureter as a stone. However, any adult over age 55 (especially a smoker or hypertensive) who develops first-time kidney stone pain with dizziness, pale clammy skin, fainting, or low blood pressure must be treated as a ruptured aortic aneurysm until an ultrasound or CT scan rules it out.

What does a blood pressure difference between the left and right arms mean during chest pain?

In a healthy adult, systolic blood pressure between the right and left arms differs by less than 10 millimeters of mercury (mmHg). If someone with sudden severe chest or back pain has a systolic blood pressure difference greater than 20 mmHg between their arms (or a weak wrist pulse on one side), an acute aortic dissection flap is likely blocking the subclavian artery supplying that arm.

If a person with sudden back or abdominal pain faints and then wakes up feeling much better on the floor, is the emergency over?

No, temporary recovery after fainting is a classic sign of a contained retroperitoneal rupture of an abdominal aortic aneurysm or a leaking thoracic dissection. When blood pressure drops during the faint and the patient lies flat, retroperitoneal tissue temporarily plugs the leak; if they stand up or delay calling emergency services, that fragile clot usually bursts into fatal hemorrhage within minutes to hours.
